Abstract

Background: Chronic kidney disease can cause physiological and psychological stress. Adaptation to changes related to disease conditions is an essential component in managing chronic kidney disease. Failure to adapt can decrease the quality of life in patients with chronic kidney disease. This study aimed to identify the factors that affect quality of life and stress adaptation in patients with chronic kidney disease. Methods: Systematic reviews using the PRISMA protocol and the JBI Critical Appraisal Tool were used to determine eligible articles from four electronic databases (PubMed, Google Scholar, Proquest, and DOAJ). Inclusion criteria include patients with chronic kidney disease quality of life, published in Indonesian and English in the last five years (2019 - 2023). Result: 14 articles were obtained according to the criteria. Factors that affect the quality of life in patients with chronic kidney disease are age, comorbidities, socioeconomic status, income, coping, anxiety and depression, family support, and drug use. Positive self-acceptance, self-efficacy, and increased self-management skills can enhance the disease adaptation process, positively impacting patients' quality of life with chronic kidney disease. Conclusion: Patient adaptation to disease processes and management changes must be made to improve treatment outcomes. Nurses can develop nursing interventions based on appropriate stress adaptation models to increase the patient's ability to adapt to disease management to improve the results of care services and ultimately improve the quality of life of patients with chronic kidney disease.

Abstract

Background: Optimal health services are one indicator of success in measuring the quality of health services. Patient-focused health services are a service model implemented in hospitals. This study aims to determine the concept of patient-centered care and its implementation in hospitals. Methods: The method of this study is a literature review. This study used electronic databases such as PubMed, Google Scholar,and Science Direct. Literature review was conducted on international journals published in the last 5 years, available in full text and not the result of a review. The keywords used are “Patient Centered care and hospital. Results: The results showed that patient-focused health services to contribute for improving patient-focused services by helping organizations target quality management efforts and can improve patient adherence in treatment so as to reduce healthcare costs. PCC also contributes to improving the performance of health service professionals and increasing patient and family participation in decision-making regarding the health services to be provided. The PCC method is very effective in palliative care and less effective in emergency care and intensive care units. Conclusion: Patient-centered care includes aspects of caring that respect values, patient needs, patient-based decision making, and patient preferences process of planning, and implementing care. Patient-centered care has advantages which include: reducing the length of stay thereby minimizing the cost of care incurred by the patient and increasing the trusting relationship between the health care team and the patient. The patient-centered care model is also an indicator to measure the level of patient satisfaction with health services.

Abstract

Background: Lung cancer is a disease that is one of the leading causes of death worldwide. Some patients experience psychological problems due to uncertainty in disease management, which can affect the treatment process and outcomes. This study analyzes the uncertainty, impact, and prevention strategies of lung cancer management. Methods: a systematic review using the PRISMA protocol and JBI critical appraisal tool to determine eligible articles. The five electronic databases used (Science Direct, PubMed, Scopus, IEEE Explore, and Google Scholar). The inclusion criteria included research on lung cancer published in English in the last ten years (2013 -2023). Result: The results showed that 11 articles met the inclusion criteria. The review results show several factors related to uncertainty related to lung cancer, including perceived physical symptoms, disease experience, cognitive factors, self-authority, social support, education, adaptation control, uncertainty inference buffering strategies, and the presence of health information. The impact of disease uncertainty is a lack of emotional control, increased stress, lifestyle changes and maladaptive behavior in health management. Nurses can implement prevention strategies by providing precise and comprehensive information, patient-centered palliative care services, improving coping through increasing family support or other social support, using positive emotional experiences of patients, evaluating service systems and planning continuous care programs. Conclusion: Patients' uncertainty in managing lung cancer can negatively impact disease management and disease management outcomes. Nurses can use appropriate strategies to improve adaptive coping abilities must be carried out to prevent adverse impacts and positively impact patient management and treatment outcomes. Abstract

Background: Chronic obstructive pulmonary disease (COPD) is one of the leading causes of morbidity and mortality worldwide. COPD causes various physical disorders, including dyspnea, physical weakness, and psychological disturbances that can impact the quality of life. This study aims to identify patients' quality of life and the factors influencing it. Methods: The method of this study is a systematic review using the PRISMA JBI Critical Appraisal Tool. The literature search used an electronic database of 3 electronic databases, namely Google Scholar, PubMed, and Scopus. The inclusion criteria include research on the quality of life of patients with COPD and articles published in English from 2018 – 2023. Data were analyzed using meta-synthesis. Results: The results of the study found 12 eligible articles. The analysis results show that the dominant factors affecting the quality of life of COPD patients are smoking status, degree of COPD, and depression. Active smoking status, severe COPD, and depression can worsen the quality of life. Family support, self-care management education, and medication adherence can improve quality of life. Family support is central because it can improve compliance and self-care management. Conclusion: Nurses' efforts to improve the quality of life of COPD patients, apart from being curative, can be carried out through preventive and promotive actions, including smoking cessation programs, increasing medication adherence, and improving self-care management through efforts to increase family social support.

Abstract

Background: Surgical safety checklistis a tool or system that is very important in implementing patient safety during surgery, especially in the operating room. One of the patient safety programs in central surgical installation is the application of a surgical safety checklist which is recognized as important by the surgical team in reducing postoperative mortality and morbidity and increasing patient safety. Objective: This study aims to determine compliance with filling out a surgical safety checklist based on the theory of planned behavior in the Central Surgery Installation (CSI). Method: The research method uses a literature review, by using Garuda, PubMed and Science Direct database searches. Selection of literature with manuscripts according to research topics with a cross sectional design. Results: A literature search showed that adherence in filling out the surgical safety checklist increased patient safety in Central Surgical Installation (CSI) programs and reduced surgical mortality worldwide. Conclusion:It can be concluded from this study that there is an analysis of adherence to the application of surgical safety checklists based on the theory of planned behavior for surgical patients in the Central Surgery Installation. Suggestion: The surgical team is expected to comply with and implement Standard Operating Procedures (SOP) by using a surgical safety checklist as an important tool or facility in implementing patient safety, especially in the operating room.

Abstract

Background: The patient mortality rate in the emergency department indicates the quality of patient care. One of the contributing factors is the nurse's response time in providing services to patients. Service delays can cause increased mortality in patients with high-priority categories. The purpose of this research is to identify how the response time of nurses in serving high-priority patients, the impact and causal factors, and strategies to improve nurse response time behavior. Methods: a systematic review using the PRISMA protocol and the JBI Critical Appraisal Tool was used to determine eligible articles. Articles were sourced from four electronic databases (Scopus, PubMed, Science Direct and Google Scholar). The inclusion criteria include research on the response time of nurses to patients in the emergency department, published in English in the last five years (2019 - 2023). Result: 19 articles were obtained according to the criteria. The analysis showed an increased risk of death in patients with the highest priority triage level, and an increase in response time positively impacted reducing the risk of death. Factors influencing response time behavior include age, gender, education level, length of work, knowledge, training, skills, self-efficacy, and work motivation. Conclusion: Efforts to increase response time are one factor in reducing the risk of death in patients with high priority. Strategies to improve nurse response time behavior in emergency services must consider factors influencing behavior to improve emergency services and reduce the risk of death.

Abstract

Background: Hypertension is a non-communicable disease known as The Silent disease. Preavlesni Hypertension in rural areas and in cities has a relatively small comparison number. The factors that cause hypertension in rural areas are different from the factors that cause hypertension in urban areas. The physical activity lifestyle of individuals with hypertension who live in rural areas has a higher life expectancy than individuals with hypertension in urban areas. Hypertension intervention is included in one of the SDGs programs. aims to provide an overview of the factors that cause hypertension and interventions carried out to treat hypertension in rural areas. Methods: scoping review is used with the PRISMA approach. Electronic databases were used such as PubMed, Google Scholar, and Science Direct. A literature review was conducted on international journals published in the last 5 years, available in full text and not the result of a review. Result : This study concludes that the causes of hypertension in rural areas include: alcohol use, occupation, smoking habits, age, zinc intake, sleep duration, exposure intensity to the use of biomass fuels, pesticides, and obesity. Interventions that can be given to people with hypertension who live in rural areas include: physical activity, family support, self-screening for routine blood pressure control, and improving sleep quality. Conclusions: In providing hypertension prevention interventions to people in rural areas, door-to-door visits are also needed to evaluate the success of the interventions that have been given, as well as seek information related to the causal factors that are obstacles to the success of the interventions that have been given Keywords: Hypertension in Rural areas, Hypertension intervention

Abstract

Diabetes mellitus is a chronic metabolic disease that requires continuous medical management and good self-care behavior. Failure in self-management will lead to complications and an increased risk of death. However, most patients do not have good self-management. One strategy to improve self-management is to conduct empowerment-based education, but its effectiveness is still unclearly. This study aims to identify the effectiveness of empowerment interventions in preventing diabetes complications. This study is a systematic review using the PRISMA protocol and JBI Critical Appraisal Tools to determine eligible articles. The articles search from five databases (PubMed, ProQuest, Science Direct, Google Scholar, and ClinicalKey for Nursing). The inclusion criteria were English, published 2012 - 2022, and experimental study. The study result showed that empowerment education could significantly increase knowledge and skills in people with diabetes so that patients can carry out self-management well, including HbA1c management, drug management, physical activity management, and diet management. Effective empowerment education interventions can reduce complications through increasing self-care behavior in diabetics, so educational interventions with empowerment can be used as an effort to prevent complications in people with diabetes mellitus. Abstrak: Diabetes melitus merupakan penyakit metabolik kronik yang membutuhkan tatalaksana medis dan perilaku perawatan diri yang baik secara berkelanjutan. Kegagalan dalam pengelolaan mandiri akan menyebabkan komplikasi dan peningkatan resiko kematian, namun demikian sebagian besar penderita tidak memiliki pengelolaan mandiri dengan baik. Salah satu strategi untuk meningkatkan pengelolaan mandiri yaitu melakukan edukasi berbasis pemberdayaan, tetapi terkait dengan kefektivitasanya belum diketahui dengan pasti. Penelitian ini bertujuan untuk mengidentifikasi efektivitas intervensi pemberdayaan dalam mencegah komplikasi diabetes. Metode penelitian ini adalah systematic review dengan PRISMA protokol dan JBI Critical Appraisal Tools untuk menentukan artikel yang eligible. Database yang digunakan adalah Pubmed, ProQuest, Science Direct, Google Scholar, dan ClinicalKey for Nursing. Studi diterbitkan dalam Bahasa Inggris pada tahun  2012-2022 dengan tipe penelitian eksperimen. Hasil analisis studi menunjukkan  bahwasanya edukasi pemberdayaan secara signifikan dapat meningkatkan pengetahuan dan keterampilan pada penderita diabetes, sehingga pasien mampu melakukan manajemen diri dengan baik meliputi manajemen HbA1c, manajemen obat, manajemen aktivitas fisik, dan manajemen diet. Abstract

Perilaku aktivitas fisik sesuai rekomendasi dapat mencegah terjadinya komplikasi akibat diabetes, serta dapat memperlambat perkembangan komplikasi. Salah satu faktor yang dapat mempengaruhi perilaku aktivitas fisik pada penderita diabetes melitus tipe 2 adalah kondisi sosiodemografi. Tujuan penelitian ini adalah menganalisis pengaruh faktor sosiodemografi terhadap aktivitas fisik pada penderita diabetes tipe 2. Metode penelitian ini adalah analitik observasional dengan pendekatan cross-sectional. Subjek penelitian ini adalah penderita diabetes melitus tipe 2 di Kabupaten Jember, dengan sample size 130 responden menggunakan teknik multistage random sampling. Variabel penelitian ini adalah perilaku aktivitas fisik sebagai variabel dependen dan faktor sosiodemografi sebagai variabel independen yang terdiri dari usia, jenis kelamin, status pernikahan, tingkat penghasilan, tingkat pendidikan, suku bangsa dan tipe keluarga. Data dianalisis secara deskriptif dan statistik dengan uji regresi logistik berganda ordinal (?= 0,05). Hasil penelitian menunjukkan bahwa sebagian besar dari responden mempunyai perilaku aktivitas fisik dalam kategori sedang (49,2%). Hasil uji goodness of fit menunjukkan model sudah fit (p = 0,045). Hasil uji parameter parsial menunjukkan bahwa faktor sosiodemografi yang signifikan mempengaruhi perilaku aktivitas fisik yaitu suku bangsa (p = 0,034) dan tipe keluarga (p = 0,002) dengan R2 = 0,186, yang berarti variabel independen mampu menjelaskan variabel perilaku aktivitas fisik sebesar 18,6%. Upaya peningkatan perilaku aktivitas fisik seyogyanya memperhatikan faktor sosiodemografi terutama suku bangsa dan tipe keluarga untuk meningkatkan pengelolaan mandiri dan kualitas hidup penderita diabetes melitus tipe 2.
