Abstract

STBM (Community-Based Total Sanitation) pillar 3 is the Management of Drinking Water and Household Food (PAM-RT) which is an important thing to pay attention to during the Covid-19 pandemic and also in stunting prevention. The purpose of this activity is to be able to improve the implementation of STBM for household-scale Drinking Water Management for STMB Cadres in the Rumbai Pesisir Health Center which is a target area of ​​community service activities for the second period of 2020 from a lecturer in Master of Public Health, Hang Tuah University Pekanbaru as an implementation Tri Dharma college. The method of this Community Service Activity is through socialization and triggering to 34 STBM cadres from 4 kelurahan and related parties in the Puskesmas and Kec. Rumbai Pesisir in February 2021. The results of socialization and discussions with cadres stated that generally the problem for drinking water is that they consume gallons of drinking water and also use drinking water that is cooked by themselves from PDAM and also wells whose quality has not been tested for quality standards so that they require supervision and also outreach from the health department. STBM, District and Community Health Center cadres need support from academics through community service activities to provide input and evaluation so that the program remains enthusiastic about running, especially during the Covid 19 period and also stunting prevention.

Abstract

Abstrak Latar Belakang :. Dalam penerapan Keselamatan Kesehatan dan Kerja (K3), dibutuhkan beberapa faktor yang perlu dipenuhi seperti unsur internal dan eksternal penerapan K3. Tujuan penelitian ini adalah untuk melakukan analisis kesiapan penerapan K3 di Puskesmas Bagan Punak Kecamatan Bangko, Kabupaten Rokan Hilir Provinsi Riau. Metode: Penelitian ini menggunakan metode kualitatif melalui wawancara mendalam, observasi dan telaah dokumen. Informan utama dalam penelitian ini sebanyak tiga orang yang terdiri dari Kepala Puskesmas, Dokter penanggung jawab terhadap K3 dan Pegawai Puskesmas, sedangkan informan pendukung sebanyak dua orang yang terdiri dari Kepala Dinas Kesehatan dan Penanggung Jawab Program K3. Analisis kesiapan penerapan K3 di Puskesmas Bagan Punak ditinjau dari aspek internal dan eksternal. Hasil: Kendala internal berupa belum adanya sumber daya kesehatan yang berlatar belakang pendidikan K3, tidak adanya rancangan dana untuk penerapan K3, kurangnya ketersediaan peralatan yang sesuai dengan peraturan pemerintah, serta kurangnya tenaga kesehatan dalam memahami buku pedoman yang telah dirancang. Kendala di eksternalnya adalah belum diterapkannya kebijakan yang harus diikuti dan kurangnya monitoring dari pihak dinas kesehatan ke puskesmas setelah dilaksanakan pelatihan untuk tenaga Kesehatan di puskesmas khususnya kepada petugas penanggung jawab K3. Kesimpulan: Disarankan kepada Puskesmas Bagan Punak adalah mengusulkan tenaga yang akan menjadi penanggung jawab K3 dan dibentuknya Tim K3 di Fasyankes, berkoordinasi dengan Dinas Kesehatan, menambah ketersediaan alat K3, dan menetapkan kebijakan penerapan K3 di puskesmas. Study of The Readiness Assessment For Occupational Health and Safety Implementation at Bagan Punak Health Center Rohil District Riau Province Abstract Background: In the implementation of Occupational Safety and Health (K3), several factors are needed that need to be fulfilled, such as the internal and external elements of the application of K3. The purpose of this study was to analyze the readiness for the application of K3 at the Bagan Punak Health Center, Bangko District, Rokan Hilir Regency, Riau Province. Methods: This study uses qualitative methods through in-depth interviews, observation and document review. The main informants in this study were three people consisting of the Head of the Health Center, the Doctor in charge of K3 and Health Center employees, while the supporting informants were two people consisting of the Head of the Health Service and the Person in Charge of the K3 Program. Analysis of readiness for the application of K3 at the Bagan Punak Health Center in terms of internal and external aspects. Result: Internal constraints in the form of the absence of health resources with an OHS educational background, the absence of a design fund for the implementation of K3, the lack of availability of equipment in accordance with government regulations, and the lack of health workers in understanding the designed guidebook. Meanwhile, the external constraints are that the policies that must be followed have not been implemented and the lack of monitoring from the health office to the puskesmas after training for health workers at the puskesmas, especially the officers in charge of K3. Conclusion: Suggested to the Bagan Punak Health Center are proposing personnel who will be in charge of K3 and the formation of an K3 Team at the Health Facilities, coordinating with the Health Office, increasing the availability of K3 equipment, and establishing policies for implementing K3 at the puskesmas.

Abstract

ABSTRAK Peningkatan mutu pelayanan seharusnya tercermin dalam setiap kegiatan dirumah sakit (RS). Untuk menjaminnya, RS wajib memiliki sistem pengawasan mutu yakni dengan penerapan standar yang terdapat pada sistem akreditasi rumah sakit untuk mencapai Indikator Nasional Mutu (INM) dari 13 Indikator Nasional Mutu yang harus dilakukan RS. Hasil survei awal yang dilakukan ada 5 indikator yang belum mencapai standar akan berdampak terhadap hasil mutu pelayanan. Penelitian ini bertujuan diperolehnya informasi mendalam tentang kecenderungan penerapan Indikator Nasional Mutu (INM) peningkatan mutu pelayanan dalam pencapaian standar akreditasi rumah sakit. Penelitian menggunakan metode kualitatif desain fenomenologi melalui wawancara mendalam, observasi, dan telaah dokumen. Komponen yang diteliti yaitu kepatuhan kebersihan tangan, kepatuhan penggunaan APD, waktu operasi sectio caesarean emergency, waktu tunggu rawat jalan, dan kepatuhan pengisian alur klinis (clinical pathway). Dari penelitian ini didapatkan hasil bahwa kepatuhan kebersihan tangan, kepatuhan penggunaan APD, waktu operasi sectio caesarean emergency, waktu tunggu rawat jalan, dan kepatuhan pengisian alur klinis (clinical pathway) belum sesuai standar. Penerapan Indikator Nasional Mutu (INM) yang dilakukan melalui peningkatan mutu pelayanan dalam pencapaian Standar Akreditasi Rumah Sakit di RSUD Bangkinang tahun 2023 masih belum sesuai standar disebabkan oleh beberapa faktor, antara lain belum ada budaya organisasi, belum ada komitmen dari petugas. Belum optimalnya penggunaan sistem aplikasi pendaftaran online, belum optimalnya penggunaan aplikasi E-RM dan dukungan dari manajemen. Disarankan agar pihak rumah sakit dalam pencapaian indikator nasional perlu menambahkan poin penilaian kepatuhan pelaksanaan penerapan Indikator Nasional Mutu (INM) di Lembar Penilaian Kinerja dan memberlakukan sistem reward dan punishment. Kata Kunci: Indikator Nasional Mutu (INM), Peningkatan Mutu Pelayanan,   Standar Akreditasi Rumah Sakit  ABSTRACT  Service quality enhancement should appear in hospital activities. Each hospital is required to have a quality control system, which is designed to ensure that the level of performance of a system remains stable, or in control. For example, to achieve 13 National Health Care Quality Indicators, some standards can be applied in the hospital accreditation system. However, we still found that there were 5 indicators that have not reached the standard. Thus, these could have a significant impact on service quality results. The purpose of this study is to obtain more in-depth information about the tendency of National Health Care Quality Indicators implementation in order to improve service quality on hospital accreditation achievement. Phenomenological Qualitative Method was used in this study. The data were collected through interviews, observation, and secondary data. The component of health care quality; such as, hand hygiene compliance, PPE compliance, timing of emergency caesarean section, patient waiting time at outpatient department, and clinical pathway compliance, were analyzed in this study. The results confirm that the five components of health care quality; such as, hand hygiene compliance, PPE compliance, timing of emergency caesarean section, patient waiting time at outpatient department, and clinical pathway compliance; had not met the standard yet. Implementation of the National Health Care Quality Indicators for improving service quality to achieve hospital accreditation in RSUD Bangkinang (2023) has not met the standard yet. It happened because of some factors; such as, there is no organizational culture yet, there are no commitments from employees yet, the online registration application system and the E-RM application, and management support have not run properly. Therefore, the researcher hopes that the hospital is necessary to add the assessment of compliance with the implementation of National Health Care Quality Indicators in performance appraisal form, and to design reward as well as punishment systems. Keywords: National Health Care Quality Indicators, Service quality improvement, Accreditation Standards for Hospital

Abstract

Perawat berisiko mengalami kelelahan kerja akibat faktor kerja, tingginya tuntutan, dan kualitas tidur yang buruk. Di IRNA Surgikal RSUD Arifin Achmad Pekanbaru, jam kerja perawat sering melebihi batas shift (41–46 jam/minggu) dengan beban tinggi, karena satu perawat menangani 7–8 pasien, terutama saat shift malam. Penelitian ini bertujuan menganalisis pengaruh faktor kerja dan kualitas tidur terhadap kelelahan kerja pada perawat IRNA Surgikal RSUD Arifin Achmad Pekanbaru tahun 2025. Desain penelitian adalah kuantitatif cross-sectional dengan 81 responden yang dipilih secara purposive. Data dikumpulkan melalui kuesioner dan dianalisis menggunakan uji Chi-Square serta regresi logistik ganda model prediksi. Hasil menunjukkan 69,1% responden mengalami kelelahan kerja. Faktor kerja (beban kerja, masa kerja, dan shift kerja) mempengaruhi kelelahan sebesar 46,5%, sedangkan kualitas tidur sebesar 53,5%. Terdapat pengaruh beban kerja (p=0,005), shift kerja (p=0,041), dan kualitas tidur (p=0,003) terhadap kelelahan kerja, namun tidak terdapat pengaruh usia (p=0,534), status pernikahan (p=1,000), dan masa kerja (p=0,148). Variabel dominan adalah kualitas tidur (POR=6,439) dengan Nagelkerke R Square = 0,395. Disarankan RSUD Arifin Achmad menambah jumlah perawat untuk menyeimbangkan rasio perawat-pasien, memperhatikan pembagian tugas, rotasi kerja, serta menerapkan program pencegahan kelelahan melalui manajemen stres, relaksasi, dan konseling.

Abstract

Pharmaceutical service is a direct and responsible service to patients related to pharmaceutical preparations with the intention of achieving definite results to improve the quality of life of patients. The purpose of this study was to obtain in-depth information on drug management in the Pharmacy Unit of RSUD Tengku Sulung. The research method used descriptive qualitative with purposive sampling with the number of informants 7 people. Data collection through in-depth interviews, direct observation and document review using content analysis. The results, on the input component obtained information that the lack of human resources, facilities and infrastructure are inadequate and there is no depot and SIM RS. Policies have been made in the form of Drug Management Guidelines in the Pharmacy Unit. In the process component, information is obtained that selection is based on the hospital formulary and national formulary, planning using the combination method, procurement using e-catalog and direct purchase, receipt is carried out by checking and adjusting the order letter, order invoice, and the drug received, storage based on dosage form, classification of types of drugs, medical devices, B3 and arranged alphabetically with the FIFO and FEFO systems, distribution using individual prescribing and Floor Stock systems, deletion is carried out by involving third parties, control is carried out by controlling drug inventory and quality, administration based on recording, reporting and financial administration. The output component obtained by the availability of drugs is still not effective and efficient. In conclusion, the pharmaceutical supply management system in the Pharmacy unit of RSUD Tengku Sulung is not in accordance with the established standards. Suggestions, the need for additional human resources, the fulfillment of infrastructure such as depot procurement and SIMRS, monitoring and evaluation, improved communication during drug selection and budget planning in the fulfillment of facilities and infrastructure.

Abstract

Patient safety stands as a vital benchmark of healthcare quality, yet reporting incidents remains a challenge influenced by multiple human and organizational factors. This study explores the elements that shape nurses’ performance in reporting patient safety incidents at Permata Hati Duri Hospital in 2025. Using a quantitative analytical observational approach with a cross-sectional design, the research involved 85 inpatient nurses. Data were analyzed through univariate, bivariate (Fisher’s Exact test), and multivariate (logistic regression) techniques. Most participants were young adults aged 20–30 years (32.9%), predominantly female (91.8%), highly educated (84.7% D4/S1/Nursing), and experienced (80.0% with 6–>10 years). Findings revealed that most nurses demonstrated strong perceptions, positive attitudes, high motivation, and favorable views of leadership. Although bivariate analysis showed no significant association between independent variables and incident reporting, multivariate results indicated that work experience played the most dominant role (OR = 3.821; 95% CI = 0.010–1.025). The study concludes that enhancing reporting behavior requires consistent training, improved digital systems, and a reinforced culture of safety across all levels of hospital practice.

Abstract

Access to safe drinking water is a key target under SDG 6.1; yet household-level risks remain under-monitored in Indonesia. This study aimed to assess household drinking-water quality in the Rumbai Bukit PHC area as part of the implementation of STBM pillar 3 by the Household Drinking Water Quality Surveillance (KAMRT). A crosssectional surveillance (July–Sept 2024) tested 24 household water samples from wells, PDAM, and refill depots for physical (colour, turbidity, odour, TDS), chemical (pH, Fe, Mn), and microbiological (E. coli) parameters. Laboratory examination showed that 22 samples (91.7%) were positive for E. coli, with the highest contamination detected in dug-well (100%) and refill-depot water (87.5%), while only one PDAM sample was positive. In addition, three parameters, pH (33.3%), Fe (25%), and Mn (20.8%), failed to meet the national standards set in the Ministry of Health Regulation No. 2/2023. No samples failed for colour, odour, turbidity, or TDS. Most household water was unsafe microbiologically. Strengthened internal/external monitoring, public education on safe storage/treatment, and multisector collaboration using community-based technologies are required to secure sustainable safe water access.

Abstract

Limbah B3 medis padat dari rumah sakit berpotensi membahayakan kesehatan dan lingkungan jika tidak dikelola dengan baik. Penelitian ini bertujuan mengetahui perilaku perawat dalam pemilahan limbah B3 medis padat. Sampel berjumlah 136 diambil dengan teknik proportional sampling, dan data dianalisis menggunakan multiple logistic regression model prediksi dengan tujuan untuk mendapatkan model yang paling baik (fit) yang dapat menggambarkan hubungan variabel independen dan dependen. Hasil penelitian menunjukkan ada hubungan antara sarana dan fasilitas (p=0,003), peran kepala ruangan (p=0,015), ketersediaan informasi (p=0,032), sikap (p=0,045) dan supervisi (p=0,046) dengan perilaku perawat dalam pemilahan limbah B3 medis padat. Variabel paling dominan adalah sarana dan fasilitas (POR=3,463). Manajemen rumah sakit disarankan untuk melengkapi setiap ruang rawat inap dengan fasilitas pemilahan limbah yang sesuai standar (kantong plastik berwarna, safety box), Menarik wadah limbah non-standar dan menggantinya dengan yang sesuai regulasi limbah B3 medis, menyediakan buku saku dan pelatihan rutin tentang limbah B3 medis, serta meningkatkan sosialisasi dan monitoring SOP pemilahan limbah kepada perawat secara berkala. Manajemen limbah B3 padat yang baik di rumah sakit mencegah penularan penyakit, melindungi tenaga kesehatan, menjaga lingkungan, memastikan kepatuhan regulasi, meningkatkan efisiensi, dan membangun kepercayaan masyarakat.

Abstract

Drinking water sampling is a critical stage in water quality testing, as it determines the reliability of laboratory results and serves as a basis for public health decision-making. This residency activity aimed to analyze the implementation of Standard Operating Procedures (SOPs) for drinking water sampling at the Environmental and Health Laboratory Unit of the Riau Provincial Health Office. The study employed a qualitative case study approach through observations, in-depth interviews, and document review. Problem prioritization was conducted using the Urgency, Seriousness, and Growth (USG) method, while root cause analysis was performed using a fishbone diagram. The findings revealed that SOP implementation by sample submitters had not fully complied with established standards, particularly regarding the use of sterile containers, sampling techniques, labeling, and sample storage and transportation. These deficiencies may compromise sample integrity and result in laboratory data that do not accurately represent actual water quality conditions. Therefore, continuous education, improved SOP dissemination, and the development of simple and practical sampling guidelines are recommended to enhance compliance and ensure the reliability of drinking water quality testing results.

Abstract

The implementation of the National Health Insurance (JKN) system through BPJS Kesehatan requires accurate diagnosis and procedure coding to ensure appropriate claim reimbursement. Inaccurate coding and incomplete medical documentation often lead to pending and disputed claims, affecting hospital financial sustainability. This study aimed to evaluate the internal verification document screening process in determining BPJS claim coding at Hospital X and to formulate improvement strategies. A qualitative approach using SWOT analysis was applied to identify internal and external factors influencing the coding and verification process. Data were collected through interviews, document review, and SWOT questionnaires, and analyzed using IFAS and EFAS matrices. The results indicated that internal weaknesses were more dominant, particularly related to coder competency, documentation completeness, and manual screening processes, while external opportunities included regulatory support, coding training availability, and health information technology development. The SWOT analysis placed Hospital X in Quadrant III (Weakness–Opportunity), indicating that leveraging external opportunities to address internal weaknesses is essential to improve coding accuracy and reduce pending and disputed BPJS claims.
