Abstract

Anemia is a condition characterized by decreased hemoglobin levels in the body. According to the World Health Organization (WHO), approximately 191 million adolescent girls worldwide suffer from anemia. West Sumatra Province had an anemia prevalence rate of 29.8% in 2024, the highest among provinces on Sumatra Island. At RSUP Dr. M. Djamil Padang, anemia ranked first with 3,073 reported cases. This study aimed to implement nursing care directly for patients with anemia through the nursing process approach. The study used a case study method with data collection techniques including interviews, observations, physical examinations, and documentation studies. The nursing diagnoses identified were ineffective peripheral perfusion, nutritional deficit, sleep pattern disturbance, and activity intolerance. After nursing interventions were implemented, the patient’s condition improved, indicated by an increase in hemoglobin level to 11.1 g/dL, blood pressure of 128/80 mmHg, oxygen saturation increasing to 96%, and improved appetite and activity tolerance. Patients are expected to maintain a healthy lifestyle, consume iron-rich foods, and attend regular health check-ups to monitor recovery progress.

Abstract

According to World Health Organization data, Indonesia ranks second in tuberculosis case globally. Hospital records at Doctor Mohammad Djamil Central General Hospital Padang registered two hundred and fifty-eight cases of pulmonary tuberculosis in two thousand and twenty-five, requiring serious management. This study aims to implement comprehensive medical surgical nursing care for a patient with pulmonary tuberculosis. The research method used was a six-day descriptive case study conducted on Mr. A in a pulmonary isolation ward. Assessment results showed that the patient experienced shortness of breath, a productive cough with yellowish-green sputum mixed with blood, nausea, and weight loss. The primary nursing problems included ineffective airway clearance, impaired gas exchange, ineffective breathing pattern, nutritional deficit, risk of infection, and knowledge deficit. Interventions performed included airway management, respiratory monitoring, ventilatory support via high flow nasal cannula, and health education. The final evaluation indicated that all nursing problems were completely resolved. This case study suggests the importance of optimizing health education for patients and families to maintain treatment compliance and prevent transmission within the home environment.

Abstract

Hypertension is a condition caused by problems in the circulatory system, resulting in blood pressure rising above normal levels. According to the World Health Organization (2025), hypertension is the leading cause of premature death worldwide. Based on the Indonesian Health Survey (2023), there were 63,309,629 cases of hypertension, resulting in 427,218 deaths. The Pariaman City Health Department (2024) reported that the prevalence of hypertension in West Sumatra is approximately 25.16%, or about 176,169 people. The highest prevalence rate was found in the city of Bukittinggi at 41.8%, and the lowest in the city of Pariaman at 20.2%. Medical records from Prof. H. Muhammad Yamin SH Pariaman General Hospital indicate that there were 5 cases in 2025. The objective of this case study is to gain an understanding of the implementation and ability to apply comprehensive nursing care for patients with hypertension. The method used in this scientific paper is the case study method, conducted in the internal medicine ward of Prof. H. Muhammad Yamin SH General Hospital in Pariaman from January 31, 2026, to February 4, 2026. The nursing issues identified in the patient were decreased cardiac output, acute pain, risk of ineffective peripheral perfusion, risk of bleeding, and knowledge deficit.  The intervention was implemented over 5 days, resulting in the following improvements for the patient: reduced pain complaints, improved pulse rate, reduced fatigue, reduced pallor, improved acral circulation, improved skin turgor, improved skin moisture, increased adherence to recommendations, and increased behavior consistent with knowledge. It is recommended that the patient take medication regularly and that the patient and family adopt a healthy lifestyle, as well as undergo routine check-ups at a healthcare facility to stabilize blood pressure.

Abstract

Chronic Obstructive Pulmonary Disease (COPD) caused 3.23 million deaths worldwide in 2024, making it the third leading cause of death. In Indonesia, COPD accounted for 1,111 deaths, while its prevalence in West Sumatra reached 3.1% or approximately 164,000 people. This case study aimed to describe the nursing care process for a patient with COPD, including assessment, nursing diagnosis, intervention planning, implementation, evaluation, and documentation. The study involved one patient (Mr. R) who was treated in the pulmonary ward of RSUD Prof. H. M. Yamin, SH Pariaman from February 2–6, 2026. Assessment findings revealed dyspnea, productive cough with difficulty expectorating sputum, sleep pattern disturbances, and decreased appetite. Nursing diagnoses included ineffective airway clearance, disturbed sleep pattern, and risk for nutritional deficit. Interventions focused on respiratory monitoring, effective coughing and deep-breathing exercises, semi-Fowler positioning, sleep support, and nutritional management. After five days of care, the patient's condition improved, with reduced dyspnea, easier sputum expectoration, better sleep quality, and increased appetite. Continuous education is essential to prevent recurrence

Abstract

Diabetes Mellitus Type II is a chronic metabolic disease characterized by elevated blood glucose levels due to insulin resistance and impaired insulin secretion. Globally, the number of people with diabetes in 2024 is estimated to reach 800 million, with more than 95% being Type II Diabetes Mellitus. At RSUD Prof. M. Yamin, SH Pariaman in 2025, there were 189 recorded cases of Type II Diabetes Mellitus without complications. The purpose of this scientific paper is to provide nursing care to a patient with Type II Diabetes Mellitus in the Internal Medicine Ward of RSUD Prof. M. Yamin, SH Pariaman. The method used is a case study with a nursing process approach, including observation, interviews, physical examination, and data collection from medical records. This study was conducted in the internal ward for 5 days, from February 6, 2026 to February 10, 2026, with the subject Mrs. N. The assessment results identified nine nursing problems: unstable blood glucose levels, acute pain, impaired skin/tissue integrity, ineffective peripheral perfusion, impaired physical mobility, sleep pattern disturbance, fatigue, risk of infection, and risk of nutritional deficit. The evaluation after 5 days of nursing care showed that five diagnoses were resolved, namely unstable blood glucose levels, ineffective peripheral perfusion, fatigue, risk of infection, and risk of nutritional deficit. Meanwhile, the other four diagnoses were not fully resolved but showed improvement. It is expected that the patient and family will maintain adherence to a healthy lifestyle, therapy, diet, and regular follow-up in order to prevent complications.
