Abstract

Kabupaten Garut has a baribis fault, especially in the South Garut region and Mount Guntur which can cause earthquakes. Preparedness needs to be had to minimize the impact that might be caused by disaster, one of them is by students because students will be involved in all phases of the disaster. This study aims to determine the level of preparedness possessed by students of the Faculty of Nursing, Universitas Padjadjaran, Garut Campus. This research used a descriptive quantitative research design with cross sectional approach. Sampling using a total sampling technique on 356 respondents. Data collection uses a preparedness questionnaire consisting of 20 questions consisting of knowledge indicators, activity plans, disaster warning systems and resource mobilization. The data obtained were analyzed using a frequency distribution then presented in the form of a percentage. The results showed 29.8% of students were at the high level, 66.9% of students included at the moderate level, and 3.4% of students were at the low level. This study shows there are indicators of preparedness that are still lacking. Therefore, it is very important for the Garut campus to make efforts to improve student preparedness in dealing with earthquakes in the indicator of disaster warning systems

Abstract

The Covid-19 pandemic has been considered a global challenge for all countries around the world. The strategy that can be done to control Covid-19 is to develop an effective vaccine. The targets for the first wave of vaccines are health workers, public officials, and the elderly. For the second wave, the targets are vulnerable groups and the general public. The lack of acceptance of the Covid-19 vaccination in the community causes the vaccination program to be less than optimal. The purpose of this study is to determine the factors that influence people's doubts about the Covid-19 vaccine. The literature review method used is a scoping review. Researchers searched for the scientific articles in the Google Scholar, Pubmed, and Ebscohost databases. The keywords that researchers used consisting of 2 languages, in English keyword Covid-19 vaccine AND doubt OR hesitation AND factor OR reason OR cause. The year published was the last 5 years, and articles were selected using critical appraisal tools from The Joanna Briggs Institute (JBI). After screening according to inclusion criteria, 8 articles were found. The conclusion from the results of the journal analysis is that there are factors that influence people to refuse vaccines, including people with low education so they believe more in negative issues about vaccines, are not sure about the side effects of vaccines, are afraid of being injected, lack of information about vaccines, believe that no need vaccines to avoid Covid-19, and people who don't believe in Covid-19.

Abstract

Introduction: A diaphragmatic rupture, which can also be called a diaphragmatic tear, is a tear that occurs in the diaphragm. The muscles at the base of the thoracic cavity play an important role when breathing. The purpose of this study is to present a nursing care plan with a focus on handling ventilation disorders in patients with diaphragmatic rupture so that it can become nursing implications in the prevention and complications of diaphragmatic rupture. Method: Researchers used the case study method which was obtained in November 2022 in the General Intensive Care Unit Room at the Hasan Sadikin General Hospital, Bandung City. Discussion: The best treatment for this diaphragmatic rupture is surgery, one of which is laparotomy, a diaphragmatic rupture will result in impaired ventilation and must be treated using a ventilator in full control. When using a ventilator, the patient's condition must always be monitored hemodynamically, especially respiration rate and oxygen saturation. The collaboration of midazolam and rocuronium drugs is useful as a sedative to make the patient drowsy and decrease anxiety. Abstract

The COVID-19 outbreak is a worrying problem worldwide and health care workers have a big responsibility in responding to this condition. The increase in the number of COVID-19 patients which causes an increase in the workload of health care workers, scarcity of personal protective equipment, and stresses during work, can lead to psychological instability in health care workers.The study aims to identify the psychological conditions experienced by health care workers during the COVID-19 pandemic.The search for articles was carried out on the online digital database, Science Direct, and PubMed with the keywords 'COVID-19', 'psychological or psychology or mental health', 'nurse'. Then the articles are selected according to the inclusion criteria, namely the results of research in 2019-2020, full text, published in English, and articles discussing the psychological and mental conditions of health care workers who are assigned to the COVID-19 pandemic. In the end, 7 articles were analyzed in this literature review.In carrying out their duties during the COVID-19 pandemic, health care workers are prone to experiencing psychological disorders such as anxiety and insomnia. High workload, lack of personal protective equipment, lack of knowledge about the prevention and transmission of COVID-19, and pressures during work, are the main triggers for psychological problems experienced by health care workers. Therefore, coping strategies that come from internal and external need to be obtained by health care workers to reduce the psychological disturbance they experience.

Abstract

Nursing profession students need readiness to attend profession learning. Optimal readiness is directly proportional to optimal capabilities. The characteristics of emergency department tend to make nursing profession students the first and most frequent resource to deal with patients entering the emergency department. This study aims to determine the readiness of nursing profession students on handling emergency patients in emergency department.This study uses quantitative descriptive methods through approaches cross sectional. The population in this      study was a nursing profession students of the Faculty of Nurisng, Universitas Padjadjaran as many as 66 respondents. The total sampling technique was used in this study. This research measuring instrument uses the READI-JV The results of the study were calculated using quantitative descriptive analysis using a cross-sectional approach.The results showed that as many as 51 (77.3%) nursing profession students are not ready to handle emergency patients. The percentage of readiness obtained in the leadership and administrative support dimensions was 85.2%, the readiness obtained in the operational competency dimension was 63.9%, the readiness obtained in the establishment dimension was 63.1%, the readiness obtained in the stress handling dimension was 57.6%, the readiness obtained in the dimensions of group integration and identification of 57.5%, readiness obtained in the survival skills dimension was 56.6%, and readiness obtained in the clinical competency dimension was 47.2%.The unpreparedness of nursing profession students is because most of the respondents have not attended emergency department training. So that it is necessary to optimize the learning methods during pre- profession by applying in accordance with emergencies and based on case studies as well as providing stress management to students to reduce stress levels in nursing profession students.

Abstract

In general, Acute Respiratory Distress Syndrome or ARDS is acknowledged to be the cause of 10% opatients requiring ICU care, and about 3 million patients worldwide experience it. Despite the data, up to these days there haven’t been many treatments that are considered to be effective in preventing or treating ARDS.This research is a literature review with a narrative approach in which the researchers analyze several research articles.  The present study aims to analyze the management of ARDS in reducing mortality, improving lung function and airway effectiveness. The articles were searched based on the publication year 2016-2020, with the keywords “Acute Respiratory Distress Syndrome” AND “Oxygen Saturation”, “Acute Respiratory Distress Syndrome” AND “Treatment”, “Acute Respiratory Distress Syndrome” AND “Management”; as well as the databases of PubMed, EBSCO and American Journal of Clinical Pathology.Based on the 14 articles selected using inclusion criteria, it was found that there were 8 treatments given to the ARDS patients to reduce mortality, namely Non-Invasive Ventilation (NIV), the use of corticosteroids, UC-MSC (umbilical cord mesenchymal stem cells), oxygen therapy, mechanical ventilation, HFNC, prone position, open lung approach (OLA), and simvastatin.Of the 8 analyzed treatments, 3 interventions were found to have the highest significance of improvement in lung function and quality improvement in several indicators such as decreased mortality rate, increased saturation, decreased hospitalization time, etc. These interventions were: (1) Non-Invasive Ventilation (p value <0.001); (2) simvastatin (p value <0.001); (3) Corticosteroid (methylprednisolone) (p value <0.001).

Abstract

Hemodynamic instability is a problem that often occurs in postoperative patients in the ICU, one of which is thoracotomy surgery. Signs of hemodynamic instability are an increase in Mean Arterial Pressure (MAP), heart rate and respiratory rate, changes  in  decreased oxygen saturation, increased pain scale and even decreased consciousness. Maintaining hemodynamic stability in post-operative patients can be done by administering pharmacological and non-pharmacological interventions. Nursing care that can be done to maintain the patient's hemodynamic stability includes oxygenation, head position adjustment, stimulation with both verbal and non-verbal communication approaches, gentle massage relaxation and music therapy. Music therapy that can make patients relax and calm, one of which is by dhikr therapy and listening murottal Al-Quran. Purpose: to analyze dhikr therapy and listening murottal Al-Quran on changes in hemodynamic status in postoperative (thoracotomy) patients. Methods: This study used the case study method to explore  the  problem  of  nursing  care.  The  approach  used  is  the  nursing   care  approach which includes: assessment, diagnoses, interventions, implementation and evaluation. Hemodynamic recording is recorded on a flipchart and pain measurement is carried out using the Critical Pain Observational Tool (CPOT) instrument. Results: The results showed that after 3 days of intervention, there was a decrease in heart rate, respiratory rate, pain scale, increase in GCS values and other hemodynamic component values within normal limits. Conclusion: Therapeutic intervention of dhikr and listening murottal Al-Quran can maintain hemodynamic stability in postoperative patients in the ICU.

Abstract

Introduction: Pain is a subjective thought and can be experienced by everyone. Patients with decreased consciousness may also feel pain. The intensity of pain experienced by patients in the intensive care unit ranges from moderate to severe pain. Previous studies stated that the phenomenon of pain in patients can cause physical and psychological complications. Thus, pain management in the scope of intensive care is crucial to do. Purpose: To identify management that can be done in patients with unconsciousness who experience pain. Methods: This case study used Indonesian Nursing Intervention Standards as a guideline for pain management. Results: A 29-year-old female patient with decreased consciousness was treated in the intensive care unit. The patient was in the intensive care unit for four months. The results showed a severe pain scale at the time of the assessment using the CPOT. This study established a nursing diagnosis of chronic pain related to treatment procedures characterized by a severe pain scale using the CPOT, invasive devices, and the presence of fourth-degree pressure sores on the sacrum. Conclusion: Pain management carried out is pain assessment, non-pharmacological and pharmacological management. In each of these interventions, nurses must pay attention to nursing ethics. Abstract

Backgorund :The mortality rate of patients who were admitted in the intensive care unit has been increased. Therefore, end of life care is needed. Goals of end of life care is to helpthe patients would die with dignity and to help the familycould receive bereavement. End of life care is related with internal and external factors.Aims :This study aims to analyze the factors that related to end of life care.Methods :Thisstudy was a quantitative research withanalytic-correlation design. The samples were 62 critical care nurse, which obtained by total sampling. This study was conducted in ICU of Kendari Hospital Southeast Sulawesi. Data collection used questionnaire thenanalyzed with bivariate throughSpearman’s Rho and multivariate through logistic regresion.Results :The implementation of end of life care in ICU of Kendari Hospitalwas optimal (74%, median = 55), which was related to several factors such as knowledge (p = 0,000), moral distress (p = 0,002), patient and family characteristics = 0.009), organizational structure and culture (p = 0,000). Knowledge was the most related factor in improving the quality of end of life care (OR = 45,542) with probality 97%.Conclusion :Optimalization of end of life care is related with the personal (internal) of the nurse factors, patient and family, and organizational structure and culture. Implication :Related factors to end of life care will enrich the thought that delivered nursing care, nurse need to align knowledge with professionalism, leaving the pressures and dilemmas due to moral stress, as well foster a deep understanding of the nature of spirituality and culture sensitivity when accompanying dying patients, grows awareness that the best preferences of critical patient care are onesbenefit the patients and familys.

Abstract

ABSTRACT Acute respiratory failure (ARF) is a disorder of the pump function of the respiratory muscles or dysfunction of the lungs. Patients with respiratory failure use mechanical ventilation for breathing apparatus. However, mechanical ventilation often interferes with airway clearance and coughing. So intervention is needed to remove mucus from the airways. This study aimed to identify interventions that can be performed to reduce and clear mucus in patients with respiratory failure who are attached to a mechanical ventilator. This study used the narrative review method with the keywords “patient with ventilator OR patient with mechanical ventilation OR mechanically ventilated patient OR patient undergoing mechanical ventilation” AND “intervention OR strategies OR best practice OR treatment OR therapy OR program OR management” AND “mucus clearance OR airway clearance OR sputum clearance”. The databases used include Scopus, PubMed, EBSCO, Oxford. The results of 5 articles were obtained which consisted of various effective interventions for removing secretions such as ventilator hyperinflation mode, head down tilt position, manual chest compression (MCC), mechanical insufflation-exsufflation (MI-E), and chest physiotherapy. This intervention is effective in expelling mucus, increases expiratory flow bias which has the potential to increase secretions removal maneuvers, facilitates mucus cleaning during suctioning, and increases aeration in the dorsal lung without affecting blood gas analysis results. These various interventions can be a choice of solutions to help problems that arise due to the use of mechanical ventilation. Keywords: Acute Respiratory Failure, Mucus Suction, Ventilator  ABSTRAK Gagal napas akut (GGA) merupakan gangguan fungsi pompa otot pernapasan atau disfungsi pada paru-paru. Penderita gagal napas menggunakan ventilasi mekanik untuk alat bantu napas. Namun, ventilasi mekanis sering mengalami gangguan pembersihan lendir saluran napas dan batuk. Sehingga diperlukan intervensi untuk mengeluarkan lendir dari saluran napas. Penelitian ini bertujuan untuk mengidentifikasi intervensi-intervensi yang dapat dilakukan dalam menurunkan dan membersihkan lendir pada pasien dengan gagal napas yang terpasang ventilator mekanik. Penelitian ini menggunakan metode narrative review dengan kata kunci “patient with ventilator OR patient with mechanical ventilation OR mechanically ventilated patient OR patient undergoing mechanical ventilation” AND “intervention OR strategies OR best practice OR treatment OR therapy OR program OR management” AND “mucus clearance OR airway clearance OR sputum clearance”. Database yang digunakan antara lain scopus, pubmed, EBSCO, oxford. Didapatkan hasil 5 artikel yang terdiri  dari berbagai intervensi yang efektif untuk mengeluarkan sekret seperti mode hiperinflasi ventilator, posisi head down tilt, manual chest compression (MCC), mechanical insufflation-exsufflation (MI-E), dan fisioterapi dada. Intervensi ini efektif dalam pengeluaran mukus, meningkatkan aliran ekspirasi bias yang berpotensi meningkatkan manuver pengeluaran sekret, mempermudah pembersihan mukus ketika suction, dan meningkatkan aerasi di bagian dorsal paru tanpa mempengaruhi hasil analisa gas darah. Berbagai intervensi ini dapat menjadi pilihan solusi untuk membantu permasalahan yang ada akibat penggunaan ventilasi mekanik. Kata Kunci: Gagal Napas Akut, Penghisapan Lendir, Ventilator
