Abstract

ABSTRACT Background: The disparity in maternal mortality rates (MMR) between developed countries and low-middle income countries was very large, including in Indonesia. One of the differences between the two was the ultrasound examination that had been used for a long time in developed countries. Semarang was one of the capital city that had implemented ultrasound examination as one of the examinations for pregnant women since 2022. To determine the implementation of ultrasound examination as an early detection of pregnancy complications at primary health care (PHC) in Semarang. Qualitative research with a case study research design. Research data were obtained through interviews, observations, and other documents collected in the period September-December 2024 at 4 PHCs in Semarang. Ultrasound examinations at PHCs were good, but there were several examination indicators that had not been carried out optimally. Two factors that influence its implementation were human resources (HR) and infrastructure. Constraints on HR include uneven distribution, mutations of trained doctors, and varying training curricula. Meanwhile, the constraints encountered in infrastructure were technical constraints on ultrasound equipment, the response of the equipment provider vendor, and the ultrasound room that were not yet up to standard. Improvement efforts were needed in the aspects of human resources and infrastructure in order to improve the quality of USG examination services at PHCs in Semarang. Keywords: Obstetric Ultrasound, Antenatal Care, Primary Health Care.  ABSTRAK Ketimpangan angka kematian ibu (AKI) antara negara maju dengan negara berkembang sangat besar, termasuk di Indonesia. Salah satu perbedaan keduanya adalah pemeriksaan USG telah digunakan sejak lama di negara maju. Kota Semarang merupakan salah satu ibu kota yang telah menerapkan pemeriksaan USG sebagai salah satu pemeriksaan bagi ibu hamil sejak 2022. Untuk mengetahui implementasi pemeriksaan USG sebagai deteksi sebagai deteksi dini komplikasi kehamilan di Puskesmas Kota Semarang. Penelitian kualitatif dengan desain penelitian studi kasus. Data penelitian diperoleh melalui wawancara, observasi, dan dokumen lainnya yang dikumpulkan pada periode September-Desember 2024 pada 4 puskesmas di Kota Semarang. Pemeriksaan USG pada ANC di Puskesmas Kota Semarang sudah baik, namun terdapat beberapa indikator pemeriksaan yang belum dijalankan secara optimal. Dua faktor yang memengaruhi pelaksanaannya adalah sumber daya manusia (SDM) dan sarana prasarana. Kendala pada SDM meliputi distribusi yang belum merata, mutasi terhadap dokter yang terlatih, dan kurikulum pelatihan yang bervariasi. Sedangkan kendala yang ditemui pada sarana prasarana adalah kendala teknis alat USG, respon vendor penyedia alat, dan ruang USG yang belum sesuai standar.Diperlukan upaya perbaikan pada aspek SDM dan sarana prasarana demi meningkatkan kualitas pelayanan pemeriksaan USG pada ANC di Puskesmas Kota Semarang. Kata Kunci: Pemeriksaan USG, ANC, Puskesmas.

Abstract

Background Healthcare workers are required to provide fast, accurate, and empathetic services, often under limited resources. The imbalance between high job demands and insufficient resources increases the risk of stress, burnout, and reduced service quality. Objective To examine the relationship between job demands and resources on work stress among healthcare workers. Method A systematic review using the PRISMA Framework was conducted on 13 national and international articles published between 2021–2025 that focused on job demands and resources in relation to work stress in the healthcare sector. Results Job demands such as physical workload, administrative tasks, emotional burden, and time pressure were found to increase stress, while resources including social support, supportive leadership, organizational policies, adequate facilities, and personal resources played a protective role. Conclusion The balance between job demands and resources determines the level of stress among healthcare workers. Integrated workload management strategies combined with strengthening organizational and personal resources are essential to maintain healthcare workers’ well-being and service quality. Abstract

Neonatal mortality in Indonesia continues to increase, especially during the first 0-6 days of life, indicating persistent gaps in the quality of maternal and neonatal care. The maternal perinatal surveillance and response audit (AMP-SR) is implemented to identify causes of death and guide preventive and curative actions. This scoping review aims to explore the causes and risk factors of neonatal mortality using the AMP-SR framework. Article searches were conducted in Google Scholar, Garuda, and PubMed using the keywords (“Neonatal Death” OR “Cause of Death” OR “Kematian Neonatal”) AND (“AMP-SR” OR “Death Notification”). Inclusion criteria comprised primary studies published in Indonesian or English between 2021 and 2025 that examined neonatal deaths using the AMP-SR approach. Seventeen articles were selected and thematically analyzed following the PRISMA guidelines. The main causes of neonatal death were clinical conditions, including asphyxia, prematurity, sepsis, hypothermia, and congenital abnormalities. Identified risk factors encompassed maternal age, pregnancy complications, referral delays, inadequate quality of care, and limited health worker competence. Most neonatal deaths occurred within the first 72 hours of life, predominantly among male infants with low birth weight or gestational age under 37 weeks. These findings demonstrate that neonatal mortality results from interconnected medical, maternal, and health system factors that can be systematically identified through AMP-SR. From a policy and practice perspective, strengthening the routine use of AMP-SR findings to inform targeted quality improvement, referral system strengthening, and workforce capacity-building is essential to reduce preventable neonatal deaths in Indonesia.

Abstract

This study aimed to analyze factors associated with the performance of childhood immunization recording and reporting using the Sehat Indonesia-Ku (ASIK) application among village midwives in Magetan District, Indonesia. The ASIK application plays an important role in strengthening Indonesia’s digital immunization reporting system by improving data accuracy, timeliness, and monitoring at primary healthcare facilities. A quantitative cross-sectional study was conducted among 228 village midwives from 22 primary health centers using a total sampling technique. Data were collected using a structured questionnaire and analyzed using univariate, bivariate (Chi-square), and multivariate analyses with binary logistic regression. The results showed that perception (p = 0.047), attitude (p = 0.002), and workload (p = 0.014) were significantly associated with performance. Midwives with positive perceptions were more likely to demonstrate good performance (OR = 1.749), while attitude emerged as the strongest predictor (OR = 2.400). In addition, a lighter workload significantly increased the likelihood of good performance (OR = 1.998). Conversely, length of service, knowledge, and motivation were not significantly associated with performance (p > 0.05). These findings suggest that immunization program managers and primary healthcare facilities should strengthen training programs, provide continuous technical support, and optimize workload distribution to enhance ASIK utilization and improve immunization data quality.
