Abstract

Background: Bronchial asthma is one of the top 10 common diseases in children in the emergency room navy hospital dr. Azhar Zahir Manokwari West Papua in the past one year. Asthma bronchial requiring therapeutic measures quickly and efficiently to improve the quality of patient care in the emergency room. Medical audit was strongly associated with improved quality and standardization for the excellent medical services in hospitals, therefore any health care providers are required to provide good service to do a medical audit. This study is to evaluate the quality of medical services asthma bronchial cases of Children in the Emergency room for the excellent medical services at Navy Hospital dr. Azhar Zahir Manokwari West Papua. Method: This study is a quantitative descriptive study, were taken in a non-probability sampling with consecutive sampling method. Criteria for sample: patients who have been diagnosed with asthma bronchial, aged one to 14 years. Result: Most patients with asthma bronchial in the emergency room was mild attacks category. The application of medical standards asthma bronchial cases in these hospitals were not good enough. It was seen from the results of the analysis, where there were some standard actions that are not performed by a doctor, among others: observation for two hours, the line installation parenteral, clinical check after 12 hours, and thoracic radiograph. Conclusion: The quality of medical care to patients who did not conform to the standard treatment of bronchial asthma in children because there were some actions that are not limited facilities and infrastructure in the emergency room.

Abstract

Background: Patient safety in hospitals is a service of a hospital system that provides care to enable patients to be more secure. Patient safety goals are to promote specific improvements in patient safety. The reduction in risk of patients falling aimed hospitals need to evaluate the risk of patient falls and take action to reduce the risk of injury when the fall is one of the goals of patient safety. This study reveals the evaluation of the application of preventive risk patient falls in Pupuk Kaltim hospital. Method: This study was a case study, in the inpatient room, the emergency room (ER) at the Pupuk Kaltim hospital. Results: This study resulted that the initial assessment in the emergency department (ED) has not been carried out, assessments of monitoring is not maximized, still found the beds are not safe as many as 26 pieces, grade three patients did not receive anti-slippery footwear. Policies and Standart Operational Procedure (SOP) has existed since 2013. Conclusion: The initial assessment of patients the risk of falling in the ER has not been carried out, assessments have not been up monitoring of risk patients, 26 beds are not safe, three unsafe gurney, patient grade three can not be footwear. Pupuk Kaltim hospital dissemination, evaluation and monitoring of the patient's fall risk assessment, infrastructure and facilities.

Abstract

Background: The incidence of IDOs in hospitals in Indonesia varies between 2-18% of all surgical procedures. No research has been done on the effects of IDO risk factors with IDO incidence in gastrophic surgery at this hospital. This study aims to determine the effect of IDO risk factors consisting of nutritional status, type of surgery, the nature of surgery, and durante surgery against the incidence of IDO in surgical patients digestif. Method: This research is an observational quantitative study. The data taken are primary (observasional) and secondary (medical record) using cross sectional method. The data were then analyzed by Chi-square bivariate and multivariate test. Result: Based on Chi-square bivariate test, the variables: operation type (p=0.000), durante surgery (p=0.000), and operational properties (p=0.029) have significant relationship while nutritional status (p=0.055) which is significant against IDO events. Conclusion: The result of multivariate test showed that the variables: nutritional status (p=0.032), type of operation (p=0.006), and durante (p=0.008) had significant influence with the type of operation is the most influential on IDO event.

Abstract

Background: Urinary tract infection (UTI) -related catheters are one of the four most common HAIs types in hospitals worldwide in addition to IDO, Pneumonia, and Primary Blood Flow Infection (IADP). UTI-related catheters are a significant cause of morbidity and mortality. The absence of data on the incidence of UTI related catheters in hospitals as an indicator is still low implementation of patient safety in hospitals. The purpose of this study to determine the incidence of urinary tract infections and germs that cause UTI. Method: This research was an analytical descriptive research using cross sectional observational quantitative design. Population and Sample of the study were catheter-attached patients as the subjects of the study through total sampling. Observation sheet urinary tract infections using this instrument are obtained from Centers for Disease Control and Prevention (CDC). Results: Of the 57 patients treated in the ward during the 209 days, seven patients with UTI were exposed, the incidence of symptomatic UTI in RS PKU Muhammadiyah Bantul was 33.49 ‰ from one culture of urine samples found Escherichia Coli and Candida Non Albicans. Conclusion: The incidence rate of symptomatic UTI in RS PKU Muhammadiyah Bantul is 33,49 ‰ and germs that cause urinary tract infection are Escherichia Coli and Candida Non Albicans.

Abstract

Background: The application of educational interprofessional education (IPE) needs to be applied in improving communication and collaboration between health professions to reduce medical errors and improve patient safety.Methods: Literature search on IPE through ScienceDirect, ProQuest, and Pubmed databases with predetermined keywords. Then screening was carried out based on inclusion and exclusion criteria, then analyzed for the feasibility of the article than a review.Results: A total of 13 of the 4,350 articles met the requirements to be analyzed in the current review. All articles provide an outline of research objectives including Testing the trends of IPE and how IPE must be designed and implemented in medicine, the effectiveness of the IPE program and the results or impacts as well as the factors that influence IPE perceptions and learning experiences. How to build interdisciplinary collaboration in a multidisciplinary team. The entire article shows that the application of interprofessional education (IPE) can improve knowledge, skills, behavior, collaboration, communication, and help understand the ideas, roles, and responsibilities of each health profession.Conclusion: Based on the analysis of the articles used as literature shows that IPE learning can increase student self-confidence, coordination (planning), control and calmness (reducing anxiety), and commitment (persistence) in supporting patient-centered care.

Abstract

Midwife is one resource that has an important role in the hospital. Midwife is very important because the job involves the safety of the mother and child during birthing. In performing its duties, midwife potentially experience of work stress are affected by the stressors of work consisting of the physical environment, individual, group and organizational. RSKIA Sadewa is one hospital that specialized in providing services for maternal and child health that attracted many people that have a lot of patients every month. A large number of patients who were not matched with adequate work environment will impact on job stress experienced by the midwives. The research was a descriptive analytic study. The populations of the study are all midwives working in Yogyakarta Sadewa RSKIA as many as 36 people. The sampling technique in this research is total sampling with amount of sample as many as 36 people. The data collect using questionnaires. Analysis data using multiple regression analysis. Physical environmental stressors have a significant effect on the performance of midwives. Individual stressors have a significant effect on the performance of midwives. Stressor groups significantly influence the performance of midwives. Organizational stressors have a significant effect on the performance of midwives.Work stressors consisting of the physical environment, individual, groups and organizations to simultaneously have a significant effect on the performance of midwives in RSKIA Sadewa Yogyakarta.   Keywords: Job stressors, physical environment, midwifery performance

Abstract

Life expectancy becomes higher and higher is one of successful indicators in development. Pulmonary system diseases, especially tuberculosis is the common disease that attacks elderly.  The aim of this research  is Obtaining comprehension about tuberculosis treatment result in elderly This research used retrospective study using non-experimental descriptive analysis designThe treatment results of the tuberculosis sufferer in Gombong Public Health Center is about 50% elderly had recovered from tuberculosis, 18,75% of elderly passed away, 6,25% moved to other health center and 6,25% had dropped out of drugs / default. There are 4 themes identified from the family consist of changes the elderly, the ability to carry out the health task of family, life quality, values and beliefs in caring for the elderly with tuberculosis. Keywords: family, caring, elderly, tuberculosis.

Abstract

Decubitus is a very serious problem especially for patients who hospitalized for long time with limited activity. Decubitus can increase the duration of hospital stay so that will increase the burden, especially the cost of hospitalization along with length of stay in hospital. This research of purpose to know the influences of nursing intervention to reduce pressure sore to reducing risk of pressure sore at RSU PKU Muhammadiyah Purworejo. This research design used “Pre and post test without control group " with intervention reduction of risk pressure ulcer. This research is conducted in July 2016 with consecutive sampling method of 40 patients. Observations of pressure sore level on bed rest patiens done before and after treatment. The evaluation was done with wilcoxon ranks test and continued with cross tabulation  for each treatment. The results showed a significant increase in risk of injury scores by 31 respondents, p = 0,000 <0.05. While the result of cross tabulation showed decreased risk of injured press on each level consisting of: high risk from 18 respondents down to 12 (66,7%) with baring and mass action. For the medium category there are 18 respondents 18 respondents dropped to 13 (72.1%) into low risk. As for the very high risk of 2 respondents who experienced a decrease in risk as much as 1 respondent while 1 other respondent remained, action at very high risk of wound care done according to grade I wound press. Likewise for low risk categories did not change, this is due to limited number of samples taken. The conclusion of this study is a decrease of risk pressure sore after applied preventive action for reduction of pressure sore in bed rest patients. The action apllied wound dressing, combination of massage and reposition, also health promotion.

Abstract

Background: Development of CAUTI bundle is very important to evaluate the nurse in implementing nursing practice related to urinary catheter which is one of prevention efforts and risk control of CAUTI. Objective: The purpose of this study is to develop CAUTI bundle that is adjusted to hospital condition in Sejiran Setason Muntok Hospital. Methods: In this study used qualitative method by conducting indepth interview and focus group discussion (FGD). It conducted for 3 weeks in Sejiran Setason Muntok Hospital, the total sample are 22 participants. The result produced the themes that used as the material for the development of the bundle or the observation sheet of CAUTI. Results: There are 10 themes which become material for the development of CAUTI bundle. The CAUTI Bundle consists of bundle components such as indication of urinary catheter insertion, urinary catheter installation must comply with SOP, monitoring of urine output by regularly, emptying urine bag regularly, performing hand hygiene before and after procedure, using masks and handscoons, patients and families must be aware of their role in minimizing the risk of CAUTI. Conclusion: Development of the bundle is very important as one of prevention of CAUTI incident in hospital. Keyword : Bundle, CAUTI, Development.

Abstract

Self care diabetes adalah tindakan yang dilakukan seseorang untuk mengontrol diabetes yang meliputi tindakan terapi obat dan pencegahan terhadap komplikasi. Pengukuran aktifitas self care diabetes meliputi pengaturan diet (pola makan), latihan fisik, monitor gula darah, dan terapi obat. Tujuan penelitian  mengetahui supportif educative system dapat meningkatkan kemandirian klien diabetes melitus tipe 2 di kecamatan Mergangsan, kota Yogyakarta. Metode penelitian ini menggunakan desain quasi experimental pre ? post test control group. Penelitian ini dilakukan di wilayah Kecamatan Mergangsan, yaitu kelurahan Wirogunan dan Brontokusuman. Jumlah sampel sebanyak 82 responden (41 kelompok intervensi dan 41 untuk kelompok kontrol). Metode yang dilakukan dengan ceramah dan pemberian modul tentang  aktifitas self care diabetes. Penelitian ini yang dilakukan selama 1 bulan, intervensi supportif educative system dilakukan 1 kali pada minggu pertama dalam waktu 120 menit pada kelompok intervensi. Hasil penelitian dengan uji Wilcoxon test kelompok intervensi didapatkan nilai bermakna (p=0,002), terdapat perbedaan kemandirian antara sebelum dan sesudah diberikan supportif educative system. Pada kelompok kontrol didapatkan nilai tidak bermakna (p=1,000), tidak terdapat perbedaan kemandirian antara sebelum dan sesudah diberikan supportif educative system. Uji beda Mann Whitney diperoleh nilai bermakna (p=0,020) terdapat perbedaan kemandirian antara kelompok intervensi dan kelompok control. Simpulannya:  supportif educative system dapat meningkatkan kemandirian pada klien diabetes melitus tipe 2. Kata Kunci: Kemandirian, Diabetes Melitus Tipe 2, Pendidikan Kesehatan Self Care Diabetes
