Abstract

Stressor of nurse work is an inability of nurse to face any condition or situation in work environment. Stressor of work consist of workload, physical environment, individual, group, and organization which is the source of stressor. The research identified the effect of work stressor on the quality of nurse care in inpatient wards of Sukoharjo Regional Public Hospital. The type of research is quantitative research with Cross-Sectional approach. The populations are all nurses who served in inpatient wards of Sukoharjo Regional Public Hospital. The samples are 81 nurses by using total sampling. There was no workload effect on the quality of nurse care (p 0,365; B -0,104). There was physical environment effect on the quality of nurse care (p 0,004; B 0,818). There was indiviual effect on the quality of nurse care (p 0,001; B 1,875). There was no group effect on the quality of nurse care (p 0,555; B 0,244). There was no organization effect on the quality of nurse care (p 0,640; B 0,184). The negative effect of stress in high level influenced the care to patient. The higher the perceived work stress would influence the quality of nurse care in inpatient wards of Sukoharjo Regional Public HospitalKey Words : Work Stressor, The Quality of Nurse Care

Abstract

Background: Urinary tract infection (UTI) -related catheters are one of the four most common HAIs types in hospitals worldwide in addition to IDO, Pneumonia, and Primary Blood Flow Infection (IADP). UTI-related catheters are a significant cause of morbidity and mortality. The absence of data on the incidence of UTI related catheters in hospitals as an indicator is still low implementation of patient safety in hospitals. The purpose of this study to determine the incidence of urinary tract infections and germs that cause UTI. Method: This research was an analytical descriptive research using cross sectional observational quantitative design. Population and Sample of the study were catheter-attached patients as the subjects of the study through total sampling. Observation sheet urinary tract infections using this instrument are obtained from Centers for Disease Control and Prevention (CDC). Results: Of the 57 patients treated in the ward during the 209 days, seven patients with UTI were exposed, the incidence of symptomatic UTI in RS PKU Muhammadiyah Bantul was 33.49 ‰ from one culture of urine samples found Escherichia Coli and Candida Non Albicans. Conclusion: The incidence rate of symptomatic UTI in RS PKU Muhammadiyah Bantul is 33,49 ‰ and germs that cause urinary tract infection are Escherichia Coli and Candida Non Albicans.

Abstract

Background: Patient safety in hospitals is a service of a hospital system that provides care to enable patients to be more secure. Patient safety goals are to promote specific improvements in patient safety. The reduction in risk of patients falling aimed hospitals need to evaluate the risk of patient falls and take action to reduce the risk of injury when the fall is one of the goals of patient safety. This study reveals the evaluation of the application of preventive risk patient falls in Pupuk Kaltim hospital. Method: This study was a case study, in the inpatient room, the emergency room (ER) at the Pupuk Kaltim hospital. Results: This study resulted that the initial assessment in the emergency department (ED) has not been carried out, assessments of monitoring is not maximized, still found the beds are not safe as many as 26 pieces, grade three patients did not receive anti-slippery footwear. Policies and Standart Operational Procedure (SOP) has existed since 2013. Conclusion: The initial assessment of patients the risk of falling in the ER has not been carried out, assessments have not been up monitoring of risk patients, 26 beds are not safe, three unsafe gurney, patient grade three can not be footwear. Pupuk Kaltim hospital dissemination, evaluation and monitoring of the patients fall risk assessment, infrastructure and facilities.

Abstract

Background: The incidence of IDOs in hospitals in Indonesia varies between 2-18% of all surgical procedures. No research has been done on the effects of IDO risk factors with IDO incidence in gastrophic surgery at this hospital. This study aims to determine the effect of IDO risk factors consisting of nutritional status, type of surgery, the nature of surgery, and durante surgery against the incidence of IDO in surgical patients digestif. Method: This research is an observational quantitative study. The data taken are primary (observasional) and secondary (medical record) using cross sectional method. The data were then analyzed by Chi-square bivariate and multivariate test. Result: Based on Chi-square bivariate test, the variables: operation type (p=0.000), durante surgery (p=0.000), and operational properties (p=0.029) have significant relationship while nutritional status (p=0.055) which is significant against IDO events. Conclusion: The result of multivariate test showed that the variables: nutritional status (p=0.032), type of operation (p=0.006), and durante (p=0.008) had significant influence with the type of operation is the most influential on IDO event.

Abstract

Background: Bronchial asthma is one of the top 10 common diseases in children in the emergency room navy hospital dr. Azhar Zahir Manokwari West Papua in the past one year. Asthma bronchial requiring therapeutic measures quickly and efficiently to improve the quality of patient care in the emergency room. Medical audit was strongly associated with improved quality and standardization for the excellent medical services in hospitals, therefore any health care providers are required to provide good service to do a medical audit. This study is to evaluate the quality of medical services asthma bronchial cases of Children in the Emergency room for the excellent medical services at Navy Hospital dr. Azhar Zahir Manokwari West Papua. Method: This study is a quantitative descriptive study, were taken in a non-probability sampling with consecutive sampling method. Criteria for sample: patients who have been diagnosed with asthma bronchial, aged one to 14 years. Result: Most patients with asthma bronchial in the emergency room was mild attacks category. The application of medical standards asthma bronchial cases in these hospitals were not good enough. It was seen from the results of the analysis, where there were some standard actions that are not performed by a doctor, among others: observation for two hours, the line installation parenteral, clinical check after 12 hours, and thoracic radiograph. Conclusion: The quality of medical care to patients who did not conform to the standard treatment of bronchial asthma in children because there were some actions that are not limited facilities and infrastructure in the emergency room.

Abstract

Background: Development of CAUTI bundle is very important to evaluate the nurse in implementing nursing practice related to urinary catheter which is one of prevention efforts and risk control of CAUTI. Objective: The purpose of this study is to develop CAUTI bundle that is adjusted to hospital condition in Sejiran Setason Muntok Hospital. Methods: In this study used qualitative method by conducting indepth interview and focus group discussion (FGD). It conducted for 3 weeks in Sejiran Setason Muntok Hospital, the total sample are 22 participants. The result produced the themes that used as the material for the development of the bundle or the observation sheet of CAUTI. Results: There are 10 themes which become material for the development of CAUTI bundle. The CAUTI Bundle consists of bundle components such as indication of urinary catheter insertion, urinary catheter installation must comply with SOP, monitoring of urine output by regularly, emptying urine bag regularly, performing hand hygiene before and after procedure, using masks and handscoons, patients and families must be aware of their role in minimizing the risk of CAUTI. Conclusion: Development of the bundle is very important as one of prevention of CAUTI incident in hospital. Keyword : Bundle, CAUTI, Development.

Abstract

Nurses have a big role in infusion installment and care according to the Standard Operating Procedure. Infection or other problems that may emerge during infusion installment can be minimized or avoided as long as the operating standard is met. Quantity work load, qualitative workload, physical workload, psychological workload, social workload and nurses’ fatigue are the major causes of infection due to working errors. This research aims at finding out the effect of nurses’ workload and fatigue on infusion care. This is a quantitative research using cross-sectional approach. The population consists of all nurses working in the third class of RSUD Sukoharjo. Using total sampling, the sampling consists of 28 nurses. Nurses’ workload that influenced infusion care comprises qualitative workload (p 0,000; B -1,847), nurse workload that did not influence infusion care comprised quantitative workload (p 0,109; B -0,243), physical workload (p 0,124; B -0,612), psychological workload (p 0,109; B -0,294), and social workload (p 0,875; B -0,053). There is no effect of nurse fatigue on infusion care (p 0,209; B -0,135). Quantitative workload did not influence nurse fatigue (p 0,196; B -0,367), qualitative workload did not influence nurse fatigue (p 0,053; B -1,331), physical workload did not influence nurse fatigue (p 0,171; B -1,061), and psychological workload did not influence nurse fatigue (p 0,980; B 0,021). The hardest nurse workload that had influenced infusion care in the third class of RSUD Sukoharjo is a qualitative workload. The more a qualitative workload that nurse felt, the worse infusion care they will treat. The workload has no effect on nurse fatigue.Key words: Infusion care, Nurses’ workload, Fatigue

Abstract

The purpose of this study is to evaluate the implementation of the supervision of nursing in Panembahan Senopati Bantul Hospital. This study is a descriptive qualitative research. All data derived from interviews, observation, and search documents. Sample of respondents is representative of the nursing supervisor scheduled in October 2015. The research variables: the implementation of nursing supervision, seen from the description of the implementation, engineering, area, barriers of implementation, supervision and evaluation program monitor. Of the 67 times the supervision scheduled in October 2015 only 17 were implemented (25.4%). Figures noncompliance supervisor in the execution of supervision is still quite high. The majority of supervisors do with indirect supervision. Supervision has not been carried out throughout the area should be. Barriers to implementation of supervision include external and internal obstacles. Program monitoring and evaluation of supervision is still not implemented. Supervision of nursing has not gone up, judging from the absence rate of non-compliance supervisor and implementation is quite high, and there are still incomprehension supervisor of engineering and supervisory area should be.

Abstract

Research on Patient Centered Care (PCC) in cataract patients is still limited, especially those that analyze the effectiveness of PCC in reducing anxiety levels and improving the patient satisfaction. This study was a quasi-experimental pre- and post-test design, which involved 30 treatment respondents and 30 control respondents. The instrument used a SAI questionnaire to measure anxiety and SERVQUAL modifications to measure patient satisfaction. The study was conducted at Nganjuk Eye Clinic in October-November 2018. Analysis of data using paired t-test and independent t-test. The results of difference tests of anxiety levels before and after PCC and difference tests of anxiety levels of the control and PCC group showed the results of sig. 2-tailed = 0,000. Whereas the difference tests of satisfaction levels between the control and PCC group were obtained sig. 2-tailed = 0.035. The PCC Effect Size obtained strong effects (2.59 and 4.31) in reducing anxiety and average effect (0.56) on improving satisfaction. There were significant differences in the level of anxiety and satisfaction in both groups. The results showed that PCC was very effective in reducing anxiety levels and quite effective in improving the satisfaction on cataract surgery patients.

Abstract

Burns are the most severe trauma impact on both physical and psychological. With the limitedtypes of advanced dressings are available in some hospitals, many alternatives were developed based treatment of burns moist (moisture balance dressings alternative) to accelerate wound healing. Nonpharmacological interventions for the treatment of psychological stress with SEFT therapy, the patient's with SEFT therapy will be relaxed and the mind becomes calm. Relaxation created very influential in the healing process. This study aim: To identify the physiological and psychological adaptation response of burn patients were given a combination of alternative SEFT moisture balance dressings and therapies. This research used action research that determine the physiological and psychological adaptation response of burn patients were given a combination of alternative moisture balance and SEFT therapy The sample is the total population that met the inclusion criteria. Result showed that There were 8 respondents (March - June 2014). Most of the 75% of men, aged between 17-51 years, extensive of wound between 6-55% TBSA, 37.5% stage II, stage III 62.5%.Physiological adaptation response with an wound healing indicator average 42.37 (36-49) showed that physiological adaptation response is adaptive, long time recovering the second degree on average 17.6 days (7-36 days), the average grade III 28 , 8 days (20-40 days). Psychological adaptation  response with an acceptance score average 44.5 (40-50) and supported by the results of interviews all  indicate an adaptive response to psychological adaptation. Conclusion: Psychological and physiological adaptation response following administration of a combination of alternative moisture balance dressings and therapies SEFT are adaptive.Keywords: Alternative moisture balance dressings, SEFT therapy, physiological adaptation,psychological
