Abstract

Pendahuluan: Subcorneal pustular dermatosis (SPD), atau penyakit Sneddon–Wilkinson, merupakan dermatosis neutrofilik kronis yang jarang, ditandai dengan pustul superfisial berulang. SPD semakin banyak dilaporkan berhubungan dengan penyakit sistemik, terutama gangguan metabolik seperti diabetes melitus tipe 2 dan dislipidemia, yang dapat berkontribusi terhadap onset dan kekambuhan penyakit. Laporan kasus: Seorang perempuan berusia 36 tahun yang datang dengan keluhan bercak kemerahan berulang disertai pustul superfisial flaksid berwarna kekuningan yang gatal pada daerah perut dan punggung bawah selama dua bulan terakhir. Lesi awalnya membaik dengan kortikosteroid topikal tetapi kambuh kembali dua minggu kemudian dengan cakupan yang lebih luas. Pasien tidak mengeluhkan demam maupun riwayat konsumsi obat sebelumnya. Pasien memiliki penyakit dasar diabetes melitus tipe 2 dan dislipidemia, keduanya dalam kondisi tidak terkontrol dengan baik. Pemeriksaan klinis menunjukkan plak eritematosa multipel dengan pustul superfisial, krusta, erosi, serta makula hiperpigmentasi pada perut dan punggung bawah. Pemeriksaan dermoskopi menunjukkan multiple yellow-white globule di atas dasar eritematosa yang dikelilingi sisik halus. Evaluasi psikologis menunjukkan tingkat kecemasan sedang dan dampak sedang terhadap kualitas hidup.  Kesimpulan: Kasus ini menekankan pentingnya mengevaluasi dan menangani komorbiditas sistemik, karena disfungsi metabolik dapat berkontribusi terhadap inflamasi persisten, kekambuhan berulang, dan respons terapi yang kurang optimal pada Subcorneal Pustular Dermatosis.

Abstract

Background: Onychomycosis and Nail Lichen Planus (NLP) are two conditions that cause nail damage and may sometimes present with overlapping clinical features, leading to difficulties in achieving an accurate diagnosis. Several ancillary examinations should be considered to avoid misdiagnosis of nail disorders. Case: A 64-year-old female patient presented to the Dermatology, Venereology, and Aesthetic Clinic at Dr. M. Djamil General Hospital Padang with complaints of progressive nail thickening and whitening affecting almost all nails, occasionally accompanied by itching, for the past 6 months. The patient had previously been diagnosed with onychomycosis and had regularly taken fluconazole 150 mg weekly for 6 months, but without improvement. Physical examination was within normal limits. Dermatological and onychoscopic examination revealed subungual hyperkeratosis, longitudinal ridging, onychotrophy, onychoschizia, trachyonychia, onycholysis, yellowish discoloration, melanonychia, and leukonychia, with an OSI score indicating severe involvement and a NALSI score of 100/120. KOH examination demonstrated the presence of hyphae, fungal culture revealed Aspergillus niger, PAS staining confirmed fungal elements, while biopsy results were consistent with NLP. The patient was diagnosed with onychomycosis with concomitant NLP and was treated with pulse itraconazole at a dose of 400 mg/day for one week each month for 2–3 months. Upon completion, treatment for NLP was initiated. Discussion: In this case, the absence of comprehensive ancillary examinations particularly biopsy, initially led to a diagnosis limited only to onychomycosis, without recognition of the coexisting NLP. This highlights the importance of thorough investigations to ensure an accurate and precise diagnosis.

Abstract

Nagari Salimpek in Alahan Panjang, West Sumatra, has a potential for wellness tourism due to its rich biodiversity, especially medicinal and cosmetic plants. However, community awareness in managing Family Medicinal Gardens (TOGA) remain limited, with only three of twenty household groups actively cultivating herbal plants. This condition emphasizes the need for community empowerment to optimize health and economic benefits from local resources. This community service program aimed to enhance local capacity in cultivating and processing TOGA as the basis for developing a Herbal Cosmetic Village. The programme began on 19 July 2025 and was evaluated over six months. Activities included educational sessions on TOGA utilization, training on herbal and cosmetic plant processing, distribution and planting of herbal seeds, and the signing of a Memorandum of Agreement (MoA) with local stakeholders. A pre-test and post-test evaluation involving 31 participants showed significant improvement in knowledge, with understanding of the “herbal center village” concept rising from 29% to 55% and awareness of Nagari Salimpek’s potential increasing from 32% to 77%. Overall, participants gained broader insights into the role of TOGA in health, cosmetics, and economic development. These results highlight the importance of continued education, mentorship, and government support to establish Nagari Salimpek as a model Herbal Cosmetic Village for wellness tourism.

Abstract

Background: Leprosy, caused by Mycobacterium leprae, persists as a global health issue where the primary challenges are not merely microbial but are deeply rooted in delayed diagnosis and poor treatment adherence. These delays, often driven by profound social stigma, lead to progressive, irreversible disability and sustain community transmission. Erythema Nodosum Leprosum (ENL), an acute immunological complication, further devastates patients' quality of life and complicates management. Case presentation: A 53-year-old Indonesian farmer presented with a 15-year history of untreated leprosy, a journey of neglect initiated by fear of treatment side effects and community ostracism. Clinical examination revealed advanced borderline lepromatous (BL) leprosy with diffuse skin infiltration, multiple anesthetic plaques, and thickened, tender peripheral nerves. He had established WHO Grade 1 disability, characterized by significant sensory loss in his hands and feet and early intrinsic muscle atrophy. A slit-skin smear confirmed a bacteriological index of +3 with a morphological index of 5%, indicating a high load of viable bacilli. Histopathology confirmed BL leprosy with a concurrent mild ENL reaction. A comprehensive, patient-centered management plan was initiated, including a 12-month course of multidrug therapy (MDT-MB), adjunctive care, and intensive counseling. Conclusion: This case powerfully illustrates the "shadow effect" of leprosy—how years of untreated disease, fueled by psychosocial barriers, culminate in a complex nexus of advanced infection, immunological reaction, and permanent neurological impairment. The patient's successful re-engagement with the health system underscores that eradicating the burden of leprosy requires a paradigm shift from a purely pharmacological approach to a deeply humanistic one. Effective control hinges on building compassionate health systems that actively dismantle stigma, empower patients with knowledge, and deliver holistic, multidisciplinary care to prevent the profound human cost of neglect.

Abstract

Background: The morphological mimicry between nummular dermatitis (ND) and tinea corporis represents a significant source of diagnostic error in clinical practice. This convergence on a similar clinical phenotype—the annular plaque—often leads to inappropriate antifungal therapy and a protracted, debilitating course for the patient, as powerfully illustrated in this case. Case presentation: We present the case of a 22-year-old female with a five-year history of chronic, recalcitrant, and intensely pruritic annular plaques on her lower legs, with a baseline pruritus score of 8/10 on a Visual Analog Scale (VAS). Previous treatments based on presumptive diagnoses, including potent topical corticosteroids and systemic antifungals, had failed. This report demonstrates a pivot from a morphology-based assessment to a multimodal diagnostic algorithm. The cornerstone of this shift was dermoscopy, which revealed patchy, irregularly distributed red dotted vessels—a classic signature of an eczematous process—and a notable absence of features suggestive of a dermatophyte infection. The diagnosis was confirmed by histopathology. A strong correlation between disease flares and psychosocial stressors was identified, and a holistic management plan targeting the cutaneous inflammation, the compromised skin barrier, and the neurogenic triggers led to a rapid and substantial improvement in her clinical status and quality of life, with the Dermatology Life Quality Index (DLQI) score decreasing from 9 to 3 and the pruritus VAS score dropping to 1/10. Conclusion: This case argues for a revised diagnostic paradigm for chronic annular lesions, one that de-emphasizes ambiguous morphology and prioritizes the integration of a detailed psychodermatological history with in-vivo microscopic analysis via dermoscopy. This approach not only prevents therapeutic futility but also underscores that successful, long-term management of ND is contingent upon a patient-centered strategy that addresses the interconnectedness of the skin barrier, the immune system, and the psycho-neuro-cutaneous axis.

Abstract

Background: The management of non-segmental vitiligo hinges on accurately assessing disease activity to guide appropriate therapy. A clinical challenge arises in patients presenting with a low vitiligo area scoring index (VASI), suggesting limited disease, which may be discordant with underlying inflammatory activity, potentially leading to therapeutic inertia. Case presentation: A 34-year-old woman with Fitzpatrick skin type III presented with a two-month history of rapidly progressing facial vitiligo, preceded by an erythematous phase. Despite a low VASI score of 1, the patient-reported vitiligo disease activity (VIDA) score was +4. Dermoscopy was instrumental, revealing definitive in-vivo evidence of inflammation and instability, including a pinkish background, telangiectasias, and a reversed pigment network. Based on this discordance between disease extent and activity, a multi-modal therapeutic regimen was initiated. A six-month follow-up demonstrated disease stabilization and significant perifollicular repigmentation, with resolution of the inflammatory dermoscopic signs. Conclusion: This case report illustrates the critical importance of an integrated diagnostic approach that moves beyond area-based assessment. It highlights how dermoscopy, when used to resolve the clinical paradox of low-extent but high-activity disease, can serve as an objective biomarker to justify timely and robust immunomodulatory intervention. This approach is crucial for altering the disease trajectory and optimizing patient outcomes.

Abstract

Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis that presents with rapidly progressive, painful ulcers and may be diagnostically challenging because it mimics infection, vasculitis, and other ulcerative dermatoses. This report describes an 88-year-old Minang woman who presented with multiple enlarging, painful ulcers on both lower legs and thighs over six months, with significant functional limitation requiring wheelchair use. Dermatological examination showed multiple ulcers with granulation to necrotic bases, irregular margins, and undermined edges, with surrounding erythema and hyperpigmentation. Laboratory evaluation demonstrated mild anaemia with otherwise unremarkable results, while Gram staining showed no bacteria. Dermoscopy revealed irregular ulcer borders with yellow-brown crusting on an erythematous background. Histopathology supported ulcerative PG by demonstrating epidermal hyperplasia with acantholysis, inflammatory cell infiltration including polymorphonuclear leukocytes and lymphocytes, mixed dermal infiltrate, and evidence of vascular destruction. The patient was treated with systemic methylprednisolone alongside topical wound care (saline compresses, topical antibiotics, and topical corticosteroids), resulting in reduced pain and lesion improvement at two-week follow-up with no new ulcers. This case highlights the importance of recognising characteristic clinical features, excluding infection, and using histopathology to support diagnosis, particularly in elderly patients where disease burden and complications may be greater and management must prioritise gentle wound care and avoidance of trauma.

Abstract

Background: Lichen simplex chronicus (LSC) is a form of eczema that damages the skin barrier due to chronic itching. Stress is a contributing factor, leading to a persistent itch–scratch cycle that worsens lesion appearance and size. Purpose: This study aimed to describe the characteristics, severity, lesion location, and size, as well as stress levels, in patients with LSC. Methods: This descriptive cross-sectional study involved 42 LSC patients from the Dermatology and Venerology Outpatient Clinic at Andalas University Teaching Hospital, diagnosed between August 2019 and August 2023. Samples were selected using simple random sampling. Severity was measured using the EASI score, lesion location was based on medical records, lesion size was measured with a tape, and stress level was assessed using the PSS-10 questionnaire. Data were analyzed using univariate analysis (age, gender, occupation, severity, lesion location, and size). Result: The majority of LSC cases occurred in the 56–65 age group (42.9%), predominantly in women (57.1%), with civil servants being the most common occupation (28.6%). Most patients had mild to moderate severity (40.5%), with the ankle as the most frequent lesion site (26.2%), plaque as the most common lesion type (90.5%), and moderate stress being most prevalent (57.1%). Conclusion: Most patients were aged 56–65, predominantly female, and civil servants. Lesions were mainly mild to moderate in severity, commonly located on the ankle, with the plaque-type being most frequent. Moderate psychological stress was most prevalent.

Abstract

AbstrakLatar belakang: Dermatitis atopik merupakan peradangan kulit kronis yang memengaruhi kualitas hidup penderita. Sejauh ini, penelitian terkait profil dermatitis atopik di berbagai kelompok usia telah dilakukan di beberapa wilayah Indonesia, namun belum ada di kota Padang.Objektif: Mengidentifikasi profil pasien dermatitis atopik di Poliklinik Kulit dan Kelamin RSUP Dr. M. Djamil Padang periode Januari 2018 - Desember 2023.Metode: Penelitian ini bersifat deskriptif observasional. Sampel diambil dengan teknik total sampling, menggunakan 31 rekam medik pasien dermatitis atopik di RSUP Dr. M. Djamil Padang periode Januari 2018 hingga Desember 2023.Hasil: Penelitian menunjukkan bahwa dermatitis atopik paling sering terjadi pada anak-anak (45,2%), dengan prevalensi lebih tinggi pada perempuan (51,6%) dan sebagian besar pasien tinggal di perkotaan (74,2%). Mayoritas pasien adalah pelajar (51,6%). Sebanyak (38,7%) pasien memiliki riwayat atopi, dengan rinitis alergi sebagai yang paling umum (41,7%), dan (67,7%) pasien memiliki riwayat atopi dalam keluarga. Gejala utama berupa gatal dan bintik merah pada kulit (64,5%). Lesi sering ditemukan di wajah pada infantil (83,3%), di tungkai pada anak-anak (85,7%), dan di tungkai serta lengan pada orang dewasa (63,6%), dengan sebagian besar kasus memiliki derajat keparahan ringan berdasarkan skor SCORAD (61,3%). Pengobatan yang paling sering digunakan adalah antihistamin dan kortikosteroid topikal (27,9%).Kesimpulan: Dermatitis atopik banyak terjadi pada anak, terutama perempuan di perkotaan. Gejala utama adalah gatal dengan bintik merah, dan kebanyakan kasus ringan. Pengobatan yang sering digunakan adalah antihistamin dan kortikosteroid topikal.Kata kunci: dermatitis atopik, profil, riwayat atopi, SCORAD
