Abstract

Introduction: Pain is a subjective thought and can be experienced by everyone. Patients with decreased consciousness may also feel pain. The intensity of pain experienced by patients in the intensive care unit ranges from moderate to severe pain. Previous studies stated that the phenomenon of pain in patients can cause physical and psychological complications. Thus, pain management in the scope of intensive care is crucial to do. Purpose: To identify management that can be done in patients with unconsciousness who experience pain. Methods: This case study used Indonesian Nursing Intervention Standards as a guideline for pain management. Results: A 29-year-old female patient with decreased consciousness was treated in the intensive care unit. The patient was in the intensive care unit for four months. The results showed a severe pain scale at the time of the assessment using the CPOT. This study established a nursing diagnosis of chronic pain related to treatment procedures characterized by a severe pain scale using the CPOT, invasive devices, and the presence of fourth-degree pressure sores on the sacrum. Conclusion: Pain management carried out is pain assessment, non-pharmacological and pharmacological management. In each of these interventions, nurses must pay attention to nursing ethics. Evaluation and documentation are carried out to review the interventions' effect and become a part of communication for interprofessional regarding the treatment process.Keywords: ICU settings, pain management, unconscious patient,

Abstract

Backgorund :The mortality rate of patients who were admitted in the intensive care unit has been increased. Therefore, end of life care is needed. Goals of end of life care is to helpthe patients would die with dignity and to help the familycould receive bereavement. End of life care is related with internal and external factors.Aims :This study aims to analyze the factors that related to end of life care.Methods :Thisstudy was a quantitative research withanalytic-correlation design. The samples were 62 critical care nurse, which obtained by total sampling. This study was conducted in ICU of Kendari Hospital Southeast Sulawesi. Data collection used questionnaire thenanalyzed with bivariate throughSpearman’s Rho and multivariate through logistic regresion.Results :The implementation of end of life care in ICU of Kendari Hospitalwas optimal (74%, median = 55), which was related to several factors such as knowledge (p = 0,000), moral distress (p = 0,002), patient and family characteristics = 0.009), organizational structure and culture (p = 0,000). Knowledge was the most related factor in improving the quality of end of life care (OR = 45,542) with probality 97%.Conclusion :Optimalization of end of life care is related with the personal (internal) of the nurse factors, patient and family, and organizational structure and culture. Implication :Related factors to end of life care will enrich the thought that delivered nursing care, nurse need to align knowledge with professionalism, leaving the pressures and dilemmas due to moral stress, as well foster a deep understanding of the nature of spirituality and culture sensitivity when accompanying dying patients, grows awareness that the best preferences of critical patient care are onesbenefit the patients and familys.

Abstract

ABSTRACT Coronary Heart Disease (CHD) is a cardiovascular disease due to atherosclerosis or atherosclerotic occlusion of the coronary arteries. Various treatments can be done to treat CHD, including Percutaneous coronary intervention (PCI). The PCI procedure can cause complications. Hematoma is a significant complication that can cause swelling, pain, and increased discomfort. One of the many actions to reduce hematomas and pain is giving cold compresses. This study aimed to determine the potential of giving cold compresses to repair hematomas in post-PCI patients. This research uses scoping review method. Article searches were carried out on databases and search engines: PubMed, ProQuest, Google Scholar, and Garuda, in the period 2017-2022 with the keywords “Percutaneous Coronary Intervention” or “Cardiac Catheterization” and “Cold Therapy” or “Ice Application” and “Hematoma.”. There were seven articles included in the qualitative analysis. All articles discuss the effect of cold compresses on hematomas in post-PCI patients. Cold compresses help vasoconstrict blood vessels and speed up blood clotting time and formation. It was concluded that cold compresses could potentially prevent hematomas in post-PCI patients. Keywords: Cold compress, Cardiac Catheterization, Hematoma, Percutaneous Coronary Intervention (PCI)  ABSTRAK Penyakit Jantung Koroner (PJK) adalah penyakit kardiovaskular yang terjadi karena aterosklerosis atau oklusi aterosklerotik pada arteri koroner. Berbagai penanganan dapat dilakukan untuk mengatasi PJK, salah satunya adalah Percutaneous coronary intervention (PCI). Prosedur PCI dapat menimbulkan komplikasi. Hematoma adalah komplikasi utama yang dapat menyebabkan bengkak, nyeri, dan meningkatkan ketidaknyaman. Salah satu tindakan yang banyak dipilih untuk mengurangi hematoma dan nyeri yaitu dengan pemberian kompres dingin. Penelitian ini bertujuan untuk mengetahui potensi pemberian kompres dingin terhadap perbaikan hematoma pada pasien post PCI. Penelitian ini menggunakan metode scoping review. Pencarian artikel dilakukan pada database dan search engine: PubMed, ProQuest, Google Scholar, Garuda, dalam rentang waktu 2017-2022 dengan kata kunci “Percutaneous Coronary Intervention” or “Cardiac Catheterization” and “Cold Therapy” or “Ice Application” and “Hematoma”. Didapatkan 7 artikel yang masuk dalam analisa kualitatif. Sseluruh artikel berfokus membahas pengaruh kompres dingin terhadap hematoma pada pasien post PCI. Kompres dingin membantu vasokonstriksi pembuluh darah, mempercepat waktu pembekuan darah, dan pembentukan bekuan darah. Didapatkan simpulan bahwa kompres dingin memiliki potensi dalam mencegah terjadinya hematoma pada pasien post PCI. Kata Kunci: Kompres dingin, Kateterisasi Jantung, Hematoma, Percutaneous Coronary Intervention (PCI)

Abstract

ABSTRACT Acute respiratory failure (ARF) is a disorder of the pump function of the respiratory muscles or dysfunction of the lungs. Patients with respiratory failure use mechanical ventilation for breathing apparatus. However, mechanical ventilation often interferes with airway clearance and coughing. So intervention is needed to remove mucus from the airways. This study aimed to identify interventions that can be performed to reduce and clear mucus in patients with respiratory failure who are attached to a mechanical ventilator. This study used the narrative review method with the keywords “patient with ventilator OR patient with mechanical ventilation OR mechanically ventilated patient OR patient undergoing mechanical ventilation” AND “intervention OR strategies OR best practice OR treatment OR therapy OR program OR management” AND “mucus clearance OR airway clearance OR sputum clearance”. The databases used include Scopus, PubMed, EBSCO, Oxford. The results of 5 articles were obtained which consisted of various effective interventions for removing secretions such as ventilator hyperinflation mode, head down tilt position, manual chest compression (MCC), mechanical insufflation-exsufflation (MI-E), and chest physiotherapy. This intervention is effective in expelling mucus, increases expiratory flow bias which has the potential to increase secretions removal maneuvers, facilitates mucus cleaning during suctioning, and increases aeration in the dorsal lung without affecting blood gas analysis results. These various interventions can be a choice of solutions to help problems that arise due to the use of mechanical ventilation. Keywords: Acute Respiratory Failure, Mucus Suction, Ventilator  ABSTRAK Gagal napas akut (GGA) merupakan gangguan fungsi pompa otot pernapasan atau disfungsi pada paru-paru. Penderita gagal napas menggunakan ventilasi mekanik untuk alat bantu napas. Namun, ventilasi mekanis sering mengalami gangguan pembersihan lendir saluran napas dan batuk. Sehingga diperlukan intervensi untuk mengeluarkan lendir dari saluran napas. Penelitian ini bertujuan untuk mengidentifikasi intervensi-intervensi yang dapat dilakukan dalam menurunkan dan membersihkan lendir pada pasien dengan gagal napas yang terpasang ventilator mekanik. Penelitian ini menggunakan metode narrative review dengan kata kunci “patient with ventilator OR patient with mechanical ventilation OR mechanically ventilated patient OR patient undergoing mechanical ventilation” AND “intervention OR strategies OR best practice OR treatment OR therapy OR program OR management” AND “mucus clearance OR airway clearance OR sputum clearance”. Didapatkan hasil 5 artikel yang terdiri  dari berbagai intervensi yang efektif untuk mengeluarkan sekret seperti mode hiperinflasi ventilator, posisi head down tilt, manual chest compression (MCC), mechanical insufflation-exsufflation (MI-E), dan fisioterapi dada. Intervensi ini efektif dalam pengeluaran mukus, meningkatkan aliran ekspirasi bias yang berpotensi meningkatkan manuver pengeluaran sekret, mempermudah pembersihan mukus ketika suction, dan meningkatkan aerasi di bagian dorsal paru tanpa mempengaruhi hasil analisa gas darah. Berbagai intervensi ini dapat menjadi pilihan solusi untuk membantu permasalahan yang ada akibat penggunaan ventilasi mekanik. Kata Kunci: Gagal Napas Akut, Penghisapan Lendir, Ventilator

Abstract

The high and varied disasters occur in Indonesia, requiring the government and the whole community to be ready for disaster. In the Faculty of Nursing Universitas Padjadjaran (Unpad), there is a group of students who do extracurricular activities to provide health aid called PNC. Training needs to be done to enhance the role of PNC as well as increasing the participation of students in the disaster relief program. This training resulted in increased knowledge and attitudes of PNC members on disaster management, and also increased the knowledge and attitude related to the role and function of students in disaster management effort. In addition, other additional results related to the effectiveness of training methods for PNC members successfully obtained. The methods compared were lecture and discussion (LD) compared with LD plus simulation and FGD methods. It is measured by using the quasi experimental method with pretest-posttest control group design. Data analysis used Mann Whitney test. The result showed that there was no difference in knowledge and attitude (p = 1,000) in both treatment groups (p = 0,424). Which means that there was no difference between LD method and LD plus simulation and FGD method.Keywords: Training, Disaster Management, Padjadjaran Nursing Corps (PNC)

Abstract

Aims: Preparedness of medical personnel in dealing with disasters is strongly influenced by several factors, one of which is knowledge, attitude, role and organization in disaster management. Nowadays, healthcare profesional is facing frequent natural disaster, thus identification on their disaster preparedness is imperative. To identify factors related to the preparedness of medical personnel in handling natural disasters at the Regional General Hospital dr. Slamet, Garut Regency, West Java. Methods: This type of research was a cross-sectional study using an online survey. The sample technique used is convenience sampling with a total number of research subjects as many as 245 nurses and 54 doctors with a total of 299 respondents. The multivariate test uses simple linear regression. Results: The mean score of disaster preparedness among nurse was 137.33 (SD=16.79) and doctor was 136.07 (SD=17.40), dengan p-value 0.799. There was a significant relationship between preparedness and knowledge (r = 0.658, p-value = 0.000), attitude (r = 0.602, p-value = 0.0000), role (r = 0.541, p-value = 0.000), and organization (r = 0.610, p-value 0.000). Knowledge (B = 1.40 (95% CI = 0.62 - 2.19, p-value = 0.00), role (B = 0.94 (95% CI = 0.50 - 1.38, p-value = 0.001), and organization (B = 1.01 (95% CI = - 0.1 - 2.13, p-value = 0.035) had a significant relationship with the preparedness of medical personnel for natural disaster management with a R2 value of 0.58. Knowledge, roles, and organization were the dominant factors that significantly correlate with the preparedness of medical personnel for natural disaster management. Conclusion: Medical personnel at General Public Hospital Dr. Slamet Garut have good preparedness for natural disasters. They have good knowledge about disaster management but lack detailed planning. Attitudes towards disaster management are good, but improvement is lacking. The role of medical personnel is good, but leadership is lacking. Organizational factors, planning, policy, and professional development are significant factors affecting preparedness.

Abstract

Introduction: The presence of family members who are treated in the intensive care unit makes the patient's familyfeel anxiety related to the fear of the death of someone theylove. In dealing with this, family-centered care is used as anapproach that has advantages for families, patients, and healthworkers. This case report aims to describe the family centeredcare towards patient and patient’s family with diabeticketoacidosis and Multiple Organ Dysfunction Syndrome. Method: This study used a case report to describe theintervention in the case. The sample of this study was an adultpatient with diabetic ketoacidosis and MODS who has a family with psychological problems in an intensive care unit. Results: Based on assessment the patient's general conditionwas weak and restless, the level of consciousness undersedation with Ramsay score was 5, the urinalysis found a ketonuria and glucosuria. The patient's family feel guilty, worried, and always asks about the patient's condition whilecrying. The interventions provided with the FCC includeinformation on the patient's condition, treatment plans, andresults after the action is taken, spiritual guidance when thefamily visit time, and provides support. The output obtainedwas the patient's family feels calmer and satisfied with thehealth services provider. Conclusions: The implementation of the FCC can decreaseanxiety and increase health care satisfaction, but theimplementation can be challenged so that it is necessary todevelop the FCC program in the ICU which requiresinvolvement and input from all parties.

Abstract

This study aims to identify the application of the Early Warning Score (EWS) system to patient safety. The method used is Literature Review. Article searches have been carried out comprehensively using the Pubmed and Google Scholar databases. Search for articles with the keywords Early Warning Score, Emergency, Patient Safety. The research results show that safety increases from early detection using EWS. EWS is very influential in improving patient safety. EWS is a scoring system that is simple, practical, easy to use and does not increase the workload of nurses. The influence of nurse competency using EWS can improve patient safety because it makes nurses compete to improve knowledge and skills, nurse communication, decision making, and collaboration with other health workers. In conclusion, the application of the Early Warning Score is very influential in supporting patient safety, where the Early Warning Score can detect patient deterioration early so that nurses are able to carry out further nursing actions. Apart from that, nurses' understanding and good nursing skills can support patient safety. Keywords: Emergency, Patient safety, Early Detection Score

Abstract

This study aims to explore the experience of nurses in monitoring Coronavirus Disease 2019 (COVID-19) patients at Tantara Hospital, Ambon City. The method used is qualitative research using a phenomenological design. Participants in the study were health workers working in the isolation room, namely nurses with the following criteria: a minimum of 2 years work experience in the isolation room and a minimum diploma of three, fulfilling the qualifications as an isolation health worker and communicating cooperatively with researchers. Research data collection was carried out using semi-structured interviews, then analyzed using the Colaizzi analysis method. The results showed that there were 5 themes related to the experience of nurses in monitoring COVID-19 patients: Anxiety, Knowledge, nurse loyalty, nurses' feelings, and efforts to reduce the risk of contracting it. In conclusion, nurses experience anxiety due to high job demands, including long working hours, nurse knowledge, loyalty, nurses' feelings and efforts to reduce the risk of infection are very important for nurses to be able to provide appropriate nursing care for patients. Abstract

One of the aims of the care service in the Intensive Care Unit (ICU) is to prepare patients in end-of-life conditions to die in a dignified manner. This can be challenging due to decision-making problems and result in a dilemma. Therefore, this study explores the ethical dilemmas experienced by nurses that provide end-of-life care in the ICU. The qualitative phenomenological methodology was used to describe the ethical dilemmas nurses face during these conditions. The data were collected through in-depth interviews and were analyzed using Colaizzi. Purposive sampling was used to select a total of eight participants, i.e., ICU nurses, who have treated end-of-life patients. Four themes were obtained from this study, namely, “the dilemma between the family’s decisions and continuing care”, “patient’s life expectancy and the family’s hope”, “DNR decisions and the nurse’s confidence”, and “the family’s understanding of the information provided”. This study recommends that the assessment of end-of-life status in critical care areas, especially the ICU, should be conducted as soon as possible to have a clearer purpose for the care provided.
