Abstract

Many techniques can be used to improve drug solubility, which is the development of the liquisolid technique. This technique has a mechanism for increasing the surface area of the drug as well as wetting from the addition of non-volatile solvents resulting in a lower surface tension and contact angle, so the solubility and drug release very increases. Liquisolid tablets show a lower contact angle compared to the conventional tablets. The liquisolid technique approach is also promising because the process is simple in making low production costs and allows the manufacturing industry, including non-volatile solvents, fillers, dryers, and disintegrants. Liquisolid characterized by specific instruments such as powder x-ray diffraction (PXRD), Fourier transforms infrared spectroscopy (FTIR), differential scanning calorimetry (DSC), and scanning electron microscope (SEM). Several liquisolid techniques are described in this review. The liquisolid technique is proven and able to change the physicochemical properties of active pharmaceutical ingredients, especially the solubility, drug release, and stability of the formula so that this technique can be a solution for class II and IV BCS pharmaceutical active drug classes.Keywords: Active Pharmaceutical Ingredients, Contact Angle, Solubility, Drug Release, Stability, Liquisolid Technique

Abstract

The halal food market has grown worldwide, including the shift from food to other products, including halal pharmaceuticals (HPC).  The growth followed by the abundance of literature has been on halal, especially pharmaceuticals. Muslim consumers need halal certification (HCT) on medicines to ensure that they do not consume or use products or services that are not halal. The government must guarantee to Muslim consumers that consumer goods or services circulating in the community are truly halal. The halal label itself will increase consumer confidence, expand the reach of the global halal food market, and increase the marketability of products in the market. Indonesia, which is predominantly Muslim, is considered a long extension in implementing HP until 2034. Different attitudes are shown by several countries with large non-Muslim majority populations but are very concerned about the implementation of HPC. The accelerated growth of knowledge of halalness, impact social media, will push the industry to implement HPC. In this review, we will discuss critical players in implementing HPC, including the ingredients that have been widely used but still doubt their halalness and what they are used for in the pharmaceutical industry. Abstract

Lisinopril is an oral angiotensin converting enzyme (ACE) long acting, based on the characteristics of biopharmaceutics, lisinopril has a high solubility, but low permeability, this causes limitations in bioavaibility. The composition of lisinopril tablets wre calcium hydrogen phosphat anhydrous and mannitol as a filler, poly vinyl pyrrolidone K 30 as binder, cros carmellose sodium as a desintegrator, edicol rasbery red as a collor, Vitamin E TPGS NF (d-tocopheryl polyethylene glycol 1000 succinate) as enhancers of absorption / permeability with various concentrations, silicon dioxide colloidal, talc and magnesium stearate as glidant and antiadherent, aquadest and alcohol 95% as wetting agent. The study used wet granulation method. Test was done for the granule and tablet tested, including the comparative dissolution test with the innovator product. The result showed that the granule and the tablet were met the requirements. Vitamin E TPGS 1000 NF wasn’t affect to the comparative dissolution test result between the tablet product and the innovator product.

Abstract

Head lice (Pediculus humanus capitis), a parasite causing hair problem,  often occurs in society.The study aimed to evaluate in vitroeffectiveness of shampoo formulation containing neem oil as an anti head lice. An experimental method was used to examine the formulation of shampoo, irritation test, testing the effectiveness of shampoo against head lice (Pediculicidal activity), and testing the effectiveness of shampoo against nits (Ovicidal activity). The effectiveness test employed an in vitromethod where neem oil shampoo were tested directly towards head lices and nits isolated from humans, then the mortality was observed. To recognize the safety of the shampoo, skin and eye irritation tests were carried out by applying 0.5 ml on the skin and dripping 0.1 ml shampoo into New Zealand white rabbit’s eyes.Data were analyzed using Kruskal Wallis and Mann Whitney. The study results showed that the primary irritation index for 5%, 10%, 15%, and 20% neem oil shampoo were 0; 0111; 0.222; and 0.222, respectively. All of the shampoos were very mild irritation response category. Eye irritation tests showed that only the 20% neem oil shampoo gave a mild irritation response with degree of corneal opacity ≥ 1 and recovered after 7 days of observation. The 5%, 10%, 15%, and 20% neem oil shampoo were effective as an anti head lice. The 15% and 20% neem oil shampoo had anti head lice effectiveness activity as same as 1% permethrin lotion. In conclusion, the 15% neem oil shampoo was chosen as the best shampoo, its high anti head lice effect and light irritation response. Keywords: Head lice, neem oil (Azadirachta indica A. Abstract

To recover good appearance of skin, dead skin cells should be removed, among others by using exfoliating agent, and to preserve the fresh skin ethyl vitamin C could be used as an antioxidant. Formulation and evaluation of skin exfoliating gel containing ethyl vitamin C in Macrobead® delivery system, using acrylates copolymer and carboxy vinyl polymer as gel bases have been carried out. The gel evaluation included physical evaluation and safety examination. The results showed that ethyl vitamin C could be trapped into Macrobead® system with good absorption inthe ratio of active content and Macrobead® of 3:4. Based on the physical evaluation of the gel formula, it was found that formula with acrylates copolymer base has altered the consistency and homogenity of gels, but not the formula with carboxy vinyl polymer base. Gels using carboxy vinyl polymer showed good physical stability for28 days of storage. The safety evaluation indicated that the preparation was safe to be used for it did not cause irritation on consumers skin. The examination showed that the preparation could exfoliate dead skin cells and thus freshen the users’ skin effectively.

Abstract

Penyakit covid-19 yang disebabkan oleh virus sarcov-2 awalnya hanya dinyatakan sebagai penyakit pernafasan, namun kemudian diketahui dampak dari penyakit ini dapat menyebabkan kerusakan multiorgan karena adanya fenomeda badai sitokin yang menyebabkan inflamasi dimana-mana termasuk pada pembuluh darah yang menuju berbagai organ penting. Tulisan ini bertujuan untuk mengungkap seberapa signifikan fenomeda koagulasi darah hasil kerja virus covid-19 ini hingga menyebabkan manifestasi tromboembolisme baik dalam terminologi tromboemboli vena, deep vein thrombosis, tromboemboli paru dan lain-lain, juga kemungkinan mekanisme yang terjadi dan yang terpenting dalam tulisan ini adalah seberapa efektifkah penggunaan obat antikoagulan pada pasien covid-19 bergejala berat.

Abstract

Ko-amorf adalah suatu sistem multikomponen padat yang mengandung zat aktif dan molekul dengan berat molekul rendah lainnya (koformer) yang dapat berupa eksipien atau zat aktif yang relevan secara farmakologis. Formulasi ko-amorf yang dibuat dengan metode preparasi dan jenis koformer yang berbeda dapat menghasilkan perbedaan yang signifikan dalam stabilitas fisik dan profil disolusi suatu bentuk ko-amorf. Tujuan penulisan dari artikel review ini adalah untuk menggali informasi lebih dalam tentang sistem ko-amorf, klasifikasi ko-amorf, karakterisasi ko-amorf serta pengaruh jenis koformer dan metode preparasi ko-amorf terhadap pembentukan ko-amorf. Artikel review ini disusun dengan literature search melalui PubMed, MDPI dan Science Direct dengan memasukkan kata kunci co-amorphous, co-amorphous formulations, co-amorphous stabilizers, co-amorphous drug formulations. Dari review ini ditemukan terdapat beberapa jenis koformer yang dapat digunakan untuk pembentukan ko-amorf yaitu dapat berupa zat aktif yang relevan secara farmakologis dan eksipien seperti diantaranya yaitu asam amino, asam karboksilat, asam tanat, quercetin, sakarin dan nikotinamid. Dan untuk metode preparasi ko-amorf yang dapat digunakan diantaranya yaitu ball milling, cryomilling, melt quenching/ quench cooling, hot melt extrusion, solvent evaporation, spray drying, freeze drying hingga teknologi seperti supercritical antisolvent dan microwave technique. Keberhasilan pembentukan ko-amorf ditentukan diantaranya oleh pemilihan jenis koformer yang melibatkan berbagai sifat yang perlu dipertimbangkan, seperti Tg, potensial ikatan hidrogen, ketercampuran/ miscibility, atau perilaku kristalisasi. Sifat zat aktif dan eksipien seperti stabilitas termal, suhu leleh dan kecenderungan kristalisasi zat aktif dan eksipien, menjadi faktor yang perlu diperhatikan dalam pemilihan metode preparasi ko-amorf.

Abstract

Seeing the development of the COVID-19 and the increasing number of new cases, WHO has published several updates, guidelines, and health protocols regarding this pandemic. The government of Indonesia has issued an appeal to the public to follow this health protocol. Various educational posters and videos have been widely published on social media, television stations and other communication media. One of the programs that the government always socializes during this pandemic is the implementation of Perilaku Hidup Bersih dan Sehat (PHBS) or Clean and Healthy Behavior. The purpose of this Virtual Integrative Community Service program is to find out the level of public knowledge about the importance of PHBS and traveling during a pandemic, especially in the AKB era. The methods used in this activity are surveys on community PHBS, surveys regarding travel activities during the pandemic, making PHBS videos and posters, and webinars on the use of herbs as an antistress and immunity enhancer. The partner community in this online KKN PPM activity is the Langensari village community, Solokanjeruk sub-district in particular and the general public who can access social media. The results of the traveling survey showed that out of 441 respondents, around 332 people did not have plans to travel. Of the 332 respondents who chose not to travel, around 229 respondents argued that they did not have a crucial interest, 142 respondents were worried about contracting COVID-19, 50 respondents said they did not have accommodation, and 102 respondents chose to continue traveling / traveling. The results of the PHBS survey are that most respondents have implemented PHBS with the most results being 99.3%, namely the choice of washing hands and using masks. Respondents who answered no to the third question gave answers to the causes of this in the form of lack of access to information (3.4%), laziness (6.9%), and lack of care (4.1%). The group activities of the 25 KKN-PPM Virtual Integrative have been carried out well and can increase community knowledge of PHBS and health protocols when traveling during a pandemic and increase knowledge about the use and manufacture of herbs to increase immunity.

Abstract

The use of warfarin was monitored using the International Normalized Ratio (INR) value. This study aims to determine the relationship between the patient’s demographic variables and INR, as well as the therapy result and differences in their INR value. A cross-sectional study was used with retrospective observational data collected by tracing secondary information of medical records from outpatients with heart disease on warfarin therapy at Dr. Hasan Sadikin hospital Bandung between 2016 to 2020. Subsequently, the difference in patients’ INR value was analyzed with the Mann-Whitney test based on dosage. At the same time, the relationship between demographic variables and INR was examined using the Chi-square test and Spearman correlation. The INR examination results in 192 subjects showed 124 patients (64.6%) reached the target with an average value of 2.37 ± 0.21 and a warfarin dose of 19.31 ± 6.25 mg weekly, while 68 (35.4%) had an average of 1.82 ± 0.73 and a dose of 20.24 ± 6.24 mg weekly. Subsequently, the difference test with Mann Whitney (p = 0.004) showed a variation in the INR value based on the warfarin dose. The Spearman correlation analysis results indicated a relationship between BMI (p = 0.009), daily (0.010), and weekly dose (0.008) on the INR value with the correlation coefficient of 0.188; 0.186; and 0.192;  indicating a negative association with very weak correlation strength. This study showed that a majority of patients reached the INR value of 2-3, and the demographic variables associated with this ratio were BMI and warfarin dose. The decision to increase and decrease the warfarin dose is determined by INR value and BMI.

Abstract

Meningkatnya minat masyarakat terhadap penggunaan bahan alam dari tanaman terjadi karena ketersediaan bahan yang mudah didapat. Cara tersebut terus dilakukan oleh masyarakat, serta diwariskan secara turuntemurun mengingat penggunaannya dianggap relatif aman serta efek samping yang minimal. Bagian akar dari pakis tangkur digunakan oleh masyarakat setempat untuk mengobati berbagai jenis penyakit, antara lain rematik, hipertensi dan sebagai afrodisiaka. Menurut United States of Food and Drug Administration (FDA) meskipun bahan alam dianggap relatif aman oleh sebagian kelompok penggunanya, tetap harus dilakukan skrining toksikologi terhadap senyawa yang berpotensi obat untuk melihat serta menggambarkan efek toksik sehingga diperoleh nilai Dosis letal (LD50). Dari hasil penelitian toksisitas akut ini nilai LD50 yang diperoleh yaitu lebih dari 5000 mg/kgBB. Dikarenakan pada dosis paling tinggi pada penelitian yaitu 5000 mg/kgBB tidak terdapat hewan yang mati, sehingga dapat disimpulkan bahwa ekstrak akar pakis tangkur termasuk kedalam kategori praktis tidak toksik.
