Abstract

Background: Pre-marital period is the best time to prepare for the quality of family health; however, In Indonesia, examinations and promotion of reproductive health and pre-marital counseling have yet to be carried out in all districts. Objectives: This study aimed to identify the description of reproductive health services and marriage counseling in Grobogan Regency, Central Java. Methods: This study used an assessment method of the implementation of the Reproductive Health Service Program for the bride, which includes IEC (Communication, Information, and Education) and medical examination of the bride and groom by health workers conducted in community health centers (Puskesmas) and other health care facilities, and office of religious affair (KUA), with the In-depth Interview Method. The subject was the KUA Officer under the Department of Religious Islamic Education in the Marriage Guidance program. Results: The interviews with KUA officers obtained results in 2017. There were still underage marriages, and the number increased in 2018. Prospective brides have submitted forms (N1, N2, N3, and N4) to KUA. Most of the brides were not physically examined (pulse, breathing frequency, blood pressure, body temperature) by health workers (63.7%), and blood groups were not examined (91.4%). Half of the respondents examined blood sugar, HIV, IMS, Hepatitis, and TORCH (56.8%). The bridegrooms were not examined for mid-upper arm circumferences (MUAC) (17.2%) and for signs of anemia (8.6%). The brides were not immunized against Hepatitis B (72.4%). Almost all brides get TT immunization at the Puskesmas (94.8%). Health workers stated that only some brides had been getting counseling about the bride's mental health (29,3%). Only 17.2% of the officers stated that the bride still needs to get a certificate of following the reproductive health IEC. Conclusions: KUA has conducted pre-marriage courses for couples according to the rules of the Ministry of Religion. Some PUSKESMAS had carried out activities for the bride and groom on a scheduled basis (for example, every Tuesday/Wednesday) but have not been carried out routinely, even stopped due to budgetary problems. The certificate is given after the bride and groom take the marriage guidance course, but it has yet to be required for marriage registration.

Abstract

Surveillance and Response Maternal Perinatal Audit (AMP-SR) is a new guideline that is used as a reference for District/City, Provincial and National Health Offices in improving the quality of Maternal Perinatal Audit (AMP) in Indonesia, but the guideline has not worked well so it has an impact in the increase in cases of maternal mortality in Indonesia over the last three years from 87.93/100,000 Live Births (LB) in 2019 to 166.5/100,000 Live Births in 2021. Objective is to analyze evidence of AMP-SR implementation in districts/cities in Indonesia through scoping review. Articles were selected from six databases: Pubmed, Clinicalkey, Scopus, Springerlink, Science Direct, and Garuda with the inclusion criteria of original articles, full text, 2012-2022, Indonesian and English, research in Indonesia, qualitative, and relevant to research purposes. The keywords used are maternal death, policy, implementation, audit, Indonesia. Article screening was carried out with PRISMA. The SPIDER method was used to construct research questions.Of the 499 articles found, only seven articles met the inclusion criteria with a high eligibility value. Seven articles discuss the implementation of AMP-SR in Regencies/Cities in Indonesia with optimal Identification 57.14%, not optimal 42.86%. Reporting is optimal 28.57%, not optimal. The optimal review is 14.29%, not yet optimal 85.71%. The optimal response is 28.57%, not yet optimal 71.43%. Most of the implementation of AMP-SR in districts/cities in Indonesia is not optimal and is still constrained, so it is necessary to strengthen the role and commitment of health workers in implementing AMP-SR so that it is better and more comprehensive. Keywords:  Audit,  Implementation, Maternal Death, Policy

Abstract

The pandemic situation led decreasing in the number of patient visits at Public Health Centers (PHC) due to concerns about contracting Covid-19, including in Demak District which experienced a decrease in visits to 71.7% from 141.2% in 2019. The lack of usage of health services contributed to the poor community health status. The purpose of the study was to analyze the influence of various factors regarding the intention of revisits health services at PHC in Demak District during the Covid-19 pandemic. This is quantitative research with a cross-sectional approach. The population were patients who visited PHC in Demak District. Samples determined by the purposive sampling technique amounted to 332 people. Partial effect test with simple linear regression and simultaneously with multiple linear regression because classical assumption test requirements were met. A total of 57.5% of respondents have a high intention to revisit health services. Partially it’s proven that knowledge, health workers' ability, infrastructure, service quality and waiting time affected intentions to revisit health services in PHC. Variables of waiting time, infrastructure, health workers' ability, and service quality were predictors for revisits with an overall effect of 27.7%. Health centres need to develop an online queuing mechanism to reduce waiting times and gradually equip infrastructure and improve service quality through health workers' discipline.Abstrak: Situasi pandemi menyebabkan penurunan jumlah kunjungan pasien di puskesmas karena kekuatiran tertular Covid-19, termasuk di Kabupaten Demak yang mengalami penurunan kunjungan menjadi 71,7% dari 141,2% di tahun 2019. Rendahnya perilaku pemanfaatan layanan kesehatan berkontribusi pada status kesehatan masyarakat yang tidak optimal. Tujuan penelitian menganalisis pengaruh berbagai faktor terhadap niat kunjungan ulang di puskesmas Kabupaten Demak selama pandemi Covid-19. Merupakan penelitian kuantitatif dengan pendekatan cross sectional. Populasi penelitian yaitu seluruh pasien yang berkunjung di puskesmas di Kabupaten Demak. Sampel ditentukan dengan teknik purposive sampling berjumlah 332 orang. Uji pengaruh secara parsial dengan regresi linier sederhana dan simultan dengan regresi linier berganda karena persyaratan uji asumsi klasik terpenuhi. Sebanyak 57,5% responden memiliki niat tinggi melakukan kunjungan ulang. Secara parsial terbukti pengetahuan, kemampuan tenaga kesehatan, sarana prasarana, kualitas layanan dan waktu tunggu mempengaruhi niat kunjungan ulang di puskesmas. Waktu tunggu, sarana prasarana, kemampuan tenaga kesehatan dan kualitas pelayanan menjadi prediktor untuk niat kunjungan ulang dengan pengaruh keseluruhan sebesar 27,7%. Puskesmas perlu mengembangkan mekanisme antrian online untuk mengurangi waktu tunggu dan secara bertahap melengkapi sarana prasarana dan meningkatkan kualitas layanan melalui kedisiplinan tenaga kesehatan.

Abstract

Angka kematian ibu (AKI) hingga saat ini masih menjadi permasalahan utama di Kabupaten Bangkalan bahkan Indonesia, AKI salah satunya disebabkan karena terjadinya preeklamsia saat periode kehamilan. Penelitian ini bertujuan untuk melakukan pengembangan sistem informasi ibu hamil sebagai upaya deteksi dini kejadian preeklamsia di Kabupaten Bangkalan. Proses pengembangan sistem informasi metode pengembangan waterfall. Proses pengujian sistem informasi dilakukan menggunakan pengujian black-box. Subjek peneltian terdiri dari bidan yang melaksanakan praktek mandiri bidan (PMB) serta 1 orang bidan koordinator. Data kualitatif diperoleh dengan menggunakan wawancara dan brainstorming sebagai bahan analisis kebutuhan user. Hasil penelitian di proses dalam bentuk flowchart sistem, context diagram, data flow diagram, entity relationship diagram. Desain di proses kedalam bahasa pemrograman PHP dan dikombinasikan dengan database MySQL. Dari hasil penelitian tersebut sistem informasi memiliki beberapa fitur diantaranya; fitur login, fitur pencatatan ibu hamil, fitur admin, fitur home, fitur deteksi dini preeklampsia, fitur catatan hasil pemeriksaan ibu hamil dan fitur laporan. Abstract

Based on a survey by the Central Bureau of Statistics in 2012, 48 out of 1,000 pregnancies occurred in adolescents aged 15-19 years. The occurrence of pregnancy at a young age & sexually transmitted diseases are the impact of having sex before marriage in adolescents. This study aims to determine the relationship between several factors with the practice of having sex for the first time and the age at first having sexual intercourse in young women in North Sumatra. The type of research used is explanatory research with a cross sectional study design. Research sampling is based on the research population, which means that all populations are samples. The sample obtained is 643 young women aged 15-24 years. The data used in this study is secondary data from the 2017 IDHS. This study found that the majority of young women aged 15-24 years in North Sumatra had never had sex, and for young women who had had sexual intercourse, it was found more in the age group ≤20 years than in the > 20 year age group. Factors that are not related to the practice of having sexual relations with young women for the first time in North Sumatra are education level (p-value = 0.089), economic status (p-value = 0.096), employment status (p-value = 0.138), and number of accesses. mass media (p-value = 0.102) and the intensity of mass media (p-value = 0.091). Factors related to the age of first sexual intercourse among young women in North Sumatra were education level (p-value=0.000) and economic status (p-value=0.007). The conclusion of this study is that most of the young women aged 15-24 years in North Sumatra have never had sex, but for the young women who have had sexual intercourse, it is found more in the age group ≤ 20 years.

Abstract

Penelitian ini mengevaluasi salah satu sistem informasi di bidang kesehatan yaitu sistem informasi manajemen Puskesmas dengan menggunakan model HOT-Fit. Metode HOT-Fit dipilih karena memberikan interpretasi penilaian yang komprehensif melalui pendekatan terhadap komponen inti suatu sistem informasi. Metode HOT-Fit mencakup: Manusia (Pengguna Sistem dan Kepuasan Pengguna), Organisasi (Struktur Organisasi dan Lingkungan Organisasi), Teknologi (Kualitas Sistem, Kualitas Informasi, dan Kualitas Layanan), serta Manfaat. Hasil penelitian menunjukkan bahwa terdapat pengaruh yang signifikan antara faktor Manusia dengan Manfaat (Sig. 0,014; Odds ratio 11,075 yang berarti faktor Manusia akan meningkatkan faktor Manfaat SIMPUS sebanyak 11,075 kali lebih tinggi). Tidak ada pengaruh yang signifikan antara Organisasi dengan Manfaat (Sig. 0,267). Tidak ada pengaruh yang signifikan antara Teknologi dengan Manfaat (Sig. 0,753).

Abstract

Penyakit tidak menular (PTM) menjadi salah satu penyebab kematian tertinggi pada semua kelompok usia di seluruh dunia dimana dari tahun ke tahun kasusnya makin meningkat. Pemerintah Indonesia melalui Permenkes No 71 tahun 2015 melakukan upaya pencegahan dan pengendalian PTM dengan melibatkan masyarakat melalui kegiatan pos pembinaan terpadu (posbindu) PTM. Kader kesehatan menjadi pelaksana kegiatan pendokumentasian hasil layanan saat pelaksanaan posbindu PTM sehingga dapat diketahui riwayat kesehatan masyarakat. Namun pencatatan dan dokumentasi hasil layanan Posbindu PTM oleh kader belum dapat dilakukan sesuai standar dan belum tersedia alat ukur untuk menilai kemampuan kader sehingga perlu pengembangan instrumen. Penelitian ini bertujuan untuk mengembangkan butir-butir pertanyaan kuesioner pengetahuan, motivasi, dan praktik dokumentasi hasil layanan Posbindu PTM. Kuesioner diuji cobakan kepada 30 kader aktif posbindu PTM di wilayah kerja puskesmas. Butir pertanyaan diuji validitasnya menggunakan uji validitas konstruk dan dilanjutkan uji reliabilitas Cronbach alpha. Sebanyak 31 pertanyaan pengetahuan, 19 butir pertanyaan motivasi, dan 11 butir pertanyaan praktik dinyatakan valid (p-value0,80). Melalui penelitian ini didapatkan butir-butir pertanyaan pengetahuan, motivasi, dan praktik dokumentasi hasil layanan Posbindu PTM yang telah valid dan reliabel. Instrumen tersebut dapat digunakan untuk menilai kemampuan praktik kader dan faktor yang memengaruhi praktiknya

Abstract

SPM Bidang Kesehatan adalah ketentuan tentang jenis dan mutu pelayanan dasar yang merupakan urusan pemerintahan wajib yang berhak diperoleh setiap warga negara Indonesia secara minimal. Capaian indikator pelayanan kesehatan penderita hipertensi di tahun 2020 sebesar 15,96% masih jauh dari target yang ditelah ditetapkan yaitu 100%. Penelitian ini bertujuan untuk menganalisis implementasi SPM bidang kesehatan pada penderita hipertensi di Puskesmas Kota Surakarta. Penelitian ini merupakan penelitian kualitatif dengan pendekatan deskriptif analitik. Subjek penelitian diambil secara purposive sampling. Hasil penelitian dapat diketahui bahwa implementasi SPM-BK pada penderita hipertensi belum dilaksanakan secara maskimal dan belum memenuhi target 100% karena adanya kendala seperti jumlah sasaran penderita hipertensi yang besar, tenaga kesehatan di Puskesmas memiliki tugas rangkap, sarana dan prasarana seperti ruang tunggu dan ruang pelayanan di Puskesmas yang masih sempit, serta faktor eksternal yaitu kesadaran masyarakat mengenai kesehatan yang masih rendah.

Abstract

Kabupaten Demak merupakan Kabupaten dengan laju pertumbuhan tertinggi (1.5%/tahun), namun jumlah peserta KB IUD mengalami penurunan. Kepesertaan IUD di Kecamatan Wonosalam masih sangat rendah, padahal jumlah peserta KB aktif baru tergolong tinggi.Fasilitas memadai sudah disediakan dan semua bidan puskesmas sudah mendapatkan pelatihan pemasangan dan pencabutan IUD. Menurut Green (1980), perilaku seseorang dipengaruhi beberapa faktor antara lain faktor prediposisi. Faktor yang termasuk dari faktor predisposisi antara lain adalah pengetahuan. Sehingga ada kemungkinan bahwa perilaku pemilihan alat kontrasepsi IUD tersebut juga dapat disebabkan oleh pengetahuan mereka tentang IUD masih rendah. Penelitian ini mengidentifikasi apakah ada hubungan antara pengetahuan dengan pemilihan kontrasepsi IUD di Kecamatan Wonosalam Kabupaten Demak 2014. Hasil penelitian ini menunjukkan bahwa pengetahuan sangat mempengaruhi praktik dalam pemilihan alat kontrasepsi.

Abstract

Stigma has become one of the aspects closely associated with human immunodeficiency virus (HIV) and (acquired immunodeficiency syndrome (AIDS), causing adverse effects for those affected. The primary goal of this scoping review is to identify various intervention models aimed at reducing HIV-related stigma. This review follows the PRISMA 2015 guidelines and utilizes sources from three online journal databases: PubMed, ScienceDirect, and Scopus. Search criteria include articles published in the last five years, from 2018 to 2023. Out of the initially retrieved 1,084 articles, 9 articles meeting inclusion criteria were selected for a more in-depth review. Most interventions typically employ a mix of strategies, incorporating diverse degrees of social-ecological involvement. Interventions employing dual strategies and extended durations with multiple sessions have the potential to more effectively reduce stigma. Supporting HIV-safe spaces and encouraging community involvement to understand the link between HIV stigma and the spread of HIV/AIDS in their surroundings can effectively reduce HIV stigma. Overall, most studies included in this review indicate fairly satisfactory outcomes. However, when implementing interventions, it remains crucial to tailor them to the specific location, considering that stigma is a social construct. The development of instruments to measure intervention effectiveness needs to be pursued to comprehensively identify significant intervention impacts.
