Abstract

Solid medical waste management remains a significant challenge in Indonesia. According to data from the 2022 Indonesian Health Profile , only 38.9% of hospitals and health centers in Indonesia adhere to established standards for medical waste management. The impact of not managing medical waste according to standards is the increased risk of health and environmental problems. The present research design is of a qualitative nature, employing a case study approach in a government hospital. The data collection methods included in-depth interviews, document analysis, and field observations. The purposive sampling technique was employed, focusing on selecting participants who deeply understand the subject matter under investigation. The sample comprised seven individuals/informants from the Government Hospital, the Health Office, and the recipient of waste treatment services. The data derived from these interviews, document analysis, and field observations were systematically analyzed until the point of saturation was reached. The analysis yielded eight themes: The management delayed enforcing green hospital policies; The management neglected organizational governance of solid medical waste management without any policy; Technical governance of solid medical waste management was perplexing: policy or opportunity?; Inadequate communication and coordination between management and units; Hospital visitors and third parties slowed down the solid medical waste management process; The impact of management delays: solid medical waste accumulated; Inefficiency due to missing all recycling opportunities for solid medical waste; and The strategies for optimizing solid medical waste management governance. It can be concluded that the management of solid medical waste in government-owned hospitals is not yet at the most effective state, and there are still wastes and constraints in its governance. To address these issues, a number of strategies have been implemented, including adjustments to medical waste management and educational initiatives targeting the hospital community.

Abstract

Tingginya prevalensi Nyeri Neuropati Diabetik (NND) menunjukkan urgensi penanganan komplikasi pada penderita Diabetes Melitus (DM) tipe 2. Faktor risiko diabetes memengaruhi kualitas hidup pasien. Di wilayah kerja Puskesmas Dukuh Kupang, sebagian besar penderita diabetes diketahui merupakan kelompok usia lanjut dan memiliki tingkat pendidikan yang rendah. Kegiatan ini bertujuan untuk meningkatkan pemahaman masyarakat tentang komplikasi NND, pemeriksaan kesehatan, serta memberikan pengobatan gratis bagi penderita DM tipe 2 dengan NND. Kegiatan dilaksanakan di Puskesmas Dukuh Kupang Surabaya yang dihadiri oleh seluruh pasien DM tipe 2 sebanyak 65 orang. Metode pelaksanaan meliputi ceramah, diskusi, pemeriksaan kesehatan, dan pengobatan gratis. Hasil evaluasi terhadap efektivitas kegiatan dilakukan menggunakan instrumen pre- dan post-test menunjukkan terdapat peningkatan yang signifikan pengetahuan peserta setelah diberikan edukasi (p<0,001). Melalui program ini disimpulkan bahwa edukasi dapat meningkatkan pengetahuan pasien DM tipe 2 dan diharapkan masyarakat dapat memperoleh layanan kesehatan dasar yang cepat dan tepat, sehingga mampu mengatasi permasalahan terkait NND.

Abstract

ABSTRAK1000 hari pertama kehidupan (HPK) mencakup fase awal dari konsepsi hingga anak mencapai usia 2 tahun, dimana masa ini dimulai dari periode kehamilan selama 270 hari. Proses perkembangan melibatkan peningkatan kompleksitas struktur dan kinerja tubuh, termasuk kemampuan motorik kasar, motorik halus, komunikasi verbal, interaksi sosial, dan kemandirian. Penelitian ini merupakan penelitian kuantitatif deskriptif metode observasional dengan pendekatan cross sectional dengan sampel menggunakan metode purposive sampling. Hasil analisis menggunakan Chi-Square memiliki nilai signifikansi pada hubungan pengetahuan 1000 HPK dengan usia ibu (P-value 0.008), pendidikan terakhir (P-value 0.000), dan pekerjaan (P-value 0.000). Pada hubungan pengetahuan stimulasi perkembangan anak dengan usia ibu (P-value 0.000), pendidikan terakhir (P-value 0.142), dan pekerjaan (P-value 0.005). Secara statistik sebagian besar terdapat hubungan antara karakteristik orang tua dengan pengetahuan orang tua mengenai 1000 HPK dan pengetahuan stimulasi perkembangan anak. Pengetahuan orang tua mengenai 1000 HPK tergolong kurang dan pengetahuan stimulasi perkembangan anak tergolong cukup. Oleh karena itu, penelitian ini dapat ditindaklanjuti untuk mengevaluasi lebih lanjut terkait pengetahuan orang tua mengenai 1000 HPK dan pengetahuan orang tua mengenai stimulasi perkembangan anak pada orang tua. Kata kunci : 1000 hari pertama kehidupan, karakteristik, pengetahuan, stimulasi perkembangan ABSTRACT             The first thousand days of life (HPK) covers the initial phase from conception until the child reaches the age of 2 years, which starts from the 270-day pregnancy period. The first thousand days of life is the golden period for children to experience the development of physical, cognitive, emotional and social aspects. This study aims to analyze the relationship between parental characteristics with knowledge of 1000 HPK and knowledge of child development stimulation in Pakis District, Malang Regency. This study is a descriptive quantitative research observational method with a cross sectional approach with a sample using purposive sampling technique with a total sample of 80 parents who have children aged 0-2 years in Pakis District, Malang Regency. The results showed that the characteristics associated with knowledge of 1000 HPK were maternal age (P-value 0.008), latest education (P-value 0.000), and occupation (P-value 0.000). In the relationship of knowledge of child development stimulation with maternal age (P-value 0.000), last education (P-value 0.142), and occupation (P-value 0.005). Statistically, there was mostly a relationship between parental characteristics and parental knowledge about 1000 HPK and knowledge of child development stimulation. Parents' knowledge of 1000 HPK was classified as insufficient and knowledge of child development stimulation was classified as moderate. Therefore, this research can be used as a government program and intensify the 1000 HPK program and the importance of stimulating child development by parents or making certain policies related to it. Abstract

Coronavirus or severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a virus that attacks the respiratory system. Coronavirus can cause mild disorders of the respiratory system, severe lung infections, and even death. This study aims to determine the relationship between knowledge, workload, an increasing number of patients, risk of exposure and infrastructure with nurses' anxiety. This study used an analytic observational design with a cross-sectional approach. The sample is 61 nurses, the sampling technique uses Total Sampling, and the analysis uses the Spearman Rank test, Pearson correlation. The results of this study indicate that there is a relationship between knowledge (p = .002, r = - .389), workload (p-value = .047, r = .256), an increase in the number of patients (p-value = .009, r = .333), risk exposure (p-value = .048, r = - .254), and infrastructure (p-value = .040, r = - .264) with anxiety. There is a significant relationship between knowledge, workload, an increasing number of patients, risk of exposure, and facilities and infrastructure with anxiety. The hospital needs to prepare adequate infrastructure and facilities for medical personnel who are on the front line, especially related to personal protective equipment.Abstrak: Virus Corona atau severe acute respiratory syndrome coronavirus 2 (SARS-CoV- 2) merupakan virus yang menyerang sistem pernapasan. Virus Corona bisa menyebabkan gangguan ringan pada sistem pernapasan, infeksi paru-paru yang berat, hingga kematian.  penelitian ini bertujuan untuk mengetahui hubungan pengetahuan, beban kerja, peningkatan jumlah pasien, resiko paparan dan sarana prasarana dengan kecemasan perawat. Penelitian ini menggunakan desain observasional analitik dengan pendekatan cross sectional. Sampel sebanyak 61 orang perawat, tehnik sampling menggunakan Total Sampling, dan analisis menggunakan uji Spearman Rank, korelasi pearson. Hasil penelitian ini menunjukkan adanya hubungan antara pengetahuan (p= .002, r= - .389), beban kerja (p value= .047, r= .256), peningkatan jumlah pasien (p value = .009, r = .333), resiko paparan (p value=.048, r = -.254), dan sarana prasarana (p value= .040, r= - .264) dengan kecemasan. Terdapat hubungan signifikan antara Pengetahuan, beban kerja, peningkatan jumlah pasien, resiko paparan, dan sarana dan prasarana dengan Kecemasan. pihak rumah sakit perlu mempersiapkan sarana dana prasarana yang memadai buat tenaga medis yang berada di garda terdepan terutama terkait dengan alat pelindung diri.

Abstract

To study the prevalence and characteristics of age related cataract in an adult population in Malang East Java. A population based, cross sectional study of 692 adults (aged > 50 years) were selected from September until December 2019 from Smarthealth Database. The presence of lens opacity was examined by slit lamp biomicroscopy and evaluated according to Lens Opacity Classification System III. The subtype of cataract present, including nuclear, cortical, and posterior subcapsular, were identified from univariate and 95% confidence interval (CI) for association of each specific type of cataract with age, health examination and medical history. A total of 692 patients with aged > 50 years old were included in this study. The prevalence of cataract was 48,7% in participants over 50 years old. From 337 respondents were in the age range 50-70 years, as many as 240 people (71,22%) patients were female, as many as 206 people (61,13%) patients with primary school education, and 151 people (44,8%) patients work as housewife. The standardized prevalences of clinically significant nuclear, cortical, and posterior subcapsular cataract (PSC) were 7,42%, 7.72%, and 5.34% in an adult population in Mendalanwangi, Sidorahayu and Cepokomulyo village in Malang districs. The prevalence of nuclear, cortical and posterior subcapsular cataract increased high prevalence of age related cataract in the adult population remains a severe public health problem.

Abstract

The hospital provides health services such as inpatient, outpatient, and emergency departments. The quality of nursing care is crucial as patients rely on nurses to address their complaints. This study aims to determine the relationship between nurse burnout and patient safety culture with the quality of nursing care. An analytical observational design with a cross-sectional study approach was conducted in two Type B Hospitals in Central Sulawesi Province with 235 nurse respondents in the inpatient room, using purposive sampling. Data analysis included the Pearson product-moment test and multivariate modeling. Results indicated a significant relationship between nurse burnout and nursing care quality (p-value <0.05), as well as between patient safety culture and nursing care quality. The dominant variable related to the quality of nursing care was patient safety culture (β = 0.723). The study highlights that nurse burnout negatively impacts the quality of nursing care, whereas a strong patient safety culture improves it. Theoretically, this research expands the understanding of the interplay between nurse burnout, patient safety culture, and nursing care quality, emphasizing the need for organizational support and effective communication. Practically, it suggests that hospital administrators should implement strategies to reduce nurse burnout and foster a positive patient safety culture, such as stress management programs and a supportive work environment, to enhance nursing care quality and improve patient outcomes.

The Confinenment action to the mental disordes suffer is the action done by the society to limited  the suffer’s mobilitas or activities its done based on the hopeless of the family which  the medical effort can not made the suffer better  than before. The economic burden is not small, additional with stigma from the society and the less  optimal of mental health services base on family. Society and medical”s crew.The result of this researchplore the family experience towar the confinement and re confinement of the disorder’s family members in job area of Bantur Community health service of Malang Regency, East Java Province.The research methodology of this thesis use qualitative research (purposive sampling)about 6 sample. The process of data taking used the deep interview with semi structure helping by interview guide and fieldmemorize, its found 7 theme; 1) The cause of mental disorder, 2) The attitude change, 3) The Family’s comment on this situation, 4) The society stigma, 5) Confinement re confinement, 6) The confinement release, 7) The suffer family and society recovery.According the family conclusion the cause of disorder mental are pfisic condition and social pressure , and the moment of confinement –free-reconfinement happened of the society coment toward the situation stigma of the society and unoptimally the medical service. The free of confinement happened because of the family readiness,society and medical service and government involment. The rehabilition of the suffer after free from the confinement need the support from the family, society, community health service and related government.

Abstract

Menopause merupakan suatu keadaan fisiologis yang pasti dialami oleh semua wanita, pada masa menopause terjadi peningkatan stres oksidatif karena penurunan hormon estrogen (hipoestrogen). Salah satu organ yang mengalami dampak akibat terjadinya hipoestrogen adalah vagina. Pada vagina terjadi atrofi dan penipisan epitel. Antosianin merupakan salah satu antioksidan yang dapat menyelamatkan sel – sel tubuh dari stres oksidatif. Penelitian ini dilakukan untuk membuktikan pengaruh antosianin terhadap ketebalan epitel, indeks apoptosis dan ekspresi p53 sel epitel vagina pada tikus putih (Rattus norvegicus) ovariektomi. Desain penelitian ini adalah True Experimental laboratory, dengan menggunakan pendekatan posttest only group control design. Penelitian ini dilakukan dengan memberikan perlakuan pada hewan coba, yaitu tikus putih (Rattus norvegicus) betina. Hasil penelitian menunjukkan pemberian antosianin terbukti berpengaruh secara signifikan (p-value = 0,000 < α=0,05) dalam menurunkan ekspresi p53 dan indeks apoptosis sel epitel vagina serta meningkatkan ketebalan epitel vagina pada tikus ovariektomi.

Abstract

Type 2 Diabetes Mellitus (T2DM) and Hypertension are chronic diseases with complex risk factors requiring comprehensive management. A holistic family medicine approach is crucial for addressing biomedical, psychological, and socio-economic aspects. Objective: This case report aims to describe the holistic management of a 62-year-old male patient with uncontrolled T2DM with complications, primary hypertension, and cutaneous candidiasis using a family medicine approach. Method: This is a case report study. Data were collected through autoanamnesis, physical examinations, and home visits. A holistic diagnosis was established using the five-axis model. Family assessment was conducted using a genogram, APGAR, and SCREEM tools. Risk factors were visualized using the Mandala of Health. Interventions were patient-centered, family-focused, and community-oriented. Results: The patient was diagnosed with T2DM with peripheral circulatory complications, primary hypertension, and candidiasis intertriginosa. Multiple internal and external risk factors were identified, including genetics, obesity, diet, smoking, sedentary lifestyle, poor sleep patterns, personal hygiene, and physical environment. Comprehensive intervention resulted in improved clinical parameters, with random blood glucose decreasing from >600 mg/dL to 189 mg/dL and blood pressure improving from 145/76 mmHg to 128/85 mmHg. Conclusion: A comprehensive family medicine approach effectively improved clinical outcomes and self-management in a complex case with multiple comorbidities.
