Abstract

Fenofibrate is a drug that can be used to treat hyperlipidemia where the drug is included in the category of Biopharmaceutical classification system II with poor solubility and high permeability. This causes the need to improve the drug delivery system (DDS) made using the solid lipid nanoparticle (SLN) method. SLN fenofibrate can be made using the high shear homogenization method by determining the formula using Factorial 22 Design Expert 12. The formula is made with a concentration of 0.31-1.25% GMS and 1.25-2% Tween-40, then SLN fenofibrate is made by mixing all ingredients until an emulsion is formed and continued with the SLN critical parameter test. From the test results, the critical parameters of SLN fenofibrate for the particle size of 8 formulas 490; 561; 601; 697; 916; 1040; 1818, and 2410 nm. The results obtained for the polydispersity index, respectively, were 0.02; 0.04; 0.08; 0.30; 0.35; 0.48; 0.51, and 0.65. The zeta potential value of the 8 formulas obtained successive values of 2.8; 3.5; 4.2; 4.8; 5.5; 5.8; 8.1, and 8.8 mV. Calculation of the efficiency of the SLN fenofibrate drug obtained successive values of 77.23; 78.53; 79.51; 80.47; 81.17; 87.38; 87.39, and 87.82%. The SLN method can improve drugs that are included in the Biopharmaceutical classification system class II category with the distribution of test results in the particle size range, and the adsorbed drug is more than 70%.

Abstract

Antibiotic resistance This causes the performance of synthetic antibiotics to decrease, so it is necessary to search for natural antibiotics. Moringa plant (Moringa oleifera Lam.) has antibacterial activity proven to inhibit and kill pathogenic bacteria and Multi-Drug Resistance (MDR)article review aims to collect data related to testing the antibacterial activity of Moringa plants as an effort to overcome antibiotic resistance. The method follows the PRISMA-2020 protocol by searching data on PubMed and ScienceDirect media using the keywords: "Antibacterial activity" and "Moringa oleifera". The data selection uses the Rayyan application by considering the inclusion and exclusion criteria that have been set. The final results obtained are 95 data which are experimental journals of the Randomized Control Trial (RCT) type published in 1981-2021. Moringa plants began to be investigated for their activity against MDR bacteria in 2012 and became a sign that research was already in the direction of finding solutions. Antibacterial testing was carried out in vitro (10 studies) and in vivo (1 study). The leaves have been thoroughly tested for their activity against Methicillin-Resistant Staphylococcus aureus (MRSA). The results of in vitro study obtained the range of zone of inhibition, minimum inhibitory concentration, and minimum kill concentration, respectively: 8.3-12 mm; 0.25-20 mg/ml; 0.50-20 mg/ml study in vivo showed that there was the healing of MRSA infection wounds in test animals.

Abstract

The lean approach can make hospitals become more organized so they can improve the quality of services to patients by reducing waste, including the occurrence of errors, movements, transportation and poor service flow that leads to long service waits which certainly is not expected by patient. The purpose of this study was to identify the waste and the root causes of waste that occurred in the Pharmacy Installation at RSUD X Jombang. This research method is a qualitative descriptive study. The stages of the research are the observation of the drug management process flow which is made in the form of value stream mapping, focus group dicussion, and fishbone diagrams to determine the root cause of the problem. The results showed that the waste in the procurement process of 96.07% included in the category of necessary but non-value added, the acceptance process amounted to 62,59% of activities included in the category of necessary but non-value added, in the storage process there was a waste of 53, 39% included in the category of non value added, and in the process of using waste by 36.22% included in the category of non value added. The root cause of the problem of procurement is the length of time it takes to request the approval of PPK and PPTK, at the reception there is no specific place for receipt of goods, in storing all records and reporting is still manual and in use there is only one prescription receipt and delivery of drugs. In the future state mapping it is known to save time by 45.60% on procurement, 58% on revenue, 53.39% on storage, and 69% on usage.

Abstract

ABSTRACT Matoa leaves (Pometia pinnata) is a plant thought to have hypertensive activity because it contains kuersetin. Research conducted by (Purwidyaningrum, 2017) extracts and fractions of matoa leaves has antihypertensive activity with an effective dose of 150 mg/kg. Other studies conducted by (Elisa, 2019) effects that can reduce blood pressure are extract doses of 300 mg and 30 mg of ethyl acetate fraction induced by angiotensin II can reduce systolic and diastolic blood pressure. This study aims to determine extracts and fractions of matoa leaves in decreasing renin levels and angiotensin II levels. The extraction method was maseration with 96% ethanol solvent and fractionated by liquids method using n-hexane solvent, ethyl acetate water. In this study used 21 male wistar strain rats divided into 7 groups, namely group I (normal), group II as negative control (CMC 1%), group III as positive control (Irbesartan), group IV (matoa leaves extract 60 mg/200g), Group V (fraction n-hexane 2,34 mg/200g), Group VI (ethyl acetate fraction 9,54 mg/200g), Group VII (water fraction 7,98 mg/200g). The results showed ethanol extract and matoa leaves fractions were not significantly difference in meaning with the positive control group different in reducing blood pressure, renin levels and angiotensin II induced by angiotensin II and were significantly from the normal and negative group. The water fraction (7,98 mg/200g) showed a decrease in systolic blood pressure of 9.0% and diastolic 7.1%, the ethyl acetate fraction showed a decrease in renin level 0f 23.6 %. EDM showed a decrease in 17.2% angiotensin II levels. Keyword : Pometia pinnata; renin levels; angiotensin II levels; induced angiotensin II
