Abstract

The Maternal Mortality Ratio (MMR) in Indonesia remains high, including in West Java province. One of the MMR causes is preeclampsia (24%). Currently, the management of preeclampsia is focused on pharmacological therapy. Preeclampsia is characterized by high blood pressure. Some studies revealed that non-pharmacological therapy was effective in dealing high blood pressure including foot massage. Foot message has benefits both physiologically and psychologically. However limited study applied this massage among pregnant women with preeclamsia. The aim of this study was to determine the effectiveness of as a complementary therapy: foot massage to reduce of blood pressure among pregnant women with preeclampsia. The study applied quasi experimental design with time series approach. The samples were chosen using consecutive sampling technique. This study involved 30 pregnant women with preeclampsia. There were two steps of data collection. First, respondents’ blood pressure was measured by researchers without any interventions for 6 days. Second, the 7th -12th respondents had 20 minutes message, and observed blood pressure at the first minute before massage (pr-etest), and the 30th minutes after massage (post-test). Data were analyzed using Friedman Test.The result showed that there were no significant differences of mean systolic and diastolic blood pressure between pre-test and post-test (p> 0.05) in control period. Whereas, in the treatment period showed that there were a significant differences of mean systolic blood pressure between pre-test and post-test on the seventh day until twelfth day (p < 0.05). While, the significantly differences in mean of diastolic blood pressure between pre-test and post-test were present during the eighth until twelfth in treatment period (p < 0.05). This study revealed that there were significant differences in decreasing of blood pressure among pregnant women with preeclampsia after treatment by foot massage. The foot massage techniques can be used as one of the interventions to lower blood pressure in pregnant women with preeclampsia, however further research is needed to support the evidence based for nursing intervention especially in complementary therapies.

Abstract

Bayi prematur yang terpasang alat bantu napas harus dalam kondisi tenang sehingga ada sinkronisasi antara napas bayi dengan alat bantu napas yang dimanifestasikan dengan perubahan heart rate, respiration rate dan saturasi oksigen. Salah satu cara membuat bayi tenang selama penggunaan alat bantu napas adalah pemberian terapi musik lullaby. Tujuan penelitian ini untuk melihat pengaruh terapi musik lullaby terhadap heart rate, respiration rate dan saturasi oksigen pada bayi prematur yang terpasang alat bantu napas. Penelitian ini menggunakan desain quasi experiment design with pre-post test without control group terhadap 22 bayi prematur yang dipilih secara non probability sampling melalui pendekatan purposive dengan kriteria bayi dipasang alat bantu napas, usia gestasi 24-36 minggu, tidak mengalami ensepalofati hipoksik iskemik. Pengumpulan data dilakukan pengukuran heart rate, respiration rate dan saturasi oksigen sebelum terapi musik lullaby diberikan dan setelah musik lullaby diberikan selama 3 hari. Analisa data yang digunakan adalah uji t dependen. Hasil menunjukan adanya perbedaan rata-rata heart rate, respiration rate dan saturasi oksigen pada hari pertama sebelum terapi musik lullaby diberikan dibandingkan dengan hari ketiga setelah terapi musik lullaby diberikan dengan nilai p value <0,05 untuk heart rate, p value <0,05 untuk respiration rate dan p value <0,05 untuk saturasi oksigen. Pemberian musik lullaby terbukti mampu membuat bayi prematur tenang dan dapat dilakukan di tempat perawatan bayi prematur lainnya yang terpasang alat bantu napas sebagai salah satu upaya mempertahankan ketenangan pada bayi prematur.Kata kunci: Bayi prematur, heart rate, respiration rate, saturasi oksigen, terapi musik lullaby Effect of  Lullaby Music Therapy on Heart Rate, Respiration Rate, Oxygen Saturation on Prematur InfantAbstractPremature infants assisted with breathing apparatus should be in calm condition so that there is synchronization between the baby’s breath and the breathing apparatus manifested by changes in heart rate, respiration rate and oxygen saturation. One way to make babies calm during the use of breathing aids is the provision of lullaby music therapy. The purpose of this study was to find out the effect of lullaby music therapy on heart rate, respiration rate and oxygen saturation in premature infants with breathing apparatus. This study used quasi experiment design with pre-post test without control group to 22 preterm babies selected by nonprobability sampling technique via purposive approach with criteria of infant with breathing apparatus, gestational age 24-36 weeks, and no ischemic hypoxic ensepalofati. Samples taken were heart rate measurement, respiration rate, and oxygen saturation before and afterlullaby music therapy was given for 3 days. Data analysis used was t test dependent. The statistical results showed the difference in heart rate, respiration rate and oxygen saturation on the first day before lullaby music therapy was administered compared to the third day after lullaby music therapy was administered with a pvalue value <0.05 for heart rate, pvalue <0.05 for respiration rate, and pvalue <0.05 for oxygen saturation. The provision of lullaby music was proven to make premature babies at peace and could be provided in other baby care unit with breathing support as an effort to sustain peace for premature babies.Keywords: Heart rate, lullaby music therapy, premature infants, respiration rate, oxygen saturation.

Abstract

The Emergency Department (ED) is a hospital service unit that provides the first service for patients with disease conditions that threaten their lives or can cause disability for 24 hours. Implementation of patient safety in the ED should be applied to minimize the risk of error handling for the patient. ED staff perceptions related to the implementation of patient safety is a factor that directly-related to his behavior in applying the implementation of patient safety. This study aimed to analyze the relationship between perceptions of staff ED and patient safety by implementing patient safety at the Regional Hospital Emergency Department Cirebon. This study was a correlational study with the cross-sectional approach of 99 emergency staff with total sampling at Cirebon. Collecting data used questionnaires of patient safety. Based on the results of the univariate analysis showed that the majority (80%) of respondents either category on the implementation of the sub-variables of patient safety team collaboration and communication, only a small proportion of respondents less category (20%) on the implementation of the sub-variables of patient safety team collaboration and communication. In addition, less than half (49.5%) category lacking in implementing patient safety, only half (50.5%) categories, both in the implementation of patient safety. Based on the results of the bivariate analysis showed that the relationship implementation of patient safety with all the variables, namely teamwork (p-value = 0.000), communications (p-value = 0.005), the concept of patient safety (p-value = 0.005), and perception (p-value = 0.005). Based on the results of the study, the researchers concluded that the relationship between staff perceptions of the emergency department (ED) on patient safety by implementing patient safety at the Regional Hospital emergency department (RSD) Cirebon. IGD support staff perceptions of patient safety, but still found lacking in the category of health workers implementation of patient safety, so the need for patient safety education and training with simulation methods to illustrate the approach in the implementation of patient safety.

Abstract

Communication among nurses, patients, and families takes an important role in the intensive care unit in which the patients are in critical condition and unable to involve in two-way communication. Research related to effective nurse-patient communication has been done extensively, but the information regarding communication in intensive care unit is still limited. This research aimed to explore nurses’ experiences in the intensive care units in effective communication to patients/patient’s families. This was a qualitative study project with phenomenology approach. The data were collected using the in-depth interview technique approximately 60 minutes involving ten nurses who were selected using the purposive sampling at Al Islam Hospital Bandung. Data were analysed using the Colaizzi method and the results were presented in themes. Based on the nurses’ experiences, four themes were emerged in this study including (1) Nurses’ dilemma of their professionalism and personal issues/matters, (2) Contextual factor affects selection of nurses’ communication technique, (3) Barriers in effective communication; difficulties in accompanying families to accept critical patient conditions, care and treatment procedures in the ICU which were complicated, and misunderstanding between nurse-patient and family (4) Compassion and patience are required in nurse-patient communication in ICU. The complex patient/family conditions in the ICU require nurses to choose the appropriate communication technique accompanied by a sense of compassion and patience. Nurses need to improve their ability to communicate effectively in order to lower the barriers in communicating between nurses-patients/families. Recommendations, training and assistance of effective communication become important for nurses in improving services in the Intensive Care Unit.

Abstract

End stage renal disease (ESRD) is a serious chronic disease that resulted from complications of chronic renal failure and a final diagnosis that requires treatment modalities such as dialysis or continuous ambulatory peritoneal dialysis (CAPD). However, CAPD is rarely selected as renal replacement therapy by ESRD patients in Indonesia. Therefore, the phenomenon associated with patients undergoing CAPD is unclear, especially in Indonesia. The purpose of this study was to explore the lived experiences of ESRD patients undergoing CAPD therapy. The research method employed qualitative descriptive phenomenology. The participants in this study were 10 ESRD patients with CAPD. Data collection utilized semi-structured interviewed and data was analyzed using the Colaizzi approach. The result of the study was explained through 6 themes. These are: 1) Condition improved with CAPD, 2) Freedom of activity, 3) Between feeling better and bored 4) Health is increased, 5) It is sustained life, 6) Resignation to accept illness. In conclusion, the patients with CAPD felt significant positive changes in their life and CAPD is considered life-sustaining for the participants, as the majority of the symptoms was decreased significantly. Therefore, the partisipants felt their conditions improved better. The unpleasant sides of CAPD are feeling bored. The support of the family is important as it produced strength and passion for patients in undergoing CAPD

Abstract

Penatalaksanaan nyeri dan kecemasan yang dialami oleh pasien dapat dilakukan dengan pendekatan farmakologis dan non-farmakologis. Pemberian analgetik dan sedatif tidak bisa langsung menghilangkan nyeri dan kecemasan pasien bahkan jika berlebihan bisa menimbulkan depresi pernapasan dan ketidakstabilan kardiovaskuler sehingga perlu dilengkapi dengan pendekatan non-farmakologis seperti penggunaan terapi musik relaksasi dan suara alam. Tujuan penelitian ini adalah untuk mengetahui sejauh mana efektifitas musik relaksasi, suara alam dan kombinasi keduanya terhadap nyeri dan kecemasan pasien. Metode yang digunakan adalah literature review dengan studi kepustakaan dan pencarian elektronik yang menggunakan search engine EBSCOhost (MEDLINE), GALE (infotract.galegroup) dan google dengan kata kunci yang digunakan yaitu patients, anxiety, pain, relaxation, music, dan nature. Kriteria pemilihan sumber antara lain artikel, karya ilmiah atau buku yang membahas tentang teknik relaksasi, terapi musik, dan suara alam yang difokuskan pada nyeri dan kecemasan serta dipublikasikan tahun 2002-2012 dengan bahasa Indonesia atau bahasa Inggris. Hasil dari pencarian didapatkan 20 artikel penelitian (dua di antaranya systematic review) yang memenuhi kriteria. Hasil review dari beberapa artikel penelitian tersebut menunjukkan bahwa 76% perawatan standar ruangan yang dikombinasikan dengan terapi musik lebih efektif menurunkan tingkat kecemasan dan 76,2% efektif menurunkan tingkat nyeri pada pasien dibandingkan tanpa terapi musik. Tujuh piluh lima persen (75%) perawatan standar yang dikombinasikan dengan terapi suara alam lebih efektif menurunkan kecemasan dan 100% efektif menurunkan tingkat nyeri pasien dibandingkan tanpa terapi suara alam. Perawatan standar yang dikombinasikan dengan gabungan antara terapi musik relaksasi dan suara alam menunjukkan bahwa 100% efektif menurunkan nyeri dan kecemasan pasien. Intervensi keperawatan non-farmakologis seperti terapi musik, suara alam dan kombinasi keduanya bukan bersifat menggantikan fungsi manajemen nyeri dan kecemasan pasien namun sebagai pelengkap intervensi farmakologi dalam tatanan klinik.Kata Kunci: kecemasan, musik relaksasi, nyeri, suara alam

Abstract

Latar Belakang: Dr. Hans Selye membagi respon terhadap stress dalam tiga tingkatan yaitu alarm (alarm), perlawanan (resitance) dan peredaan (exhaustation), Ada beberapa penyakit yang dimulai dari stress, seperti hipertensi. Hipertensi di desa Ciganjeng dan Karangsari, angka kejadian hipertensinya cukup tinggi, untuk dikarangsari ada sekitar 43orang, di ciganjeng ada sekitar 54orang, data didapat dari bidan desa ciganjeng dan karangsari. Menurut petugas kesehatan di kedua desa tersebut baru ada penanganan dalam bentuk pemberian obat hipertensi saja, itupun tidak semua pasien berobat rutin. kami menganggap perlu melakukan pemberdayaan masyarakat tentang cara perawatan hipertensi dengan manajemen stress di desa ciganjeng dan karangsari kecamatan Padaherang kabupaten Pangandaran.   Metode: Metode penelitian yang digunakan adalah deskriptif kuantitatif serta analisa data dilakukan dengan mencari nilai rata-rata pada data pre-test dan post-test. Tehnik sampel yang digunakan adalah total sampling, yaitu sampel yang terkumpul adalah 59 untuk desa Ciganjeng dan 51 untuk Desa Karangsari. Kuesioner yang digunakan adalah 10 item pertanyaan pilihan tunggal. Intervensi yang dilakukan untuk pemberdayaan masyarakat desa Karang sari dan Desa Ciganjeng Kecamatan Padaherang adalah dengan cara melakukan penyuluhan tentang managemen stress dan juga hipertensi, karena sesuai dengan teori yang akan diambil adalah theory self care, yaitu dengan mamandirikan masyarakat dan memberdayakan masyarakat untuk meningkatkan derajat kesehatannya. terjadi peningkatan pengetahuan masyarakat desa Ciganjeng tentang masalah psikososial dan hipertensi.   Hasil: Perubahan penilaian pengetahuan pre dan post lebih dari 100% peningkatannya, hal ini membuktikan bahwa masyarakat desa Ciganjeng memiliki kemampuan untuk dibina dalam membuat desa siaga, sehingga masyarakat desa Ciganjeng akan menjadi masyarakat atau desa siaga.

Abstract

Tujuan: Penyakit gagal ginjal kronis menyebabkan masalah kesehatan seperti manifestasi klinis, komplikasi dan efek samping terapi yang akan mempengaruhi status fungsional yang berhubungan dengan kinerja, kemandirian, pemeliharaan kesehatan, dan kesejahteraan. Penelitian ini bertujuan untuk mengetahui gambaran status fungsional pada pasien gagal ginjal kronis yang menjalani hemodialisis di rsup dr. hasan sadikin bandung.   Metode: Penelitian ini menggunakan metode deskriptif kuantitatif dengan jumlah subyek penelitian sebanyak 80 orang yang diambil dengan menggunakan teknik purposive sampling. Penelitian menggunakan instrumen (FACIT), data dianalisis menggunakan nilai mean. Semakin tinggi skor semakin baik status fungsional atau kesejahteraan.   Hasil: Nilai mean status fungsional pada pasien gagal ginjal kronis yang menjalani hemodialisis adalah 2,65 (±3,08) dengan nilai kesejahteraan emosional 2,36 (±0,48), kesejahteraan fisik 2,40 (±0,60), kesejahteraan fungsional 2,58 (±0,72), kesejahteraan sosial/keluarga 2,87 (±0,78) dan kesejahteraan spiritual 3,07 (±0,41).   Simpulan: Dari hasil tersebut terjadi penurunan status fungsional pada kesejahteraan emosional, fisik, dan fungsional. Upaya meningkatkan status fungsional tersebut dapat dilakukan dengan terapi aktivitas kelompok agar pasien dapat mengungkapkan perasaan dan pengalaman selama menjalani hemodialisis.

Abstract

AbstrakTujuan: Sistem code blue merupakan sistem komunikasi yang mampu memastikan dilakukannya panggilancepat dan efektif untuk melakukan resusitasi pasien henti jantung paru. Sistem ini dibangun sebagai responcepat terhadap kegawatan jantung paru di area rumah sakit, sehingga mampu memberikan resusitasi danstabilisasi sesegera mungkin.Tim ini harus bekerja cepat dan efisien untuk mengoptimalkan pertolongan. Disisi lain, kondisi kegawatan jantung paru seringkali menimbulkan stressor yang tinggi pada tim sehinggacukup mengganggu pelaksanaan. Literature review ini dilakukan untuk mengidentifikasi pelaksanaan codeblue.Metode: Metode yang dilakukan dengan melakukan analisis kritis pada artikel penelitian berbahasa inggristentang identifikasi pelaksanaan code blue tahun 2013-2018. Pencarian artikel dilakukan pada mesinpencarian Google Scholar, PubMed, serta Sciencedirect, dan dipilih dengan kriteria mudah diakses fulltexnya.Kata kunci yang digunakan adalah “Evaluation of Code blue”, “Implementation of Code blue”. Artikeldiseleksi secara bertahap dengan menggunakan metode PRISMA dan didapatkan 10 artikel yang relevan.Hasil: Hasil menunjukkan tiga hal penting dalam pelaksanaan code blue, meliputi : identifikasi pasien yangtidak tepat, code blue efektif untuk meningkatkan ROSC (return of spontaneous circulation), danpelaksanaan sesuai protokol akan memberi hasil lebih baik.Simpulan: Identifikasi pelaksanaan code blue menunjukkan tiga hal ini sangat bermanfaatbagi pelaksanaancode blue. Bluecode team must bework fastly and efisienly to get maximal outcome. Beside that,bluecode situation is full of stressor conditions,that obstruct the implementation. This literature review aims to identify the implementation of bluecodesystem.Method: A method used is critical review full text of 2013-2018 period in English language. The multipledata bases used is Google Scholar, PubMed, and Sciencedirect with keyword “Evalution of code blue”, and“Implementation of Code blue”. The article select gradually by using of appraisal tool of PRISMA and obtained 10 relevan article.Results: The literature review obtained 3 important point on the implementation of code blue. First,Identification patient not properly, second, blucode is effective for increasing of rosc event,third,implementation base on protocol give better outcome .Conclusion: This literature review give us three important point on bluecode implementation. Abstract

Introduction: Stress experienced by the baby will affect the body’s function by increasing the body’s metabolism. Nesting is used to reduce stress in premature babies. Nesting can be done in a supine or prone position. Few studies have examined the effects of body position on body weight and oxygen saturation. The objective of the study was to determine the difference in oxygen saturation and weight change on the use of nesting in the prone and supine positions in premature babies.Methods: The research used a quasi-experimental design. The sample consisted of 30 premature babies, which was obtained using a consecutive sample technique. The independent variables were nesting positioning (supine and prone), and the dependent variables were oxygen saturation and body weight. The data of oxygen saturation and the baby’s weight were collected using pulse oximetry; the baby’s weight scale used observation sheets. The data was analysed using the t-test, Wilcoxon Sign Ranks Test, and Mann Whitney U Test.Results: The results showed that there was a difference in oxygen saturation before and after the use of nesting in the supine (p=0.001) and prone position (p=0.000). There was a weight difference before and after the use of nesting in both supine (p=0.000) and prone position (p=0.000). There was no difference in oxygen saturation value and infant weight, before or after, between the supine position and the prone position (p=0.18; p=0.9).Conclusion: The use of nesting in both positions (supine or prone) can increase oxygen saturation and infant weight. Researchers recommend the use of nesting with supine or prone positions routinely in premature babies.
