Abstract

Tujuan: Mengidentifikasi masalah penerapan fungsi pengendalian mutu di ruang rawat inap dan mengembangkan solusi pemecahan masalah. Metode: Metode yang digunakan berupa mini project serta analisis hasil dan gap implementasi dengan pembahasan berdasarkan kajian literatur. Permasalahan dianalisis menggunakan diagram fishbone. Penyelesaian masalah menggunakan tools Plan Do Study Action (PDSA) dimulai dari Plan of Action (POA), implementasi, evaluasi, dan tindak lanjut. Data diambil melalui wawancara terstruktur, survei, dan observasi lapangan dengan responden 11 Kepala Ruangan dan 88 Staf Perawat. Hasil: Fungsi pengendalian mutu keperawatan belum dilaksanakan optimal pada tahap perencanaan, pemantauan dan tindak lanjut masalah (45,45%). Implementasi penyelesaian masalah berupa sosialisasi dan workshop penyusunan panduan, prosedur, kamus dan instrumen pemantauan indikator mutu keperawatan. Hasil evaluasi menunjukkan 62% Kepala Ruangan meningkat pengetahuannya tentang pengendalian mutu dengan rata-rata skor pre-post test meningkat 1,33 poin dari 6,10 menjadi 7,43. Survei pasca implementasi menghasilkan persepsi yang baik dari 86,67% Kepala Ruangan mengenai pengendalian mutu keperawatan. Diskusi: Pengendalian mutu merupakan salah satu fungsi utama Kepala Ruangan. Rumah sakit telah memiliki program pengendalian mutu yang dipersyaratkan oleh standar akreditasi rumah sakit, namun compliance penerapannya masih perlu dipertahankan. Rumah sakit dipersyaratkan secara mutlak untuk dapat mempertahankan kepatuhan dan kesinambungan pengendalian mutu untuk mengevaluasi proses kerja secara berkelanjutan. Kesimpulan: Sosialisasi, workshop, dan simulasi langkah-langkah dalam penerapan pengendalian kualitas keperawatan cukup efektif meningkatkan pengetahuan Kepala Ruangan dan membuka wawasan yang lebih luas tentang pemantauan mutu yang dipersyaratkan akreditasi rumah sakit. Persepsi yang baik dari Kepala Ruangan tentang pentingnya fungsi pengendalian mutu menjadi modal awal dalam menjalankan fungsinya dengan baik. Kata Kunci: management function, nurse manager, nursing quality, quality control.

Abstract

Perencanaan strategis memegang peranan penting dalam menentukan masa depan rumah sakit. Pentingnya perencanaan strategis bagi RS dapat membantu memaksimalkan kekuatan RS untuk meningkatkan kualitas layanan dan keselamatan pasien serta memperbaiki pelayanan sesuai kebutuhan pasien. Adapun tujuan kajian ini adalah untuk mendeskripsikan rencana strategi keperawatan RS X mencakup SDM, SIM dan keuangan. Metode yang digunakan studi kasus pendekatan analisis Strength-Weakness-Opportunity-Threat (SWOT). Pengumpulan data dilakukan Mei 2022 melalui telaah dokumen dan wawancara. Hasil yang diperoleh posisi organisasi berada pada sel I, growth and building dengan skor EFE 3,21 dan IFE 3,04. Perumusan strategi terpilih difokuskan pada peningkatan SDM melalui pendidikan lanjut formal dan informal, peningkatan pelayanan melalui pengambangan pelayanan keperawatan berbasis IT dan perencanaan anggaran serta pengendalian keuangan.

Abstract

Background: The non-optimal implementation of patient discharge planning is still an important problem in reforming the care system. This non-optimality will have an impact on increasing the length of stay, high patient recurrence rates, and decreasing levels of patient satisfaction and loyalty.Purpose: to explore in-depth experiences of nurses related to nurses' perceptions, support and obstacles in carrying out patient discharge planning.Methods: This research is a qualitative research with a phenomenological approach. Participants in this study were the implementing nurses and team leaders who worked in inpatient rooms. Recruitment of participants is carried out by providing electronic recruitment leaflets to the head of the room. Data analysis used the thematic analysis of the Collaizi method which consisted of six stages, namely preparation of transcripts, coding, categories, themes, interpretation and validation.Results: The results of this study contained 6 (six) themes, namely: 1) The purpose of the patient discharge action plan; 2) The process of carrying out discharge planning at the hospital; 3) SOP for implementation of discharge planning is carried out from the beginning of the patient's treatment; 4) Factors influencing discharge planning; 5) Nurse's motivation in carrying out discharge planning; 6) Obstacles for nurses related to planning to go home which is often overlooked due to the high mobility of nurses and not having time to write.Conclusion: The most important continuity of nursing care activities, one of which is the implementation of discharge planning in accordance with the SOP by involving all health workers and documented from the time the patient enters the treatment room until the patient is planned to go home.Keywords: Patient Discharge Planning, Nursing Care Management, Military Hospital, Qualitative

Abstract

Latar Belakang: Pengalaman menderita kanker dapat mengakibatkan tanda, gejala, efek samping dan toksisitas. Pemahaman prinsip pengelolaan gejala dapat mengoptimalkan paliasi, meringankan gejala, dan meningkatkan kualitas hidup. Integrasi teknologi dalam manajemen gejala dapat berupa penggunaan aplikasi seluler. Tujuan: Untuk mengeksplorasi dan mensintesis literatur ilmiah mengenai penggunaan aplikasi seluler untuk pengelolaan gejala pada pasien kanker. Metodologi: Studi ini menggunakan kerangka kerja scoping review dengan menggunakan basis data Pubmed, Science Direct, dan CINAHL. Kata kunci yang digunakan adalah “cancer”, “mobile application”, “symptom management” dan “palliative care”. Sebanyak 77 artikel ditemukan, 17 artikel dipilih. Hasil: Aplikasi seluler pengelolaan gejala secara umum berisi tentang pengkajian gejala (dilengkapi dengan tingkat keparahan), pengelolaan gejala, dan kontak yang bisa dihubungi. Fitur tambahan antara lain avatar, video tutorial, fitur audiovisual, modul, dan integrasi dengan Internet of Things (IoT) dapat meningkatkan daya tarik aplikasi dan bermanfaat untuk meningkatkan pengetahuan, keterampilan, literasi kesehatan pasien dan/atau keluarga, serta berdampak positif pada luaran klinis pasien. Kesimpulan: Penggunaan aplikasi seluler untuk pengelolaan gejala pada pasien kanker berdampak positif pada luaran pasien. Aplikasi ini dapat menjadi sarana komunikasi, edukasi, dan koordinasi. Perawat dapat berperan dalam mengembangkan, memberikan umpan balik, dan memanfaatkan aplikasi pengelolaan gejala

Abstract

Aims: This literature review aims to theoretically analyze the rationale for developing this electronic application to cover all aspects of adolescent mental health life and as a promotive and preventive tool for adolescents with alcohol abusers. Methods: The literature review method was carried out on ten articles which were obtained from five databases namely PUBMED, Google Scholar, ClinicalKey Nursing, ProQuest, Science Direct. The keywords for article search were "FAST (Fast alcohol screening test)", "electronic-based nursing care", "alcohol abuse", "adolescent". The criteria for articles considered included articles available in English and Indonesian and published between 2017 and 2023. Results: This review shows that the use of FAST and electronic-based nursing care can improve the quality of nursing care. Conclusion: The utilization of technology in the screening test process and electronic based nursing care in nursing care is one of the alternative problem solving and especially can improve the quality of nursing care.

Abstract

This study aims to provide recommendations for the implementation of COVID-19 patient transfers and handovers between rooms carried out by nurses to be more optimal. The method used is a case study at the Jakarta Military Hospital. The results of the study through interviews and document searches found that in the planning function, there is no SOP for handovers between rooms, the SOP for handovers between rooms is combined with the SOP for handovers between shifts where the SOP contains more of the handover process between shifts. In addition, there is no monitoring and evaluation (money) form for the implementation of handovers and documentation of handovers between rooms. In conclusion, the situation analysis obtained at the Jakarta Military Hospital, the handover process between rooms, and the COVID-19 patient transfer process still needs to be developed. Abstract

This research aims to discuss the methods used to implement patient safety standards by assessing the fall risk in hospitals integrated with electronic medical records obtained from several research journals. The technique used is a literature review from journal searches in online databases, including Pubmed, ProQuest, Clinical Key Nursing, and Science. The research results show that using technology to prevent patient falls generally indicates positive results, which are very beneficial for patients and improve the quality of patient safety in hospitals. In conclusion, ways to prevent falls in hospital patients need to be further developed so that they can show better results. Abstract

This study aims to describe the implementation of planned changes in an effort to improve patient safety through optimizing monitoring and evaluation of the implementation of risk management countermeasures in inpatient rooms. The method used is to use a problem solving cycle approach which includes identifying problems, analysis and diagnosis, designing solutions, implementation and evaluation. The assessment was carried out by means of structured interviews, observation, and distributing questionnaires. Implementation of planned changes using Kurt Lewin's change model, namely unfreezing, movement and refreezing. The results of problem identification showed that the implementation of risk management in the inpatient unit was not optimal, so innovation was needed to motivate staff to implement patient safety by distributing online flyers and creating an instrument to monitor the implementation of risk management. In conclusion, monitoring using good monitoring instruments and distributing online flyers can increase nurses' understanding of implementing risk management so that patient safety can be realized. Abstract

This study aims to encourage optimization of the implementation of nursing supervision for TB program activities in hospitals. The method used is a case report, which was carried out in one of the inpatient rooms at Hospital X. Data collection techniques were through observation, interviews and questionnaires. The total sample was 22 executive nurses. The results of the assessment are analyzed to determine the problem, determine interventions to solve the problem, and prepare a Plan of Action. The results of the survey showed that the planning function of the head of the ward was not optimal, as evidenced by the lack of nursing supervision regarding the implementation of the national tuberculosis program, namely education, recording and reporting as well as Infection Prevention and Control. Nursing supervision guidelines have been prepared and follow-up plans have been made for their implementation. In conclusion, the implementation of nursing supervision needs to be optimized, including nursing supervision of national Tuberculosis program activities, in order to ensure the quality of nursing care. Abstract

This research aims to identify the relationship between the application of nursing ethical principles and nurse performance. The method used was quantitative with a cross-sectional approach to 129 nurses on duty in the Inpatient Room at RSAL Dr. Mintohardjo. Data analysis was carried out using a linear regression test. The research results show that factors related to performance include rank or class (p=0.037), respect for others (r=0.621), compassion (r=0.594), advocacy (r=0.610), intimacy (r=0.608). The regression test shows that 7 variables have a joint influence with the equation Y: 31.864+1.127*Respect to Others + 1.689*Education + 1.179 Length of Work - 1.919 *class rank + 0.755 Intimacy – 1.975*career level + 1.054 Advocacy. Respect to others partially has a positive effect on nurses' performance in nursing care (t=2.433, p=0.016), while the other variables are confounding variables (p>0.05, t<1.98). Variables namely education, rank/class, length of service, career level, advocacy, respect for others and intimacy influence nurse performance with a determined influence of 39.3%. Conclusions from the 4 sub-variables of ethical implementation where these four variables have a significant relationship independently to nurse performance.
