Abstract

This study examines the readiness of public health laboratories in Indonesia's primary healthcare services, focusing on resource distribution, critical result reporting, and health policy implementation. A review of several studies reveals that laboratory readiness is limited by facility constraints, drug availability, and healthcare workers' understanding of critical result handling. Policies like BLUD and lean hospital approaches can enhance operational flexibility in laboratories. Recommendations are directed toward strengthening resource distribution and staff training to improve laboratory service quality in primary healthcare.

Abstract

Hospital is a health service institution that provides comprehensive personal health services, including medical services, medical rehabilitation and maintenance services. These services are carried out through the emergency department, outpatient unit and inpatient unit. Specialists and subspecialist doctors in hospitals provide outpatient services. By reviewing published works in 10 journals from 2015 to 2022, we examine the relationship between BPJS patient satisfaction level and service quality with the keywords service satisfaction journal search, BPJS, and Hospitals. The analytical tool used was PICO. The results of the study found that based on satisfaction indicators, there were seven dissatisfied journals (66.7%) on all the hands described (Tangibility, Reliability, Responsiveness, Assurance and Empathy) from 9 journals (90%) dissatisfied with BPJS services in hospitals. Patient satisfaction will impact the quality of service in a hospital.

Abstract

Background: The Standard Inpatient Class Policy (KRIS) in Indonesia aims to provide equitable healthcare services, ensuring the same standards in inpatient wards across all hospitals in Indonesia. This policy has been issued since 2021 and is mandatory for all hospitals to implement by December 2024. The implementation of the KRIS policy impacts hospitals, necessitating analysis and mitigation of risks arising from its implementation. This study aims to provide an overview of the analysis and mitigation of risks in the standard inpatient class implemented by Hospital X. Method: Qualitative research using Focus Group Discussions (FGDs) in November 2023 involving 7 key informants, including Hospital Directors and Department Heads, regarding the mitigation of risks in the standard inpatient class using risk analysis matrices. Results: There are two criteria for the standard inpatient class that pose a very high level of risk, namely the building components lacking high porosity and the density of ward spaces and bed spacing. Conclusion: Hospital X implements risk mitigation by taking actions to reduce the impact of risks through room improvement based on related room reports, risk reduction mitigation by planning inpatient building renovations, and risk acceptance mitigation by allowing ward areas to be 9.8 square meters per bed.

Abstract

Good control of the supply of drugs and consumables (BMHP) by Hospital Pharmaceutical Installations (IFRS) can avoid shortages or overstocks in pharmaceutical warehouses. The Type C "X" Special Hospital in Semarang City experienced a stockout of IVF drugs and embryo laboratory BMHP which resulted in delays in medical procedures and a decrease in the quality of service. The purpose of this study is to determine the factors that affect the efficiency and effectiveness of pharmaceutical preparations using the MMSL method. This study is a quasi-experimental non-equivalent without control group, samples were taken purposively using retrospective data from October – December 2023 with a stable supply and the research was applied prospectively from January – March 2024. Twelve types of IVF drugs and 56 types of BMHP embryo laboratories were used as research samples. The statistical test used cross-tabulation between efficiency variables (inventory value, inventory turnover ratio [ITOR], stockout value) and effectiveness (number of stockout events). The test results showed that the lower the price of IVF drugs and BMHP in embryo laboratories led to lower inventory values. The more expensive the BMHP price of embryo laboratories results in higher ITOR. The faster the lead time supplier BMHP embryo laboratory, the smaller the stockout will occur. The larger the Smin, Smax, and BMHP safety stock of the embryo laboratory, the lower the incidence of stockout. This study provides an insight into the factors that support the application of MMSL to inventory control in IFRS "X".

Abstract

This study investigates the implementation of clinical pathways (CPs) for pediatric Dengue Hemorrhagic Fever (DHF) inpatients at Hospital X, a Type C hospital in Indonesia, focusing on the role of management in CP development and compliance. Clinical pathways aim to enhance hospital efficiency, quality of care, and patient satisfaction. However, the study identified several issues in CP implementation, including incomplete documentation, lack of socialization and training among staff, and inadequate managerial involvement in CP development and oversight. Data were collected through in-depth interviews with attending physicians, nurses, pharmacy staff, and medical management, along with analysis of 53 pediatric DHF CPs. Results showed that CP compliance rates were below 80%, and the hospital's organizational role in CP development, as assessed by the ICPAT dimension 6 instrument, was insufficient. Key areas of non-compliance included lack of evidence of clinical governance, integration with other hospital initiatives, and adequate support for CP documentation and variation reporting. Recommendations include increasing management involvement, providing comprehensive training for staff, and implementing regular CP evaluations. The findings highlight the need for stronger multidisciplinary collaboration and hospital management support to optimize CPs, ensure compliance, and improve service quality.

Abstract

The emergency department (ED) of a hospital plays a crucial role in managing emergency cases, including initial examination, resuscitation, and stabilization. Timely and precise intervention in the ED is vital, as prompt treatment can reduce mortality by up to 30%. Conversely, delayed response times can lead to permanent disability or death. An emergency is defined as a clinical situation requiring immediate medical action to save lives and prevent disability. In this study, the majority of patients (over 80%) admitted to the ED were victims of traffic accidents, including motorcycle, car, bicycle, and pedestrian incidents, while the remainder involved falls, impacts with objects, and violence-related injuries. This research utilized a total sampling method, where all members of the population were included as samples. The study results indicate that training significantly improves nurse response times in handling traffic accident cases in the ED of Semarang Regency Hospital. Additionally, triage skills have a significant positive impact on nurse response times, as does the work environment. However, the workload was found to have an insignificant positive effect on response times. These findings underscore the importance of training, triage proficiency, and a supportive work environment in enhancing nurse response times in emergency situations, particularly in traffic accident cases. Further research may explore additional factors influencing response times and the potential for improving patient outcomes in emergency settings.

Abstract

Traditional health care remains the community's choice in the midst of the modern health care system. However, their utilization is not optimal due to various factors. This study aims to analyze the influence of these three factors on the utilization of traditional health services in Pekalongan City. This study used a cross-sectional design with a quantitative approach. A sample of 393 respondents was selected using proportional random sampling technique. Data were collected through a structured questionnaire and analyzed using the Chi-square test and multiple logistic regression. The results of the analysis showed that cost and perception variables were related to utilization, but the information exposure factor was not statistically proven to be related. Cost was the largest influencing factor, with respondents who rated the cost of services as affordable being 3.5 times more likely to utilize traditional services. Cost and perception contribute to the utilization of traditional health services. Policies that focus on improving community perceptions and affordability are needed.
