Abstract

The mostly caused death on chronic renal failure are overhydration and hypercalemia. Therefore, the compliance of patients with chronic renal failure recognized as attempt to decrease mortality rate of it. The aim of this study was to describe compliance of fluid restriction and a low salt (sodium) diet in patients with chronic renal failure undergoing regular hemodialysis at Hospital dr. Hasan Sadikin. This research used descriptive method with cross-sectional approach. The samples of this research were regular hemodialysis patients who were selected by using consecutive sampling technique of 93 people. The research instrument used questionnaire to measure compliance of fluid restriction and low salt (sodium) diet. Analysis used univariate analysis with median values.The analysis result showed that respondents who are compliance to the restriction of fluid were 35 people or 37.6%, while non-compliance is 58 people or 62.4%. Respondent who were comply of a diet low in salt (sodium) were 31 people or 33.3%, while non-compliance is 62 people or 66.7%. This study showed that most of patients with chronic renal failure did not adhere to restrictions of fluid and low-salt diet (sodium). The increasing of frequency education motivation and dietary fluid counseling in each hemodialysis schedule are nessesary.

Abstract

Nurses’ preparedness of facing disasters plays an important role in determining the success rate of disaster management. Nurses have an important role in providing nursing actions in the phase of disaster mitigation, preparedness, response, recovery and rehabilitation. The purpose of this Literature Review was to identify nurses’ preparedness in dealing with disasters. The data collection technique was by searching for articles published in 2010 – 2020 on Pubmed, Proquest, Ebsco and Google Scholar with keyword Nurse, disaster, preparedness, disaster preparedness, and disaster management. The inclusion criteria are abstract and full text, the type of search in the database with journal articles, cross-sectional study design, published in Indonesian and English, articles published from 2010 to 2020 and articles discussing the preparedness of nurses to face disasters. The article search used PRISMA flow diagram and obtained 1859 articles, but only 12 articles were included in the article analysis review. Of the 12 articles analyzed, they show that the majority of nurses were not ready in facing disasters with a range of 46.5% - 97.5%. However, 69% of nurses were interested to participate in disaster training for preparedness of self-protection. The results of the review concluded that nurses are less than optimal in dealing with disasters, so every agency must encourage the nurse preparedness training in dealing with disasters.

Abstract

The high mortality rate in the Emergency Department (ED) is consistent with the high demand for the end of life (EOL) care. However, ED work system required to live-saving quickly and precisely. It affects the quality of EOL care. The purpose of this study was to determine nurses’ perceptions in ED related to supportive behaviors (SB) and burdens in providing EOL care to dying patients. The descriptive-quantitative method was used with a cross-sectional approach in 50 nurses Emergency Department at one of the tertiary hospitals in Indonesia. The samples were chosen using the total sampling technique. Data collection used emergency nurse perceptions of end of life care questionnaire. Data were analyzed using univariate analysis in the form of distribution, frequency, and mean. The result of the study showed that the three largest obstacles in EOL care were:  ED nurses’ work loads (mean=10.626), intra-family disagreements (mean=9.802), and deal with angry of family members (mean=9.472). The three largest supportive behaviors were teaching families how to act (mean=11.962), allowing family members adequate time with the patient after a patient died (mean=11.09), and good collaboration from health professionals (mean=10.886). According to this research, further studies are needed to minimize obstacles and improve supportive behaviors related to the nurse’s workload, the provision of spiritual teams, special training for the nurse and the provision of special rooms for EOL patients and their families.

Abstract

Chronic renal failure (CRF) is one of chronic diseases that can decrease the quality of life and trigger anxiety that is formed by negative thinking, including thought of death. The excessive thought of death will cause stress. This situation will have an impact on care planning, preparation for facing death and difficulties in achieving a dignified quality of death. This study aimed to identify the level of death anxiety among CRF patients undergoing haemodialysis. This study used a quantitative descriptive method involving 73 patients recruited by using a consecutive sampling technique. Data were collected by using a Death Anxiety Scale (DAS) Templer instrument that was adapted for haemodialysis patient. Data were analyzed using descriptive statistics (mean, frequency, and percentage).The results of this study showed that nearly half of the patients had moderate death anxiety (n=35), 27.4% respondents (n=20) experienced low death anxiety, and 24.7% respondents (n=18) experienced high death anxiety. Based on the results, it is necessary to manage the death anxiety of CRF patients undergoing haemodialysis. Some potential approaches may include spiritual therapy, group counselling during haemodialysis, cognitive therapy (positive perception strengthening), and health education related to haemodialysis procedures and chronic renal failure.Keyword : Chronic renal failure, death anxiety, haemodialysis. Tingkat Kecemasan Pasien Gagal Ginjal Kronik yang Menjalani Hemodialisis dalam Menghadapi KematianAbstrakPenyakit gagal ginjal kronik dapat menurunkan kualitas hidup sehingga memicu kecemasan akibat dari pemikiran negatif termasuk pemikiran akan kematian. Pemikiran kematian yang berlebihan menimbulkan stres tersendiri sehingga berdampak pada perencanaan perawatan, persiapan menghadapi kematian dan sulitnya mencapai kualitas kematian yang bermartabat. Penelitian ini bertujuan untuk mengetahui tingkat kecemasan pasien gagal ginjal kronik yang menjalani hemodialisis dalam menghadapi kematian. Penelitian deskriptif kuantitatif ini melibatkan 73 pasien gagal ginjal kronik yang menjalani hemodialisis diambil dengan teknik consecutive sampling. Pengumpulan data menggunakan instrumen Death Anxiety Scale (DAS) Templer yang diadaptasi untuk pasien hemodialisis. Data dianalisis dengan statistik deskriptif (frekuensi dan persentasi). Hasil penelitian menunjukkan separuh dari jumlah pasien gagal ginjal kronik yang menjalani hemodialisis mengalami kecemasan kematian tingkat sedang (n=35), 27.4% responden (n=20) mengalami kecemasan rendah, dan 24.7% responden (n=18) mengalami kecemasan kematian tinggi. Berdasarkan hasil tersebut perlu dilakukan pengelolaan terhadap kecemasan menghadapi kematian pasien gagal ginjal kronik yang menjalani hemodialisis. Bentuk pengelolaan kecemasan menghadapi kematian diantaranya dengan terapi spiritualitas, konseling kelompok, terapi kognitif (penguatan persepsi positif), dan pemberian pendidikan kesehatan terkait dengan prosedur hemodialisis dan penyakit gagal ginjal kronik.Kata kunci : Gagal ginjal kronik, hemodialisis, kecemasan menghadapi kematian.

Abstract

Introduction, the success of treatment in chronic renal failure patients with hemodialysis required the existence of self-management and self-efficacy. Previous studies showed that self-efficacy had the influence to bring up self-management behavior. The purpose of this study was to determine patients’ self-management and self-efficacy in chronic renal failure patients who had hemodialysis at Advent’s Hospital Bandung. Methods, this study was a quantitative descriptive research.  The study samples were chosen using consecutive sampling technique. The number of samples was 75 patients. Data were collected using the SMBQ and PEPPI+Life Options DeNovo instruments. Data were analyzed using the quantitative descriptive approach with mean value and frequency distribution. The results of self-efficacy in patients with chronic renal failure with hemodialysis was   high (54.7%) and the self-management behavior was in a good category (53.3%). The conclusion of this study is that the patient has a good self-management as well as self-efficacy.  However, this study found the poor category of the partnership in caring aspect. Recommendation There is a need for a strategy to improve the self-management in the partnership in caring aspect by providing emotional support, educating patients related to treatment and care, and evaluating patients’ health development periodically.

Abstract

Emergency departments nurses are prone to burnout because they are dealing with emotional patients and families, and the number of visits is more than the number of nurses on duty. The purpose of this study was to identify the relationship between burnout levels and perceptions of caring for nurses in the emergency departments. The research design used a descriptive correlational. The research instrument used 2 questionnaires, namely the Maslach Burnout Inventory-Health Service Survey, to measure the level of burnout (low, medium, and high) and the Caring Behavior Inventory-24 to measure perceptions of caring (good and poor). The study population consisted of nurses in the Emergency Departments in Dr. Hasan Sadikin Bandung Hospital with a sample size using a total sampling technique of 50 nurses. Univariate analysis for each variable is presented with a frequency distribution table and bivariate analysis using the Rank Spearman test. The results showed that nurses’ burnout level was in a low category (60%), and the perception of nurses caring was in a good category (64%). The relationship test results obtained a negative correlation (p-value = 0.000; r = -0.607), which means that the higher the burnout level, the lower the caring perception that is owned. The study concludes that most nurses have a low level of burnout and a good perception of caring. Hospitals are advised to maintain or eliminate low burnout by carrying out a sharing program between nurses or counseling and maintaining good caring by conducting caring simulation training.

Abstract

Background: Mechanical ventilation is very important for the world of critical care. However, the use of mechanical ventilation also affected respiratory muscle weakness. Objective: The purpose of this study was to identify the implementation of Inspiratory muscle training for weaning mechanical ventilation in patients with mechanical ventilation in the Intensive Care Unit. Design: A systematic literature review study design was used. Data Sources: The databases used are CINAHL, PubMed, MEDLINE, Sage Journal, and ScienceDirect. Review Methods: Articles published from 2009-2020 with the type of randomized research-controlled trial (RCT) in English. The keywords "Inspiratory Muscle Exercise", "Inspiratory Muscle Training", "weaning", "mechanically ventilated", "critical care", and "intensive care". Results: From four articles analyzed, we found an effect of inspiratory muscle training on ventilator weaning, which was carried out on therapy five times a week, and some were carried out for seven days for two weeks and increased respiratory muscle strength. Conclusion: Inspiratory muscle training in mechanically ventilated patients is a nursing intervention that requires evidence-based preparation of guidelines to provide best practice. The optimalization of inspiratory muscles exercise can help the weaning process and be implemented further.

Abstract

The flash flood that occurred in the Garut Regency three years ago brought about devastating effects, one of which was the psychological impact in adolescents, namely PTSD. This study aims at detecting the presence of PTSD in adolescent victims of the flash flood that occurred in Garut, Indonesia three years ago. PTSD screening was conducted using the PTSD Checklist (PCL) questionnaire for 102 teenagers of the State Junior High-School (SMPN) 3 Tarogong Kidul who were victims of the flood and selected using a purposive sampling technique. This study used a quantitative descriptive method with cross-sectional approach. The results showed that three years after the Garut flash flood, a total of 80% of adolescent victims did not experience PTSD and 20 (20%) of the adolescents were identified having PTSD. The highest arising symptom responses were re-experiencing (90%) and hyperarousal (80%), which involve feeling intruded by memories of disasters and increased alertness. It is expected that nurses and experts can perform early and continuous screening for PTSD following a traumatic event, both in adolescents and other age groups to prevent another more severe impact.

Abstract

Pasien COVID-19 dengan gejala ringan hingga sedang menjalani isolasi mandiri sesuai dengan kebijakan terbaru yang dikeluarkan oleh pemerintah karena keterbatasan ruang perawatan. Pendidikan kesehatan tentang perawatan pasien COVID-19 selama menjalani isolasi mandiri (ISOMAN), menjadi sangat penting untuk mendukung keberhasilan ISOMAN pasien Covid 19. Tujuan dari kegiatan ini untuk meningkatkan pengetahuan tentang self care pada pasien COVID-19 selama menjalani isolasi mandiri dirumah. Sasaran kegiatan adalah seluruh masyarakat terutama pasien COVID-19 yang sedang menjalani isolasi mandiri dirumah. Jumlah partisipan yang terlibat sebanyak 74 peserta. Kegiatan pengabdian kepada masyarakat ini diawali dengan pretest, pemaparan materi, sesi diskusi dan tanya jawab dengan menggunakan aplikasi zoom, kemudian diakhiri dengan mengisi post test. Data univariat dan bivariat dianalisis dengan menggunakan uji t test berpasangan. Hasil kegiatan ini menunjukkan bahwa terjadi peningkatan pengetahuan rata-rata responden secara signifikan pretest 42.77 dan post test 83,24 dengan signifikansi 0,00. Hal ini menujukkan bahwa penggunaan metode daring dapat meningkatkan pengetahuan responden terkait topik pendidikan kesehatan yang telah diberikan. Kesimpulan pelaksanaan pendidikan kesehatan dengan medode daring dapat meningkatkan pengetahuan responden tentang self care pada pasien Covid-19 selama isolasi mandiri. Diharapkan pelaksanaan pendidikan kesehatan dengan metode ini dapat dilakukan untuk memberikan informasi bagi pasien COVID-19 yang sedang menjalani isolasi mandiri. Kata kunci: Covid-19, isolasi mandiri, pendidikan kesehatan, tingkat pengetahuan.

Abstract

Pendahuluan : Identifikasi faktor-faktor yang berhubungan dengan kejadian hipoglikemia penting dilakukan untuk mengetahui faktor resiko kejadian hipoglikemia, sehingga dapat digunakan sebagai dasar acuan pencegahan hipoglikemia berat dan hipoglikemia berulang. Objektif : Bertujuan untuk melakukan Literature review mengenai faktor-faktor yang berhubungan dengan kejadian hipoglikemia pada pasien diabetes mellitus. Metode : Metode penelusuran sumber data pada Literature review ini yaitu melakukan penulusuran database terdiri dari Google Scholar, Pubmed, Ebsco dengan kata kunci yang digunakan untuk pencarian pada databased Internasioanal yang pertama yaitu “Diabetes mellitus” AND “Risk factor hypoglycemia” “OR” Predictor hypoglycemia” Sedangkan pada databased Nasional menggunakan kata kunci yang pertama yaitu “Diabetes Mellitus “ dan” Faktor resiko hipoglikemia.”. Artikel diseleksi berdasarkan kriteria inklusi dan melakukan critical apraisel pada setiap artikel. Hasil : Berdasarkan hasil pencarian ditetapkan 8 artikel dengan design Kuantitatif publikasi 10 tahun terakhir (2009-2019). Faktor resiko yang berkaitan dengan kejadian hipoglikemia pada pasien diabetes mellitus yaitu terapi insulin, sulfolinurea, dan terapi kombinasi merupakan prediktor kuat terjadinya hipoglikemia, namun ada beberapa faktor lain yang berkaitan dengan kejadian hipoglikemia diantaranya nilai HbA1c, BMI, usia, jenis kealmin, pekerjaan, tingkat pendidikan, lama menderita diabetes mellitus, penyakit penyerta (penyakit ginjal kronis), dukungan keluarga, dan gaya hidup (konsumsi alcohol). Kesimpulan: Faktor resiko kejadian hipoglikemia pada pasien diabetes mellitus berkaitan dengan faktor medikasi (pemberian insulin, sulfolinurea, terapi kombinasi) dan faktor demografi pasien, oleh karena itu pengkajian resiko hipoglikemia pada pasien diabetes mellitus penting untuk diketahui, sehingga dapat berpotensi mencegah kejadian atau menurunkan kejadian hipoglikemi dan dapat meningkatkan kualitas hidup pasien diabetes mellitus
