Abstract

Pada tahun 2011-2013 terjadi penurunan cakupan K4 di Kabupaten Sumba Timur dan hanya mencapai 44,7%, jauh dari target 85%. Hasil studi pendahuluan menunjukkan bahwa ibu hamil tidak mengetahui manfaat pelayanan K4 dan mempunyai kesulitan dalam akses pelayanan kesehatan. Penelitian ini bertujuan untuk menganalisis faktor-faktor yang berhubungan dengan pemanfaatan pelayanan K4 oleh ibu hamil. Penelitianobservasional analitik ini menggunakan metode survey dengan desain cross sectional. Subjek penelitian adalah 96 ibu hamil yang dipilih secara multistage random sampling pada tingkat Puskesmas dan Desa. Pengambilan data dilakukan dengan wawancara menggunakan kuesioner terstruktur. Analisis bivariat dengan uji Chi Square dan multivariat dengan regresi logistik. Hasil penelitian menunjukkan 71,8% responden tidak memanfaatkan pelayanan K4, 66,7% pengetahuan kurang, 66,7% persepsi kurang terhadap pelayanan bidan, 52,1% tersedia sarana prasarana, 75,0% sulit untuk mencapai Puskesmas, 56,3% tidak tersedia biaya, 65,6% persepsi terhadap kebutuhan pelayanan K4 masih rendah. Ada hubungan positif antara pengetahuan (p=0,004), persepsi terhadap pelayanan bidan (p=0,004), sarana prasarana (p=0,444), kemudahan mencapai Puskesmas (p=0,001), ketersediaan biaya (p=0,006), persepsi terhadap kebutuhan (p=0,001) dengan pemanfaatan pelayanan K4. Tidak ada hubungan antara ketersediaan sarana prasarana Puskemas dengan pemanfaatan pelayanan K4. Faktor-faktor yang berpengaruh bersama-sama terhadap pemanfaatan pelayanan K4 adalah kemudahan mencapai Puskesmas (Exp-B=4,728, p=0,001) dan persepsi terhadap kebutuhan (Exp-B=4,177 p=0,001). Akses dan persepsi kebutuhan merupakan dua faktor utama dalam pemanfaatan pelayanan K4 di Kabupaten Sumba Timur. Disarankan Dinas Kesehatan untuk membangun Poskesdes di setiap Desa, agar pelayanan K4 lebih dapat dijangkau oleh ibu hamil.

Abstract

The development of online-based information technology in the health sector can improve the quality of hospital services. Currently, Diponegoro National Hospital has implemented outpatient registration based on the "RSND-Ku" android application, and 70% of patients use the application. This study aims to describe the effectiveness and factors that influence the implementation of online registration services for outpatients at the Diponegoro National Hospital Semarang by applying a quantitative method with a descriptive analytic approach to 100 patients who use online registration services aged 20-40 years. The results showed that there is a relationship between the factors that influence the implementation of outpatient online registration services, namely the p-value of system quality of 0.000, the correlation coefficient of 0.460, the p-value of the information quality value of 0.000 and the correlation of 0.472, the p-value of service quality of 0.000 and the correlation of 0.498, and use with a p value of 0.000 and a correlation of 0.441. As for further research, the researcher suggests improvements to optimize the smoothness of the system, completeness of information, updating services, and socializing the flow of online registration services.

Abstract

Sejak pelaksanaan Jaminan Kesehatan Nasional (JKN) beberapa dokter di Rumah sakit mengalami penurunan pendapatan, akibatnya timbul ketidakpuasan dan turunnya motivasi sehingga berakibat pada menurunnya kinerja dokter yang dimanifestasikan  berupa pembatasan jumlah kunjungan pasien, pengalihan  pasien dari RS ke praktek pribadi atau ke rumah sakit lainnya dan manifestasi penurunan kinerja lainnya. Tujuan penelitian ini untuk menganalisis hubungan antara kompensasi, motivasi dan kepuasan kerja dengan kinerja dokter pada BLUD RS dr. Loekmono Hadi Kudus.    Penelitian ini menggunakan metode kuantitaif dengan pendekatan cross sectional. Sumber data dalam penelitian dibagi menjadi dua yaitu sumber data primer dan data sekunder. Jumlah sampel pada penelitian ini sebanyak 49 orang.Analisa penelitian ini menggunakan analisa univariat didapatkan hasil variabel kompensasi kategori tinggi sebesar 53.1 %, variabel kepuasan kerja sebesar 51,0% dan variabel motivasi kerja sebesar 53,1%. Sedangkan hasil analisa bivariate didapatkan hasil bahwa variabel kompensasi, kepuasan kerja, dan motivasi memiliki nilai p value kurang dari 0,05 yang menunjukkan ketiga variabel tersebut berhubungan dengan kinerja dokter dan memiliki p value kurang dari 0,25, sehingga dapat dilanjutkan ke analisa multivariat.  Sedangkan hasil uji multivariate  dengan menggunakan uji regresi logistik didapatkan pada variabel motivasi kerja didapatkan hasil p value =0.000 dengan Exp(B) = 8,155.Kesimpulan penelitian ini adalah motivasi dokter memiliki pengaruh 8,155 kali lebih baik dibandingkan dengan variabel independen lainnya.Penelitian ini merekomendasikan agar pihak manajemen RS memperhatikan sistem pengembangan karir dan promosi, meningkatkan komunikasi interpersonal, memberikan reward pada dokter dan memberikan tanggung jawab sesuai tugas dan kewenangan dokter untuk dapat meningkatkan kepuasan kerja dokter.

Abstract

ABSTRACTThe use of preventive services has an effect on improving health because people who are identified as having symptoms of the disease can be detected early so that they can be anticipated immediately. Preventive services in the Nusantara Sehat program run efficiently, of course, it requires good management.The purpose and scope of the research is to raise the problem of implementing prevention services in the Nusantara Sehat program at the Waitii Health Center, Wakatobi Regency from the management program, namely planning, organizing, implementing, and monitoring the program from Nusantara Sehat on preventive services.The method used in this research is qualitative with case study design, data analysis, triangulation of sources and triangulation of findings. The research subjects were the head of the puskesmas, village head, health officer, person in charge of essential fields and the community. The data collection instrument as a guide for observations and interviews was developed from Terry's (2010) theory.The findings in this study are the management of the preventive service program implemented by the Nusantara Sehat team at the Waitii Health Center, Tomia District, for the organizational aspect, it is carried out in a structured manner starting from the way of working, division of tasks between health workers, involvement and coordination with stakeholders in preventive service activities as well as in creating cooperation with stake holders. In the implementation aspect, a motivation is applied which is reflected in the program vision. In the aspect of supervision, through a good process, starting from the criteria for setting implementation standards, determining the measurement of the implementation of activities, comparing the implementation of activities with analyzing deviations and taking corrective actions.The conclusion in this study is that all aspects of management, both planning, organizing, implementing and supervising proceed well according to Terry's theory.

Abstract

Ketersediaan sumber daya manusia yang sesuai kebutuhan dengan kemampuan dan kualitas, serta professional merupakan indikator keberhasilan farmasi yang efektif dan efisien. Kecukupan tenaga masih kurang dikarenakan peningkatan jumlah resep namun tidak diimbangi jumlah tenaga sehingga meningkatkan beban kerja dan komplain pasien terkait lamanya waktu tunggu obat. Penelitian bertujuan menganalisis kesesuaian beban kerja dengan jumlah SDM dan mengetahui kendala pemenuhan jumlah tenaga.Penelitian dilakukan dengan metode deskriptif kualitatif melalui wawancara dan observasi langsung. Informan utama adalah apoteker dan tenaga teknis kefarmasian. Informan triangulasi adalah kepala seksi penunjang medis,kepala bidang kepegawaian dan direktur rumah sakit.  Analisis data menggunakan rumus WISN berdasarkan data selama observasi, melakukan kategorisasi data, melakukan verifikasi dan menarik kesimpulan.Hasil penelitian menunjukkan instalasi farmasi masih kekurangan SDM diketahui dari hasil perhitungan WISN dan wawancara dengan informan. Beban kerja farmasi belum sesuai karena jumlah pelayanan resep tidak diimbangi dengan jumlah tenaga, SPO pola dan distribusi ketenagaan belum dijalankan dengan baik dan jumlah sarana prasarana belum mencukupi. Pemenuhan tenaga sesuai beban kerja masih memiliki kendala disebabkan efisiensi biaya dan melakukan modifikasi tenaga untuk mengatasi beban kerja yang ada.Penelitian ini merekomendasikan penambahan tenaga disesuaikan beban kerja, menjalankan SPO dengan baik dan melengkapi sarana prasarana untuk meningkatkan pelayanan farmasi.

Abstract

Background: As a form of health promotion, Antenatal Education (AE) class is expected to improve the knowledge, attitudes, and practices of mothers facing their pregnancy. Access to antenatal classes for mothers has not been optimal because of the low level of participation. Not all mothers are willing to join antenatal classes for various reasons.Aim: This study aimed at analyzing the determinant factors that influence the mother’s participation in antenatal classes.Method: This study was a quantitative study with a cross-sectional approach. The population was all pregnant women in Semarang City with 140 pregnant mothers as samples from 3 Primary Healthcare Centers including Rowosari, Purwoyoso, and Gayamsari which were selected by using a purposive sampling technique. Data were collected through an interview using a structured questionnaire.Results: The findings showed that only 54.3% of mothers attended the antenatal classes. Knowledge, support from health personnel, facilities, and infrastructure, as well as a history of illness and pregnancy,  affect mothers’ participation in antenatal classes where the support of health workers had the most dominant influence (OR 5.394). The overall influence was 39.2%.Conclusion: The low mothers’ participation in antenatal classes was influenced by the lack of knowledge and support, inadequate facilities as well as the low health status of mothers. Further socialization and engagement across related sectors with strong networking are needed to solve this problem.Keywords:  antenatal education, participation, primary healthcare center.

Abstract

Background: A strategic plan is the main reference for the management in carrying out activities to be better and more developed according to the customer’s needs. A preliminary study on documents was conducted at the District General Hospital of Dr. (H.C) Ir. Soekarno in Bangka Belitung Province which has implemented a strategic plan. It showed that there were gaps between expectation and implementation of strategic plan.Aim: This study aimed to analyze aspects affecting the strategic plan implementation, including communication, resources, disposition, and bureaucratic structure.Methods: This study was a descriptive study using a qualitative approach. Primary data were obtained through in-depth interview with 10 (ten) informants while secondary data were obtained from document analysis. The data processing and analysis were done using content analysis. The research was conducted from 2 to 31 January 2019 at the District General Hospital of Dr. (H.C) Ir. Soekarno.Results: The findings showed that the implementation process of strategic plans has some gaps. They include (1) ineffective communication, (2) resource factor (lack of quality and quantity of Human Resources, inadequate information, insufficient effectiveness of budget utilization), (3) weak disposition of some implementers, and (4) irrelevant and unadjusted bureaucratic structure.Conclusion: The implementation process of strategic plan has not worked appropriately because some aspects, such as communication, resources, disposition, and bureaucratic structure are not well-provided. This study suggests that formulators should represent all elements. The strategic planning has to be formulated by involving internal and external parties.  Monitoring, evaluation, communication, and coordination among stakeholders have to be carried out. Resources need to be improved, and the hospital needs to develop organization structure and conduct disposition guide for executives and implementors.Keywords: implementation, strategic plan, hospital.

Abstract

Background: Radiotherapy is an important cancer therapy in Indonesia. For hospitals which have provided radiotherapy tools for more than five years, they need to evaluate its utilization and influence on patients’ condition.Aim: This study aims to analyze the use of Linac for radiating breast cancers in one of a type-C private hospital in Central Java by using Health Technology Assessment.Method: This study is an observational and descriptive study with an in-depth interview. There were 72 medical record documents examined. Furthermore, the researchers calculated the profits from the financial feasibility of tool investment gained by the hospital. This study involved six Key Informants and four triangulation informants.Results: This study shows that in terms of effectivity aspect, one Linac can prolong patient waiting time about 2-4 weeks. Such a long waiting time may cause disease progression to increase. Meanwhile, seen from the technical characteristics, the tool is not well-maintained by the internal and external parties. It causes the tool’s performance worse. In terms of the economic aspect, the tool has lasted for 7.5 years, but it technically has been utilized for ten years. Therefore, the hospital needs to supply more radiation tools.Conclusion: The Linac utilization in a year increased, and the ca mammae patient visits were high. In addition to those aspects, the profits gained from the health services were high as well. The hospital should add radiation tools to improve the radiation capacity and decrease patient waiting time.Keywords: linac, economic evaluation, Ca Mammae.

Abstract

Breastfeeding is proven to have long-term health benefits for both mothers and infants. The advancement of mobile technology is very useful in promoting health that can change health behaviors. The success of breast milk is not separated from the methods and media used. The study aimed to develop MIB-Model and to examine the effect of MIB-Model on behavior and self-efficacy of breastfeeding among mothers in providing breast milk. The application development method with the software development Live cycle (SDLC) with the waterfall model. The test model is conducted with Quasi-experiment with pre-test, post-test, and control group design. The number of respondents in this study amounted to 40 0rang, 20 for the experimental group, and 20 for the control group. MIB-Model proved its worth having performed with the ISO 9126 standard and can improve knowledge, attitudes, actions, and the baby's weight to the P-value <0.05. In unpaired data analysis, self-efficacy with P-value> 0.05 showed no differences between the intervention and control, support, and experience of being part of self-efficacy formation. MIB-Model is feasible to be utilized and improved primiparous breastfeeding mothers' behavior and the baby's weight and does not increase the self-efficacy mother

Abstract

Background: Planning becomes the most important management function because it arranges all management activities. Non-compliance with the standards of the planning process has an impact on the quality of planning that is unable to bridge needs which ultimately lead to dissatisfaction. PHC as public organizations must meet public service standards. Organizational factors are determinants of organizational performance as they relate to the provision and arrangement of vaious resourcesAims: The purpose of study was to determine affect of organizational factors on compliance with the standards of PHC planning process.Methods: An observational study with cross sectional design. All 56 PHC in the 2 districts in Central Java province (Semarang and Kudus) became the samples. The respondents are officer in charge of the PHC planning programme.. Data collection by interviews using questionnaires and observations. Data were analyzed descriptively and statistically.Results: Only 55.4% of PHCs comply with standard planning process. Weaknesses and inconsistencies still occur at every stage, especially with regard to inaccuracy of time, incomplete and inappropriate data, and low understanding of members of the team. Study shows that the aspect of manpower, infrastructure, organizing and monitoring-evaluation function are positively correlated with adherence to standard planning processes, where monitoring-evaluation factors are the dominant factor.Conclusion: All the gaps identified as the cause of puskesmas non-compliance to the standard planning process can be overcome through increasing staff commitment and competency by providing structured managerial training and routine socialization. Strengthening the monitoring-evaluation function needs to be pursued consistently to ensure the quality of the puskesmas planning process and the implementation of the work plan produced
