Abstract

Background: Non-Alcoholic Fatty Liver Disease (NAFLD) is emerging as chronic liver disease, both in developed and developing countries. NAFLD affects up to 25% population worldwide. The incidence of NAFLD associated with various risk factors supporting the development of the fatty liver. This study aim is to know the characteristics of NAFLD patients.Method: Retrospective study was conducted from medical records to find the characteristics of patients diagnosed with NAFLD  at Dr. M. Djamil General Hospital from January 2016 – December 2018.Results: Seventy-seven patients were diagnosed with NAFLD. The majority of  NAFLD patients were male and female at 36-45 years old and female at 65 years old. Unemployed females were more prevalent compared to males. Dyslipidemia was the most component of a metabolic syndrome found in NAFLD patients. From laboratory findings, 55.56% of patients have increased aspartate aminotransferase (AST) levels, while 52.78%  have increased alanine aminotransferase (ALT) levels. Ultrasonography used widely as a diagnostic device to detect NAFLD. Pharmacological therapy based on American Asociation for the Study of Liver Disease (AASLD) recommendation that used widely to treat patients with NAFLD was statin.Conclusion: The incidence of NAFLD was found to vary in different age ranges and sexes. Dyslipidemia occurred in most of NAFLD patients. AST and ALT levels increased in about half of the patients. Abdomen ultrasound as a diagnostic modality that was widely used.

Abstract

Background: Parathyroid hormone is known as one of the main uremic toxicants, which, if the amount increases in plasma, will affect motor conduction in patients with chronic kidney disease. This study aims to determine the relationship between Parathyroid hormone levels with uremic neuropathy in patients with chronic kidney disease. Methods: This study was an analytic observational study with a cross-sectional approach, where as many as 42 subjects participated in this study. Data analysis was performed using SPSS using univariate and bivariate, with p<0.05. Results: There was a difference in the average thyroid hormone levels in neuropathy and without neuropathy, but statistically, it was not different p>0.05. Conclusion: There is no relationship between parathyroid hormone and uremic neuropathy in patients with chronic kidney disease.

Abstract

Spasmofilia merupakan keadaan patologis dimana terjadi hiperiritabilitas saraf dan otot (neuromuskular) akibat adanya gangguan keseimbangan elektrolit, terutama ion kalsium (Ca++) dan ion magnesium (Mg++) yang ditandai dengan munculnya kedutan otot, kesemutan dan spasme karpopedal. Hipoparatiroidisme ditandai dengan hipokalsemia dan hiperfosfatemia yang dapat menyebabkan hiperiritabilitas neuromuskular dan menimbulkan gejala spasmofilia. Kami laporkan sebuah laporan kasus seorang wanita usia 47 tahun yang didiagnosis dengan spasmofilia akibat hipoparatiroidisme didapat. Dari laporan kasus ini dapat disimpulkan bahwa diagnosis spasmofilia pada pasien ini ditegakkan berdasarkan anamnesis, pemeriksaan fisik, laboratorium dan pemeriksaan Elektromiografi (EMG) yang merupakan gold standard pada spasmofilia. Dapat disimpulkan bahwa penyebab munculnya spasmofilia pada pasien ini adalah hipoparatiroidisme post tiroidektomi.

Abstract

Latar Belakang: Kemoterapi pada pasien keganasan payudara dapat menyebabkan gangguan kognitif. Sebuah studi menjelaskan bahwa terdapat 65% pasien dengan keganasan payudara mengalami gangguan kognitif akut dan 60% dari mereka mengalami penurunan kognitif terlambat Objektif: Mengetahui gambaran fungsi kognitif pada penderita keganasan payudara yang menjalani kemoterapi di RSUP Dr. M. Djamil Padang. Metode: Penelitian ini adalah penelitian deskriptif dengan desain cross-sectional. Pengambilan sampel dilakukan dengan teknik simple random sampling sebanyak 44 orang. Penelitian ini berlangsung dari Maret 2020-Januari 2021. Hasil: Sebanyak 75% subjek penelitian mengalami gangguan kognitif. Gangguan kognitif paling banyak ditemukan pada kelompok usia pertengahan dan pada tingkat pendidikan rendah, yaitu 100%. Gangguan kognitif mulai ditemukan pada kemoterapi kedua, paling banyak ditemukan pada kemoterapi ketiga dan keempat (18,2 %). Gangguan kognitif terjadi pada semua obat kemoterapi dan paling banyak ditemukan pada penggunaan obat kombinasi CAF. Kesimpulan: Gangguan kognitif terbanyak ditemukan pada usia pertengahan dengan tingkat pendidikan yang rendah, pada subjek yang menjalani kemoterapi yang ketiga dan keempat, serta pada pemberian terapi kombinasi 5-Flourouracil-Doxorubicin-Cyclophosphamide

Abstract

Background: Dickkopf-related protein – 1 (DKK-1) is an antagonist protein to the Wingless (Wnt)-β catenin signal. Dysregulation of Wnt-β catenin signaling by DKK-1 causes disruption of neuronal synapses, which can result in impaired cognitive function. Impaired cognitive function in people with HIV or HIV-associated neurocognitive disorder (HAND) is a spectrum of cognitive disorders related to HIV neuroinvasion and neuroinflammation, which significantly results in impaired cognitive function and daily activity. This study aimed to assess the relationship between plasma DKK-1 levels and impaired cognitive function in HIV patients. Methods: This study used a cross-sectional design, consisting of 84 HIV sufferers who went to the voluntary counseling and testing (VCT) polyclinic at Dr. M. Djamil General Hospital, Padang, Indonesia, in the period December 2022 - March 2023, who met the inclusion and exclusion criteria. Cognitive function was assessed using the Montreal Cognitive Assessment Indonesian version (MoCa-Ina), and daily activity function using the instrumental activity of daily living (IADL) examination. Plasma DKK-1 levels were measured using the enzyme-linked immunosorbent assay (ELISA) method. Results: The mean plasma DKK-1 level of HIV sufferers was 358.4 (+ 157.6) pg/mL. ANI type disorder was found in 78.6%, MND in 21.4%, and no HAD type. There was a significant relationship between plasma DKK-1 levels and impaired cognitive function in HIV patients (OR = 2.82, CI95% = 1.128-7.043, p = 0.025), but there was no significant difference in plasma DKK-1 levels between types of cognitive impairment ANI and MND (p = 0.858). Conclusion: Plasma DKK-1 levels are significantly associated with impaired cognitive function in HIV patients.

Abstract

Latar Belakang: Glaukoma juveinil adalah bagian dari glaukoma primer sudut terbuka atau primary open angle glaucoma (POAG) dengan karakteristik penurunan secara autosomal dominan dan terjadi pada usia muda, sekitar 4 hingga 35 tahun. Glaukoma juveinil berkaitan dengan peningkatan tekanan intraokular (TIO) akibat struktur trabecular meshwork yang abnormal sehingga menyebabkan gangguan aliran aqueous humor. Peningkatan tekanan intraokular dapat mengakibatkan kerusakan permanen pada saraf optik yang menyebabkan perubahan ukuran cup disk ratio (CDR) sehingga dapat berujung pada kebutaan. Objektif: Untuk mengetahui profil pasien glaukoma juveinil di Poliklinik Mata RS Dr. M. Djamil Padang Tahun 2017-2020 Metode: Jenis penelitian ini adalah deskriptif. Populasi penelitian ini adalah seluruh pasien yang terdiagnosis glaukoma juveinil oleh klinisi di Polikinik Mata RS Dr. M. Djamil Padang dengan jumlah sampel sebanyak 42 orang dengan total 84 mata pasien dari tanggal 1 Januari 2017 – 31 Desember 2020. Hasil: Hasil penelitian didapatkan pasien glaukoma juvenil sebagian besar laki-laki sebanyak 52,4% dengan kelompok umur terbanyak ≤20 tahun sebanyak 50%. Faktor resiko terbanyak ditemukan adalah dengan ada riwayat keluarga 64,3%, miopia 33.3%, dan TIO ≤21 mmHg 77,4%. 47,6% sampel memiliki visus normal. Stadium terbanyak adalah stadium sedang sebesar 79,8%. Tatalaksana terbanyak yaitu dengan monoterapi 76,2%. Kesimpulan: Karakteristik pasien glaukoma juveinil paling banyak yaitu laki-laki dan berada pada kelompok usia ≤20 tahun. Faktor resiko yang terbanyak adalah pada yang memilki riwayat keluarga dan miopia. Pasien terbanyak dengan visus normal dengan stadium sedang. Tatatalaksana yang paling banyak dilakukan adalah dengan monoterapi.

Abstract

Latar Belakang : Stroke merupakan penyakit dengan angka kematian tertinggi kedua di dunia. Pada massa pandemi COVID-19 berbagai temuan neurologis termasuk stroke ditemukan pada penderita COVID-19. Terjadinya stroke pada pasien COVID-19 dipicu keadaan hiperinflamasi dan hiperkoagulasi yang umum terjadi pada pasien COVID-19. Objektif : Mengetahui gambaran faktor risiko penderita stroke dengan COVID-19 di RSUP Dr. M. Djamil Padang periode Maret 2020-Desember 2021. Metode : Penelitian ini menggunakan metode cross sectional menggunakan data rekam medis pasien yang terdiagnosis stroke dengan COVID-19. Pengambilan sampel dengan metode total sampling. Analisis data secara univarian untuk mencari distribusi frekuensi dan bivariat menggunakan uji fisher exact untuk mengetahui hubungan faktor risiko dengan penyakit storke pada pasien COVID-19. Hasil : Hasil penelitian didapatkan 381 pasien stroke. Pasien stroke dengan COVID- 19 sebanyak 36 orang (9,4%). Stroke iskemik dengan COVID-19 sebanyak 26 orang (72,22%) dan stroke hemoragik dengan COVID-19 sebanyak 10 orang (27,78%). Kelompok usia terbanyak ≥65 tahun (47,2%) dan lebih banyak berjenis kelamin laki-laki (55,5%). Urutan faktor risiko mulai dari yang terbanyak yaitu hiperkoagulsi, hipertensi, diabetes melitus, dislipidemia, obesitas, dan riwayat penyakit jantung. Penelitian ini menunjukkan adanya hubungan antara hipertensi dengan terjadinya stroke dengan COVID-19. Kesimpulan : Frekuensi pasien stroke dengan COVID-19 sebesar (9,4%). Pasien umumnya mengalami gangguan koagulasi, ditandai dengan peningkatan nilai D-dimer. Pada penelitian ini terdapat hubungan antara faktor risiko hipertensi dengan kejadian stroke pada pasien COVID-19. Kata kunci: Faktor risiko, Stroke, Iskemik, Hemoragik, COVID-19 Background : Stroke is the sceond leading highest mortality rate in the world. During the COVID-19 pandemic, various neurological findings including stroke in patient with COVID-19 disease. Stroke in COVID-19 patients is triggered by hiperinflammation and hypercoagulability which are common in COVID-19 patients. Objective : To describe the risk factors of stroke on COVID-19 Patients hospitalized in DR. M. Djamil Padang during Maret 2020-Desember 2021 period. Methods : This study used a cross-sectional method using medical record data of patients diagnosed with stroke in COVID-19 patients. The sample was taken using the total sampling method. The data analysis was conducted Univariate to find the frequency distribution and bivariate using the Fisher's exact test to determine the relationship between risk factors and stroke in COVID-19 patients. Results : The result of study were 381 patients strokes. There are 36 patients strokes with COVID-19. Patient ischemic stroke with COVID-19 were 26 (72,22%) and hemorrhagic stroke with COVID-19 were 10 (27,78%). The most age group is ≥65 years old (47,2%) and the gender are more male (55,5%). The sequence of risk factors starting from the most is hypercoagulation, hypertension, diabetes mellitus, dyslipedimia, obesity, and history of heart disease. This study showed relationship between hypertension and the occurence of stroke in COVID-19 patients. Conclusion : The frequency of stroke in COVID-19 patient is 9,4%. Patient generally had coagulation disorders which are showed by increased D-dimer values. This study also showed the relationship between hypertension and the occurence of stroke in COVID-19 patients while other risk factors have no relationship. Abstract

Latar Belakang: Peningkatan angka harapan hidup yang terjadi menyebabkan peningkatan penyakit degeneratif pada lansia, salah satunya hipertensi. Proses penuaan pada lansia menyebabkan lansia mengalami beberapa perubahan fisiologi salah satunya fisiologi tidur, struktur, durasi, kedalaman, dan kontinuitas tidur akan pada lansia. Kualitas tidur buruk dapat menyebabkan meningkatnya tekanan darah melalui aktivasi sistem saraf simpatis. Objektif: Mengetahui hubungan kualitas tidur dengan tekanan darah pada lansia di kecamatan Kuranji Kota Padang. Metode: Jenis penelitian ini merupakan penelitian analitik dengan desain studi cross-sectional. Sampling from the population using consecutive sampling techniques in the elderly recorded in Kuranji District, Padang City and conducting interviews with PSQI questionnaires and blood pressure measurements directly on the samples. Hasil: Terdapat sebanyak 67 responden (69,1%) memiliki kualitas tidur buruk dan 30 responden (36,1%) memiliki kualitas tidur baik. Sebanyak 62 responden (63,9%) mengalami hipertensi. Hasil analisis data dengan SPSS menggunakan uji Chi-square diperoleh nilai p = 0,000 < 0.05 ( α = 0,05 ). Kesimpulan: Terdapat hubungan antara kualitas tidur dan tekanan darah pada lansia di Kecamatan Kuranji Kota Padang sehingga H1 ditolak.
