Abstract

Insufficient breast milk production often occurs during the first week postpartum due to psychological stress, inadequate stimulation of prolactin and oxytocin hormones, and nutritional aspects of the mother’s diet. Efforts to improve and increase breast milk production include providing back massage to induce a relaxing sensation and fulfilling the needs for a balanced nutritious diet, which can effectively stimulate the release of prolactin and oxytocin hormones. This research aims to compare the effectiveness of oxytocin massage and banana flower consumption in increasing breast milk volume. This study used true experimental with a pre-post test design (excluding a control group) and involved 32 postpartum mothers with low-stress levels and low to medium socioeconomic status. Breast milk volume was evaluated by breast pumping for 30 minutes and measuring the output with a measuring cup. Data were analyzed using an independent t-test. There was a 6.375 cc increase in breast milk volume in the banana flower consumption group compared to the oxytocin massage group. However, statistically, both treatments had similar effects, as there were no significant differences between the groups (p = 0.519). Therefore, both oxytocin massage and banana flower consumption can be considered effective alternatives for increasing breast milk volume.

Abstract

Mental health disorders are a significant issue, yet there is currently no consistent data regarding the factors that may contribute to their occurrence. The aim of this study was to identify the factors that may influence the occurrence of mental health disorders in the undergraduate student population. This meta-analysis, registered in PROSPERO, was conducted as of September 5, 2024. The systematic searches were conducted on three databases (Scopus, Embase, and PubMed). Data on factors related to the occurrence of mental health disorders in undergraduate students were collected to calculate pooled point estimates using the Mantel-Haenszel test. Out of 6,837 articles, 16 articles were included in the analysis. Our findings revealed that female students had a higher chance of experiencing mental health disorders compared to male students (OR: 1.43; 95%CI: 1.14–1.80; p-Egger of 0.1041; p-Heterogeneity of <0.0001; p=0.0020). The chance of having mental health disorders was also higher among single students compared to those in relationships (OR: 1.20; 95%CI: 1.02–1.43; p-Egger of 0.0535; p-Heterogeneity of 0.1130; p=0.0180). This study did not find any associations between age group, family income, smoking, living arrangements, years of study, or regularity of physical activity with the occurrence of mental health disorders. These findings could serve as a basis for improving policies aimed at reducing the rates of mental health disorders in undergraduate students.

Abstract

Chronic Kidney Disease (CKD) is a significant global health issue, with hypertension being one of its major risk factors. This article explores a comprehensive Family Medicine-based approach to managing a 54-year-old male patient with CKD accompanied by hypertension. Through a holistic strategy, emphasis is placed on blood pressure control, lifestyle modifications, and strengthening the family's role in supporting the patient’s adherence to therapy. This approach aims to enhance the patient's quality of life, prevent further complications, and optimize remaining kidney function. The study underscores the importance of interprofessional collaboration and effective communication between the patient, family, and healthcare providers for successful CKD management within a primary care context.

Abstract

To study the prevalence and characteristics of age related cataract in an adult population in Malang East Java. A population based, cross sectional study of 692 adults (aged > 50 years) were selected from September until December 2019 from Smarthealth Database. The presence of lens opacity was examined by slit lamp biomicroscopy and evaluated according to Lens Opacity Classification System III. The subtype of cataract present, including nuclear, cortical, and posterior subcapsular, were identified from univariate and 95% confidence interval (CI) for association of each specific type of cataract with age, health examination and medical history. A total of 692 patients with aged > 50 years old were included in this study. The prevalence of cataract was 48,7% in participants over 50 years old. From 337 respondents were in the age range 50-70 years, as many as 240 people (71,22%) patients were female, as many as 206 people (61,13%) patients with primary school education, and 151 people (44,8%) patients work as housewife. The standardized prevalences of clinically significant nuclear, cortical, and posterior subcapsular cataract (PSC) were 7,42%, 7.72%, and 5.34% in an adult population in Mendalanwangi, Sidorahayu and Cepokomulyo village in Malang districs. The prevalence of nuclear, cortical and posterior subcapsular cataract increased high prevalence of age related cataract in the adult population remains a severe public health problem.

Abstract

Bacterial vaginosis is a reproductive system infection that causes a variety of symptoms. This condition can be caused by a variety of risk factors, including Vulvovaginal Flora Disruption. This case was detected in a Malang City primary health care. An in-depth history is collected to identify risk factors, followed by a home visit to detect risk factors. We shall explore the role and use of family medicine in cases of bacterial vaginosis in this case report. Because of the complicated interplay of biopsychosocial variables, comprehensive and holistic treatment is required.

Abstract

The Confinenment action to the mental disordes suffer is the action done by the society to limited  the suffer’s mobilitas or activities its done based on the hopeless of the family which  the medical effort can not made the suffer better  than before. The economic burden is not small, additional with stigma from the society and the less  optimal of mental health services base on family. Society and medical”s crew.The result of this researchplore the family experience towar the confinement and re confinement of the disorder’s family members in job area of Bantur Community health service of Malang Regency, East Java Province.The research methodology of this thesis use qualitative research (purposive sampling)about 6 sample. The process of data taking used the deep interview with semi structure helping by interview guide and fieldmemorize, its found 7 theme; 1) The cause of mental disorder, 2) The attitude change, 3) The Family’s comment on this situation, 4) The society stigma, 5) Confinement re confinement, 6) The confinement release, 7) The suffer family and society recovery.According the family conclusion the cause of disorder mental are pfisic condition and social pressure , and the moment of confinement –free-reconfinement happened of the society coment toward the situation stigma of the society and unoptimally the medical service. The free of confinement happened because of the family readiness,society and medical service and government involment. The rehabilition of the suffer after free from the confinement need the support from the family, society, community health service and related government.

Abstract

Menopause merupakan suatu keadaan fisiologis yang pasti dialami oleh semua wanita, pada masa menopause terjadi peningkatan stres oksidatif karena penurunan hormon estrogen (hipoestrogen). Salah satu organ yang mengalami dampak akibat terjadinya hipoestrogen adalah vagina. Pada vagina terjadi atrofi dan penipisan epitel. Antosianin merupakan salah satu antioksidan yang dapat menyelamatkan sel – sel tubuh dari stres oksidatif. Penelitian ini dilakukan untuk membuktikan pengaruh antosianin terhadap ketebalan epitel, indeks apoptosis dan ekspresi p53 sel epitel vagina pada tikus putih (Rattus norvegicus) ovariektomi. Desain penelitian ini adalah True Experimental laboratory, dengan menggunakan pendekatan posttest only group control design. Penelitian ini dilakukan dengan memberikan perlakuan pada hewan coba, yaitu tikus putih (Rattus norvegicus) betina. Hasil penelitian menunjukkan pemberian antosianin terbukti berpengaruh secara signifikan (p-value = 0,000 < α=0,05) dalam menurunkan ekspresi p53 dan indeks apoptosis sel epitel vagina serta meningkatkan ketebalan epitel vagina pada tikus ovariektomi.

Abstract

This study was aimed to examine the influence of clarity of waiting time information and the comfort of waiting room towardssatisfaction through waiting time perception of drug services. Chosen observational analytical study by cross-sectional study was used as methods in this study. Sampling technique used in this study was purposive sampling. 120 respondents were chosen as study sample among all of the outpatients out of pocket which waiting for drug services on March - April 2017. Path analysis was used as data analysis method. Study result showed that clarity of waiting time information and the comfort of waiting room had a positive and significant influence on waiting time perception and satisfaction awaiting drug services. As of intervening variable, waiting time perception was contributed increasing influence clarity of waiting time information and the comfort of waiting room towardssatisfaction with drug services. The most predominant variable in constructing satisfaction with drug servicesthrough waiting time perception was the comfort of the waiting room.

Abstract

Osteoarthritis (OA), a prevalent degenerative joint disease, poses significant challenges in primary care due to its chronic nature and multifaceted impact on patients' lives. This case report highlights the importance of a comprehensive, patient-centered approach to managing OA, integrating biomedical, psychological, and social considerations within the family medicine framework. A 60-year-old male, a civil servant and single parent, presented with chronic, worsening left knee pain and concurrent left shoulder pain of three months' duration, alongside a history of controlled hypertension. Initial self-management with topical analgesics and knee support provided minimal relief. Clinical assessment, based on ACR and ICPC criteria, led to a diagnosis of left genu osteoarthritis and left shoulder osteoarthritis. The management strategy adopted a holistic perspective, acknowledging the patient's physical symptoms, his concerns as a single parent, and the psychological burden of his condition. Non-pharmacological interventions, including lifestyle modifications, ergonomic advice, and regular light exercise, were emphasized. Pharmacological treatment involved analgesics, while a referral to an orthopedic specialist was initiated due to persistent symptoms. The Family APGAR score indicated good family function, crucial for supporting comprehensive interventions. This case underscores how family dynamics and psychosocial stressors influence chronic disease presentation and management in primary care. This case illustrates that effective OA management extends beyond pharmacological interventions to encompass a thorough understanding of the patient's biopsychosocial context. A robust family medicine approach, integrating patient education, lifestyle modification, and psychosocial support, is crucial for improving outcomes and enhancing the quality of life for individuals with chronic conditions, such as osteoarthritis.

Abstract

Atopic dermatitis (AD) is a chronic inflammatory skin condition often exacerbated by psychosocial stressors, significantly impacting patients' quality of life. This case highlights the intricate relationship between psychological burden and AD pathogenesis, emphasizing the crucial role of a holistic family medicine approach in its comprehensive management. We present the case of a 54-year-old male, a motivational trainer, who presented with recurrent pruritic erythematous plaques on both ankles, consistent with chronic atopic dermatitis. His symptoms were significantly aggravated by psychological stress stemming from a substantial decline in work engagements and income during the recent pandemic, which also restricted his stress-coping hobbies. Clinical evaluation, guided by Hanifin Rajka criteria, confirmed the diagnosis of atopic dermatitis, with psychological stress identified as a key exacerbating factor through neuro-immuno-cutaneous mechanisms. A comprehensive family medicine intervention was employed, integrating pharmacological management with topical corticosteroids, non-pharmacological strategies like skin barrier optimization, and extensive psychosocial counseling. The patient's functional family dynamics, as assessed by a high Family APGAR score, supported the feasibility of a multi-faceted approach, addressing both physical symptoms and underlying stressors. This case underscores the necessity of a biopsychosocial model in managing chronic conditions like atopic dermatitis. A holistic family medicine perspective, incorporating patient education, stress management, and collaborative care, is vital for achieving optimal outcomes and improving patient well-being.
