Abstract

Cardiac arrest is a sudden cessation of heart activity that requires immediate treatment in the form of Basic Life Support (BLS). Students who are members of the Student Activity Unit (UKM) in the humanitarian field at Unpad are potential first aiders to provide basic life support in the event of cardiac arrest. This study aims to determine the level of knowledge of basic life support in students who are members of UKM in the humanitarian field at Universitas Padjadjaran. The method used in this research is quantitative descriptive with a cross sectional approach. The population in this study amounted to 158 active members of the humanitarian student activity unit and the sample was taken using convenience sampling type (n = 120). Data were collected using a questionnaire containing 30 questions that have been declared valid and reliable with a Cronbach's alpha value of 0.797 and data were analysed using computer software with univariate analysis techniques. The results showed that the level of knowledge of humanitarian UKM members at Unpad regarding basic life support was classified into the good category with a total of 108 respondents (90%). The conclusion in this study is that the level of knowledge of basic life support in members of humanitarian UKM at Universitas Padjadjaran is in the good category, but it is necessary to increase knowledge, especially on procedural topics related to basic life support.

Abstract

Chronic renal failure patients undergoing haemodialysis therapy experience various conditions and require long-term care which can cause a burden on the families who care for patients. This family caregiver burden can affect the quality of life of both patients and family caregivers, and also affect the care provided to patients. The purpose of the study was to determine the description of caregiver burden in families with haemodialysis patients. This study is a quantitative descriptive study and the sample was taken using purposive sampling technique (n = 107) with the inclusion criteria of respondents as the main caregiver with family members undergoing haemodialysis therapy at RSUD Sumedang, and physically and mentally healthy. Exclusion criteria in the study were potential respondents who refused to participate and respondents who decided to stop in the middle of the study. The burden of caregivers on the family was measured using the Burden Scale for Family Caregivers Short Scale (BSFC-s) questionnaire with test results, namely r results (0.634) > r table (0.786) as well as Cronbach alpha value of 0.900 which means this questionnaire is valid and reliable. The picture of caregiver burden shows that almost half of family caregivers are in the high category (49.5%) and most experience heavy burden in the physical dimension. The results of this study are expected to be used as a reference for learning and evaluation for nurses, family caregivers, and educational institutions to be able to carry out effective interventions that can be carried out to reduce the burden of care experienced by family caregivers.

Abstract

ABSTRACT Pain is an unpleasant sensory and emotional experience resulting from tissue damage. Unconscious patients cannot directly communicate their pain to nurse, but this doesn’t meant they cannot feel pain. Pain in unconscious patients can arise from routine procedures such as prolonged immobilization to invasive procedures like surgery. Nursing interventions to manage pain are necessary to enhance patient comfort. This study aims to examine pain management in unconscious patients in the Intensive Care Unit. This type of research is a scoping review, searching for articles in the PubMed, SCOPUS, and EBSCO databases. The search process used keywords (unconscious patient OR coma OR sopor OR somnolent) AND (pain management OR pain relief OR pain control) AND (intensive care unit OR critical care OR HCU). 45 articles were found that matched the keywords, but only 5 articles were relevant to the review's objectives. Pain management for unconscious patients starts with recognizing pain symptoms, assessing pain, and followed by nursing interventions. Interventions that can be provided include TENS (Transcutaneous Electrical Nerve Stimulation), foot massage, and music therapy.  Keywords: Pain Management, Unconscious patient, ICU   ABSTRAK  Nyeri merupakan pengalaman sensorik dan emosional yang tidak menyenangkan akibat adanya kerusakan jaringan. Pasien yang tidak sadar tidak dapat mengatakan rasa nyeri yang dirasakan secara langsung pada perawat namun bukan berarti tidak dapat merasakan nyeri. Rasa nyeri pada pasien tidak sadar dapat berasal dari tindakan rutin seperti imobilisasi lama, hingga tindakan invasif yang seperti tindakan pembedahan. Intervensi perawat untuk mengatasi nyeri perlu dilakukan untuk meningkatkan kenyamanan pasien. Penelitian ini bertujuan untuk melihat manajemen nyeri pada pasien penurunan kesadaran di ruang Intensive Care. Jenis penelitian ini adalah scooping review, dengan mencari artikel pada database Pubmed, SCOPUS dan EBSCO. Proses pencarian menggunakan kata kunci (unconscious patient OR coma OR soppor OR somnolent) AND (pain management OR pain relief OR pain control) AND (intensive care unit OR critical care OR HCU).  Didapatkan 45 artikel sesuai kata kunci namun hanya didapatkab 5 artikel yang sesuai dengan tujuan review. Manajemen nyeri pasien tidak sadar dimulai dari mengenali gejala nyeri, pengkajian nyeri dan dilanjutkan dengan intervensi keperawatan. Intervensi yang dapat diberikan adalah TENS (Transcutaneous Electrical Nerve Stimulation), pijat kaki dan terapi musik.  Kata Kunci: Manajemen Nyeri, Pasien Tidak Sadar, ICU

Abstract

ABSTRACT Prolonged Mechanical Venilation (PMV) in the Intensive Care Unit can cause several problems including causing various diseases that are not expected, increasing the length of stay (LOS) of patients which ultimately increases the cost of care.This study aims to determine the limitations of the definition and factors associated with the incidence of Prolonged Mechanical Venilation (PMV) in patients in the ICU room. A scoping review was conducted using two databases, PubMed and CINAHL, with keywords Population: adult patients, Concept: Prolonged Mechanical Ventilation (PMV), Context: Intensive Care Unit. Articles included with the criteria are English articles, published in 2012-2022 and have full-text articles. Based on the search results, 38 articles were obtained which were then analyzed and 17 articles were reviewed. The results of the scoping review found that 3 out of 5 articles mentioned PMV was ventilator use above 21 days. Analysis of factors associated with PMV was seen from several approaches such as demographics. Three studies revealed that advanced age as a related factor. 14 out of 17 studies examined factors related to specific body conditions such as anemia, HAP, ARDS, Hematocytes and so on. While three other researchers discussed systems or tools. It can be concluded that almost all the analyzed articles mention that poor health conditions increase the duration of mechanical ventilation use. Keywords: Adult, Prolonged Mechanical Ventilation (PMV), Intensive Care Unit  ABSTRAK Prolonged Mechanical Venilation (PMV) di ruang Intensive Care Unit (ICU) dapat menimbulkan beberapa masalah diantaranya menimbulkan berbagai penyakit yang tidak di harapkan, meningkatkan lama rawat pasien length of stay (LOS) pasien yang pada akhirnya meningkatkan biaya perawatan. Penelitian ini bertujuan untuk mengetahui batasan definisi serta faktor-faktor yang berhubungan dengan kejadian PMV pada pasien di ruang ICU.  Metode dilakukan dengan scoping review menggunakan dua database yaitu PubMed dan CINAHLdengan kata kunci yaitu Population: pasien dewasa, Concept: Prolonged Mechanical Ventilation (PMV), Context: ruang Intensive Care Unit. Artikel yang masuk dengan kriteria adalah artikel berbahasa inggris, terbit tahun 2012-2022 dan memiliki artikel full-text. Berdasarkan hasil pencarian didapatkan 38 artikel yang selanjutnya dilakukan analisa data dan artikel yang akan di review 17 artikel. Hasil scoping review didapatkan bahwa 3 dari 5 artikel menyebutkan PMV merupakan penggunaan ventilator diatas 21 hari. Analisis faktor yang berhubungan dengan PMV dilihat dari beberapa pendekatan seperti demografi. Tiga penelitian megungkapkan bahwa usia lanjut sebagai factor yang berhubungan. 14 dari 17 penelitian meneliti faktor yang berkaitan dengan kondisi tubuh secara spesifik seperti anemia, HAP, ARDS, Hematoktit dan lain sebagainya. Sedangkan tiga peneliti lain membahas mengenai sistem atau tools. Kesimpulan studi ini adalah kondisi kesehatan yang buruk meningkatkan durasi penggunaan ventilasi mekanik. Kata Kunci: Adult, Prolonged Mechanical Ventilation (PMV), Intensive Care Unit.

Abstract

Penilaian agitasi merupakan salah satu penilaian yang dilakukan untuk menilai status agitasi dan sedatif pada pasien perawatan khusus. Penggunaan ventilasi mekanis menyebabkan terjadinya agitasi pada pasien yang mempengaruhi manajamen sedatif pada pasien. Instrumen agitasi dapat menjadi acuan secara objektif, namun pengaplikasiannya masih jarang digunakan pada pasien perawatan khusus. Casse report ini bertujuan untuk menggambarkan risiko yang didapat pada Ny. L di ruang perawatan khusus tanpa penilaian agitasi secara objektif.  Ny. L berusia 52 tahun yang di rawat di ruang Medical Intensive Care Unit (MICU) RSUP Dr. Hasan Sadikin Kota Bandung Provinsi Jawa Barat. Pasien mengalami gagal nafas dan terpasang ventilator dengan terapi sedatif dexmedetomidine. Pasien terpasang restrain pada kedua tangannya dan tampak gelisah. Penilaian agitasi secara objektif dan berkala tidak dilakukan oleh perawat. Hasil penelitian mendapatkan kegagalan dalam proses penyapihan pada Ny. L dengan ditandai adanya penurunan kondisi pasien hingga terjadi perubahan ventilator pada pasien yang memperpanjang Length Of Stay pasien di ruang intensive. Sehingga dapat disimpulkan risiko perburukan dapat terjadi pada pasien intensive Ketika pemantauan agitasi tidak akurat dan tidak valid diantaranya  meningkatkan ketergantungan terapi sedatif dan ventilasi, Length Of Stay pasien lebih panjang, hingga proses penyapihan ventilator jangka panjang lebih dari 7 hari.

Abstract

ABSTRACT There are many cardiac arrest events in the community that can not be saved because they do not get the right and quick action due to the ignorance and the inability of the nurses of the puskesmas to provide emergency response to cardiac pulmonary resuscitation. Cardiopulmonary resuscitation (CPR) is the main action in cardiac arrest and stop breath, which can increase life expectation. Goal to be achieved in the implementation of this program is Increased ability of nurse puskesmas in performing action of heart resuscitation of lung. The specific targets of the activities are the increased ability of health center nurses on the ability to perform cardiopulmonary resuscitation with the latest guidelines. and the implementation of a structured training program on pulmonary cardiac resuscitation at community health center nurses on an ongoing basis. Solutions offered to overcome the problem is the training of Lung Heart Resuscitation (RJP) to nurses Puskesmas. Training activities conducted for one day include pre-test evaluation, material exposure, rjp skills training and post training evacuation. The result showed that there was significant difference between attitude value and perceived behavioral control between before and after training, whereas there was no difference of subjective norm value between before and after training. Training on updating and increasing the competence of cardiopulmonary resuscitation should be done regularly and periodically. Routine and periodic training needs to be done to improve nursing services, especially cardiovascular emergency. Key Word : Nursing Public Health ,cardiac pulmonary resuscitation ABSTRAK Terdapat banyak kejadian henti jantung di masyarakat yang tidak dapat diselamatkan karena tidak mendapatkan tindakan yang tepat dan cepat akibat ketidaktahuan dan ketidakmampuan perawat puskesmas untuk memberikan tindakan tanggap darurat resusitasi jantung paru.. Tindakan resusitasi jantung paru (RJP) merupakan tindakan utama pada henti jantung dan henti napas, yang dapat meningkatkan harapan hidup.Tujuan yang ingin dicapai dalam pelaksanaan program ini adalah Meningkatnya kemampuan perawat puskesmas dalam melakukan tindakan resusitasi jantung paru. Target khusus kegiatan adalah Meningkatnya kemampuan perawat puskesmas tentang kemampuan melakukan resusitasi jantung paru dengan pedoman terbaru. dan terselenggaranya program pelatihan yang terstruktur mengenai resusitasi jantung paru pada perawat puskesmas secara berkesinambungan. Solusi yang ditawarkan untuk mengatasi permasalahan adalah dilakukannya pelatihan Resusitasi Jantung Paru (RJP) kepada perawat Puskesmas. Kegiatan pelatihan dilakukan selama satu hari meliputi evaluasi pra test, paparan materi, pelatihan skill rjp dan evaliasi post pelatihan. Hasil kegiatan menunjukan adanya perbedaan yang significant antara nilai sikap dan perceived behavioral control antara sebelum dan sesudah pelatihan, sedangkan tidak terdapat perbedaan nilai norma subjektif antara sebelum dan sesudah pelatihan. Pelatihan updating dan peningkatan kompetensi resusitasi jantung paru perlu dilakukan secara rutin dan berkala. Abstract

ABSTRACT Patients in the Intensive Care Unit (ICU) often experience limitations in verbal communication due to ventilator use, sedation, or other medical conditions. These communication barriers can interfere with the delivery of medical information and affect the quality of patient care. Augmentative and Alternative Communication (AAC) is a potential solution to improve interactions between patients, healthcare providers, and families in the ICU environment. This review aimed to explore the implementation of AAC in the ICU, including its effectiveness, challenges, and impact on the quality of communication and patient care. This study used a scoping review approach following the PRISMA-ScR guidelines. A literature search was conducted through the EBSCOhost Medline, PubMed, and Google Scholar databases, using relevant keywords related to AAC and ICU. Studies that met the inclusion criteria were analyzed thematically to identify key findings. Of the 386 articles identified, 11 articles met the criteria for further analysis. The results showed that implementing AAC in the ICU can improve communication between patients and healthcare providers, reduce patient anxiety, and strengthen the therapeutic relationship. However, there are obstacles to its implementation, such as a lack of training for healthcare providers, time constraints, and a lack of institutional policies that support the systematic use of AAC. Despite the challenges in implementing AAC in the ICU, this technology has great potential to improve patient communication and quality of care. Therefore, a more comprehensive strategy is needed, including training health workers, developing institutional policies, and procuring communication devices that are appropriate to patient needs to support optimal implementation of AAC in the ICU. Keywords: Augmentative and Alternative Communication, ICU, Communication, Critical Care, Scoping Review.

Abstract

Introduction: Academic resilience defined as the ability to survive and increase success in terms of education despite facing severe problems. Academic resilience in students can be formed if students are able to break down these problems into smaller problems and then deal with them properly. To be able to overcome these small or minor problems, students need good academic buoyancy. This study aims to analyze the relationship between academic buoyancy and academic resilience in first-year students at the faculty of health Universitas Padjadjaran.Method: The research design is a quantitative correlation with a cross-sectional time approach. The instruments used were the Indonesian version of the Academic Buoyancy Scale and the Academic Resilience Scale-30. The research population was 1082 first-year students of the 2022 class of the Faculty of Health. The sampling technique used was proportional stratified random sampling (n=162).Results: The results of the correlation analysis show that there is a strong and positive relationship between academic buoyancy and academic resilience (p = 0.000 and r = 0.679).Conclusions: There is a strong and positive relationship between academic buoyancy and academic resilience. Students should strive to maintain and improve their academic buoyancy and academic resilience, for example by building social connections, maintaining health, developing positive thoughts, and seeking help when needed.

Abstract

Background: The incidence of cardiac arrest or Sudden Cardiac Arrest (SCA) in the community that cannot be saved increases every year. This is because they do not get the right and fast action due to the community's ignorance and inability to assist. Objective: This study aimed to evaluate health education on increasing knowledge and readiness of respondents in helping cardiac arrest victims. Methods: This study used a pre-experimental design with a one-group pre-post test approach to 52 respondents, namely the general public. The research sample was recruited using convenience sampling techniques. The sample was given health education on basic life support (BLS) and cardiac arrest management directly with several interactive sessions using visual aids such as posters and presentation slides. Data collection used a valid and reliable questionnaire (Knowledge and Readiness to Help). Data were analyzed univariately through frequency distribution, central tendency, mean difference, and bivariate tests using paired sample t-tests. Results: There was an increase in respondents' BLS knowledge from 6.87 to 11 (p<0.001) and readiness to help, with an average score increasing from 60.4 to 63.3 (p=0.002). These results indicate that health education can increase knowledge about BLS and readiness to help respondents. Research results show that readiness to act in emergencies depends on more than theoretical understanding; it also requires practical training, self-confidence, and direct experience. Conclusion: Health education significantly improved public knowledge and readiness to provide first aid for cardiac arrest victims. These findings indicate that structured Basic Life Support (BLS) education is effective in strengthening community preparedness and timely emergency response. Integrating regular and practical BLS training into community health programs may further enhance public capacity to respond to cardiac arrest events.

Abstract

ABSTRAK Keterlambatan kedatangan pasien dengan penyakit jantung koroner (PJK) ke rumah sakit menjadi masalah utama dalam penanganan medis yang tepat waktu. Keterlambatan ini dapat mengakibatkan kerusakan jantung permanen, tingginya angka kematian, dan morbiditas jangka panjang. Namun, faktor-faktor yang menyebabkan keterlambatan ini sering kali kompleks dan berkaitan dengan berbagai aspek seperti sosial demografis, akses ke layanan kesehatan, dan perilaku kesehatan pasien. Untuk mengeksplorasi faktor-faktor yang berhubungan dengan keterlambatan kedatangan pasien penyakit jantung koroner.  Penelitian ini menggunakan scoping review berdasarkan kerangka Arksey dan O’Malley serta panduan PRISMA-ScR. pencarian artikel yang dilakukan di tiga database yaitu CINAHL, PubMed, Scopusdan search engine google scholar. Kata kunci yang digunakan meliputi "factors", "delay", "coronary heart disease", "myocardial infarction", dan "pre-hospital delay". Kriteria inklusi mencakup artikel yang diterbitkan dalam bahasa Inggris atau bahasa indonesia antara tahun 2015-2025 dan membahas faktor-faktor yang mempengaruhi keterlambatan kedatangan pasien dengan penyakit jantung koroner. Pertanyaan penelitian utama adalah; Apa saja faktor-faktor yang berhubungan dengan keterlambatan kedatangan pasien dengan penyakit jantung koroner?. Terdapat 12 artikel yang memenuhi kriteria inklusi dan membahas faktor-faktor yang berhubungan dengan keterlambatan kedatangan pasien dengan penyakit jantung koroner. Temanya meliputi sosial demografis, gejala klinis, akses ke layanan kesehatan, perilaku kesehatan, dan faktor budaya. Faktor-faktor utama yang ditemukan meliputi jenis kelamin, usia, tingkat pendidikan, status sosial ekonomi, akses transportasi, serta kesadaran gejala penyakit jantung. Faktor-faktor ini berhubungan dengan kurangnya pemahaman tentang gejala atau peran gender dalam pengambilan keputusan medis, serta keterbatasan akses ke layanan medis yang memadai, terutama di daerah pedesaan. Keterlambatan kedatangan pasien dengan penyakit jantung koroner dipengaruhi oleh berbagai faktor yang saling terkait. Untuk mengurangi keterlambatan, diperlukan pendidikan masyarakat tentang gejala penyakit jantung, serta peningkatan akses ke layanan kesehatan dan perbaikan sistem transportasi medis. Kata Kunci: Faktor, Keterlambatan Kedatangan, Penyakit Jantung Coroner. ABSTRACT Delayed hospitalization of patients with coronary heart disease (CHD) is a major obstacle to timely medical care. This delay can result in permanent heart damage, high mortality, and long-term morbidity. However, the factors causing this delay are often complex and related to various aspects such as sociodemographics, access to healthcare, and patient health behaviors. To explore factors associated with delayed hospitalization for patients with coronary heart disease, this study used a scoping review based on the Arksey and O'Malley framework and the PRISMA-ScR guidelines. Articles were searched in three databases: CINAHL, PubMed, Scopus, and the Google Scholar search engine. Keywords used included "factors," "delay," "coronary heart disease," "myocardial infarction," and "prehospital delay." Inclusion criteria included articles published in English or Indonesian between 2015 and 2025 that discussed factors influencing delayed hospitalization for patients with coronary heart disease. The primary research question was: What are the factors associated with late arrival of patients with coronary heart disease?. Twelve articles met the inclusion criteria and discussed factors associated with delayed arrivals for patients with coronary heart disease. The topics covered sociodemographics, clinical symptoms, access to healthcare, health behaviors, and cultural factors. Key factors identified included gender, age, education level, socioeconomic status, access to transportation, and awareness of heart disease symptoms. These factors are associated with a lack of understanding of symptoms or the role of gender in medical decision-making, as well as limited access to adequate medical services, particularly in rural areas. Delayed arrivals for patients with coronary heart disease are influenced by various interrelated factors. To reduce delays, public education about heart disease symptoms, increased access to healthcare, and improvements to medical transportation systems are needed. Keywords: Factors, Delayed Arrivals, Coronary Heart Disease.
