Abstract

AbstractThis article discusses about Indonesian influence diagnostic related group (ina-drg) to resume charging completeness medical patients in hospitals Kalianda jamkesmas South Lampung. In the health care system was an enacted JAMKESMAS payment pattern by applying INA-DRG system. Financing system diagnosis related group (DRG) is a system or method of payment by the funders of health care providers (health providers) for convening services, large costs are not calculated based on the type or amount of health services being offered to each patient, but by agreement price according to the diagnosis of diseases in which a group of patients who are being treated are located. INA-DRG (Diagnostic Related Group Indonesian) using the basics in coding using ICD 9 CM to ICD 10 and the action to diagnose the disease, the pattern of the rates specified amount of an average patient stay and long stay patients in accordance with the code of the disease suffered by patients treated so long hospitalization was determined and adjusted to the prevailing rates in the INA-DRG. Medical resumes charging completeness prior to implementation of INA-DRG still shows that there is still low awareness of health practitioners in the medical resume charging completeness. This is because there is no requirement in terms of the completeness of filling the whole medical resume. Completeness of medical resumes charging after INA-DRG implementation has already shown that there are very significant and meaningful changes, but there are certain parts that are still not discovered the change, namely the charging of physical examination items. Abstract

AbstractThe Health Care Referral System is a service system that controls the delegation of authority and responsibility for health problems which is carried out reciprocally vertically or horizontally, in the sense of a unit with less capability to a more capable unit. The purpose of this study was to determine the implementation of the health service referral system in terms of regulations, health personnel resources and infrastructure. This research is a systematic review (Systematic Literature Review). The data synthesis process was carried out by comparing 15 literatures that met the quality assessment as well as inclusion and exclusion criteria with the database sources used, namely, Google Scholar, Pubmed, ScienceDirect. The data is processed and presented in tabulated form. The results showed that several countries such as Indonesia, Iran, Northern Tanzania, Riyadh, Ghana, Zambia and Hoduras from the articles reviewed used a referral system starting from the basic level (Primary), secondary level (Secondary) and tertiary level (Tertiary). Judging from the three aspects of the implementation of the health service referral system reviewed, it shows that there are problems in its implementation, including; the regulatory aspect found only 2 literatures that discussed regulations in the implementation of the referral system, the aspect of health personnel resources was found to be unavailability of health workers in health services causing patient referral and infrastructure aspects found obstacles in inadequate infrastructure resulting in referral.Keywords: implementation, referral system, health care AbstrakSistem Rujukan Pelayanan Kesehatan merupakan sistem pelayanan yang mengendalikan pelimpahan wewenang serta tanggung jawab atas permasalahan kesehatan yang dilakukan secara timbal balik secara vertikal ataupun horizontal, dalam makna dari unit yang berkemampuan kurang ke unit yang lebih, terjangkau dan rasional. Tujuan penelitian ini untuk mengetahui implementasi sistem rujukan layanan kesehatan dilihat dari aspek regulasi, sumber daya tenaga kesehatan dan sarana prasarana. Penelitian ini merupakan tinjauan sistematis (Systematic Literature Review). Proses data sintesis dalam penelitian ini dilakukan dengan cara membandingkan 15 literature yang memenuhi penilaian kualitas serta kriteria inklusi dan ekslusi dengan sumber database yang digunakan yaitu, Google Scholar, Pubmed, ScienceDirect. Data diolah dan disajikan dalam bentuk tabulasi. Hasil penelitian menunjukan beberapa negara seperti Indonesia, Iran, Tanzania Utara, Riyadh, Ghana, Zambia dan Hoduras dari artikel yang direview menggunakan sistem rujukan yang dimulai dari tingkat dasar (Primary), tingkat menengah (Secondary) dan tingkat tersier (Tertiary). Dilihat dari ketiga aspek implementasi sistem rujukan layanan kesehatan yang direview menunjukkan adanya permasalahan dalam pengimplementasiannya diantaranya; aspek regulasi ditemukan hanya 2 literature yang membahas regulasi dalam pengimplementasian sistem rujukan, aspek sumber daya tenaga kesehatan ditemukannya ketidaktersediaan tenaga kesehatan di layanan kesehatan sehingga menyebabkan terjadinya rujukan pasien dan aspek sarana prasarana ditemukannya hambatan pada infrastruktur yang tidak memadai sehingga menyebabkan terjadinya rujukanKata Kunci: implementasi, sistem rujukan, layanan kesehatan

Abstract

AbstractSultan Thaha Saifuddin Hospital is heading towards the implementation of SNARS-1. The first phase of accreditation in 2016 uses the 2012 version with only four services (SKP, HPK, KPS, PPI) so that there is no medical record review assessment as in the MIRM 13.4 SNARS-1 assessment element. Currently, Sultan Thaha Saifuddin Hospital must adjust the medical record review with the MIRM standard on SNARS-1. Field Study Practices at the Sultan Thaha Saifuddin Hospital, Tebo Jambi Regency on April 1, 2019 to April 20, 2019 with the aim of knowing the implementation of medical record reviews based on the SNARS-1 accreditation standard. Methods of data collection are carried out by observation and interviews using observation sheets and interview guides. The results of the field study practice are: there is a team responsible for reviewing medical records called the medical record committee; there is an SPO regarding medical record review; review of medical records using an open, closed, and recapitulation review form; There was an increase in the quality of medical records based on the results of reviews between period 1 and period 2, namely from 89.65% to 96.8% on timeliness, an increase from 95.4% to 97.6% in legibility, and an increase from 97, 6% to 99.2% on completeness of medical records. The implementation of reviewing medical records at Sultan Thaha Saifuddin Hospital is in accordance with the applicable SPO. The implementation of the review is also in accordance with the SNARS-1 assessment elements.Keyword: medical record, completness, Accreditation Assessment SNARS-1                                                                AbstrakRSUD Sultan Thaha Saifuddin sedang menuju implementasi SNARS-1. Akreditasi tahap pertama tahun 2016 menggunakan versi 2012 baru sebatas empat pelayanan (SKP, HPK, KPS, PPI) sehingga belum ada penilaian review rekam medis seperti pada elemen penilaian MIRM 13.4 SNARS-1. Saat ini, RSUD Sultan Thaha Saifuddin harus menyesuaikan review rekam medis dengan standar MIRM pada SNARS-1. Praktik Belajar Lapangan dilakukan di RSUD Sultan Thaha Saifuddin Kabupaten Tebo Jambi pada tanggal 01 April 2019 sampai 20 April 2019 dengan tujuan untuk mengetahui pelaksanaan review rekam medis berdasarkan standar akreditasi SNARS-1. Metode pengumpulan data dilakukan dengan observasi dan wawancara menggunakan lembar observasi dan panduan wawancara. Hasil praktik belajar lapangan yaitu: terdapat tim yang bertanggung jawab pada review rekam medis disebut panitia rekam medis; terdapat SPO tentang review rekam medis; review rekam medis menggunakan form review terbuka, tertutup, serta rekapitulasi; terdapat kenaikan kualitas rekam medis berdasarkan hasil review antara periode 1 dan periode 2 yaitu dari 89,65% menjadi 96,8% pada ketepatan waktu, terjadi kenaikan  dari 95,4% menjadi 97,6% pada keterbacaan, serta terjadi kenaikan dari 97,6% menjadi 99,2% pada kelengkapan rekam medis. Pelaksanaan review rekam medis di RSUD Sultan Thaha Saifuddin telah sesuai dengan SPO yang berlaku. Abstract

Latar belakang: Tahun 2015-2019 ditargetkan kepemilikan akta kelahiran anak-anak di Indonesia sebanyak 85% (RPJMN), namun sampai tahun 2016 prosentase pencapaian hanya menyentuh angka 62%. Persyaratan untuk membuat akta kelahiran yaitu Surat Keterangan Kelahiran (SKK) yang dikeluarkan oleh dokter/bidan dan saat ini pembuatannya masih dilakukan secara manual (paper based). Sejalan dengan pengembangan e-iANC yang dikhususkan untuk digitalisasi pencatatan dan pelaporan Antenatal Care (ANC) maka diperlukan adanya pengembangan SKK. Tujuan: Membuat surat keterangan kelahiran secara digital dalam rangka optimalisasi Electronic Integrated Antenatal Care (e-iANC). Metode: Mengunakan metode waterfall dengan tahapan: 1. Perencanaan Sistem, 2. Analisis Sistem, 3. Desain Sistem, 4. Penerapan Sistem. Informan dalam penelitian yaitu founder dan developer e-iANC serta bidan sebagai user e-iANC. Hasil: Kebutuhan data dalam SKK meliputi: nomor KTP, nama, alamat, pekerjaan ibu dan ayah, nama dan jenis kelamin bayi, jam dan tanggal lahir, jenis kelahiran, berat dan panjang bayi, serta nama bidan. Fitur SKK berfungsi untuk menghasilkan SKK digital. Beberapa elemen data dalam SKK digital diakses dari database e-iANC yaitu nomor KTP ibu, alamat dan nama ibu dan ayah. Elemen data lainnya perlu di-entry yaitu: nomor KTP ayah, jam dan tanggal lahir bayi, jenis kelamin, berat dan panjang bayi, serta jenis kelahiran. Kesimpulan: Fitur SKK pada e-iANC diperlukan bidan untuk membuat SKK digital yang dapat mengakselerasi kepemilikan akta kelahiran anak-anak di Indonesia. Abstract

The number of delays in the cause of inpatient medical records in Indonesia is still high, in the range of 45.45% - 100%. Where the highest delay in return is caused by incompleteness in filling out the medical resume, this can potentially cause losses to the hospital due to delayed claim collection. This study aims to analyze the causes of the problem to develop an inpatient medical record control system model using the waterfall model method, including Requirements, Analysis, and Design at Aqidah Hospital, Tangerang City. This case study uses a qualitative approach with in-depth interviews, FGD (Focus Group Discussion), Observation and Document Review of inpatient medical records for October – December 2021. The results showed that the delay in returning inpatient medical records in 2019 was 185 out of 280 (66.10%). In 2020 as many as 280 out of 410 (68.29%), and in 2021 as many as 228 out of 320 (71.25%). ) thus, the highest percentage of delays in returning inpatient medical records will occur in 2021. While the writing of the primary diagnosis only reached the percentage of 1.25%, and the secondary diagnosis only reached 0.93%. For this reason, the researcher proposes a solution to the problem of delaying the return of medical records at Aqidah Hospital to develop a medical record control system model to improve the medical record management process.

Abstract

Aplikasi e-iANC merupakan sebuah aplikasi aplikasi sistem informasi berbasis web yang dapat digunakan oleh bidan dalam manajemen data dan informasi antenatal care, hanya dengan satu kali entry data antenatal dapat mengakses informasi Kesehatan secara individual dan aggregate secara real time. Aplikasi sangat dibutuhkan oleh bidan dalam membantu entry data antenatal dan mempermudah bidan dalam proses pembuatan kohort ibu. Bidan adalah seorang yang telah diakui secara regular dalam program pendidikan kebidanan sebagaimana yang diakui yuridis. Bidan yang mempunyai surat ijin praktek bidan (SIPB) disebut dengan bidan praktek mandiri. saat ini proses pelaporan dan penginputan data di setiap bidan praktek mandiri masih berbasis paper based, sehingga mengakibatkan dokumentasi data menjadi tidak berurutan dan masalah – masalah yang terjadi seperti data pasien yang hilang, tidak akurat dan bidan tidak bisa memprediksi resiko kehamilan yang dialami oleh seorang ibu yang sedang hamil. Dengan adanya aplikasi e-iANC yang telah dikembangkan, maka diharapkan dapat membantu setiap bidan praktek mandiri agar dapat memprediksi resiko kehamilan dan mempermudah pendataan kelahiran dari seorang ibu hamil. Sehingga tim peneliti dari Universitas Esa Unggul mengadakan bimbingan teknis untuk penggunaan aplikasi pada setiap bidan praktek mandiri yang dilaksanakan pada bulan Agustus – November 2021 dengan total peserta bidan praktek mandiri sebanyak 14 orang, hasil dari kegiatan ini adalah setiap bidan praktek mandiri yang diberikan pelatihan dan bimbingan teknis sangat antusias untuk segera memakai aplikasi e-iANC.

Abstract

AbstractThe development of increasingly advanced information technology in various fields has become common in today's digital era. The health sector is no exception. One of the developments in information technology is using information systems in health services in electronic medical records. Electronic medical records are expected to be able to provide benefits in overall health services. However, until now, the application of electronic medical records is still assisted by paper due to limited development costs and users. This study aimed to determine the implementation of electronic medical records in pediatric dental clinics per the five components of electronic medical records: hardware, software, policies, users, and processes. The method used for implementing RME is descriptive, with data collection obtained from observations, interviews, and document review. RME was implemented for one month at the pediatric dental clinic by involving the clinic director, clinic manager, IT officer, dentist, registration officer, and two nurses. The RME application has been integrated with medical support applications. It can be implemented in pediatric dental clinics as patient registration, medical record documentation, patient visit lists, payments, stock of goods and medicines, and clinical reports. However, this system is not entirely electronic, and there are still some manual services such as making drug prescriptions, informed consent, and signing the approval for visits.Keywords: implementation, electronic medical records, electronic medical records, dental clinicsAbstrakPerkembangan teknologi informasi yang semakin maju di berbagai bidang menjadi hal yang biasa terjadi pada era digital saat ini. Tidak terkecuali pada bidang kesehatan, salah satu perkembangan teknologi informasi adalah penggunaan sistem informasi dalam layanan kesehatan yang berbentuk rekam medis elektronik. Rekam medis elektronik diharapkan mampu memberikan manfaat dalam pelayanan kesehatan secara keseluruhan, namun sampai saat ini penerapan rekam medis elektronik masih dibantu dengan kertas karena keterbatasan biaya pengembangan maupun pengguna. Tujuan penelitian ini untuk mengetahui implementasi rekam medis elektronik di klinik gigi anak sesuai dengan kelima komponen dalam rekam medis elektronik yaitu perangkat keras, perangkat lunak, kebijakan, pengguna, dan proses. Metode yang digunakan untuk implementasi RME adalah metode deskriptif dengan pengumpulan data didapatkan dari hasil observasi, wawancara, dan telaah dokumen. Implementasi RME dilakukan selama 1 bulan di klinik gigi anak dengan melibatkan direktur klinik, manajer klinik, 1 orang petugas IT, 1 orang dokter gigi, 1 orang petugas pendaftaran, dan 2 orang perawat. Aplikasi RME sudah terintegrasi dengan aplikasi penunjang medis dan dapat diimplementasikan di klinik gigi anak sebagai pendaftaran pasien, dokumentasi rekam medis, daftar kunjungan pasien, pembayaran, stok barang dan obat-obatan, serta laporan klinik. Namun, sistem ini belum seluruhnya menggunakan elektronik dan masih terdapat beberapa pelayanan yang manual seperti pembuatan resep obat, informed consent, serta tanda tangan persetujuan kunjungan.Kata kunci : implementasi, rekam medis elektronik, rekam medis elektronik, klinik gigi

Abstract

Every country needs to develop Universal Health Coverage (UHC) to promote optimal levels of public health. But in realizing UHC, there must be some problems, one of which is fraud. Based on the Corruption Eradication Commission (KPK) data, potential fraud is detected from 175,774 claims of Advanced Referral Health Facilities (FKRTL) or worth Rp. 440 billion until June 2015. This review article describes the incidence of fraud in health care facilities. Out of a total of 12,736 cases of fraud, readmission occupies the most cases of fraud, which is 4,827 cases or 37.9%.

Abstract

Clinical Coding will be accurate if the proper medical language is used while writing. Clinical coding that is appropriate may help healthcare organizations with things like accurate care, billing for services, strategic planning, and statistical and financial analysis. The purpose of the study, which employed a methodical literature analysis, was to ascertain the link between the clinical coding accuracy in healthcare institutions and the correctness of medical terminology. This study is an organized review of the literature. Google Scholar, Garuda, Neliti, the Indonesian Scientific Journal Database (ISJD), Semantic Scholar, PubMed, and ScienceDirect are the databases that were used. PRISMA (Preferred Reporting Items for Systematic Review and Meta-analysis) is the strategy used for the literature selection. The Reinforcement of Observational investigation Reporting in Epidemiology (STROBE) tool was used to evaluate the caliber of the literature in this investigation. The study's findings show that the average value of clinical coding accuracy in healthcare facilities is 48%, with 14 articles having accuracy levels above or below 50%. The average value of medical terminology accuracy in healthcare facilities is 55%, with 11 articles having accuracy levels below 50. Up to 79% of the study's findings indicated a connection between the precision of clinical coding and the precision of medical language. Comparatively, 21% of respondents claimed there was no connection between clinical coding accuracy and medical terminology correctness.

Abstract

The acceleration of technological developments affects the management of information systems within the hospital, especially in using of Electronic Medical Records (EMR). Behavioral intention to use EMR is the essential factor, the role of user is one of the keys to successful EMR implementation. This study aims to analyze the factors that influence behavioral intention of specialist doctors to use inpatient EMR. Data were obtained through questionnaires using the Likert measuring scale and data analysis was processed using SEM (Structural Equation Modelling). Quantitative analytical research method with cross-sectional design using a total sampling of 144 specialists. Of the total 144 respondents, the majority of respondents were male (59%), aged 40-50 years (30.6%) and had a working period of more than 20 years (25.7%). Hypothesis analysis found that there was an effect of system and information quality on perceived ease of use and usefulness. System quality, perceived ease of use and usefulness influence behavioral intention, while information quality has no effect, but indirectly influences behavioral intention through perceived ease of use and usefulness. Service quality has no effect on perceived ease of use, perceived usefulness, and behavioral intention. Perceived usefulness has the greatest influence on behavioral intentions to use inpatient EMR. As an implication of this study, it was found that it is necessary to optimize the inpatient EMR system in the aspects of reliability and connectivity.
