Abstract

Patient satisfaction is an essential indicator in assessing the quality of health services, especially in outpatient services at afternoon clinics designed to meet community needs. This study aims to describe the level of patient satisfaction with the services of the Afternoon Clinic at X Regional General Hospital in Semarang Regency, using the HEALTQUAL service quality dimensions. The study used a cross-sectional, quantitative design with 227 respondents selected by proportional sampling from a total population of 1,315 patients during the period January–June 2025. Data were collected through a questionnaire that had been tested for validity and reliability and then analysed descriptively using SPSS. The results showed that the communication dimension received 75.8% strongly agree, the physical evidence 79.3% strongly agree, the efficiency 62.6%–67.8% strongly agree, and the safety 60.8%–70.9% strongly agree. In contrast, the empathy dimension was dominated by strongly agree and agree responses. The conclusion of this study indicates that the services provided by the Afternoon Polyclinic at X Regional General Hospital in Semarang Regency have met the expectations of the majority of patients. It is recommended that the hospital conduct continuous evaluations, particularly in terms of physical facilities and optimisation of technology-based service systems, to maintain and improve patient satisfaction.

Abstract

Introduction: The presence of Escherichia coli strains in freshwater fish like catfish (Clarias gariepinus) populations has caused contamination, particularly in Indonesian aquaculture areas where domestic garbage is the primary source of contamination. In order to determine the appropriate handling of E. coli and fecal Coliform contamination before and after harvesting, this study examined the effects of treatment on safety measures for consumption. Methods: Between March and November 2022, a posttest-only control design was used in 126 catfish samples from Bumiayu, Central Java for an experimental study that was quasi-quantitative. There were two types of feed used during growth, followed by six treatments post-harvest (washing; soaking; fry; grill). The MPN method was employed in microbiology testing. Results and Discussion: There was no significant difference in microbial load variation before harvesting the food, with a pre-harvest feed variation of p > 0.05 (indicating that environmental contamination may have been more important). Thermal treatments that completely eradicated E. coli and fecal coliforms (0 MPN/g) by cooking at high temperature for 4-6 minutes and grilling for 5-7 minutes, were the only alternative to post-harvest treatment. Washing and soaking alone remained ineffective. Conclusion: This study demonstrates that high-temperature processing is a reliable method to ensure microbiological safety of catfish, whereas feed variation before harvest has minimal influence under environmentally contaminated conditions. The findings provide practical guidance on safe cooking time–temperature combinations to prevent E. coli exposure from aquaculture products.

Abstract

Chronic disease management programs (CDMPs) in primary health care aim to improve outcomes and reduce avoidable utilization for people with type 2 diabetes, yet enrollment, engagement, and sustained use vary widely across settings. Objective: To map factors influencing utilization of CDMPs among adults with diabetes in primary care and identify leverage points to improve uptake and sustained participation. Methods: We conducted a scoping review of 12 original studies across diverse health systems, synthesizing patient-, program-, provider-, system-, and technology-level determinants of utilization. Results: Utilization was shaped by insura nce coverage and costs, care access and convenience, service quality, referral processes, provider capacity, health literacy, motivation, social support, and fit with clinic workflows. Evidence from Indonesia’s Prolanis linked program engagement to national insurance membership, primary care use, and glycemic monitoring, with active participation associated with fewer outpatient visits (Putra et al., 2024). Family support and perceived service quality strongly predicted active participation (Cahyaningrum et al., 2024). Technology-enabled programs such as remote patient monitoring (RPM) and telemedicine showed promise for widening access when adequately resourced and integrated, but sites reported variable delivery strategies and implementation barriers (staffing, leadership buy-in, reimbursement, and EHR workflows) (Kirkland et al., 2023). Conclusions: Multi-level, context-adapted strategies are needed to optimize CDMP utilization, including streamlined EHR-enabled referrals, aligned incentives, family-engaged care, provider training, and flexible digital options that fit clinic workflows and patient preferences.

Abstract

Penata anestesi memberikan pelayanan di rumah sakit selama 24 jam sehari. Oleh karena itu, pelayanan penata anestesi merupakan bagian intergral dari pelayanan kesehatan Rumah Sakit serta memberikan kontribusi yang sangat besar dalam menentukan kualitas pelayanan Sentral. Tujuan penelitian menganalisis beban kerja tenaga Penata Anestesi di instalasi bedah sentral (IBS) di rumah sakit pindad turen kota malang pada tahun 2025. Penelitian ini bertujuan untuk mengetahui bagaimana gambaran beban kerja penata anestesi pada preoperatif di rumah sakit pindad turen malang. Metode pengumpulan data menggunakan desain deskriptif dengan pendekatan kualitatif dan rancangan multiple case study. Kemudian hasil dari beban kerja yang didapatkan digunakan untuk perhitungan kebutuhan tenaga penata anestesi dengan metode Workload Indicator Staff Need (WISN). Sampel sebagai subjek personel yang akan diamati sebanyak 50 orang perawat. Hasil Berdasarkan hasil penelitian diketahui Jumlah beban kerja tenaga perawat yaitu (84,48%). Perhitungan kebutuhan tenaga perawat berdasarkan perhitungan WISN di ruang instalasi bedah sentral (IBS) 50 tenaga penata anestesi. Kesimpulan di harapkan kepada pihak manajemen penata anestesi rumah sakit pindad turen kota malang dapat merencanakan jumlah kebutuhan tenaga penata anestesi dengan mempertimbangkan beban kerja penata anestesi.

Abstract

Kesehatan merupakan hak dasar manusia yang harus dijamin negara melalui sistem pelayanan yang adil, merata, dan berkualitas. Penelitian ini bertujuan untuk menganalisis perbandingan transformasi kebijakan sistem kesehatan antara Thailand dan Indonesia dalam upaya mencapai Universal Health Coverage (UHC). Metode yang digunakan adalah kualitatif dengan pendekatan tinjauan pustaka, yang bersumber dari jurnal ilmiah, laporan resmi pemerintah, serta publikasi dari lembaga internasional terkait. Hasil penelitian menunjukkan bahwa Thailand menerapkan reformasi besar-besaran (big bang reform) pada tahun 2002 melalui Universal Coverage Scheme (UCS) dengan pendekatan terpusat, sistem kapitasi, dan dukungan kuat relawan kesehatan desa. Reformasi tersebut berhasil meningkatkan cakupan layanan kesehatan dari 76% menjadi 99,5% dalam satu dekade dan menurunkan beban biaya kesehatan masyarakat secara signifikan. Sebaliknya, Indonesia menempuh pendekatan transformasi bertahap melalui enam pilar transformasi sistem kesehatan nasional yang menekankan penguatan layanan primer, digitalisasi melalui SATUSEHAT, serta peningkatan kompetensi sumber daya manusia kesehatan. Meskipun Jaminan Kesehatan Nasional (JKN) telah mencakup lebih dari 96% penduduk, tantangan masih terdapat pada kesenjangan infrastruktur, ketimpangan distribusi tenaga kesehatan, dan koordinasi lintas sektor. Penelitian ini menyimpulkan bahwa keberhasilan transformasi sistem kesehatan sangat dipengaruhi oleh tata kelola yang efektif, keterlibatan komunitas, serta kesiapan layanan primer dan digitalisasi sebagai fondasi reformasi berkelanjutan menuju pencapaian UHC.

Abstract

This research aims to analyze the implementation of the Youth Posyandu/Integrated Health Post program in Pekalongan city. The research design used qualitative with in-depth interview techniques and data triangulation. The informants consisted of relevant officials, Posyandu cadres, and stakeholders. The results indicate that strategies for increasing youth participation include intensifying education, involving community leaders, creating a youth-friendly atmosphere, and strengthening the role of cadres. The barriers to participation can be resolved through a creative and collaborative approach. Adolescents' health awareness is increased through understanding their needs. Health information was delivered using an attractive digital platform according to teenagers' interests. Overcoming access barriers required stigma-free educational campaigns, supportive environments, and active youth involvement. The positive role of parents and the social environment creates norms that support adolescent participation. A youth-friendly environment was created through participatory design, adequate facilities, and organizing activities by teenagers. Moreover, evaluation of program effectiveness involved measurable performance indicators, comprehensive data analysis, and ongoing feedback to optimize impact on youth knowledge, behavior, and well-being.

Abstract

The use of Artificial Intelligence (AI) in healthcare facilities has begun to be implemented at both the hospital and primary healthcare facilities levels, although it is still limited and simple. This study aims to analyze the understanding of staff, the readiness of staff and organizations, and the potential use of AI to support healthcare services in primary clinics. This is a qualitative study conducted from March to October 2025 at two primary clinics in Semarang City. Informants in this study were clinic managers, doctors, and clinic administrators. Data collection was conducted through in-depth interviews with informants. Data analysis used content analysis techniques. The results show that the basic understanding of staff at the clinics is quite good, but detailed technical instructions and training are still needed. The clinics are quite ready in terms of infrastructure to support the use of AI. The potential for AI utilization is still vast, for example, for early detection, patient education, and other health and support services, if detailed guidelines and limitations are in place to ensure quality and safe healthcare. Staff at primary clinics have begun to utilize AI in several aspects, but still need expansion and more in-depth training to optimize the use of AI to provide effective, efficient, and safe healthcare.

Abstract

This study examines the readiness of public health laboratories in Indonesia's primary healthcare services, focusing on resource distribution, critical result reporting, and health policy implementation. A review of several studies reveals that laboratory readiness is limited by facility constraints, drug availability, and healthcare workers' understanding of critical result handling. Policies like BLUD and lean hospital approaches can enhance operational flexibility in laboratories. Recommendations are directed toward strengthening resource distribution and staff training to improve laboratory service quality in primary healthcare.

Abstract

Top management decisions play a crucial role in shaping hospital risk management. Effective leadership, resource allocation, and a strong safety culture are essential to minimize risks and improve healthcare quality. This study aims to explore the impact of top management decisions on hospital risk management and to identify key factors that influence the success of patient safety programs. This scoping review followed the PRISMA framework. Articles were systematically searched from PubMed, Scopus, and Google Scholar, focusing on publications from 2005 to 2025. Keywords included hospital risk management, top management, decision making. Ten relevant articles were selected and analyzed thematically to identify key patterns related to leadership decisions in hospital risk management. The study found that top management influences hospital risk management through leadership support, resource allocation, communication, safety culture development, and staff training. However, challenges such as limited resources, inadequate staffing, and underreporting of incidents remain. Top management is crucial in ensuring hospital safety through strong leadership, clear communication, sufficient resources, and ongoing staff training to enhance patient safety and risk management. Overcoming organizational barriers is essential for sustaining long-term safety improvements.

Abstract

Hospitals play a strategic role in supporting promotive, preventive, curative, and rehabilitative health efforts. The Emergency Department (ED) and Radiology Unit are critical services that directly affect the speed and accuracy of diagnosis, especially in emergency cases. This study aims to evaluate the efficiency of these two units using a Lean Management approach. A systematic literature review was conducted through databases including Scopus, PubMed, and Google Scholar using the keywords “emergency department,” “radiology,” and “lean management.” Inclusion criteria consisted of English- and Indonesian-language articles published between 2015 and 2024. Article selection followed the PRISMA guidelines. A total of 250 records were initially identified, with 10 high-quality articles ultimately meeting all inclusion and quality appraisal criteria for analysis. Findings indicate that Lean Management effectively identifies process waste, particularly prolonged patient waiting times and delays in radiology reporting. Techniques such as Value Stream Mapping and Root Cause Analysis were found to streamline workflows and reduce non-productive time. The implementation of Lean Management significantly enhances the efficiency of radiology services in emergency settings by optimizing processes and minimizing waste. Continuous outcome monitoring, regular training, multidisciplinary collaboration, and information technology integration are essential to ensure sustainable and system-wide improvements.
