Abstract

Maternal and child health was a priority of health program in Indonesia. Maternal Mortality Rate in Semarang was ranked second highest in Central Java. The highest proportion of maternal deaths occurred during puerperium. That’s indicates, there was a problem in a process of maternal health services during puerperium period in health facilities. This study was conducted to examine constraints on health systems that limit range of interventions or health services that were important for postpartum, bottlenecks related to postpartum services in Public Health Center (PHC), especially infrastructure, human resources, access to PHC, post partum visits (KF1 and KF3), as well as quality of post partum services on risk reduction of maternal mortality. This case study used a qualitative approach. Data collection through interviews to five midwives as main informants, 5 midwives coordinator and 5 heads of PHC as informant triangulation. Data analsyis used content analysis method, then assigned priority bottleneck through MCUA (Multiple Criteria Utility Assessment) techniques. WHO's scale-up BNA plan to analyze bottleneck causes. Results showed, there was a bottleneck on childbirth services in PHC. The causes of bottleneck risk reduction efforts of maternal mortality incidence in puerperium period has never been analyzed workload of health personnel in PHC, lack of monitoring and evaluation of an availability infrastructure facilities in PHC, there has not been regular training, especially on delivery until puerperium services, and PHC has not received more detailed and operational information about puerperium so their maternal knowledge about puerperium has not increased much and couldn’t raise mother's awareness to do so. Semarang Public Health Office (Dinas Kesehatan Kota Semarang) needs to conduct periodic monitoring and evaluation implementation of postpartum services and improve quality of childbirth services in PHC.Keywords: Bottleneck analysis, health services, post partum, Primary Health Care, Puskesmas, Maternal Mortality Rate

Abstract

To improve health development in Jambi Province, it needs to be supported by qualified human resource that can improve performance of employees. The decrease in performance might be caused by no work satisfaction among them. One of the problems faced by Health Office of Jambi Province was decreasing a level of work satisfaction among employees from time to time. Promotion, equity in sharing either financial or non-financial incentive, and sharing tasks equally needed to be done in order improve their work satisfaction. The aim of this study was to figure out the influence of promotion, effectiveness in sharing tasks, and equity in sharing incentive towards work satisfaction of employees at the Health Office of Jambi Province. This was an observational study using cross-sectional approach. Number of population was 277 employees of the Health Office of Jambi Province. Meanwhile, number of samples was 74 persons. Data were collected using a questionnaire and analysed quantitatively using methods of univariate, bivariate (a Chi Square test), and multivariate (a Logistic Regression test). The results of this research showed that more than half of the respondents were satisfied (56.8%), had perception of balanced promotion (50%), had good effectiveness in sharing tasks (55.4%), and felt equal in sharing incentive (51.4%). There were any significant relationships between these three variables and work satisfaction of the employees. The results of multivariate analysis demonstrated that variables of promotion (p=0.015) and equity in sharing incentive (p=0.151) jointly influenced work satisfaction of the employees. As suggestions, employees need to pay more attention to the factors of promotion, effectiveness in sharing tasks, and equity in sharing incentive.

Abstract

Maternal Mortality Rate mostly occurred during postnatal period. Midwives played an important role and had responsibility in providing maternal services. Postnatal services in Blora District had not been optimally implemented. This condition was influenced by motivation of midwives in providing postnatal services. Five of seven midwives said that they were lazy to provide postnatal services particularly in visiting maternal. Motivation was influenced by intrinsic and extrinsic factors. Therefore, factors influencing motivation of midwives in providing maternal services needed to be investigated. This was a quantitative-analytic study using cross sectional approach. Data were collected using a questionnaire. Number of population was 359 midwives in Blora District. Number of samples were 76 respondents calculated using minimal sample size. A validity test used pearson product moment whereas alpha Cronbach test was used to analyse reliability. Data were analysed using methods of univariate (Kolmogorov-Smirnov test), bivariate (Chi-Square test), and multivariate (logistic regression test). The results of this research showed that factors of perception of administration and policy (p=0.043; C=0.227), perception of supervision (p=0.001; C=0.346), perception of work (p=0.000; C=0.503) and perception of environmental condition (p=0.001; C=0.350) statistically significantly related to motivation of midwives. Perception of work was the most influenced factor of motivation (p=0.000; Exp(B)=11.655). As suggestions, other researchers need to investigate the factor of perception of work. District Health Office needs to evaluate main and additional tasks and to consider increasing incentive for activities of maternal services. Indonesian Midwives Association of Blora needs to guide and to improve motivation of midwives in providing maternal services. Health Centres (Coordinator Midwives) need to involve midwives in improving perception of work (maternal services) by conducting training.

Abstract

Based on the medical records of the Banyumanik Hospital, the number of visits in the Banyumanik Hospital dental clinic in 2015 was 766 patients (90%), increasing in 2016 to 781 (91%), but decreased in 2017 to 724 patients (85%) one factor the cause is the quality of service. This study aimed  to analyze the effect of quality outpatient services on patient satisfaction in dental polyclinics in hospitals. Banyumanik Semarang. This was quantitative research with cross sectional design. The number of samples in this study were 54 respondents. Research variables were 5 dimensions of service quality. The results of the study showed the dimensions of the quality of outpatient care in dental poly: Real Evidence had a poor result of 68.5, reliability had poor results of 32 respondents 59.3%, poor responsiveness 40.7%, poor guarantees 59.3%, poor empathy at 55.6%, and less patient satisfaction at 44.4%. In the results of multivariate analysis that the responsiveness variable has the highest significance of 0.039 with the value of Exp (B) 4,566. The conclusion of this study was that the dimensions of service quality namely Real Evidence, Responsiveness, and Empathy affect patient satisfaction, while the guarantee does not affect the satisfaction of outpatients in the Dental Polyclinic Hospital. Abstract

ABSTRAKBPJS kesehatan merupakan perlindungan berupa jaminan sosial untuk melindungi masyarakat Indonesia dari masalah kesehatan. Masalah kesehatan pada masa ini membutuhkan biaya yang besar. Tenaga kerja perlu mendapat perhatian dan perlindungan dari resiko sakit atau kecelakaan kerja yang dapat menyebabkan menurunnya tingkat produktivitas. Oleh karena itu pemerintah didalam UU Jaminan Sosial Tenaga Kerja mewajibkan pengusaha untuk mendaftarkan pekerjanya untuk mengikuti program-program jaminan yang diberlakukan salah satunya program jaminan pemeliharaan kesehatan yang akan bertransformasi kedalam program BPJS. Perkembangan jumlah perusahaan yang beropersi di Kota Mataram dan telah terdaftar pada PT. Jamsostek dari tahun 1993 sampai dengan tahun 2012 sebanyak 1595 unit perusahaan. Masih ada beberapa 101 unit perusahaan yang tidak aktif. Hal ini disebabkan karena masih rendah kepesertaan pekerja formal swasta dari perusahaan menengah, kecil dan mikro di Kota Mataram. Dalam hal ini peserta belum mengetahui manfaat dari jaminan social, tidak mengetahui kepengurusan kepesertaan, iuran yang harus dibayarkan dan pelayanan kesehatan yang diterima sebagai peserta program jaminan social. Hal ini disebabkan karena kurangnya sosialisasi yang dilakukan terkait masalah kepesertaan, iuran dan pelayanan kesehatan yang diperoleh. Tujuan penelitian ini adalah untuk mengetahui factor-faktor yang mempengaruhi kesiapan perusahaan dalam pelaksanaan BPJS. Jenis penelitian survey analitik dengan pendekatan cross sectional. Instrument penelitian menggunakan kuesioner pada 113 unit perusahaan dan pimpinan perusahaan sebagai responden. Data penelitian diolah secara kuantitatif dengan uji univariat, bivariat dan multivariate dengan menggunakan uji analisis regresi linier. Hasil penelitian menunjukkan bahwa responden berpendidikan D3 (27,4%), jenis usaha menurut tingkat pendapatan perusahaan tertinggi usaha micro (64,6%), jenis usaha bila dilihat dari jumlah tenaga kerja termasuk dalam usaha kecil sebesar (90,3%). Berdasarkan pengelompokkan jawaban diperoleh hasil bahwa pengetahuan kurang baik (52,2%), karakteristik inovasi kurang baik (50,4%), saluran komunikasi baik (52,2%), system social kurang baik (54%), promosi agen pembaharuan kurang baik (50,4%) dan kesiapan perusahaan dengan tidak siap (54%). Hasil analisis hubungan menunjukkan ada hubungan antara pengetahuan (p=0,0001, r =0,898 ), karakteristik inovasi (p=0,0001, r = 0,888), saluran komunikasi (p=0,0001, r = 0,886), system social (p=0,0001, r = 0,894) dan promosi agen pembaharuan (p=0,0001, r = 0,704) dengan kesiapan perusahaan. Hasil analisis pengaruh bersama-sama berdasarkan nilai p value d” 0,05, F = 223,404 didapatkan bahwa pengetahuan, karakteristik inovasi, saluran komunikasi dan system social berpengaruh secara bersama-sama terhadap kesiapan perusahaan dalam pelaksanaan BPJS. Disarankan untuk melakukan sosialisasi pada perusahaan. Didalam lingkungan perusahaan penyampaian informasi dengan komunikasi interpersonal. Until 2010, many companies in Mataram City had not been a member of social insurance for workers because there was a lack of socialization in terms of the importance of health insurance for companies’ sustainability. This research aimed to identify factors influencing readiness of companies in implementing Social Insurance Organizing Body.This was analytic survey with cross-sectional approach. Samples were 113 company managers. Data were collected using a questionnaire and analyzed using the methods of univariate, bivariate, and multivariate (Linear Regression test). The result of this research revealed that percentage of respondents graduated from Diploma III was 27.4%, type of business based on the highest income level of a micro business (64.6%), and type of small business based on number of employees (90.3%). In addition, mostly respondents had low knowledge (52.2%), bad characteristics of innovation (50.4%), good communication channel(52.2%), bad social system (54%), bad promotion of reforming agent (50.4%), and unready company (54%). Furthermore, variables of knowledge (p=0.0001, r=0.898), characteristics of innovation (p=0.0001, r=0.888), communication channel (p=0.0001, r=0.886), social system (p=0.0001, r=0.894), and promotion of reforming agent (p=0.0001, r=0.704) had significant correlation with readiness of companies in implementing Social Insurance Organizing Body. Multivariate analysis showed that variables of knowledge, characteristics of innovation, communication channel and social system jointly influenced readiness of companies (F test= 223.404). As a suggestion, dissemination of information needs to be conducted among companies. Additionally, providing information in companies needs to be done by using a method of interpersonal communication. Abstract

Patients who should be able to be treated at the health center (puskesmas) but because of some matters referred to and handled in a hospital, it is a waste of health costs. Hence, BPJS will pay more according to INA CBG's tariff. 3       Advanced Health Facilities, in this case a hospital, is also object to having to deal with non-specialist cases, while the case can actually be completed at the first health facility. Handling non-specialist cases in hospitals leads to health services being less well targeted. The hospital will be very busy handling patients who are not supposed to, while patients who really need treatment in the hospital can be neglected. 2       This study aims to analyze factors which influence the commitment of doctors not to refer to non-specialist cases in health centers in the city of Semarang       The study was conducted with a cross sectional quantitative research design. The data were collected by calculating the total results of the questionnaire measurements on each variable with a Likert scale on 73 doctors in 37 health centers spread across the city of Semarang. The study was conducted during March to May 2018.       The results showed that the number of non-specialist cases of BPJS Health participant patients was still relatively high. This has to do with the commitment of doctors at the health center not to refer to non-specialist cases. The most influence factor to the commitment of doctors not to refer to non-specialist cases was the doctor's perception of the completeness of facilities and infrastructure and the doctor's perception of his competence

Abstract

Traditional services or service by dukun is a phenomenon of community tradition that needs to be evaluated, especially related to birth assisstance. The scope of delivery by health workers was still low of 68% in 2016.This was qualitative research with phenomenological approach. The subjects of this study were maternal mothers who gave birth within the last 1 (one) year selected from the highland region and lowland region in the Uesesi Puskesmas area. Data were collected by in-depth interview techniques. Processing and data analysis using content analysis. Results showed that there were predisposition factors, enabling factors and reinforcing factors which influenced the behavior. The predisposition factors were 1) the informant is able to explain about healthy labor but unable to explain the safe delivery 2) The informant is still more inclined to the service of dukun maternity 3) The high trust of informant to the dukun maternity. The enabling factors were 1) access to utilize the service of dukun maternity is easy because dukun apply method "pick up ball" 2) Cost of labor can be reached by informant. The reinforcing factors were family and some toma support while some other toma leads to no longer utilize the services of dukun maternity.It is advisable to develop strategies to minimize the number of shamans who perform delivery services, provide education and understanding to the community about delivery by health personnel, and to update and conduct regular meetings on issues related to midwifery science so as to create a quality profession.

Abstract

Pecatatan pengobatan Pasien TB paru tentang jadwal pengobatan pasien TB Paru di puskemas Kabupaten Kudus pada lembar catatan pengobatan. Dimana ada lembar yang disimpan petugas dan ada lembar yang dibawa oleh pasien. selain itu, dalam mengirimkan pean petugas harus mencari kontak pasien melihat catatan pasien yang diimpan dalam box file. Dari kondisi tersebut menyebabkan petugas mengalami kendala dalam memantau pasien dalam jadwal pengobatan. Tujuan penelitian ini untuk menghasilkan Sistem Informasi pemantauan pengobatan pasien TB Paru yang dapat digunakan untuk memantau pengobatan pasien, pengiriman pesan pengingat dan menghasilkan rekap laporan mendukung evaluasi Program TB Paru di Kabupaten Kudus.              Jenis penelitian yang digunakan adalah actions research berupa pengembangan sistem (system development). Pengembangan sistem menggunakan metode FAST (Framework for the Application of System Techniques). Subjek penelitian terdiri dari 5 responden yang terdiri dari kepala seksi penanggulanagan penyakit menular, pengelola TB Paru di Dinas Kesehatan dan 3 orang petugas TB Paru Puskemas.            Hasil dari pengembangan ini adalah sitem informasi berbasis web dengan fitur SMS gateway. Dengan sistem ini petugas lebih mudah dalam memantau  pengobatan pasien. dalam list pasien dapat dilihat jadwal pengobatan berdasarkan tanggal yang di inginkan oleh petugas. Dalam pengiriman pesan pengingat lebih mudah dilakukan karena nomor handphone pasien tersimpan dalam basis data sistem. Selain itu rekap laporan pengobatan pasien TB Paru, dan grafik pengobatan pasien berdasarkan  puskesmas.

Abstract

        Pencegahan dan penanggulangan medication error perlu dilakukan untuk menurunkan insiden terutama KTD dan mengoreksi sistem dalam rangka meningkatkan keselamatan pasien . Di RSI NU Demak pada 5 tahun terakhir  2012-2016 masih ditemukan medication error . Oleh karena itu perlu dilakukan penelitian peran dokter dalam pencegahan dan penanggulangan medication error di RSI NU Demak. Desain penelitian ini bersifat deskriptif analitik dengan pendekatan kualitatif. Subjek penelitian secara purposive sebagai informan utama 5 dokter yang memberikan pelayanan di RSI NU Demak, informan triangulasi 2 apoteker, 2 perawat dan 2 keluarga pasien. Data dikumpulkan dengan teknik wawancara mendalam, focus group discussion dan observasi. Pengolahan dan analisis data menggunakan analisis isi.       Hasil survei dari 100 lembar resep didapatkan hasil:  penulisan resep yang sulit terbaca ada 14 lembar resep (14%), dosis kurang tepat ada 75 lembar resep (75%),  polifarmasi) ada 55 lembar resep (55%) , interaksi minor ada 28 lembar resep (28%), dan interaksi mayor ada 1 lembar resep (1%). Hal tersebut dilatarbelakangi oleh :1) Predisposisi factor dari aspek pengetahuan terutama interaksi obat masih ada dokter yang kurang memahaminya, aspek beban kerja era BPJS pasiennya semakin banyak tenaga terbatas; 2) Enabling factor ; komitmen dan ketrampilan dalam pencegahan dan penanggulangan medication error masih kurang 3) Reinforcing factor dari aspek managemen RS perencanaan program belum dilakukan sesuai pedoman, sosialisasi SPO terkait kelengkapan administratif resep, HAM, Polifarmasi dan interaksi obat kepada semua dokter, apoteker dan perawat belum dilakukan sesuai pedoman, pelaksanaan  monev terhadap indikator mutu patieny safety ( medication error) belum dilakukan sesuai pedoman.        Disarankan untuk advokasi dan sosialisasi kepada pengambil kebijakan diikuti deklarasi komitmen bersama dari pihak managemen, dokter, apoteker, perawat untuk mensukseskan program patient safety, pelaksanaan program patient safety dilakukan dengan system pencacatan dan pelaporan,monitoring dan evaluasi agar dapat dilaksanakan sesuai pedoman.

Abstract

ABSTRAKABSTRAK Implementasi Perda Kabupaten HSS Nomor 4 Tahun 2012 tentang KIBBLA belum dilaksanakan secara optimal, hal ini dikarenakan selama kurun waktu 2 tahun sejak disahkannya Perda KIBBLA, DKK HSS hanya melakukan kegiatan-kegiatan yang masih bersifat himbauan saja tanpa didukung peraturan lain di bawah Perda yang lebih mengikat. Tujuan penelitian mengevaluasi seberapa jauh kinerja implementasi Perda Nomor 4 Tahun 2012 tentang KIBBLA di Kabupaten HSS terkait penyusunan outline draft Peraturan Bupati tentang KIBBLA. Desain penelitian kualitatif disajikan secara deskriptif eksploratif, pendekatan menggunakan teknik non probability sampling dengan purposive sampling. Subjek penelitian 4 orang informan utama yaitu Kepala Dinas Kesehatan, Kabid Kesga, Kasi KIA dan Kasi Gizi, total informan triangulasi sebanyak 24 orang, yang terbagi dalam 2 kelompok yaitu 12 orang berasal dari tenaga kesehatan pelaksana Perda KIBBLA dan 12 orang sasaran Perda KIBBLA. Data dikumpulkan dengan wawancara mendalam (indepth interview) FGD dan workshop, pengolahan data dengan metode analisis isi  (content analysis). Hasil penelitian menunjukkan bahwa: 1) evaluasi kinerja implementasi Perda KIBBLA melalui indikator sebagai berikut: a) akses sasaran Perda KIBBLA untuk menjangkau fasilitas dan pelayanan KIBBLA bervariasi tergantung dari letak geografis; b) pelayanan KIBBLA masih belum dapat dijangkau oleh semua kelompok sasaran; c) frekuensi sasaran mendapat pelayanan KIBBLA beragam tergantung dari jenis pemberi pelayanan; d) masih terdapat penyimpangan terhadap penerapan Perda KIBBLA di lapangan; e) pelayanan yang diberikan masih belum sesuai standar; f) akuntabilitas yang dilaksanakan sebatas pada kegiatan rutin; g) keluaran kebijakan yang diterima oleh sasaran masih belum sesuai dengan kebutuhan; 2) telah dilakukan proses identifikasi masalah dan masukan untuk penyusunan Peraturan Bupati dari sasaran dan petugas kesehatan; 3) telah dilakukan perumusan outline draft Perbup, mengenai: wewenang, tanggung jawab Pemda, pelayanan persalinan, jenis tenaga KIBBLA, jenis sarana pelayanan kesehatan dan kualifikasi yang dibutuhkan, tata cara pelaporan, pengaduan, pembentukan unit pengaduan masyarakat dan tata cara verifikasi dan tingkat kesiapan.Kata kunci : Evaluasi kinerja implementasi, Perda KIBBLA, Outline Draft Peraturan Bupati.ABSTRACTThe local regulation Number 4 Year 2012 about health of maternal, infant, baby, and childrenunder 5 years old in District of Hulu Sungai Selatan had not been optimally implemented because it was not supported by other regulation below its regulation since it was approved two years ago. This research aimed to arrange a draft outline of Regent regulation as an operational regulation of the local regulation Number 4 Year 2012 about Health of Maternal, Infant, Baby, and Children under 5 years old in District of Hulu Sungai Selatan in Province of South Borneo. This was qualitative research presented using descriptive-explorative methods. Number of main informants were 4 persons selected using non probability sampling with purposive sampling. They consisted of head of District Health Office, head of family health department, and heads of maternal and child health section and nutritional section. Informants for triangulation purpose were divided into two groups namely 12 health workers and 12 people of target of the local regulation. Data were collected using in-depth interview, FGD, Brainstorming, and workshop. Furthermore, data were analyzed using content analysis.The result of this research showed that: 1) the indicators to evaluate the local regulationimplementation were as follows: a) access to reach service facilities and obtaining the variousservices depended on geographical location; b) the services of Health of Maternal, Infant, Baby, and Children under 5 years old had not been used by all target people; c) frequency of target people who obtained the services varied depending on types of providers; d) there was any irrelevance in implementing the local regulation; e) there was any unstandardized service; f) a form of accountability was only as a routine activity; g) a released policy was not appropriate with necessity of target people. 2) There had been done a process of problem identification and input to arrange the draft outline of the regent regulation from target people and health workers. 3) Some important points that would be written on the draft outline were: authority, local government responsibility, delivery service, types of workers, types of health service means and qualification needed for the services, reporting procedure, complaint, forming a complaint unit and a procedure to verify and level of readiness.Keywords : Performance Evaluation of Implementation, Local Regulation of Health of Maternal, Infant, Baby, and Children under 5 years old, Draft Outline, Regent Regulation
