Abstract

Diabetes mellitus negatively affects bone tissue and unfavourably impacts bone mineral density (BMD), therfore increasing the probability of fractures through pathological mechanisms. This study aimed to explore the factors associated with BMD and osteoporosis risk in individuals with a diagnosis of type 2 diabetes mellitus (T2DM). An investigation was undertaken involving 49 patients diagnosed with T2DM who fulfilled the specified criteria. These individuals underwent BMD assessment as part of a cross-sectional study. The analysis encompassed both univariate and bivariate approaches, utilizing the Chi-square test (X2) and binary logistic regression methods. A total of 30 participants (61%) have decreased BMD. Among the participants aged 60 years and above, 83.4% exhibited a decreased BMD status (osteopenia and osteoporosis), in contrast to the under 60 years age group, in which 40% displayed decreased BMD status. Older age (>60 years) is a risk factor for decreasing bone density onset of diabetes mellitus (DM) diagnosis, duration of DM, glycemic control, body mass index (BMI), use of thiazolidinediones (TZD) drugs, kidney function, were not associated with lower BMD in T2DM patients.

Abstract

Perinatal Mental Health (PMH) is a mental health disorder during pregnancy and postpartum. Untreated PMH will have serious consequences for mothers, children, their families, and society as a whole. This study utilized the Systematic review method by analyzing articles from PubMed, Scopus, and Science Direct as a database with a range of publications between 2018-2023 and using postpartum keywords of risk factors and mental health. Twelve articles met the inclusion criteria. Based on these articles analyzed the findings reveal that risk factors influencing the occurrence of PMH in postpartum mothers are violent factors (physical, psychological, and sexual violence), type of childbirth, history of childhood trauma, history of PMH during pregnancy, social factors (husband, family, community support), economy, demographics (age, marital status, education, family type, number of children), breastfeeding satisfaction, obstetric history, medical illness, food insecurity, and life-threatening events. Based on these risk factors, proactive efforts are required to provide an early understanding of interventions to pregnant women related to PMH to prevent it from occurring in postpartum mothers

Abstract

The Law on Marriage has been amended, but the prevalence of child marriage has not been reduced significantly. Grobogan is one of the areas with the highest prevalence of married and pregnant women at child age (less than 19 years) in Central Java, and is always above the national prevalence. During 2018 the prevalence of cases reached 59.88%, in 2019 it was 51.24%, in 2020 it was 52.81%, in 2021 it was 54.33%, and in 2022 it was 52.15%. Marriage of women at child age becomessocial phenomena that often occur in society. This research is an analytical survey with a cross-sectional design. The research uses a purposive sampling technique, namelyof all women who married at the age of less than 19 years in Grobogan district in the last 5 years (2018-2022), namely 2,676 cases. Data were analyzed to describe trends in cases and motives for marriage between women as children, as well as bivariate analysis using the Chi-square test to analyze the relationship between parents' marital history and children's marriages. The results of the research are that cases of child marriage (less than 19 years old) are more common among girls. Women are the ones who mostly marry under the age of 19. The most common cause (76.8%) underlying applications for marriage dispensation is dating behavior. Based on the pattern of child marriage couples that occurred in the last 5 years in Grobogan district, the most frequent marriages were women aged children (less than 19 years) marrying men aged adults (more than 19 years), namely 82.5%, there were 15.5 % of marriages that occurred between women and men who were both under 19 years of age, there were 1.8% of marriages that occurred between adult women (over 19 years) and men under 19 years of age, and 3 couples (0 .1%) married when both were 19 years old. There is a significant relationship between mothers' marital history and child marriages that occur in their daughters (p value 0.0002). This means that mothers who marry at a young age (less than 19 years) influence the occurrence of child marriage in their daughters.

Abstract

Covid-19 for three years has become an inseparable part of the lives of urban and rural communities. The impact occurs, such as the impact of Covid-19 on children. Children are a vulnerable part of society during a pandemic, about the conditions for their health, education, and protection. The role of the city in carrying out its duties as a public servant is important, where all stakeholders have an important role. Central Java, one of the provinces affected by Covid-19 has responded well with its adaptive programs. This research was conducted with a quantitative approach with a quantitative descriptive method, starting with an analysis of the role of stakeholders and programs related to Covid-19 prevention for children. The results of the analysis are continued by looking at the spatial pattern between cities and districts. Research data is taken from primary surveys through FGD and document review. The results of the study identify that not all city/district areas have a response to child protection during the Covid-19 pandemic, urban areas are more responsive to this condition. Increasing the role of protection for children during a pandemic is an important part of the process of returning to the new normal.

Abstract

Introduction: Indonesian Ministry of Health has emphasized the need to effectively identify and respond to emergent threats (ET) to maternal and newborn health (MNH), including non-communicable and infectious diseases. Improved ET screening, surveillance, and data management will reduce poor MNH outcomes including mortality, stillbirth, prematurity, and low birthweight. This scoping review examines practices of surveillance of ET to MNH, both globally and in Indonesia. Methods: Fifty-nine articles published between 2011 to 2024, sourced from PubMed and Google Scholar were reviewed. Thirteen articles were included in the final scoping review. The key search terms included “emergent threat surveillance”, “maternal and newborn health”, “MNH surveillance”, “PWS-KIA”, and “SatuSehat”.Results: The review identified numerous gaps in MNH surveillance, including a need for comprehensive data sharing between health facility, district, provincial, and national levels. Additionally, midwives reported that the current MNH monitoring system, PWS-KIA (Local Area Monitoring Health of Mother and Child), is time-consuming and confusing, leading to poor data reporting. Finally, due to limited training of health workers, there is a lack of data analyzation at each level, leading to poor response. Conclusion: An integrated, standardized, and user-friendly system is needed to detect and mitigate emergent threat to maternal and neonatal health. Data analysis and response to adequately identify and reduce emergent threat at each level is critical.  Routine training is needed to improve health provider knowledge on data interpretation and use.

Abstract

Electronic Medical Record (EMR) revolutionize patient information management in healthcare but pose challenges for nurses, who play a crucial role in their utilization. Despite perceived benefits, nurses face significant mental burdens navigating these systems, requiring tailored strategies for successful integration. This article explores nurses' challenges with EMRs and proposes solutions to alleviate their burden while ensuring efficient patient care. This study employed a systematic literature review following the PRISMA method. Conducting searches across databases Scopus and PubMed using keywords “electronic medical record” and “nurse”. Total of 22 selected articles show that Nurses' cognitive challenges with EMRs in hospitals stem from design complexities, impacting workflow and decision-making, exacerbated by factors like experience levels and external events like the COVID-19 pandemic, while usability, system quality, and participatory design approaches can mitigate stressors and enhance nurses' engagement and efficiency. Nurses' EMR experiences impact their well-being and patient care. Addressing these challenges requires optimizing design, usability, training, and support. Supporting nurses in EMR use enhances patient care and well-being, necessitating ongoing research and collaboration for improvement.

Abstract

ntroduction: Dehydration can affect body functions related to temperature regulation and blood circulation. Persistent dehydration can lead to fatigue, impaired concentration, and kidney dysfunction. Workers in labor-intensive environments, such as the brickmaking industry, are at a higher risk due to strenuous physical activity and prolonged exposure to high temperatures. Direct sunlight and the brick-firing process further increase the risk of fluid loss, highlighting the need to assess hydration status among brickmaking workers. Methods: This study employed a cross-sectional design involving 105 brickmaking workers. Individual characteristic factors, worker habit factors, work factors and work environment factors as independent variables, while the dependent variable was hydration status. Hydration status was measured using reagent strips for urinalysis, and other variables were collected through structured interviews using a questionnaire. Data collection was conducted over a period of 14 days. Data were analyzed using the Spearman rank correlation test and multiple linear regression to identify the most influential factors. Results: The majority of study subjects were severely dehydrated, as indicated by Urine Specific Gravity (USG) measurements ranging from 1021 to 1030. The highest distribution was found in the USG range of 1026–1030, with a percentage of 36.2%. Significant factors associated with hydration status included water consumption (p = 0.000), physical workload (p = 0.000), and clothing use (p = 0.005). Other factors, such as age, nutritional status, work duration, years of experience, and heat stress, did not show a significant relationship with hydration status. Multiple linear regression analysis revealed that water consumption had the most significant influence on hydration status (p=0,000). Conclusion: Water consumption significantly impacts hydration status. These findings highlight the need for hydration education and workplace interventions, such as accessible drinking water and awareness programs, to enhance worker safety and health in brickmaking environments.
