Abstract

Meningioma merupakan tumor otak primer yang paling sering dijumpai,1 namun meningioma angiomatosa merupakan subgrup meningioma yang sangat jarang (2,1%).2 Meningioma umumnya terjadi pada perempuan, namun khusus tipe angiomatosa lebih sering pada laki-laki. Dilaporkan kasus meningioma angiomatosa pada laki-laki berusia 56 tahun, yang didiagnosis berdasarkan gejela klinis, pencitraan, dan gambaran histopatologi yang khas. Pasien dilakukan operasi kraniektomi reseksi tumor total dilanjutkan pemasangan tulang kranium kembali 6 bulan kemudian yang mengalami perbaikan klinis.

Abstract

Mahasiswa kedokteran merupakan kelompok yang paling rentan mengalami gangguan tidur. Hal ini dapat menurunkan pencapaian akademik dan mengganggu kesehatan serta mood individu. Sleep hygiene yang tidak benar merupakan salah satu penyebabnya. Tujuan penelitian ini adalah menentukan hubungan antara sleep hygiene dan hasil belajar blok pada mahasiswa tahap akademik Fakultas Kedokteran Universitas Andalas (Unand). Jenis penelitian ini adalah analitik dengan desain potong lintang. Responden penelitian adalah 65 orang mahasiswa tahun ketiga Fakultas Kedokteran Unand. Instrumen penelitian ini menggunakan kuesioner Sleep Hygiene Index (SHI) yang telah divalidasi. Hasil belajar diambil dari nilai ujian blok dengan kategori baik (skor≥70) dan kurang baik (skor<70). Data dianalisis menggunakan uji bivariat Fisher’s exact. Hasil penelitian menunjukkan 51 orang (78,5%) adalah perempuan dan 14 orang (21,5%) adalah laki-laki. Usia terbanyak adalah 20 tahun (67,7%). Tingkat sleep hygiene 52 orang (80%) sedang dan 1 orang (1,5%). Tingkat hasil belajar 54 orang (83,1%) kurang baik. Sebanyak 46 orang memiliki sleep hygiene sedang-buruk dan hasil belajar buruk. Analisis bivariat menunjukkan nilai p=0,194 (p>0,05), berarti tidak terdapat hubungan yang signifikan antara sleep hygiene dan hasil belajar blok. Simpulan studi ini adalah sebagian besar tingkat sleep hygiene dan hasil belajar mahasiswa kedokteran tidak adekuat, maka edukasi sleep hygiene perlu direkomendasikan untuk mencapai kualitas tidur yang lebih baik.

Abstract

Tumor ekstramedula intradural merupakan kasus yang jarang. Gambaran klinis sangat bervariasi, mulai dari gejala ringan dan tidak spesifik hingga komplikasi neurologis yang berat. Dilaporkan seorang pasien laki-laki umur 14 tahun dengan keluhan lemah kedua tungkai sejak 5 hari sebelum masuk RS. Awalnya pasien merasakan tungkai kanan terasa lemah sehingga pasien berjalan dengan menyeret. Kelemahan bertambah berat sehingga pasien hanya bisa berbaring di tempat tidur. Nyeri di punggung setinggi puting susu sejak 1 minggu sebelum masuk RS, nyeri dirasakan seperti terikat, nyeri kadang dirasakan hilang timbul. Keluhan juga diikuti rasa kebas yang dirasakan setinggi puting susu kebawah. Tidak ada keluhan buang air besar dan buang air kecil. Sensasi nyeri, suhu raba berkurang setinggi dermatom torakal IV kebawah. Hasil MRI didapatkan suatu gambaran kista. Tindakan operasi dilakukan pada pasien dengan ektirpasi kista dengan hasil biopsi pilositic astrocitoma (WHO grade 1) dan pasien dianjurkan untuk radioterapi.

Abstract

Personal protective equipment (PPE) in the COVID-19 pandemic era is essential for healthcare workers to decrease the risk of infection. The PPE, such as N95 masks and goggles, can trigger external-compression headache (ECH). This study aimed to determine the factors associated with the prevalence of ECH attributed to PPE usage. It was an analytic-observational study with a cross-sectional design conducted at Dr. M. Djamil Hospital Padang in May 2020. The subjects were healthcare workers (doctors and nurses) who used level 3 PPE for a minimum of 4 hours. The diagnosis of ECH was determined by the International Classification of Headache Disorders (ICHD) 3rd edition criteria. The Headache Screening Questionnaire (HSQ) determined the pre-existing primary headaches. The association between variables was analyzed using chi-square and Kruskal-Wallis tests. P-value<0.05 was considered statistically significant. A total of 113 healthcare workers participated in this study, consisting of 46 (40.7%) males and 67 (59.3%) females. The median age was 30 (23–46) years. ECH occurred in 102 (90.3%) respondents. Pre-existing primary headaches were present in 79 (69.9%) respondents, including migraine in 28 (23.6%) and tension-type headaches (TTH) in 46 (40.7%) respondents. The frontalis (69%) and temporalis (50.4%) were the most affected muscles. In this study, the significant factor associated with ECH was pre-existing primary headache (p=0.001, OR=7.795). There was a significant association between the pre-existing TTH (p=0.022) and ECH. There was a non-significant association between pre-existing migraine and ECH (p=0.284). In conclusion, the pre-existing primary headache was associated with the prevalence of ECH attributed to PPE usage.

Abstract

Background. Lumbar disc herniation (LDH) is a disorder that involves rupture of the annulus fibrosus so that the nucleus pulposus protrudes and compresses the lumbar canal. LDH is the most common cause of lumbar radiculopathy. LDH causes functional disorders in patients, causing disability to carry out daily activities. The purpose of this study was to determine the risk factors that influence the degree of disability in lumbar nucleus pulposus herniation. Methods. Analytical research with a consecutive cross-sectional study approach in patients with lumbar LDH at the Neurology Outpatient Clinic, Dr. M. Djamil General Hospital Padang from March to November 2021. The risk factors assessed were age, gender, occupation, length of illness, and body mass index (BMI). The measurement of the degree of disability uses the Oswestry disability index (ODI) score. The relationship between risk factors and the degree of disability was analyzed using chi-square test. Differences in the mean age of groups with mild and moderate degrees of disability using unpaired t-test. Results. 40 subjects met the inclusion and exclusion criteria. Mean age 57 ± 10,976 years, male and female ratio 1:2, work with heavy lifting activities was found in 70% of subjects, duration of suffering from LDH more than one year in 60% of subjects. BMI overweight and obesity as many as 77.5% subjects. As many as 60% of the subjects experienced mild to moderate degrees of disability. There is a difference in the average age of the subjects with mild to moderate degree of disability with a severe degree of p-value of 0.044. There was no significant relationship between gender (p=0.054), occupation (p=0.398), length of illness (p=0.503) and body mass index (p=0.757), and the degree of disability. Conclusion. The degree of disability in patients with LDH is influenced by age. Meanwhile, gender, occupation, length of illness and body mass index were not significantly related to the degree of disability in patients.

Abstract

Short-lasting unilateral neuralgiform headache attacks with conjunctival injection and tearing (SUNCT) are a subset of short-lasting unilateral neuralgiform headache attacks. SUNCT is a condition that is rarely encountered and is characterized by typical moderate to severe neuralgia pain located in the first division area of the trigeminal nerve that can be felt in any part of the head. SUNCT can be primary and secondary. Typical SUNCT symptoms include severe unilateral headache with autonomic symptoms with bilateral conjunctival injection and lacrimation with an attack frequency more than 60 times per day (range 3 to 200) which can occur spontaneously or be triggered by a mechanical stimulus in an area that is innervated by the trigeminal nerve , and without a refractory period thereafter. The current treatment is still diverse, including therapy with anti-epileptic drugs, interventional therapy, or invasive procedures. The diagnosis and management of complex therapies in patients with SUNCT still need further understanding.

Abstract

Post-Traumatic Headache (PTH), is the most frequent complaint after Traumatic Brain Injury (TBI). PTH affects the quality of life of the sufferer. Post-traumatic headache is defined as secondary headache within 7 days after head trauma or after regained consciousness after head trauma. PTH's definitive mechanisms are still unknown, but hypotheses can be brought from clinical findings and diagnostic tests that have been conducted. PTH can originate from peripheral or central. A multidisciplinary approach is emphasized in the treatment of PTH due to the many associated symptoms

Abstract

Abstrak  Migren merupakan salah satu jenis nyeri kepala primer yang banyak dikeluhkan. Migren yang tidak ditangani dengan baik akan menjadi kronis dan menimbulkan disabilitas yang akan mempengaruhi kualitas hidup penderitanya selain pembiayaan yang besar. Mengenali pencetus, tata laksana akut yang tepat dan pemberian terapi preventif harus dilakukan. Terapi preventif bertujuan untuk mengurangi frekuensi, severitas, durasi serangan, meningkatkan respon terapi akut sehingga akan meningkatkan kualitas hidup dan mengurangi disabilitas. Terapi preventif dikatakan berhasil bila frekuensi serangan berkurang 50% setiap bulannya selama tiga bulan. Kata kunci — Luaran, migren, terapi preventif Abstract  Migraine is one of primary headache which is often complained by patients. Migraines that are not treated properly will become chronic and cause disabilities that will affect the quality of life of sufferers in addition to large financial expenditure. Physicians have to be able to recognize the triggers, performe appropriate acute management, and give preventive therapy must be carried out. Preventive therapy aims to reduce the frequency, severity, duration of attacks, increase the response of acute therapy so that it will improve quality of life and reduce disability. The succesful preventive therapy is successful if the frequency of attacks is reduced by 50% every month for three months.Keywords — Migraine, outcomes, preventive therapy

Abstract

Tension type headache is the most common primary headache. Tension type headache can be triggered by anxiety. Anxiety often occurs in medical students, especially in new students. The aim of this  study was to determine the correlation between anxiety and tension-type headache among medical students batch 2019 in Andalas University. This was an analytic study using cross-sectional design. Sampling was done by stratified random sampling technique and we obtained sample from 79 respondents. Headache diagnosis was based on the consensus of PERDOSSI 2018 which in accordance with International Classification of Headache Disorder 3rd Edition (Beta version). Anxiety was diagnosed by using Hamilton Rating Scale for Anxiety questionnaire that had been validated. The correlation between variabels was analyzed using Spearman Rho test. The results of univariate analysis showed that the majority of respondent were women (69,6%). The most common anxiety category was mild anxiety (57%). The most common tension-type headache type was infrequent type (62%). The results of bivariate analysis showed 8 sample (34,8%) which experienced moderate-severe anxiety had infrequent tension-type  headache and 41 samples (73,2 %) which experienced mild anxiety also had infrequent tension type headache. This study showed that anxiety was positively correlated with tension-type headache (r=0,327 p=0.003)
