Abstract

Background: Diabetes mellitus (DM) is a chronic disease that is a global public health problem that has a social impact, economic impact, and quality of life for patients, which leads to increased morbidity and mortality. Uncontrolled blood sugar levels and long-term DM, affecting the pathophysiology of disorders including diabetic retinopathy, heart disease, kidney failure, hyperglycemia and hypoglycemia are needed interventions that can help regulate glycemic (blood sugar levels and HbA1c). Objective: To find out various therapies non-smartphone and smarthphone to regulate glycaemic control (blood glucose and hba1c) in patients with diabetes mellitus. Method: This systematic review was based on preferred reporting items for systematic reviews and meta-analyzes (prisma). The database used in this studied was scopus, proquest and pubmed were limited to the last 5 years of publication from 2015 to 2020 and full-text article. The keywords used were "diabetes mellitus, “glycaemic control”, health education” and “m-health”. This systematic review conist of 15 articles that fit the inclusion criteria. Results: Non-smartphone management in diabetic patients to help regulate glycemic (blood sugar and HbA1c levels) is peer support and booklets, while those using smartphones are mobile application and short message service (SMS). The treatment provided includes 4 pillars that become guidelines, namely education, eating arrangements, exercise, medication adherence given in stages according to the educational media used. Keywords: diabetes mellitus; glycaemic control; health education; m-health ABSTRAK Latar belakang: Diabetes Melitus (DM) adalah penyakit kronis yang menjadi masalah kesehatan publik global yang memberikan dampak sosial,dampak ekonomi, dan kualitas hidup bagi pasien, yang mengarah ke peningkatan morbiditas dan mortalitas. Kadar gula darah yang tidak terkontrol dan lama menderita DM, mempengaruhi patofisiologi terjadinya gangguan diantaranya adalah retinopati diabetik, jantung, gagal ginjal, hiperglikemia dan hipoglikemia adalah sehingga dibutuhkan intervensi yang dapat membantu meregulasi glikemik (kadar gula darah dan HbA1c). Tujuan: Untuk mengetahui berbagai macam tatalaksana diabetes mellitus non smartphone dan smartphone untuk meregulasi kadar glikemik (kadar gula darah dan HbA1c). Metode: Systematic Review ini disusun berdasarkan Preferred Reporting Items for Systematic Reviews and Meta-Analyzes (PRISMA). Database yang digunakan dalam penelitian ini adalah Scopus, Proquest dan Pubmed terbatas untuk publikasi 5 tahun terakhir dari tahun 2015 hingga 2020 dan full text article. Hasil: Tatalaksana non smartphone pada pasien diabetes untuk membantu meregulasi glikemik (kadar gula darah dan HbA1c) adalah peer support dan booklet, sedangkan yang menggunakan smartphone yaitu mobile application dan short message service (SMS). Tatalaksana yang diberikan meliputi 4 pilar yang menjadi pedoman, yaitu edukasi, pengaturan makan, olahraga, kepatuhan pengobatan yang diberikan dengan tahapan-tahapan sesuai media edukasi yang digunakan. Abstract

Melalui kemampuan berempati, perawat dapat menurunkan stereotip dan memberikan evaluasi positif. Tujuan penelitian ini untuk mengetahui pengaruh training komunikasi interpersonal dengan Model of Structural Reflection (MSR) terhadap persepsi keluarga klien di ruang ICCU tentang empati perawat, menggunakan desain pre and post test with control group. Sampel penelitian terdiri atas keluarga klien kelompok perlakuan di ruang ICCU dengan jumlah responden masingmasing kelompok kontrol dan perlakuan sebanyak 48 keluarga klien. Alat ukur menggunakan kuisioner. Data dianalisis menggunakan uji Man-Whitney dan uji regresi multinomial berganda. Berdasarkan hasil analisis diperoleh hasil bahwa training komunikasi interpersonal MSR berpengaruh terhadap persepsi keluarga klien dengan peningkatan persepsi keluarga klien pada kelompok perlakuan dibanding kelompok kontrol sebesar 0,01. Training komunikasi interpersonal efektif dilaksanakan dengan menggunakan metode MSR, karena dengan komunikasi interpersonal yang efektif dapat memberikan dampak meningkatkan kualitas hubungan yang saling memuaskan, adanya pengertian dan empati.

Abstract

Background: Communication is an important element of quality care that can help build a nurse-patient relationship. Good nurse-patient communication has the potential to increase patient understanding of treatment plans and reduce behaviors that lead to poor outcomes. Not appreciating the importance of people-centered communication and improper communication training can result in unsatisfactory communication performance. Objective: Knowing the effect of therapeutic communication training on patient satisfaction Methods: The database used in this systematic review were taken from Scopus, Science direct, Proquest and Pubmed, journals were limited to the 2013-2019 publication year in the area of nursing medicine journals, as well as English journals. This systematic review used 9 articles that fitted the inclusion criteria. Critical appraisal and data extraction were performed independently by two different authors, there was a difference in results, the consensus was opted to solve the differences. Results: From 9 selected articles found that training varies between 50%-80% there was an influence on patient satisfaction. Therapeutic communication training had a positive impact on hospital services. Conclusion: Therapeutic communication training improves skills, encourages, supports, strengthens therapeutic communication, communication training reduces poor nurse interaction behavior and can increase patient satisfaction. Keywords: training; therapeutic communication; patient's satisfaction ABSTRAK Latar belakang: Komunikasi adalah elemen penting dari perawatan yang berkualitas dapat membantu membangun hubungan perawat-pasien. Komunikasi perawat-pasien yang baik memiliki potensi untuk meningkatkan pemahaman pasien tentang rencana perawatan dan mengurangi perilaku yang mengarah pada hasil yang buruk. Kurang menghargai pentingnya komunikasi yang berpusat pada orang dan pelatihan komunikasi yang tidak tepat dapat mengakibatkan kinerja komunikasi yang tidak memuaskan. Tujuan: Mengetahui pengaruh pelatihan komunikasi terapeutik terhadap kepuasan pasien Metode: Database yang digunakan dalam systematic review ini adalah scopus, science direct, proques dan pubmed, journal dibatasi dengan tahun publikasi 2013-2019 dengan area jurnal nursing medicine, serta jurnal berbahasa Inggris. Systematic review ini menggunakan 9 artikel yang sesuai dengan kriteria inklusi. Telaah artikel dan ekstraksi data dilakukan secara terpisah oleh dua peneliti, jika terdapat perbedaan, maka hasil diambil secara consensus. Hasil Dari 9 artikel terpilih ditemukan bahwa pelatihan bervariasi antara50%-80% ada pengaruh terhadap kepuasan pasien. Pelatihan komunikasi terapeutik memberikan dampak yang positif terhadap pelayanan rumah sakit. Simpulan: pelatihan komunikasi terapeutik meningkatkan keterampilan, mendorong, mendukung, memperkuat komunikasi terapeutik, pelatihan komunikasi mengurangi perilaku interaksi perawat yang buruk dan bisa meningkatkan kepuasan pasien Kata kunci: pelatihan; komunikasi terapeutik; kepuasan pasien

Abstract

Diabetes mellitus has now become a major problem in non-communicable diseases whose prevalence continues to increase. Lack of family independence and client non-compliance with the diet program greatly affects blood sugar levels. This study needs to evaluate the effectiveness of family empowerment towards diet compliance and family independence in caring for family members with diabetes mellitus. A systematic review searched five electronic databases (Scopus, sciencedirect, pubmed, research gate, and google scholar) with the last ten years (2011-2021). The quality of the articles used in this study was assessed using the Joanna Briggs Institute Guideline. The prism protocol is also used to review each journal. We found 15 studies out of a total of 5,865 studies that discussed family empowerment in increasing family independence and dietary adherence in family members suffering from diabetes mellitus. Family empowerment is one method that is quite effective in increasing family independence and adherence to diet programs. There are six articles (40%) that state that planned behavior theory is effective in increasing family independence and dietary adherence to family members suffering from diabetes mellitus. From all the articles analyzed, there are several effective theories to empower families to change the behavior of family independence and individual diet compliance, namely the theory of planned behavior. However, this study is not strong enough to provide an assessment that family empowerment based on the theory of planned behavior and empowerment is the best way to increase independence, therefore further research is needed.

Abstract

Pendahuluan: Diabetes melitus tipe 2 (DMT2) merupakan penyakit kronis yang menyebabkan komplikasi berat dan berkontribusi dalam menaikkan angka morbiditas dan mortalitas. Penelitian ini bertujuan untuk mengevaluasi efektivitas mobile health (kesehatan seluler) dalam manajemen diri pasien diabetes mellitus tipe 2. Metode: Sebuah tinjauan sistematis dilakukan pada penelitian ini menggunakan PRISMA. Lima database digunakan dalam penelitian ini yang diterbitkan antara tahun 2016 dan 2020 dan menargetkan pasien dengan DMT2. Hasil: Tiga belas artikel terdaftar dalam penelitian yang dilakukan di negara maju (61,5%). Hasil juga melaporkan tiga jenis intervensi kesehatan seluler, yaitu aplikasi (46,2%), pesan teks (46,2%), dan panggilan telepon (7,6%). Intervensi diberikan oleh perawat dan tim multidisiplin, dengan durasi studi yang paling sering adalah 4-6 bulan (38,5%). Simpulan: Meskipun intervensi yang berbeda memiliki efek yang berbeda pada manajemen diri dan nilai glikemik, aplikasi seluler dan pesan teks berpotensi diimplementasikan di negara maju dan berkembang.

Abstract

Pengetahuan sangat penting dalam mengembangkan diabetes self-management untuk mencegah resiko komplikasi jangka panjang dengan diet yang ketat. Tujuan penelitian ini untuk mengetahui hubungan antara pengetahuan dan diabetes self-management dengan tingkat stress pasien diabetes melitus yang menjalani diet. Penelitian ini menggunakan desain cross-sectional. Jumlah total sampel didapatkan sebesar 106 responden dengan multistage sampling, Kriteria inklusi meliputi pasien DM <5 tahun dengan rentang usia 35-55 tahun, tidak menggunakan insulin injeksi, Pasien DM masih berkeluarga dan tinggal dengan keluarganya. Data didapatkan dengan kuesioner Knowledge of Diabetic Diet Questionnaire, Self-Management Dietary Behaviors Questionnaire, dan tingkat stress menjalani diet. Kemudian dianalisis dengan uji Spearman test. Hasil penelitian menunjukkan bahwa tingkat pengetahuan (p=0,049; r=-0,192) dan diabetes self-management (p=0,000; r=-0,341) memiliki hubungan terhadap tingkat stress saat menjalani diet. Diabetes self-management memiliki hubungan yang sangat kuat dari pada tingkat pengetahuan terhadap tingkat stress pasien diabetes yang menjalani diet. Penelitian selanjutnya diharapkan bisa menggunakan responden dengan wilayah yang lebih luas dan serta mengembangkan intervensi yang lebih baik untuk meningkatkan pengetahuan tentang diet pada pasien diabetes melitus. Kata Kunci: diabetes melitus, diet, self-management, tingkat stres Abstract The correlation between knowledge level and diabetes self-management towards stress level of patients DM with diet. Knowledge is very important in developing diabetes self-management to prevent the risk of long-term complications with a strict diet. The purpose of this study was to determine the relationship between knowledge and diabetes self-management with stress levels of people with diabetes mellitus who went on a diet. This study uses a cross-sectional design. The total samples obtained were 106 respondents with multistage sampling, inclusion criteria included patients with DM <5 years with a range of ages 35-55 years, did not use insulin injection, DM patients were still married and lived with their families. Data are obtained with questionnaire of Knowledge of Diabetic Diet Questionnaire (KDDQ), a Self-Management Dietary Behaviors Questionnaire (SMDBQ), and a stress level on a diet. Data analyzed by the Spearman Rho. The results showed that the level of knowledge (p=0.049; r=-0.192) and diabetes self-management (p=0,000; r=-0,341) had a relationship to stress levels while on a diet. Diabetes self-management has a very strong relationship to the level of knowledge of the stress level of diabetics with diet. Future studies are expected to be able to use respondents with a wider area and develop better interventions to increase knowledge about diets in people with diabetes mellitus. Keywords: diabetes mellitus, diet, self-management, stress level

Abstract

Introduction: Cancer is a chronic disease that affects physical, psychological, social, and economic life of individual and then contributes to life quality. Psychotherapy is important in improving the life quality of cancer patients. One of psychotherapy that can be given is the implementation of ACT (Acceptance and Commitment Therapy). The aim of this study is to analyze the application of ACT in improving the life quality of cancer patients at the Puskesmas Pacarkeling Surabaya. Methods: This research method used pre-experimental with one group pre-post test design. The population of cancer patients enrolled in the Puskesmas Pacar Keling. Samples were 12 people whom are determined based on the inclusion criteria. The Independent variable is application of ACT and the dependent variable is life quality. Data collection and implementation of ACT were conducted in puskemas and home visits. The collected data was analyzed by Paired T test with significance level α=0.05.Results: Result showed that there was change in increasing life quality of cancer patients significantly after giving implementation of the ACT. The result of Paired t-test showed the life quality of patients had a significance level of p=0.000. These result demonstrate the value of p <0.05, so the research hypothesis is accepted.Discussion: It can be concluded that the application of ACT can improve the life quality of cancer patients. Health workers (nurses) need to master the implementation of the ACT as a therapeutic modality.

Abstract

Introduction:Type 2 diabetic patients usually unsuccessful to follow the diet recommendation due to lack of motivation, memory and intention. This study attempts to increase the motivation and also to improve intention in dietary adherence through the implementation of Health Action Process Approach (HAPA).Method: This study was a quasy-experiment. The population were type 2 diabetic patients in Puskesmas Krian Sidoarjo in March-April 2015. Respondents were only 16 and had been divided into experiment and control group. The independent variable was the implementation of HAPA. The dependent variable were self-efficacy, dietary adherence and blood sugar levels. The instruments in this study were questionnaires and blood sugar monitoring devices. Data were analyzed using statistical wilcoxon sign rank test and mann whitney u  test with significance level α ≤ 0.05.Result: Wilcoxon sign rank test showed there were differences between pre and post test significantly on self-efficacy (p=0.014), dietary adherence  (p=0.025), blood sugar levels (p=0.009) in  experiment group, while no significant differences in control group. Mann Witney U test showed that there was significant difference on dietary adherence (p=0.002) between two groups.Discussion: In conclusion, the implementation of HAPA can improve dietary adherence in type 2 diabetic patient. Further, following studies are expected with large number respondents and identify the whole variables in the HAPA theory.

Abstract

Introduction: Diabetes mellitus (DM) is a chronic disease with many complications. Self-efficacy is a psychosocial aspects refer to patient’s belief about their ability to do Diabetes managements. Self-efficacy can be enhanced by providing a mutual support from other Diabetic patients. This study aimed to analyze the influence of peer group support on self-efficacy, glicemic control and self-care activities of Diabetic patients.Method: This research was a pre-experimental with one-group pre-post test design and was evolved 29 respondents. Peer Group Support indicators were checking in, presenting problems, clarifying issues, sharing experiences, action planning and checking out. Implementation of peer group support activity was 60 minutes meeting, once a week for totally 6 weeks. The datas were analyzed by Wilcoxon signed rank test and Spearman’s Rho test with significance values p≤ 0.05.Result: Self efficacy, self care activities, and blood glucose level were increased (p=0.000).Discussion: Peer group support was recommended to enhance self-efficacy and self-management of diabetic patients. Further research is needed about health promotion for improving self-efficacy.

Abstract

Introduction: Living with chronic diseases such as Diabetes mellitus type 2 will make patients experience change or imbalance include biological, psychological, social and spiritual. One of psychology aspects in patients with Diabetes mellitus type 2 is psychological well being (PWB). Emotional response of type 2 DM patients since the early diagnosis to begin undergoing the treatment will be different for each person. Type 2 DM patients need a good transition process to achieve well being state. The transition from a healthy to a diseased condition is needed for the successful self care management of type 2 DM patients. The purpose of this research was to explore the description of PWB in patients of type 2 Diabetes mellitus in six aspects of PWB and PWB facilitate and inhibitor factors in type 2 DM patients.Methods: This research used qualitative design research with case studies approach. The subject of research was seven participants who met the inclusion criteria. Data collection was done by structured interview and observation. Data analysis was done by thematic analysis.Result and Analysis: This study generated 14 themes. The result showed that the process of type 2 DM patients subjected to the process of transition from a healthy condition into ill condition. The transition process started with cyclic lose response which influence type 2 DM patient to self control and make a right decision-making to self care. Self-control would make type 2 DM patients able to adapt and engage with new experiences that become a new habit for type 2 DM patients and will facilitate type 2 DM patients in adapting to the internal and external environment and make type 2 DM patients have a positive hope in their life.Conclusion: finding in this study would hopefully be beneficial for professional health staff to make assessment about PWB in type 2 DM patients, nurse hopefully can assist patients in transition with the condition of type 2 DM. Need further research about the relation between self acceptance and self control, activation process till engagement to new habit in type 2 DM patients, perceived support to PWB in type 2 DM patient DM.
