Abstract

Family members who take the patient to ED have high expectation but sometimes what happens if lose their beloved one. ED busy situations, small room with many patients, and lack of time to communicate with the medical team during the EOL process. When patients are in the EOL phase, the necessary treatment focuses at improving patients' life quality in order to welcome the death as they wish, die with dignity. This research applies qualitative design with a phenomenology approach. It involves participants who are 7 of nuclear family members who accompany patients during the EOL process in ED. Data were collected using an in-depth interview, analyzed using Colaizzy method. Five themes are obtained from this research namely dilemma in making a decision to stop life support, fear of being left by beloved one, guiding beloved one to pray in their final seconds, Life and Death in Allah’s Control, and dissatisfaction towards the service. This research provides a description of how the family feels after accompanying the patient during the EOL process in ED through the themes and sub-themes in this research. There is a new phenomenon emerging in this research which is not found in other research. It is the feeling of being neglected and discriminated when it comes to medical cost and BPJS insurance, and feeling of gratitude and pride of the family members who can accompany the patient till this patient die with khusnul khotimah or peacefull.

Abstract

Kesiapsiagaan terhadap bencana merupakan aspek yang sangat penting penentu keberhasilan dalam penanggulangan bencana. Untuk mengurangi dampak bencana di seluruh fase yang berbeda, banyak tindakan keperawatan yang diperlukan, termasuk pencegahan atau mitigasi, kesiapsiagaan, respon, pemulihan, dan rekonstruksi atau rehabilitasi. Akan tetapi informasi tentang gambaran kesiapsiagaan perawat terhadap bencana secara sistematis masih terbatas. Sehingga tinjauan ini bertujuan untuk mengidentifikasi hasil penelitian yang ada tentang kesiapan perawat di rumah sakit dalam menghadapi bencana secara lebih komprehensif. Metode: Pencarian literature dilakukan terhadap artikel yang diterbitkan dari 2014 sampai 2019 menggunakan PubMed dan Google Scholar dengan kombinasi kata kunci readiness atau preparedness dan disaster dan healthcare professional atau nursing. Kriteria inklusi pencarian adalah studi yang berfokus pada analisis kesiapsiagaan pada perawat, jenis studi cross sectional, diterbitkan dalam bahasa Inggris dan bahasa Indonesia. Semua makalah yang terpilih dimasukkan berdasarkan penilaian independen berdasarkan kriteria dari JBI. Hasil: Pencarian awal menghasilkan 1.143 artikel dan hanya 9 artikel yang termasuk dalam tinjauan artikel, dari 9 artikel menunjukan  mayoritas perawat rumah sakit menunjukkan kesiapan yang buruk terhadap respon bencana rentang dari 45,8% sampai 78,5%. Mereka juga tidak percaya diri dengan kemampuannya dalam menghadapi bencana yang besar (52,1% - 72,4%). Akan tetapi, sebanyak (48,3% - 82,3%) perawat menunjukkan kesiapan yang baik terhadap respon bencana dalam hal manajemen klinis dan kesiapan dalam perlindungan diri. Kesimpulan : Hasil review menunjukan bahwa sebagian besar perawat memiliki kesiapan yang kurang optimal dalam menangani bencana. Pembuat kebijakan harus mendorong diadakanya pelatihan untuk perawat mengenai kesiapsiagaan bencana di rumah sakit.

Abstract

Aims: End-of-life care in ICU involves a wide range of staff from every discipline including nurses, who have a lot of interaction time with patients. Most of the patients in the intensive care room have unconscious characteristics and family acts as the holder of autonomy in decision making, so nurses must have competence and experience in caring for patients in the end-of-life phase. However, nurses feel many barriers and challenges in facing the end-of- life in the intensive care environment. This study aimed to identify barriers and challenges of end-of-life care implementation in the ICU Designs: This study used literature review that analyzes articles from five reputable databases such as Pubmed, Proquest, CINAHL, Science Direct, and Google Scholar with the publication year between 2010 - 2021. The keywords used are 'critical care', 'critical care nursing', 'intensive care unit', 'end-of-life care', 'barriers', and 'challenges' with the help of bolean operators 'OR' and 'AND', articles were selected in stages using PRISMA. From the results of 2,186 articles, only 11 articles were identified that met the inclusion and exclusion criteria. Results: The results of the review showed that some barriers of end-of-life care implementation in the ICU are lack of communication, lack of knowledge and training about end of life care, differences in religious and cultural backgrounds, medical team work in the ICU, patient and family factors, and environmental factors. Meanwhile, the challenges of end-of-life care in ICU are communication and decision making in end-of-life care and unrealistic family expectations. Conclusions: There are still many barriers and challenges identified on implementation of end-of-life care in intensive care units, mostly due to lack of knowledge and communication during end-of-life care.  As a recommendation in improving end-of-life care services in the ICU, health service agencies need to increase knowledge and training of health workers about communication and end-of-life care in critical patients.

Abstract

Spiritual care is a part of end of life care, which role is to provide a sense of comfort and peace to the patient and ease the grieving process for the family. Nevertheless, COVID-19 pandemic caused many challenges experienced by ICU nurses in the implementation of spiritual care. This study aimed to identify empirical evidences of ICU nurses experience in providing spiritual care to end-of-life care patients during COVID-19 pandemic. It used a literature review method, analyzing articles from 5 reputable databases, namely Pubmed, Proquest, CINAHL, Science Direct, and Google Scholar with the range of publication years between 2020 and 2022. The keywords used were 'pandemic COVID-19', 'critical care nursing’, 'end of life care', 'dying', 'spiritual care', and 'nursing experience'. The articles were selected in stages by PRISMA flow. 2,727 articles were identified and selected based on the duplication and the inclusion criteria until 6 analysis-deserve articles were obtained. The analysis results showed that during COVID-19 pandemic spiritual care was an important domain in end of life care. Spiritual care for end-of-life patients can reduce anxiety and ease the grieving process for the family. The implementation of spiritual care during pandemic era experienced many challenges; thus, an innovative strategy was required to implement it. Health care facilities should provide supports for nurses to carry out spiritual care by increasing their competences through education and training.

Abstract

ABSTRAK Salah satu dampak negatif penggunaan internet, khususnya pada kalangan remaja adalah kekerasan melalui media sosial yang umumnya dikenal dengan cyberbullying. Cyberbullying bisa menjadi pemicu stressor yang mengancam untuk remaja karena dapat menimbulkan gangguan psikologis. Salah satu dampak yang terjadi pada korban cyberbullying adalah kecemasan. Penelitian ini bertujuan untuk mengetahui gambaran tingkat kecemasan siswa SMA pada korban cyberbullying. Jenis penelitian ini yaitu deskriptif kuantitatif yang menggunakan teknik total sampling dengan jumlah sampel yaitu 123 siswa SMA Negeri 27 Kota Bandung. Alat ukur yang digunakan menggunakan ZSAR – S (Zung Self Anxiety Rating – Scale) yang sudah diuji validitaskan dengan Rhasil 0,81 dan dinyatakan valid. Hasil data penelitian ini didapatkan kemudian dianalisis menggunakan analisa deksriptif dengan bentuk persentase.Berdasarkan hasil penelitian didapatkan sebagian besar responden mengalami tingkat kecemasan ringan (82,9%), sedang (16,3%) dan berat (0,8%). Maka kesimpulan dari penelitian ini didapatkan bahwa kecemasan korban cyberbullying dari 123 siswa sebagian besar mengalami kecemasan ringan. Meskipun hasil yang didapat yaitu kecemasan ringan tetapi jika dibiarkan saja akan bisa menjadi kecemasan sedang, berat bahkan panik. Sehingga penelitian ini memiliki rekomendasi untuk siswa yang mengalami kecemasan, siswa tersebut harus mendapatkan perhatian khusus dari lingkungan sekolahnya, dan sekolah memberikan fasilitasi untuk konsultasi yang membuat siswa tersebut terbuka tentang masalah cyberbullying yang dialami dan memberikan pendidikan dan pengetahuan tentang pencegahan tindakan cyberbullying dimana tindakan tersebut merupakan tindakan kriminalitas yang melanggar UU ITE sehingga siswa yang menggunakan media sosial akan lebih berhati – hati dan menggunakannya dengan baik sesuai kebutuhan. Kata kunci : Cyberbullying, Kecemasan, Remaja ABSTRACK One of the negative impacts of internet use, especially among teenagers, is violence through social media, commonly known as cyberbullying. Cyberbullying can be a trigger for a threatening stressor for teens because it can cause psychological disorders. One of the impacts that occur on victims of cyberbullying is anxiety. This study aimed to describe the anxiety level of high school students in victims of cyberbullying. The type of this research was quantitative descriptive which used total sampling technique with a sample size of 123 students of SMA Negeri 27 Bandung. The measuring instrument used was ZSAR-S (Zung Self Anxiety Rating - Scale) which had been tested for validity with Rhasil 0.81 and declared valid. The results of this research data were then analyzed using descriptive analysis in the form of percentages.Based on the results of the study, the majority of respondents experienced mild anxiety levels (82.9%), moderate (16.3%) and severe (0.8%). Then the conclusion of this study found that the anxiety of victims of cyberbullying from 123 students mostly experienced mild anxiety. Although the results obtained were mild anxiety, it could be potentially be a moderate anxiety, even panic. So that this study has recommendations for students who experience anxiet to get special attention from their school environment, and schools provide facilitation for consultations that make these students open about the problem of cyberbullying experienced and provide education and knowledge about prevention of cyberbullying actions where the action is criminal acts that violate the ITE Law so students who use social media will be more careful and use it properly as needed. Abstract

Dampak yang disebabkan oleh tindakan hemodialisis akan terasa oleh keluarga pasien sehingga perlu adanya strategi koping dari keluarga pasien tersebut agar perawatan kepada pasien dapat optimal. Tujuan dari penelitian ini adalah untuk mengetahui gambaran strategi koping keluarga pasien gagal ginjal kronis yang menjalani hemodialisis di RSUD dr.Slamet Garut. Penelitian ini merupakan penelitian deskriptif kuantitatif dengan teknik total sampling pada 111 responden menggunakan alat ukur dari Lazarus dan Folkman yaitu Ways of Coping Questionnaire. Penelitian ini dilakukan pada bulan April-Mei 2019. Hasil penelitian menunjukkan hampir setengah dari responden (27,9%) cenderung menggunakan strategi koping distancing (membuat jarak) dan sebagian besar dari responden (64%) cenderung menggunakan emotion focus coping pada anggota keluarga yang menjalani hemodialisis >6 bulan. Kedua strategi koping tersebut kurang baik digunakan karena masalah tidak akan langsung tertangani. Saran untuk RSUD dr.Slamet Garut diharapkan memberikan pendidikan kesehatan atau konsultasi terkait penggunaan strategi koping efektif kepada keluarga pasien yang menggunakan strategi koping distancing serta keluarga pasien dengan hemodialisis >6 bulan yang masih menggunakan koping emotion focused coping.

Abstract

Latar Belakang: Dr. Hans Selye membagi respon terhadap stress dalam tiga tingkatan yaitu alarm (alarm), perlawanan (resitance) dan peredaan (exhaustation), Ada beberapa penyakit yang dimulai dari stress, seperti hipertensi. Hipertensi di desa Ciganjeng dan Karangsari, angka kejadian hipertensinya cukup tinggi, untuk dikarangsari ada sekitar 43orang, di ciganjeng ada sekitar 54orang, data didapat dari bidan desa ciganjeng dan karangsari. Menurut petugas kesehatan di kedua desa tersebut baru ada penanganan dalam bentuk pemberian obat hipertensi saja, itupun tidak semua pasien berobat rutin. kami menganggap perlu melakukan pemberdayaan masyarakat tentang cara perawatan hipertensi dengan manajemen stress di desa ciganjeng dan karangsari kecamatan Padaherang kabupaten Pangandaran. Metode: Metode penelitian yang digunakan adalah deskriptif kuantitatif serta analisa data dilakukan dengan mencari nilai rata-rata pada data pre-test dan post-test. Tehnik sampel yang digunakan adalah total sampling, yaitu sampel yang terkumpul adalah 59 untuk desa Ciganjeng dan 51 untuk Desa Karangsari. Kuesioner yang digunakan adalah 10 item pertanyaan pilihan tunggal. Intervensi yang dilakukan untuk pemberdayaan masyarakat desa Karang sari dan Desa Ciganjeng Kecamatan Padaherang adalah dengan cara melakukan penyuluhan tentang managemen stress dan juga hipertensi, karena sesuai dengan teori yang akan diambil adalah theory self care, yaitu dengan mamandirikan masyarakat dan memberdayakan masyarakat untuk meningkatkan derajat kesehatannya. terjadi peningkatan pengetahuan masyarakat desa Ciganjeng tentang masalah psikososial dan hipertensi. Hasil: Perubahan penilaian pengetahuan pre dan post lebih dari 100% peningkatannya, hal ini membuktikan bahwa masyarakat desa Ciganjeng memiliki kemampuan untuk dibina dalam membuat desa siaga, sehingga masyarakat desa Ciganjeng akan menjadi masyarakat atau desa siaga

Abstract

The Emergency Department (ED) is a hospital service unit that provides the first service for patients with disease conditions that threaten their lives or can cause disability for 24 hours. Implementation of patient safety in the ED should be applied to minimize the risk of error handling for the patient. ED staff perceptions related to the implementation of patient safety is a factor that directly-related to his behavior in applying the implementation of patient safety. This study aimed to analyze the relationship between perceptions of staff ED and patient safety by implementing patient safety at the Regional Hospital Emergency Department Cirebon. This study was a correlational study with the cross-sectional approach of 99 emergency staff with total sampling at Cirebon. Collecting data used questionnaires of patient safety. Based on the results of the univariate analysis showed that the majority (80%) of respondents either category on the implementation of the sub-variables of patient safety team collaboration and communication, only a small proportion of respondents less category (20%) on the implementation of the sub-variables of patient safety team collaboration and communication. In addition, less than half (49.5%) category lacking in implementing patient safety, only half (50.5%) categories, both in the implementation of patient safety. Based on the results of the bivariate analysis showed that the relationship implementation of patient safety with all the variables, namely teamwork (p-value = 0.000), communications (p-value = 0.005), the concept of patient safety (p-value = 0.005), and perception (p-value = 0.005). Based on the results of the study, the researchers concluded that the relationship between staff perceptions of the emergency department (ED) on patient safety by implementing patient safety at the Regional Hospital emergency department (RSD) Cirebon. IGD support staff perceptions of patient safety, but still found lacking in the category of health workers implementation of patient safety, so the need for patient safety education and training with simulation methods to illustrate the approach in the implementation of patient safety.

Abstract

Communication among nurses, patients, and families takes an important role in the intensive care unit in which the patients are in critical condition and unable to involve in two-way communication. Research related to effective nurse-patient communication has been done extensively, but the information regarding communication in intensive care unit is still limited. This research aimed to explore nurses’ experiences in the intensive care units in effective communication to patients/patient’s families. This was a qualitative study project with phenomenology approach. The data were collected using the in-depth interview technique approximately 60 minutes involving ten nurses who were selected using the purposive sampling at Al Islam Hospital Bandung. Data were analysed using the Colaizzi method and the results were presented in themes. Based on the nurses’ experiences, four themes were emerged in this study including (1) Nurses’ dilemma of their professionalism and personal issues/matters, (2) Contextual factor affects selection of nurses’ communication technique, (3) Barriers in effective communication; difficulties in accompanying families to accept critical patient conditions, care and treatment procedures in the ICU which were complicated, and misunderstanding between nurse-patient and family (4) Compassion and patience are required in nurse-patient communication in ICU. The complex patient/family conditions in the ICU require nurses to choose the appropriate communication technique accompanied by a sense of compassion and patience. Nurses need to improve their ability to communicate effectively in order to lower the barriers in communicating between nurses-patients/families. Recommendations, training and assistance of effective communication become important for nurses in improving services in the Intensive Care Unit.

Abstract

Aims: Natural disasters can have an impact on the material (physical), psychological and social aspects. The traumatic experience is a psychological impact on victims that are often found. traumatic experience or traumatic stress if it lasts for 6 months will result in PTSD (Post Traumatic Stress Disorder). There are various interventions to treat PTSD, this intervention is known as trauma healing. Trauma healing can be one of the appropriate rehabilitation tools for disaster victims, especially to heal post-disaster trauma. The purpose of the study is to find out what trauma healing interventions are in handling PTSD in post-disaster victims. Method: The method used is a narrative review with PRISMA 2009 guidelines. The search is based on Garuda, EBSCO, Proquest, and PubMed. The studies reviewed were RCT research, quasi-experiment, and qualitative research in English and Indonesian with full text, as well as intervention studies within the last 7 years of publication from 2015  to 2021. Result: Trauma healing that can be given and proven effective to treat or reduce PTSD symptoms is CBT (Cognitive Behavioral Therapy), remembrance therapy, dhikr and play therapy. The results of the search used turned out to be for CBT therapy and play therapy were more effective in reducing PTSD in children, while dhikr was effective in adults, and remembrance therapy was very suitable for adults who experienced PTSD with other physical problems such as hypertension. Conclusions: Therapy as post-disaster trauma healing to overcome PTSD is divided into two, namely therapy for children and adults. Therapy for children is playing therapy and CBT, while for adults there is dhikr, and remembrance therapy is used for adults who experience physical disorders such as hypertension.
