Abstract

Background: Drug-resistant tuberculosis (DR-TB) remains a major challenge in global TB control efforts, particularly in middle-income countries such as Indonesia. Long-term regimens lasting 18–24 months have been associated with low treatment success rates due to their prolonged duration and high incidence of adverse drug reactions. More recent developments, namely the bedaquiline–pretomanid–linezolid regimen with or without moxifloxacin (BPaL/M), have demonstrated treatment success rates of up to 89–90% in international clinical trials. Purpose: To describe the treatment success of short-term, long-term, and BPaL/M regimens among patients with drug-resistant tuberculosis. Method: A retrospective descriptive study was conducted using medical record data of DR-TB patients who received long-term, short-term, or BPaL/M regimens during 2023–2024. Treatment outcome classification followed the WHO 2022 guidelines. The variables collected included patient characteristics, type of regimen, and treatment outcomes based on WHO standards: cured, treatment completed, treatment failed, lost to follow-up, and died. Results: Of the 148 DR-TB patients who met the inclusion criteria, 98 patients (66.2%) received long-term regimens, 17 patients (11.5%) received short-term regimens, and 33 patients (22.3%) were treated with the BPaL/M regimen. The treatment success rate (cured + treatment completed) for the short-term regimen was 76.5%, which was lower compared to the long-term regimen at 83.7%. The BPaL/M regimen demonstrated the highest treatment success rate at 87.9%. Conclusion: The treatment success rates for DR-TB patients at Dr. H. Abdul Moeloek Hospital indicate that the BPaL/M regimen provides the most favorable outcomes compared to long-term and short-term regimens. Abstract

Pendahuluan: Infeksi Malassezia furfur yang umum di Indonesia memerlukan alternatif pengobatan alami karena efek samping dan kasus resistensi pengobatan antijamur. Penelitian ini bertujuan membandingkan potensi antijamur ekstrak etanol 96% biji kopi robusta (Coffea canephora) dan lada hitam (Piper nigrum) yang merupakan komoditas lokal Provinsi Lampung dalam menghambat pertumbuhan Malassezia furfur secara in vitro. Metode: Studi eksperimental Post-test Only Control Group Design ini menguji ekstrak hasil maserasi etanol 96% menggunakan metode difusi sumuran pada media SDA. Perlakuan meliputi lima konsentrasi (100%, 50%, 25%, 12,5%, 6,25%), kontrol positif (Terbinafine), dan kontrol negatif (Akuades). Diameter zona hambat yang terbentuk diukur dan dianalisis secara statistik. Hasil: Ekstrak kopi robusta konsentrasi 100%; 50%; 25%; 12,5%; 6,25% menghasilkan zona hambat berturut-turut 13,39 mm; 11,25 mm; 5,72 mm; 1,99 mm dan 0 mm. Sedangkan ekstrak lada hitam menghasilkan 7,75 mm; 6,94 mm; 1,60 mm; dan 0 mm pada konsentrasi 12,5% dan 6,25%. Kontrol positif (Terbinafine) sebesar 23,32 mm dan kontrol negatif 0 mm. Pembahasan: Ekstrak kopi robusta lebih unggul karena kandungan asam klorogenat dan kafeinnya mampu melisiskan sel jamur Malassezia furfur dan berdifusi baik. Berbeda dengan lada hitam, senyawa piperin yang bersifat lipofilik memiliki keterbatasan difusi pada media uji, sehingga zona hambatnya terbatas. Simpulan: Ekstrak etanol 96% biji kopi robusta memiliki daya hambat yang lebih besar dan signifikan dibandingkan ekstrak buah lada hitam dalam menghambat pertumbuhan jamur Malassezia furfur.

Abstract

Background: Drug-resistant tuberculosis (DR-TB) is an increasingly complex health issue, especially when accompanied by chronic obstructive pulmonary disease (COPD) comorbidity, which can exacerbate lung conditions, affect therapeutic response, and reduce treatment success. This study aims to describe the characteristics of DR-TB patients with COPD comorbidity at RSUD Dr. H. Abdul Moeloek from 2022 to 2024. Method: This study utilized a retrospective descriptive design. Data used were secondary data from patient medical records. Univariate analysis was performed to describe subject characteristics. Results: Among 46 patients, the majority were male (78.3%) aged 41–60 years (50%). RR-TB classification (76.1%) and long-term regimens (78.3%) dominated the findings. Therapeutic outcomes recorded 54.3% cured, 30.4% treatment completed, 8.7% failed, and 6.5% deceased. Discussion: DR-TB patients with COPD are more frequently observed in middle-aged males due to smoking exposure and occupational factors. The majority were classified as RR-TB, consistent with gradual resistance patterns. Long-term regimens were preferred as they are considered safer for lung function. Although COPD aggravates the condition, therapy remains likely to succeed with appropriate drug selection and patient adherence. Conclusion: DR-TB patients with COPD comorbidity are generally middle-aged males with a relatively good proportion of treatment success. Appropriate regimen selection and strict monitoring are required to optimize treatment outcomes.

Abstract

Acute respiratory tract infection (ARI) is one of the leading causes of morbidity that often requires antibiotic therapy, with Klebsiella pneumoniae being one of the most commonly found bacterial pathogens and known to have a high level of antibiotic resistance. Irrational use of antibiotics can accelerate the development of resistance, so the effectiveness of antibiotics needs to be evaluated periodically. Amoxicillin and piperacillin are commonly used antibiotics in the management of ARTI. This study aimed to evaluate the effectiveness of amoxicillin and piperacillin against Klebsiella pneumoniae in cases of ARTI at Dr. H. Abdul Moeloek General Hospital from 2019 to 2023. This study is an observational analytical study with a cross-sectional design using secondary data in the form of culture results and antibiotic sensitivity tests from patient medical records. The sampling technique used total sampling with a total of 106 Klebsiella pneumoniae isolates. Data analysis was performed using the Chi-Square test. The results showed that the resistance rate of Klebsiella pneumoniae to amoxicillin was 92.5% with a sensitivity of 7.5%. In contrast, piperacillin showed a higher sensitivity of 58.5% although resistance was still found at 41.5%. Statistical tests showed a significant relationship between the type of antibiotic and the level of bacterial sensitivity (p-value < 0.001). The conclusion of this study indicates that piperacillin is more effective than amoxicillin in the management of ARI caused by Klebsiella pneumoniae and can be considered in the selection of empirical antibiotic therapy.

Abstract

Alopecia is a disorder of the hair follicles that causes hair loss, either in limited areas or throughout the entire body. This study aims to evaluate the potential and molecular interactions of compounds found in the Rampai plant (Lycopersicon esculentum Mill.). The Rampai plant is known to contain various secondary metabolites such as alkaloids, flavonoids, arbutin, amygdalin, and pectin, which have the potential to be developed as antialopecia drug candidates through an in-silico approach to androgen receptors (PDB ID: 4K7A) and ADME-Tox profile predictions. The in-silico approach was conducted using the molecular docking method to predict the interaction between the active compounds from the Rampai plant and the androgen receptor using Autodock Tools 1.5.7 and Vina software, while the ADME-Tox analysis was conducted through the pkCSM platform. The molecular docking results showed that the reference ligand (Minoxidil) had an affinity energy of −7.353 kcal/mol, while the test compound with the best affinity was isorhamnetin with a value of −8.398 kcal/mol, which was lower (more stable) than Minoxidil. In addition, the ADME-Tox prediction results for isorhamnetin show favorable pharmacokinetic characteristics, especially in terms of skin permeability, absorption and distribution. Thus, isorhamnetin has the potential as an androgen receptor antagonist and a role in the development of therapies for alopecia.
