Abstract

Type 2 diabetes is a chronic disease caused by insulin resistance in the body. Diabetes Diet for a client with diabetes mellitus type 2 is used to look at dietary self-care behaviors and control the total cholesterol is a risk factor for cardiovascular disease. The purpose of the study was to analyze the effect of the diet plan for diabetes self-care behaviors and total cholesterol diets client type 2 diabetes mellitus in Puskesmas Rambipuji Jember. The study design was quasi-experimental with equivalent control group pretest posttest design. The treatment group was given treatment (application of diet diabetes mellitus), in the untreated control group. The results showed significant differences in dietary self-care behaviors between the treatment group and the control group, but for total cholesterol was no significant difference between the treatment and control groups. The results of the independent t-test on the variable dietary self-care behaviors scored 2,555 t (p = 0.016), while the value of t -0997 total cholesterol (p = 0328). There is influence of a diet plan to diet diabetes with self care model for self-care behaviors and total cholesterol in type 2 diabetes mellitus client.   Keywords: diabetes mellitus type 2, diabetes diet plan, dietary self-care behavior, total cholesterol

Abstract

Diabetes mellitus type 2  (DM type 2) is chronic disease that needed long term treatment. Diet plays an important role in the management of diabetes, and a suboptimal diet is a commonly identified problem. The objectives of the research are analized factors that related to diet barrier on diabetic patient tipe 2 (individual characteristic, family function and family support). The research is corelational study with 30 outpatient in Wonosari Public Health Center, Bondowoso District.  Patients reported on their perception of family functioning, family support for diet, and barriers to diet self-care.The Result of the study indicated, family functioning and  family support for diet self-care was related to perceived barriers to diet  with negative correlation. (R value = 0,899, double correlation family functioning and family support with perceived barriers to diet).  Adjusted R value = 0,794, indicated family functioning and family support contributed 79,4 % to perceived barriers to diet. F value 0,00 <0,05  on double regression explained that  family functioning and family support,  had influence to perceived barriers to diet. Although individual characterictic have correlation with perceived barriers to diet, but it is not significant (p value > 0,05). The conclusions of the research are family functioning and family support have negative correlation with perceived barriers to diet. As family support for diet increased, perceived barriers to diet decreased. Family approach on diabetic management, especially diet DM education can improve patient self care diabetic management and their family.

Abstract

Introduction: The successful management of Type 2 diabetes mellitus is determined by the role of the family in self-management as a family caregiver. Many factors influence the capability of the family caregivers to carry out diabetes self-management. The purpose of this study was to analyze the factors that influence the family caregiver capability of performing diabetes self-management in people with Type 2 diabetes mellitus.Methods: The study design was an analytical observation using a cross-sectional approach. The sample was 220 family caregivers of people with Type 2 diabetes mellitus in Jember Regency, East Java Province, Indonesia. The multistage random sampling technique was used as the sampling technique. The research instrument was a questionnaire. The questionnaires used in this study were the Diabetes Management Self Efficacy Scale, the Spoken Knowledge in Low Literacy Patients with Diabetes Questionnaire, the Motives for Caregiving Scale, the Spirituality Index of Well-Being, the Sense of Coherence Scale, the Coping Scale, the Hensarling Diabetes Family Support Scale, and the Family Caregiver's Perception of the Role of the Nurse Questionnaire. The data analysis was performed using the Pearson correlation test, the Spearman rank test, and the multiple linear regression test.Results: The results of the Pearson correlation test and Spearman rank test showed that the sociodemographic factors associated with diabetes self management were age (p=0.000), gender (p=0.016), education (p=0.000), income (p=0.000), and kinship (p=0.000). The psychosocial factors associated with diabetes self management were diabetes knowledge (p=0.000), motivation (p=0.000), coping skills (p=0.000), spirituality (p=0.000), family coherence (p=0.000), family support (p=0.000) and the role of the nurses (p=0.000). The multiple linear regression test showed that the factors associated with diabetes self management were diabetes knowledge (β=0.047), motivation (β=0.094), coping (β=0.188), spirituality (β= -0.082), family coherence (β= −0.043), family support (β= 0.296) and the role of the nurses (β= 0.512).Conclusion: Efforts to increase the family caregiver’s capabilities in terms of diabetes self-management should pay attention to the socio-demographic and psychosocial factors to prevent complications and to improve the health status, and quality of life of people with Type 2 diabetes mellitus.

Abstract

Damage to motor nerves in people with diabetes increases the risk of a foot injury. This occurs because the damage results in changes in foot biomechanics, muscle atrophy, deformity, and increased pressure on the feet. So far, the examination of motor nerve damage in people with diabetes in agricultural areas such as the Jember Regency is still rarely touched by primary health services. The purpose of this study was to identify motor nerve damage in people with diabetes in the agriculture area of ​​the Jember Regency. This type of research is exploratory, descriptive, 102 respondents obtained by purposive sampling technique. Inclusion criteria in this study are age> 18 years; do not have DFU active, and did not experience amputation in both legs. Data collection uses instrument modification from MNSI (Michigan Neuropathy Screening Instrument) and MDNS (Michigan Diabetic Neuropathy Score). MNSI using in inspecting for deformity, while MDNS using for forms of physiological reflex damage and muscle strength. The analysis used in univariate and displayed in the frequency distribution. The results showed that the majority of respondents who suffer from DM are experienced in the middle adulthood category (71.6%), with a length of suffering more than five years (95.1%), and have a history of comorbidities (68.6%). The most common form of motor deformity damage was hallux valgus on the right and left legs (38.2%; 26.4%). Forms of motor damage in the form of muscle strength, severe damage to the abduction of the right and left legs (7.8%), and were found right or left toe extensions (1.9%; 2.9%). Motor damage in the form of no physiological reflexes was found in the right and left Quadriceps femoral (2%; 1%), and right or left leg Achilles (1%). The result of total motor damage assessment is that people with diabetes have decreased muscle strength in the right and left extremities (74.5%; 72.5%). This research shows that the majority of people with diabetes in the agricultur area of ​​the Jember Regency suffer motor nerve damage. Therefore, there is a need for preventive measures to prevent the worsening condition of people with diabetes.

Abstract

Diabetes mellitus type 2 is a chronic disease that requires self-management. Self-efficacy of patients with type 2diabetes focuses on improving management education and support services. This study aimed to determine theeffect of diabetes self-management education and support on self-efficacy in patients with type 2 diabetes mellitusin Dr. Soebandi hospital Jember. This type of research was a quasi-experiment using a two-group pre-post-testdesign. The research sample consisted of 30 respondents from 15 controls and 15 consecutive sampling groups.Technique sampling used. Inclusion criteria included 1) diagnosed with type 2 diabetes mellitus; the level ofconsciousness is compos mentis; 2) aged 30–65 years. The statistical test used t-tests dependent and independentt-test. Stages there were 6 sessions. Implementation of 1-4 sessions in the hospital and at home 5–6 sessions patients.The results showed that a score of self-efficacy increased by 17:53 in the treatment group and 4.93 in the controlgroup (p=0.001). The results showed that there was a significant difference between self-efficacy, the treatmentgroup and the control group after administration amounted to 10.495 (p=0.001). The conclusion suggests that thereis a significant influence on the self-efficacy of patients with type 2 diabetes in Dr. Soebandi hospital Jember. It isexpected that this study found is a recommended intervention for health workers to improve the self-efficacy ofpatients with type 2 diabetes so as to improve healthy lifestyles, reduce complications, and improve quality of life.

Abstract

Surgery is one of procedur to cure of the disease with method cut and slice of the body. Almost surgery use general aenesthesia before the procedure began. After general anesthesia procedure, the acumulation of mucus secretion in respiratory tract have occured. The accamulatin of mucus secretion it it cause by intubation endotracheal tube procedure or effect of anesthetic agens itself. The mucus accumulation can lead improving of bacterial colonization, especially Staphylococcus aureus which was a normal flora and might become pathogenic when amount more than usual. Staphylococcus aureus were increasingly more and colonized in respiratory tract, especially in pharix, would getted into the lungs and caused pneumonia nosocomial. One of nurse efforts to protect the patient from pneumonia nosocomial is minimized the colonization of Staphylococcus aureus by implementing effective cough and deep breath. The procedure can do after surgery in post operative phase to help patient removed the excessive mucus secretions, it's mean amount of bacteria that contained in the mucus can minimized. This research is quasy experiment study. The design of the study is posttest only experimental design with non-equivalent control group design. The samples in this study were 20 respondents postoperative patients with general aenesthesia divided into 2 groups (control group and the experimental group). The intervention (effective cough and deep breath procedure) is given to the experimental group. Data were analyzed by independent t-test. The results showed that t value = 4.405 (p value = 0.000 < 0.05), that mean the procedure of effective coughing and deep breathing have effected to reduce the colonization of Staphylococcus aureus in secret of patients post surgery with general aenesthesia in Soebandi Jember Hospital and could have minimized risk of pneumonia nosocomial.

Abstract

Post-operative patients with general anesthesia are highly risk of mucus hypersecretion and respiratory distress as the effect of anesthesia caused by secretions accumulation in oropharynx. One of nursing interventions to decrease accumulation of mucus is by oral hygiene. The purpose of this study was to determine the differences of using chlorhexidine 0.2% and povidone iodine 1% as oral decontamination to the colonization of Staphylo-coccus aureus of post-operative patients in mawar ward at Dr. Abdoer Rahem Hospital Situbondo. This study was a quasy experimental research with non-equivalent control group design. The samples in this study were 20 samples divided into two groups 10 as the treatment group was given chlorhexidine 0.2% and 10 as the control group was  given povidone iodine 1%. Data were analyzed by using Mann-Whitney. The result of Mann-Whitney test showed p value (0.490) > alpha (0.05). However, there was no difference between chlorhexidine 0.2% and povidone iodine 1% as oral decontamination for colonization of Staphylococcus aureus of post-operative patients in mawar ward at Dr. Abdoer Rahem Hospital Situbondo. The conclusion is chlorhexidine 0.2% and povidone iodine 1% can be used as oral decontamination for post-operative patients with general anesthesia.

Abstract

Diabetes diet self-management is an important component in type 2 diabetes mellitus (T2DM) management to improve diabetes outcomes, reduce the risk of complications and disability, improve quality of life, and prevent premature death. The purpose of this study was to describe diabetes diet self-management among people with T2DM. The research method is descriptive-analytic with a cross-sectional design. The subjects were people with T2DM in Jember Regency. The sample size is 180 respondents with multistage random sampling. The instrument is Self-Management Dietary Behaviors Questionnaire. Data were analyzed by descriptive and inferential analysis (Kruskal-Wallis test and Mann-Whitney U test). The results showed that the average age of the respondents was 55.33 ± 6.98 years. The majority of respondents are women (67.8%), married (83.9%), basic education (46.1%), low income level (66.1%), and Madurese (51.7%). The majority of respondents had the ability to self-manage a diabetic diet in the moderate category (61.7%), with an average score of 42.89±9.063. There are differences in the self-management of diabetes diet based on education level, income level, and ethnicity (p=0.002; p=0.004; p=0.001 < =0.05). Meanwhile, based on age, gender, and marital status there was no significant difference (p=0.706; p=0.168; and p=0.217 > =0.05). Interventions for self-management of diabetes diet in people with T2DM should pay attention to sociodemographic conditions, especially education level, income, and ethnicity to improve adherence and diabetes outcomes. Abstract

Introduction:  Nurses have a responsibility to empower the family that is a preventive promotion effort to improve the ability of the family caregiver to manage diabetes. The purpose of this study was to analyze the role of nurses on the ability of family caregivers through family caregiver empowerment. Methods: This study was an analytic observational study with a cross-sectional design. This study was conducted in the Jember Regency, East Java Province of Indonesia, with a sample size of 150 respondents using a multi-stage random sampling technique. The instrument were namely the Role of the Nurse Perception Questionnaire, Psychological Empowerment Scale, and Diabetes Management Self Efficacy Scale. The data analysis used descriptive analysis and inferential analysis with Structural Equation Modeling-Partial Least Square (SEM-PLS). Results: The results showed that nurse's role had a direct effect on family caregiver empowerment by 93.80% (t = 89.558; p = 0.000 <0.005), but did not have a direct effect on the diabetes self-management capabilities (t = 0.508; p = 0,600> 0,005). However, nurse's role has an indirect effect on the diabetes self-management capabilities through family caregiver empowerment of 64.50% (t = 4.036; p = 0,000 <0.005). This path analysis model is fit model and  has an accurate predictive relevance to the existing constructor has a relevant ability to predict (SRMR Value = 0.053 <0.8; Q2=0.331 >0). Conclusion: The nurse's role has a direct influence on family caregiver empowerment and has an indirect effect on the diabetes self-management capabilities through family caregiver empowerment. The efforts to empower family caregivers by nurses through enabling, reinforcing, and supporting as a preventive-promotion intervention need to be improved to enhance the ability of family caregivers in type 2 diabetes management.Keywords: Diabetes Self-Management Capabilities; Family Caregiver Empowerment; Nurse's Role; Type 2 diabetes.

Abstract

Increasing the number of cancer patients need exploration of the impact of cancer and chemotherapy. Care dependency is one impact of chemotherapy treatment and can affect the quality of life. Therefore, the prevalence and factor associated with care dependency of cancer patients undergoing chemotherapy need to be explored. This study aimed to determine the prevalence of care dependency and correlation among depression, hemoglobin level, pain, and fatigue to care dependency of cancer patients undergoing chemotherapy. Cross-sectional study was performed. 388 cancer patient undergoing chemotherapy in Baladhika Husada Hospital Jember, Indonesia was recruited using a consecutive sampling technique. Multivariate logistic regression was used to determine the association..The age of the samples was 51.49 (± 9.71) years old. This study found 29.9% of people have care-dependent conditions. After multivariate analysis, hemoglobin, pain, fatigue, and depression were statistically significant with care dependency. Conclusion: Fatigue has the most significant influence on care dependency of cancer patients undergoing chemotherapy. Further research needs to be analyzed care dependency in cancer patients using multi-center chemotherapy service. Also, the development of nursing interventions to reduce care-dependent conditions needs to be conducted.
