Abstract

ABSTRACT Patients in the Intensive Care Unit (ICU) often experience limitations in verbal communication due to ventilator use, sedation, or other medical conditions. These communication barriers can interfere with the delivery of medical information and affect the quality of patient care. Augmentative and Alternative Communication (AAC) is a potential solution to improve interactions between patients, healthcare providers, and families in the ICU environment. This review aimed to explore the implementation of AAC in the ICU, including its effectiveness, challenges, and impact on the quality of communication and patient care. This study used a scoping review approach following the PRISMA-ScR guidelines. A literature search was conducted through the EBSCOhost Medline, PubMed, and Google Scholar databases, using relevant keywords related to AAC and ICU. Studies that met the inclusion criteria were analyzed thematically to identify key findings. Of the 386 articles identified, 11 articles met the criteria for further analysis. The results showed that implementing AAC in the ICU can improve communication between patients and healthcare providers, reduce patient anxiety, and strengthen the therapeutic relationship. However, there are obstacles to its implementation, such as a lack of training for healthcare providers, time constraints, and a lack of institutional policies that support the systematic use of AAC. Despite the challenges in implementing AAC in the ICU, this technology has great potential to improve patient communication and quality of care. Therefore, a more comprehensive strategy is needed, including training health workers, developing institutional policies, and procuring communication devices that are appropriate to patient needs to support optimal implementation of AAC in the ICU. Keywords: Augmentative and Alternative Communication, ICU, Communication, Critical Care, Scoping Review.

Abstract

Introduction: Academic resilience defined as the ability to survive and increase success in terms of education despite facing severe problems. Academic resilience in students can be formed if students are able to break down these problems into smaller problems and then deal with them properly. To be able to overcome these small or minor problems, students need good academic buoyancy. This study aims to analyze the relationship between academic buoyancy and academic resilience in first-year students at the faculty of health Universitas Padjadjaran.Method: The research design is a quantitative correlation with a cross-sectional time approach. The instruments used were the Indonesian version of the Academic Buoyancy Scale and the Academic Resilience Scale-30. The research population was 1082 first-year students of the 2022 class of the Faculty of Health. The sampling technique used was proportional stratified random sampling (n=162).Results: The results of the correlation analysis show that there is a strong and positive relationship between academic buoyancy and academic resilience (p = 0.000 and r = 0.679).Conclusions: There is a strong and positive relationship between academic buoyancy and academic resilience. Students should strive to maintain and improve their academic buoyancy and academic resilience, for example by building social connections, maintaining health, developing positive thoughts, and seeking help when needed.

Abstract

Background: The incidence of cardiac arrest or Sudden Cardiac Arrest (SCA) in the community that cannot be saved increases every year. This is because they do not get the right and fast action due to the community's ignorance and inability to assist. Objective: This study aimed to evaluate health education on increasing knowledge and readiness of respondents in helping cardiac arrest victims. Methods: This study used a pre-experimental design with a one-group pre-post test approach to 52 respondents, namely the general public. The research sample was recruited using convenience sampling techniques. The sample was given health education on basic life support (BLS) and cardiac arrest management directly with several interactive sessions using visual aids such as posters and presentation slides. Data collection used a valid and reliable questionnaire (Knowledge and Readiness to Help). Data were analyzed univariately through frequency distribution, central tendency, mean difference, and bivariate tests using paired sample t-tests. Results: There was an increase in respondents' BLS knowledge from 6.87 to 11 (p<0.001) and readiness to help, with an average score increasing from 60.4 to 63.3 (p=0.002). These results indicate that health education can increase knowledge about BLS and readiness to help respondents. Research results show that readiness to act in emergencies depends on more than theoretical understanding; it also requires practical training, self-confidence, and direct experience. Conclusion: Health education significantly improved public knowledge and readiness to provide first aid for cardiac arrest victims. These findings indicate that structured Basic Life Support (BLS) education is effective in strengthening community preparedness and timely emergency response. Integrating regular and practical BLS training into community health programs may further enhance public capacity to respond to cardiac arrest events.

Abstract

ABSTRAK Keterlambatan kedatangan pasien dengan penyakit jantung koroner (PJK) ke rumah sakit menjadi masalah utama dalam penanganan medis yang tepat waktu. Keterlambatan ini dapat mengakibatkan kerusakan jantung permanen, tingginya angka kematian, dan morbiditas jangka panjang. Namun, faktor-faktor yang menyebabkan keterlambatan ini sering kali kompleks dan berkaitan dengan berbagai aspek seperti sosial demografis, akses ke layanan kesehatan, dan perilaku kesehatan pasien. Untuk mengeksplorasi faktor-faktor yang berhubungan dengan keterlambatan kedatangan pasien penyakit jantung koroner.  Penelitian ini menggunakan scoping review berdasarkan kerangka Arksey dan O’Malley serta panduan PRISMA-ScR. pencarian artikel yang dilakukan di tiga database yaitu CINAHL, PubMed, Scopusdan search engine google scholar. Kata kunci yang digunakan meliputi "factors", "delay", "coronary heart disease", "myocardial infarction", dan "pre-hospital delay". Kriteria inklusi mencakup artikel yang diterbitkan dalam bahasa Inggris atau bahasa indonesia antara tahun 2015-2025 dan membahas faktor-faktor yang mempengaruhi keterlambatan kedatangan pasien dengan penyakit jantung koroner. Pertanyaan penelitian utama adalah; Apa saja faktor-faktor yang berhubungan dengan keterlambatan kedatangan pasien dengan penyakit jantung koroner?. Terdapat 12 artikel yang memenuhi kriteria inklusi dan membahas faktor-faktor yang berhubungan dengan keterlambatan kedatangan pasien dengan penyakit jantung koroner. Temanya meliputi sosial demografis, gejala klinis, akses ke layanan kesehatan, perilaku kesehatan, dan faktor budaya. Faktor-faktor utama yang ditemukan meliputi jenis kelamin, usia, tingkat pendidikan, status sosial ekonomi, akses transportasi, serta kesadaran gejala penyakit jantung. Faktor-faktor ini berhubungan dengan kurangnya pemahaman tentang gejala atau peran gender dalam pengambilan keputusan medis, serta keterbatasan akses ke layanan medis yang memadai, terutama di daerah pedesaan. Keterlambatan kedatangan pasien dengan penyakit jantung koroner dipengaruhi oleh berbagai faktor yang saling terkait. Untuk mengurangi keterlambatan, diperlukan pendidikan masyarakat tentang gejala penyakit jantung, serta peningkatan akses ke layanan kesehatan dan perbaikan sistem transportasi medis. Kata Kunci: Faktor, Keterlambatan Kedatangan, Penyakit Jantung Coroner. ABSTRACT Delayed hospitalization of patients with coronary heart disease (CHD) is a major obstacle to timely medical care. This delay can result in permanent heart damage, high mortality, and long-term morbidity. However, the factors causing this delay are often complex and related to various aspects such as sociodemographics, access to healthcare, and patient health behaviors. To explore factors associated with delayed hospitalization for patients with coronary heart disease, this study used a scoping review based on the Arksey and O'Malley framework and the PRISMA-ScR guidelines. Articles were searched in three databases: CINAHL, PubMed, Scopus, and the Google Scholar search engine. Keywords used included "factors," "delay," "coronary heart disease," "myocardial infarction," and "prehospital delay." Inclusion criteria included articles published in English or Indonesian between 2015 and 2025 that discussed factors influencing delayed hospitalization for patients with coronary heart disease. The primary research question was: What are the factors associated with late arrival of patients with coronary heart disease?. Twelve articles met the inclusion criteria and discussed factors associated with delayed arrivals for patients with coronary heart disease. The topics covered sociodemographics, clinical symptoms, access to healthcare, health behaviors, and cultural factors. Key factors identified included gender, age, education level, socioeconomic status, access to transportation, and awareness of heart disease symptoms. These factors are associated with a lack of understanding of symptoms or the role of gender in medical decision-making, as well as limited access to adequate medical services, particularly in rural areas. Delayed arrivals for patients with coronary heart disease are influenced by various interrelated factors. To reduce delays, public education about heart disease symptoms, increased access to healthcare, and improvements to medical transportation systems are needed. Keywords: Factors, Delayed Arrivals, Coronary Heart Disease.

Abstract

The mostly caused death on chronic renal failure are overhydration and hypercalemia. Therefore, the compliance of patients with chronic renal failure recognized as attempt to decrease mortality rate of it. The aim of this study was to describe compliance of fluid restriction and a low salt (sodium) diet in patients with chronic renal failure undergoing regular hemodialysis at Hospital dr. Hasan Sadikin. This research used descriptive method with cross-sectional approach. The samples of this research were regular hemodialysis patients who were selected by using consecutive sampling technique of 93 people. The research instrument used questionnaire to measure compliance of fluid restriction and low salt (sodium) diet. Analysis used univariate analysis with median values.The analysis result showed that respondents who are compliance to the restriction of fluid were 35 people or 37.6%, while non-compliance is 58 people or 62.4%. Respondent who were comply of a diet low in salt (sodium) were 31 people or 33.3%, while non-compliance is 62 people or 66.7%. This study showed that most of patients with chronic renal failure did not adhere to restrictions of fluid and low-salt diet (sodium). The increasing of frequency education motivation and dietary fluid counseling in each hemodialysis schedule are nessesary.

Abstract

Nurses’ preparedness of facing disasters plays an important role in determining the success rate of disaster management. Nurses have an important role in providing nursing actions in the phase of disaster mitigation, preparedness, response, recovery and rehabilitation. The purpose of this Literature Review was to identify nurses’ preparedness in dealing with disasters. The data collection technique was by searching for articles published in 2010 – 2020 on Pubmed, Proquest, Ebsco and Google Scholar with keyword Nurse, disaster, preparedness, disaster preparedness, and disaster management. The inclusion criteria are abstract and full text, the type of search in the database with journal articles, cross-sectional study design, published in Indonesian and English, articles published from 2010 to 2020 and articles discussing the preparedness of nurses to face disasters. The article search used PRISMA flow diagram and obtained 1859 articles, but only 12 articles were included in the article analysis review. Of the 12 articles analyzed, they show that the majority of nurses were not ready in facing disasters with a range of 46.5% - 97.5%. However, 69% of nurses were interested to participate in disaster training for preparedness of self-protection. The results of the review concluded that nurses are less than optimal in dealing with disasters, so every agency must encourage the nurse preparedness training in dealing with disasters.

Abstract

Spiritual care is a part of end of life care, which role is to provide a sense of comfort and peace to the patient and ease the grieving process for the family. Nevertheless, COVID-19 pandemic caused many challenges experienced by ICU nurses in the implementation of spiritual care. This study aimed to identify empirical evidences of ICU nurses experience in providing spiritual care to end-of-life care patients during COVID-19 pandemic. It used a literature review method, analyzing articles from 5 reputable databases, namely Pubmed, Proquest, CINAHL, Science Direct, and Google Scholar with the range of publication years between 2020 and 2022. The keywords used were 'pandemic COVID-19', 'critical care nursing’, 'end of life care', 'dying', 'spiritual care', and 'nursing experience'. The articles were selected in stages by PRISMA flow. 2,727 articles were identified and selected based on the duplication and the inclusion criteria until 6 analysis-deserve articles were obtained. The analysis results showed that during COVID-19 pandemic spiritual care was an important domain in end of life care. Spiritual care for end-of-life patients can reduce anxiety and ease the grieving process for the family. The implementation of spiritual care during pandemic era experienced many challenges; thus, an innovative strategy was required to implement it. Health care facilities should provide supports for nurses to carry out spiritual care by increasing their competences through education and training.
