Abstract

High-risk pregnancy is a condition that can negatively impact the health of both the mother and the fetus. Social and demographic factors such as maternal age, educational level, economic status, employment, and place of residence contribute to pregnancy risk levels. Indonesia continues to experience a high maternal mortality rate (MMR), necessitating an in-depth analysis of the social determinants affecting high-risk pregnancies. This study employs a cross-sectional design and utilizes secondary data from the 2023 Indonesian Health Survey (SKI). The study sample consists of 46,699 pregnant women from 38 provinces across Indonesia. The independent variables include maternal age, education, employment, economic status, and place of residence, while the dependent variable is high-risk pregnancy. Data analysis was conducted using the chi-square test to identify associations between social determinants and pregnancy risk. The findings indicate that 15.4% of pregnant women experience risk factors in their pregnancy. Significant social factors associated with high-risk pregnancy include maternal age, education level, employment, place of residence, and economic status (p < 0.05). Adolescent mothers (aged 10–19 years) and older mothers (>35 years) are at higher risk. Women with low education levels, unemployment, and lower economic status are also more vulnerable to high-risk pregnancies. High-risk pregnancy is influenced by various social and demographic factors. Therefore, inclusive health policies are needed to improve maternal healthcare access, especially for high-risk groups. Health education programs on reproductive health and increased access to prenatal healthcare services can help reduce the prevalence of high-risk pregnancies in Indonesia.

Abstract

Diarrhea is still a challenging health problem for children under five years of age in Indonesia. Diarrhea due to waterborne diseases is still prevalent in Samarinda, and this environmental health problem could trigger outbreaks in multiple regions. The objective of this study was to analyze the effect of child characteristics, mother characteristics, and environmental characteristics in Samarinda. The research method used a case-control design with a sample of 124 (62 cases and 62 controls), and the data was obtained by interview using a questionnaire. The sampling technique was consecutive sampling. Data analysis was carried out, including univariate and bivariate, by chi-square statistical tests. And multivariate analysis by multiple logistic regression. The results showed that the incidence of diarrhea with nutritional status P = 0.027 (OR = 2.292; 95% CI: 1.092-4.807); history of exclusive breast-feeding P = 0.040 (OR = 2.180; 95% CI: 1.030-4.612); history of immunization status P = 0.014 (OR = 2.860; 95% CI: 1.218-6.716); worker mother P = 0.08 (OR = 0.354; 95% CI: 0.163-0.771); and clean water source P = 0.09 (OR = 2.754; 95% CI: 1.280-5.923); and drinking water management P = 0.017 (OR = 2.444, 95% CI: 1.166-5.127). The factors that did not relate significantly in this research were the child’s age (P = 0.402), mother’s age (P = 0.467), mother’s education (P = 0.534), and family income (P = 0.579). The result from the multivariate shows the variable that had the most significant association with diarrhea was the mother’s occupation. The accessibility to clean water sources was an intervening variable that played an important role in diarrhea cases in Samarinda. Health institutions also need to conduct periodic water quality checks and increase community knowledge about water quality and diarrhea prevention. This study could provide valuable input for diarrhea prevention and control programs.

Abstract

Binge eating behavior is described as a loss of control over eating large amounts of food compared to ordinary people over a period of time. Binge eating is common among adolescents and poses significant health risks. In Indonesia, research on this issue is limited. This study explores how self-esteem, dietary habits, BMI, and emotional control relate to binge eating. This research uses a quantitative method with a cross-sectional study. The sample in the study amounted to 346 participants. Data collection techniques using the Modified BES (Binge Eating Scale), DERS-SF (Difficulty Emotional Regulation - Short Form), RSES (Rosenberg Self Esteem Scale), and dietary restraint questionnaire that the researcher made himself. The participants with binge eating behavior were 118 (33.5%). A significant relationship exists between binge eating and self-esteem, emotion regulation, and dietary restraint, but not with BMI. Dietary restraint has the most vital relationship. This study concludes that binge eating behavior has a high prevalence among adolescents. Emotion regulation, self-esteem, and dietary restraint were proven as risk factors in this study, and the most significant factor was dietary restraint. BMI Z-score showed a negative correlation with the incidence of binge eating in adolescents. Suggestion: Schools are expected to provide emotional assistance through counseling guidance to students related to mental health and knowledge about eating behavior and proper diet.

Abstract

Stunting remains a major public health concern in Indonesia, including in densely populated urban areas such as Bandarharjo, Semarang. Stunting is caused by chronic malnutrition and recurrent infections during the first 1,000 days of life (WHO, 2015). This study aims to evaluate the implementation of health promotion programs in reducing stunting rates in the Bandarharjo Community Health Center (Puskesmas) service area. This study employs a qualitative approach using the CIPP evaluation model (Context, Input, Process, Product) to assess the extent to which the program is implemented and effective in practice. Data were collected through in-depth interviews with key informants and triangulation, including health promotion officers, nutritionists, midwives, environmental health workers, Posyandu cadres, and mothers of infants. The results indicate that the program is functioning reasonably well in terms of education and cross-sectoral collaboration; however, challenges remain, including low community awareness, physical environmental barriers, and limitations in personnel and facilities. Innovations such as “Srigunting” add value to program implementation. The CIPP evaluation revealed that interventions are occurring at all stages; however, strengthening is needed in the process and monitoring aspects to ensure more optimal and sustainable outcomes.

Abstract

Introduction: Improper infant positioning and attachment during breastfeeding is a common problem among mothers, mainly due to a lack of knowledge about breastfeeding techniques. This lack of understanding regarding correct breastfeeding technique contributes to nipple fissures and breast engorgement, ultimately impacting infant nutrition. This study aimed to assess how LATCH-based education method influences the proper latching of breastfeeding mothers in Magelang City Methods: This study employed a quasi-experimental pre-post design conducted in Magelang City. The participants were breastfeeding mothers who were doing it for the first time. A total of 44 participants were enrolled as samples through purposive sampling. The tools used included a questionnaire and an evaluation observation sheet to gauge the effectiveness of breastfeeding techniques. The data analysis was performed using the Wilcoxon Signed-Rank Test. Results: The results showed that three respondents had good attachment accuracy (6.8%), 38 mothers had sufficient latch (86.6%), and 3 had inadequate latch (6.8%). After receiving education, 32 respondents (72.7%) had a good latch, while 12 respondents (27.3%) had a sufficient latch. Respondents who achieved LATCH-Score indicators increased, including LATCH-on, audible swallowing comfort, and hold with a total increase of respondents of 5 (12%), 24 (76%), 20 (46%), and 15 (34%), respectively. There was a significant relationship between latch accuracy before and after being given education about breastfeeding techniques in primigravida breastfeeding mothers in the Magelang City area (P value= 0.000). Conclusion: Latch score-based breastfeeding education on breastfeeding techniques affects how well new mothers attach their infants during breastfeeding. It is potentially used to support successful breastfeeding.

Abstract

Stunting is a complex public health problem in Indonesia. The incidence of stunting in Kudus City was 3,601 toddlers (5.85%), with the highest prevalence in Dawe District (13.32%), Undaan District (11.13%), Gebog District (10.66%), and 238 children under five, accompanied by comorbidities such as acute respiratory tract infections (ARI), diarrhea, tuberculosis (TB), and worms. One of the determining factors for success in combating stunting is family independence in managing the health of toddlers. However, family independence is constrained by limited access to information, lack of balanced nutrition education, sanitation, personal hygiene, symptoms of comorbidities, monitoring and development of toddlers not routinely, non-compliance with immunization and treatment. Smartphone applications can be used to overcome these obstacles. This study aims to develop and evaluate the effectiveness of the “Family Care Stunting” smartphone application in improving the health independence of families of stunted toddlers with comorbidities. This study used a research and development design, with 201 respondents from families of stunted toddlers who had comorbidities. Effectiveness was analyzed using the Wilcoxon nonparametric differential test to compare the level of family health independence before and after the intervention. There was a significant increase in the level of family health independence after the intervention of the “Family Care Stunting” smartphone application (p-value = 0.000). This application has the potential to be an innovative tool that supports families to play an active role in child health management, thereby contributing to more comprehensive stunting prevention and handling efforts.
