Abstract

The coexistence of depression and type 2 diabetes mellitus (T2DM) can significantly worsen disease prognosis and lower quality of life. Emerging evidence suggests that vitamin D deficiency contributes to the progression of T2DM and is closely associated with the development of depression. The aim of this study was to investigate the effects of cholecalciferol on depression in patients with T2DM, exploring its mechanisms by analyzing its impact on C-peptide, serotonin, and neurotrophin-3 levels. A double-blind, randomized, placebo-controlled clinical trial was conducted at Cipto Mangunkusumo General Hospital, Jakarta, Indonesia, from April 2021 to September 2022. Patients with T2DM and depressive symptoms were randomly assigned to two groups: received 4000 IU of cholecalciferol daily and received a placebo for 12 weeks. Depression was assessed using the Beck Depression Inventory-II (BDI-II) before and 12 weeks after the intervention. The levels of C-peptide, serotonin, and neurotrophin-3 were measured at the end of the fourth week of intervention using the enzyme-linked immunosorbent assay (ELISA) method. Between-group comparisons were made using independent Student t-tests and Mann-Whitney U tests. Paired Student t-tests or Wilcoxon tests were applied for within-group comparisons between pre- and post-intervention. A total of 70 T2DM patients with depression were included in this study, comprising 38 patients in the cholecalciferol group and 32 in the placebo group. C-peptide levels increased significantly in the cholecalciferol group compared to the placebo group (p=0.006). No significant differences were observed in serotonin and NT-3 levels between the cholecalciferol group compared to the placebo group. The cholecalciferol group had a significantly greater reduction in BDI-II scores compared to the placebo group (p<0.001). This trial highlights that taking cholecalciferol might help ease mild to moderate depression symptoms in patients with T2DM by enhancing c-peptide levels, though its effects on serotonin and neurotrophin-3 are still unclear.

Abstract

Background: Inflammatory Bowel Disease (IBD), encompassing ulcerative colitis (UC) and Crohn's disease (CD), is an inflammatory digestive tract condition with unknown causes. Its unpredictable symptoms affect quality of life. In Indonesia, the quality of life of IBD patients remains unreported. Factors such as advanced age, long disease duration, active disease, corticosteroid use, comorbidities, unemployment, and poor sleep quality may reduce quality of life. This study aims to assess the quality of life profile of IBD patients and its associated factors.Methods: This cross-sectional study collected data from October to November 2024 through interviews at the outpatient unit of Dr. Cipto Mangunkusumo Hospital (RSCM). Quality of life was measured using the Inflammatory Bowel Disease Questionnaire 9 (IBDQ-9), and sleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI). Both tools were validated in Indonesian. Eligible participants met inclusion and exclusion criteria. Bivariate and multivariate logistic regression analyses identified factors associated with quality of life.Results: Among 201 participants, 95% reported a good quality of life. Multivariate analysis identified active disease (PR 4.072 [1.133–14.633], p = 0.031) and combination therapy (PR 12.803 [1.423–115.147], p = 0.023) as factors associated with poor quality of life. Age, disease duration, comorbidities, employment status, and sleep quality showed no significant associations.Conclusion: Most IBD patients (95%) in the RSCM outpatient unit reported a good quality of life. Active disease and combination therapy were linked to poorer quality of life.

Abstract

Berbagai penelitian menunjukkan bahwa defisiensi vitamin D dan hiperpara-tiroidisme sekunder menimbulkan dampak serius pada kesehatan, antara lain meliputi osteoporosis, osteomalasia, kelemahan otot, jatuh dan fraktur osteoporotik. Tujuan penelitian ini adalah (1) mengetahui pengaruh pajanan UVB sinar matahari pada konsentrasi 25(OH)D dan hormon paratiroid (PTH) perempuan usia lanjut Indonesia. (2) mendapatkan saat dan lama pemajanan yang optimal. Penelitian uji klinik acak terbuka ini melibatkan 74 perempuan berusia 60 - 90 tahun yang tinggal di 4 panti werda di Jakarta dan Bekasi. Randomisasi dilakukan untuk memisahkan kelompok studi dan kontrol. Kelompok kontrol hanya mendapat kalsium 1000 mg/hari, sedang kelompok intervensi dipajankan dengan matahari selama 6 minggu. Hasil yang diukur sebelum dan sesudah 6 minggu pemajanan adalah konsentrasi 25(OH)D, PTH, dan ion kalsium. Ditemukan bahwa, waktu pemajanan yang optimal adalah 1 jam sebelum dan sesudah tengah hari. Prevalensi defisiensi vitamin D pada wanita usia lanjut adalah 35,1%. Pada kelompok terpajan, konsentrasi 25(OH)D meningkat lebih tinggi daripada yang tidak dipajan (51,8% vs 12,5%). Hasil tambahan adalah rerata asupan kalsium 248 mg/hari, dan rerata asupan vitamin D 28 IU/hari. Many studies showed that vitamin D deficiency and secondary hyper-parathyroidism cause serious impact on health including osteoporosis, osteomalacia, paralysis, fall, and osteoporotic fracture. This study was conducted to compare the effect of UVB from sunlight exposure in combination with calcium supplementation, and control (calcium only) on the vitamin D status and parathyroid hormone (PTH) concentration in Indonesian elderly women. This study was a randomized clinical trial in institutionalized care unit. Subjects included 74 elderly women with a mean age 71 years. Intervention was random allocation of UVB from sunlight exposure at 0.6 MED/hour noted in the UV meter on the face and both arms and calcium 1000 mg, three times per week for 6 weeks, and without treatment (calcium 1000 mg only). Main outcome measured were fasting serum levels of 25(OH) D, PTH, and calcium ion at 0 and 6 weeks in both treatment and control groups. The incidence of vitamin D deficiency in this population study was 35.1 %. In the treatment group, 25(OH) D increased from 59.1 nmol/L to 84.3 nmol/L (mean value after 6 weeks of sunlight exposure) with only a slight increase of 25(OH) D in the control group (51.8% vs 12.5%). 25(OH)D deficient levels in 15 out 16 subjects became normal after 6 weeks of sun exposure. There was no change of PTH levels in both groups. Additional results of this study are mean calcium intake of 248 mg/day and vitamin D intake of 28 IU/day.

Abstract

BACKGROUND Stress urinary incontinence (SUI) is uncontrollable urine leakage when intra-abdominal pressure increases. Behavioral therapy with Kegel exercise is currently the best conservative management recommendation for treating SUI patients. This study aimed to investigate the success of supervised Kegel exercises using the Kegel exercises guidebook in women with SUI. METHODS This quasi-experimental study involved both groups being taught the same regiment of Kegel exercises, but the intervention group was given the Kegel exercises guidebook. After 12 weeks, the compliance, subjective, and objective success rates were evaluated through the patient’s notes, Urinary Distress Inventory-6, Incontinence Impact Questionnaire-7, perineometer, and pad test. RESULTS Patients with a higher level of education had lower compliance to Kegel exercises (p = 0.01; odds ratio [OR] 0.38; 0.18–0.79). No significant difference was observed in the subjective symptom improvement between the two groups. The intervention group had a significantly higher success rate based on objective success by analyzing the 1-hour pad test results. The intervention group was also more adherent (p<0.001; OR 4.78; 2.51–9.0). CONCLUSIONS Patients who received the Kegel exercises guidebook were more compliant and more objectively successful than those who did not receive the manuals.

Abstract

BACKGROUND Nephrolithiasis or kidney stone disease (KSD) is common worldwide. Despite various effective treatment strategies, KSD recurrence remains a problem. This study aimed to investigate the risk factors of KSD recurrence. METHODS This retrospective cohort study used medical records of all patients who came to the Department of Urology, Cipto Mangunkusumo Hospital, Jakarta, from January 2014 to December 2019, with asymptomatic and symptomatic KSD. Demographic information, clinical data, exposure to risk factors, and recurrent KSD diagnosis were collected. Univariate and multivariate analyses using logistic regression were performed to determine the significant risk factors. RESULTS We reported 325 patients with a median age of 52 years. More than half of the patients were males and from Java. Staghorn stone dominated the KSD types found in 181 patients (55.7%). After undergoing percutaneous nephrolithotomy, 214 patients (65.8%) became stone-free. However, about 40.6% of them later developed recurrent KSD. The adjusted odds ratio in recurrent KSD were 1.46 (95% confidence interval [CI] 1.33–1.59) for younger age, 1.86 (95% CI 1.61–2.07) for overweight–obese, 2.13 (95% CI 1.89–2.31) for less fluid intake, 1.81 (95% CI 0.97–2.12) for routine tea consumption, 1.24 (95% CI 1.06–1.84) for routine vegetables consumption, 2.27 (95% CI 1.83–2.84) for a family history of KSD, and 2.08 (95% CI 1.77–2.39) for diabetes mellitus (DM). CONCLUSIONS Most patients with recurrent KSD were younger, overweight/obese, had less fluid intake, a family history of KSD, and DM. Modifying a healthy lifestyle and a balanced diet is important to prevent KSD recurrence.

Abstract

BACKGROUND Vitamin D deficiency has been linked to autoimmune diseases, cancer, and cardiovascular diseases. Although 1 study attempted to elucidate the ingredients required to make this diet, the process remained unclear. Hence, this study aimed to customize a low cholecalciferol diet with good tolerability in mice. METHODS We customized a diet containing a normal cholecalciferol content (1 IU/g diet) and another with a low cholecalciferol content (0.05 IU/g diet). Samples from both diets were sent to an independent laboratory to ensure that the levels of cholecalciferol, phosphorus, and calcium present in the custom diets matched our calculations. 5 mice were fed the customized normal cholecalciferol diet for 1 week to assess tolerability. Tolerability was assessed by measuring the amount of food consumed, weight gained, and the presence of any adverse events. RESULTS Cholecalciferol, phosphorous, and calcium levels in both diets satisfactorily matched our calculations. The diet was well tolerated without any adverse events or mortalities. The mice consumed an adequate amount of food (mean: 5.34 [0.08] g diet/day, 95% confidence interval [CI]: 5.12–5.56; 19.38 kcal, fat: 0.43 g, protein: 0.14 g, carbohydrates: 3.16 g, and cholecalciferol: 0.007 mg) and gained a slight amount of weight by the end of the experiment (mean: 1.86 [0.46] g, 95% CI: 0.58–3.14). CONCLUSIONS This study successfully created 2 custom diets with quantified cholecalciferol contents. This animal model may prove valuable for studies involving vitamin D.
