Abstract

ABSTRACTBackground: The decision to upgrade service level in the era of National Health Insurance is a demand arising for health services. There are several factors that influence the demand for health services.Objectives: The aims of the study are to determine the influence of income level, availability of care classes, hospital common rates, service quality, comfort, privacy, the completeness of facilities and additional insurance against the upgrading service level selection of Healthcare and Social Security Agency (BPJS Kesehatan) inpatients from class I to VIP class and the most dominant factor.Research Methods: This study was an observational study with cross-sectional design of the 284 respondents were divided proportionally from 6 hospitals in Kediri in collaboration with the BPJS Kesehatan Kediri Branch. The research was conducted by interviewing the respondents using a questionnaire at the time of going home from the hospital or during outpatient control.Results: The results showed eight independent variables can influence on the model simultaneously. Factors that are statistically significant influence patient choice grade is hospital common rates factor (p=0.001); (²=0.208). This may imply that the choice of grade-patient tends to grow at 20.8% every hospitals common rates reduction.Conclusions: Hospitals reasonable rates will make patients reconsider their ability to pay. Rates adjusment and standardization of hospital services, monitoring of class availability in hospital, as well as the National Health Insurance premium adjustment is required in order to implement better social security. Keywords: inpatients, service level upgrading, BPJS Kesehatan ABSTRAKLatar belakang: Keputusan untuk memilih naik kelas rawat pada era Jaminan Kesehatan Nasional merupakan permintaan yang timbul terhadap pelayanan kesehatan. Terdapat beberapa faktor yang mempengaruhi dalam permintaan pelayanan kesehatan.Tujuan: Mengetahui pengaruh faktor tingkat pendapatan, ketersediaan kelas perawatan, tarif rumah sakit, kualitas pelayanan, kenyamanan, privasi, kelengkapan fasilitas dan asuransi tambahan terhadap pilihan pasien rawat inap BPJS Kesehatan naik kelas perawatan dari kelas I ke kelas VIP dan faktor yang paling dominan.Metode Penelitian: Penelitian ini adalah penelitian observasional dengan rancangan cross-sectional pada 284 responden yang dibagi secara proporsional dari 6 rumah sakit di Kota Kediri yang bekerja sama dengan BPJS Kesehatan Cabang Kediri. Penelitian dilaksanakan dengan melakukan wawancara pada responden menggunakan kuisioner pada saat hendak pulang dari rumah sakit maupun pada saat kontrol rawat jalan.Hasil: Penelitian menunjukkan kedelapan variabel independen dapat memberikan pengaruh terhadap model secara simultan. Faktor yang secara statistik signifikan mempengaruhi pilihan naik kelas rawat adalah faktor tarif umum rumah sakit (p=0.001); (²=0.208). Hal ini dapat diartikan bahwa pilihan naik kelas rawat cenderung bertambah sebesar 20,8% setiap ada penurunan tarif umum rumah sakit.Kesimpulan: Tarif rumah sakit yang wajar akan membuat pasien berpikir ulang tentang kemampuannya dalam membayar. Penyesuaian tarif dan standarisasi pelayanan rumah sakit, monitoring ketersediaan kelas rawat di rumah sakit, serta penyesuaian iuran Jaminan Kesehatan Nasional diperlukan dalam rangka pelaksanaan jaminan sosial yang lebih baik. Kata kunci: pasien rawat inap, naik kelas rawat, BPJS Kesehatan

Abstract

BACKGROUND: Obesity, especially obesity in adolescent, is a worldwide health problem needing much of our attention because it can continue to be obesity in adulthood. About 50% obese adolescents grew up to be obese adults. It was a concern since it is one of risk factor associated with cardiovascular events including hypertension, dyslipidemia, insulin resistance and stroke. Visceral obesity is correlated with diabetogenic, atherogenic, prothrombotic, pro-inflammation, and abnormal metabolism. The objective of this study was to assess the prevalence of obese adolescents in Malang and to identitfy the differences in food compositions between metabolic syndrome and non-metabolic adolescents.METHODS: Prevalence of obesity was determined by assessing BMI in 20 Senior and Junior High Schools. Metabolic syndrome was diagnosed using IDF criteria; waist circumference of >80cm and >90cm for female and male, respectively, and increased triglyceride and decreased HDL concentration levels. The food composition was assessed using food recalls, and then regression linier test was done to define the correlation between food intake and the components of metabolic syndrome.RESULTS: The prevalence of adolescent obesity in Malang had reached 3.32%, with the prevalence of obesity in male subjects higher than in female subjects, i.e. 54.1% compared to 45.9%. The boys had higher mean for height and weight than the girls did, however, the BMI was higher in girls rather than boys. The difference of fat in food composition was significantly higher for the metabolic groups (p=0.031), but the carbohydrate did not significantly differ between the two groups (p=0.407).CONCLUSIONS: The prevalence of adolescent obesity in Malang had reached 3.32%, with the prevalence of 54.1% in male and 45.9% in female. From the statistics test, fat intake showed a significant difference between metabolic syndrome and non-metabolic syndrome groups, but other food compositions didn’t.KEYWORDS: obesity, metabolic syndrome, adolescent

Abstract

BACKGROUND: Consumption of foods and drinks high in energy, fat, and/or sugar beyond the recommended quantities can cause obesity, which triggers the incidence of brain nerve cell death related to oxidative stress, high levels of tumor necrosis factor (TNF)-α and triglycerides, and low high-density lipoprotein (HDL) levels. Progressive nerve cell death causes decreasing cognitive performance. This study aims to prove that an American Institute of Nutrition committee in 1993 (AIN-93M) diet modified with high-fat-high-fructose (HFHF) can decrease the number of hippocampal neurons. A decrease in the number of hippocampal neurons indicates progressive nerve cell death.METHODS: An experimental study using a post-test control group design was carried out using male Sprague Dawley rats. Samples were selected using simple random sampling to divide them into two groups, Group I was AIN-93M-modified HFHF diet (n=14) and Group II was AIN-93M standard (n=16). The number of visible neurons was measured in the hippocampus area of Sprague Dawley rats’ brains, stained with haemotoxylin and eosin (H&E) and scanned under 400x magnification. Neurons were counted in 10 visual fields using the "Cell_Count" application.RESULTS: The data were analysed by Pearson’s correlation test using SPSS. The results show that rats in Group I had a greater weight gain and fewer neurons than those in the Group II (p=0.023, r=-0.413).CONCLUSION: The consumption of foods high in fat and fructose can cause an increase in nerve cell death, as shown by the decrease in the number of hippocampal neurons.KEYWORDS: brain nerve cells, high fat, high fructose, increased body weight

Abstract

One of forensic identification that can be done is through DNA testing. FBI recommends to use 13 short tandem repeat loci known as CODIS 13 for forensic DNA testing. We tested such system to characterize and determine whether individuals with Madura ethnic background (Madurese) resides in different region differ in CODIS 13 pattern. Employing standard PCR-RFLP for STR CODIS 13 our results showed that indeed the CODIS 13 pattern between the two regions were distinct. Most frequent alleles appeared in Madura ethnic from Bangkalan were at locus D3S1358: allele 16; VWA: 16; FGA: 24 and 26; TH01: 8; TPOX: 9; CSF1PO: 10; D5S818: 11 and 13; D13S317: 14; D7S820: 9; D8S1179: 15; D21S11: 27, 28 and 30; D18S51: 16; and D16S539: 15. Most frequent alleles appeared in each locus of Madura ethnic from Probolinggo were locus D3S1358: allele 17; VWA: 17;  FGA: 23 and  26;  TH01: 7;  TPOX: 10;  CSF1PO: 10, 11 and 13; D5S818: 11 and 12;  D13S317: 11;  D7S820: 11;  D8S1179: 11 and 12;  D21S11: 30; D18S51: 14; and D16S539: 8. The heterozygosity levels of Madura ethnic from Bangkalan’s CODIS 13 varied from 0.4180 (D5S818) to 0.9102 (FGA). The power of exclusion was between 0.1569 (D5S818) to 0.7667 (FGA). The power of discrimination was between 0.2816 (D5S818) to 0.9811 (FGA). The paternity index was between 0.8591 (D5S818) to 5.5653 (FGA). The heterozygosity index of Madura ethnic from Probolinggo varied between 0.63021 (VWA) to 0.94445 (FGA), power of exclusion was between 0.33418 (VWA) to 0.84779 (FGA), and power of discrimination ranged from 0.60501 (TPOX) to 0.99305 (FGA).

Abstract

The World Health Organization (WHO) estimates a continuous increase in the number of hypertension cases alongside the expanding population. It is projected that approximately 29%, or around 1.6 billion people worldwide, will be afflicted with hypertension by 2025. The findings of the 2018 Riskesdas survey reveal a prevalence of hypertension in the population aged > 18 years based on national measurements, standing at 34.11%. This represents a 9% surge from the 2013 Riskesdas data, which recorded a prevalence of 25.8%. East Java Province ranks seventh with a prevalence rate of 36.32%, surpassing the national average. In the pursuit of hypertension control, it has been integrated into the Preventive Efforts of the Healthy Indonesia Family Approach Programs (PIS-PK) under point 7, mandating continuous monitoring of hypertension status for family members with a history of hypertension.  This study aims to identify determinant factors associated with enhanced services and visits for hypertension patients at Universitas Brawijaya Hospital in Malang amidst the pandemic. This study employs an observational analytic design, aiming to analyze the relationship between education level and medication adherence among hypertension patients during the COVID-19 pandemic. Temporally, this research adopts a cross-sectional approach, where both dependent and independent variable data are collected simultaneously. The study was conducted at Brawijaya University Malang, utilizing data from hypertensive patients who visited the hospital between March 2020 and March 2022. Logistic regression analysis with α = 0.05 will be employed for analytical purposes. The variable most strongly associated with hypertension patient visits is insurance ownership, exhibiting a Chi-Square value of 26.716. Additionally, the age variable demonstrates a correlation with patient visits, with a Chi-Square value of 4.312. The corresponding P-values for insurance ownership (p = 0.000) and age (p = 0.038) indicate a significant relationship between insurance ownership, age, and patients' motivation to seek healthcare. The determinant factors that positively influence hypertensive patient visits during the pandemic at Brawijaya University Hospital are insurance ownership and age. On the other hand, gender, regional affordability, and occupation exert a lesser impact on patient visits.

Abstract

One of the biggest cost control problems among hospitals in the National Health Insurance is chronic disease drug services. Billing fee delays or unpaid billing fees often occur due to patient failure to revisit the doctor according to the requirement of 30 (thirty) days from the last visit. It resulted in the hospital's inability to collect the financing for chronic disease drugs from Social Security Administrative Agency (BPJS). This study aimed to discover the root cause of the pending billing of chronic disease drug costs and executable solutions in hospitals. This research was conducted at X Hospital Malang. This research used mix-method of qualitative and quantitative. The qualitative method were observations of the service flow for chronic disease patients, interviews with all unit coordinators, and focus group discussion with the Pharmacy Unit and Casemix Unit coordinators. The quantitative method was done using CARL scoring to determine the priority of the problem solution. The root of the problem was the lack of adequate systems and resources in chronic disease drug services. The possible solution is to modify the flow in the outpatient pharmacy. It is necessary to apply patient screening through the Online Pharmacy Application at the outpatient pharmacy to prevent patients who visit for less than 30 days. The results of the solution implementation are expected to reduce the amount of money from pending billing for chronic disease drug costs, thereby minimizing hospital financial losses.

Abstract

Banyak tenaga kesehatan profesional yang mengalami burnout selama pandemi COVID-19, termasuk psikiater sebagai pemberi layanan kesehatan jiwa. Meningkatnya jumlah kasus, konsulan, rujukan, serta keterbatasan sumber daya membuat karakteristik kerja psikiater penuh tekanan dan menurunkan kualitas kehidupan profesional psikiater. Penelitian ini bertujuan mengeksplorasi karakteristik pekerjaan dan kualitas hidup profesional psikiater selama pandemi COVID-19. Responden dalam penelitian ini berjumlah sepuluh orang psikiater anggota Perhimpunan Dokter Spesialis Kedokteran Jiwa Indonesia (PDSKJI) cabang Malang yang bertugas dalam pelayanan klinis selama pandemi COVID-19 dan dipilih menggunakan purposive sampling. Penelitian ini menggunakan pendekatan fenomenologi interpretatif dengan metode wawancara mendalam. Didapatkan dua tema terkait karakteristik pekerjaan psikiater selama pandemi COVID-19, yaitu peran penting dalam mendukung kesejahteraan pasien, keluarga pasien, staf tenaga kesehatan lain yang berisiko, dan masyarakat, serta dualitas tanggung jawab menghadapi masalah neuropsikiatri maupun kondisi medis umum. Didapatkan tiga tema terkait kualitas hidup psikiater selama pandemi COVID-19, yaitu dilema antara hak atas keselamatan dan tanggung jawab, kepuasan kerja, serta kelelahan dalam bekerja. Kesimpulannya, karakteristik kerja psikiater selama pandemi COVID-19 cukup kompleks karena memainkan peran penting bagi pasien dan keluarganya, tenaga kesehatan lain, serta masyarakat, dan juga menghadapi dualitas tanggung jawab. Kualitas hidup psikiater selama pandemi COVID-19 dipengaruhi oleh dilema antara tanggung jawab dan hak atas keselamatan. Psikiater menemukan rasa kepuasan, walaupun secara umum kepuasan kerja menurun karena tidak bisa melakukan pelayanan maksimal. Meningkatnya rasa cemas, menurunnya kepuasan kerja, serta peningkatan beban kerja menimbulkan kelelahan pada psikiater, sehingga menurunkan kualitas hidup profesional psikiater.

Abstract

he construction sector is a dynamic industry that produces projects in terms of infrastructure and buildings. This sector faces challenges, particularly in worker safety and health (K3) in every project. Statistics show that the increase in fatalities and permanent disability cases due to accidents at construction sites in Indonesia is among the highest compared to other industrial sectors. This study examines the key factors and challenges in meeting occupational safety and health (K3) standards to achieve quality infrastructure. The method used in this study is a literature review conducted through databases such as PubMed, Science Direct, and Google Scholar. After selection, 42 articles were used as sources for analysis in this study. The results indicate that technical aspects, both Internal Risks and Project Risks, are the most influential causes of health and safety risks in a building construction project. Meanwhile, External Non-Technical Risks are the least influential causes of health and safety risks in a building construction project. Risk control can be carried out through good working methods, the use of tools according to warning signs, the appropriate use of personal protective equipment (PPE), and clear explanations of accident risks and risk control methods by the contractor.

Abstract

Insufficient breast milk production often occurs during the first week postpartum due to psychological stress, inadequate stimulation of prolactin and oxytocin hormones, and nutritional aspects of the mother’s diet. Efforts to improve and increase breast milk production include providing back massage to induce a relaxing sensation and fulfilling the needs for a balanced nutritious diet, which can effectively stimulate the release of prolactin and oxytocin hormones. This research aims to compare the effectiveness of oxytocin massage and banana flower consumption in increasing breast milk volume. This study used true experimental with a pre-post test design (excluding a control group) and involved 32 postpartum mothers with low-stress levels and low to medium socioeconomic status. Breast milk volume was evaluated by breast pumping for 30 minutes and measuring the output with a measuring cup. Data were analyzed using an independent t-test. There was a 6.375 cc increase in breast milk volume in the banana flower consumption group compared to the oxytocin massage group. However, statistically, both treatments had similar effects, as there were no significant differences between the groups (p = 0.519). Therefore, both oxytocin massage and banana flower consumption can be considered effective alternatives for increasing breast milk volume.

Abstract

During the first 6 month period, newborns who are exclusively breastfed consume solely breast milk and abstain from any other form of nourishment, such as food or liquids, including water. The government Indonesia has pledged to attain a target proportion of exclusive breastfeeding, with a goal of reaching up to 80%. The X Community Health Centre in Malang City achieved a relatively low level of exclusive breastfeeding coverage in 2022, with only 51.18%. This falls far short of the established target. Internal variables and the mother's opinion of the involvement of health personnel are influential in determining exclusive breastfeeding. The aims of this research were to analyses the influence of internal factors and maternal perspectives on the role of healthcare providers in promoting exclusive breastfeeding. This cross-sectional study was conducted in January-March 2024 at the X Health Centre Working Area in Malang City. The study utilized observational analytics. The study's sample consisted of one hundred moms with children older than six months. Chi-square test yields identify there wasn't significant association between employment also having multiple children (p-value>0.05). However, various factors related to exclusive breastfeeding, like as age, education, family support, and mother's intellectual and perception about the role of health workers, were found to be significantly associated (p-value <0.05). The multivariate analysis using multiple logistic regression revealed that family support has a significant impact on exclusive breastfeeding. The p-value of 0.006 and an odds ratio of 26,595 indicate that family support increased the likelihood of exclusive breastfeeding by 26-fold.
