Abstract

The use of trauma scoring in the hospital is very important because it has a significant impact both clinically and economically. However, not all trauma scoring that we know today is suitable for use in low to middle-income countries or applied in hospitals with limited resources. The purpose of compiling a systematic review is to identify which trauma scoring is most appropriate for use in low to middle-income countries. This systematic review was built by identifying scientific articles published between 2010-2019. Search for articles was carried out with the keywords "Trauma Scoring" and "Low And Middle-Income Country". The search was carried out with the help of databases including ProQuest, science direct, and PubMed and SpringerLink. From this search, we found 3802 articles. Furthermore, the 3802 articles were screened with the help of PRISMA flow diagrams and criticized with the JBI tool. After that, 9 articles relevant to the research question were obtained to be analyzed into a systematic review. The results showed that 4 articles were stating that the Kampala Trauma Score (KTS) had a higher AUC ROC value when compared to another trauma scoring. Also, in these four articles, KTS had the lowest AIC score. Thus, it can be concluded that VCT is trauma scoring which is most suitable to be applied in low to middle-income countries or hospitals in rural areas with limited resources.

Abstract

The late arrival of stroke patients to the hospital is still a problem that still occurs in hospitals in several regions in Indonesia. Delay in treatment of stroke patients causes permanent disability and death. The time it takes for stroke patients is less than 3 hours to get thrombolytic therapy, which is started when an attack occurs. Where, the effectiveness of therapy is determined by the time the patient arrives at the hospital. Patient arrival to the hospital affects patient outcomes. Purpose: to analyze the factors associated with family delays in bringing stroke patients to the ER at Labuha Hospital. This study used an observational analytic study design with a cross sectional design approach. The research location is in the Emergency Room of the Labuha Hospital. Research time was September-November, 2020. The sampling technique used nonprobability sampling with purposive sampling technique totaling 42 stroke patients. The measuring instrument used is a questionnaire. Based on the Spearman test, it shows the relationship between the factors of knowledge (p = 0.01), awareness (p = 0.03), decision making (p = 0.00), economy (p = 0.00) and behavior (p = 0.00) with family delays in bringing stroke patients to the ER at Labuha Hospital. The multiple linear regression test shows that awareness is the most dominant factor associated with family delay with the coefficient value β -11.051 compared to other factors. The time delay of stroke patients decreased when knowledge, awareness, decision making, economy and family behavior were good.

Abstract

The surface dose on electron irradiation which is received by the skin does not reach 100%, so a bolus is needed as a compensator material in order to reach or approach 100%. This study aims to create, test, and describe the effect of different thicknesses of boluses that are made of 3D printed TPU, silicone sealant and resin on equivalence with tissue and the percentage of surface dose produced. A bolus with a size of 15x15 cm2 and with variations in thickness of 0.3 cm, 0.5 cm, and 1 cm was imaged by a CT-Scan to analyze the CT-Number value and relative electron density using imageJ software. After that, the bolus was irradiated by a Linac with an energy of 10 MeV and 12 MeV to measure the surface dose using an advances marcus detector. The result of this study showed that 3D printed TPU, silicone sealant and resin are similar to some soft tissues. Silicone sealant has the highest flexibility of the two boluses. In addition, silicone sealant also produces the highest increase in the percentage of surface dose in phantom.

Abstract

The prevalence of malocclusion and the need for orthodontic treatment in Indonesia is relatively very high. Orthodontic treatment that transmits mechanical forces to the periodontal ligament and alveolar bone cells can stimulate the periodontal ligament and alveolar bone remodeling. While this time, orthodontic treatment requires a long time, resulting in several adverse effects and discomfort. For this reason, a method that can accelerate orthodontic treatment is needed which aims to increase stability after active orthodontic treatment, to reduce the potential for relapse, and to accelerate retention time. Pulsed Electromagnetic Field (PEMF) stimulation is one of the methods that have this goal. This aim to study the stimulation mechanism of PEMF on the osteogenesis process of orthodontic tooth movement. Alveolar bone remodeling played an essential role in orthodontic tooth movement, is an active and dynamic process. The dynamic process depends on a balance between resorption and bone formation (osteogenesis). PEMF stimulation is a nonsurgical method that can promote osteogenesis through activation of Wnt signaling. Several studies showed that this method could accelerate retention time and orthodontic treatment. PEMF stimulated osteogenesis through activation of Wnt signaling in orthodontic tooth movement.

Abstract

Background: Times New Roman 9, single space, contains the brief description of the research. Stroke is the greatest cause of disability and mortality worldwide. Several biological mechanisms underlying this disease such as failure of glutamate reuptake and ATP synthesis, resulting in high levels of reactive oxygen species (ROS), neuroinflammatory responses, and apoptosis, resulted in cell death and brain tissue damage. Neurotherapeutics agents are given to affect the pathophysiological pathways and prevent expanding infarct area. Objective: This study aims to analyze the modulation of Gluthatione S-Transferase (GST) and Tyrosine kinase receptor C (TrkC) by 7,8-DHF as neuroprotective agent in ischemic in silico. Methods: This study used in silico simulation to predict 7,8-dihydroxyflavone (DHF) as neuroprotective agent by using PubChem, RCSB, Biovia Discovery Studio, PyRx, and PyMol. This study analyzes the pharmacodynamics, pharmacokinetics, and molecular interactions between 7,8-DHF as a ligand with GST (13GS) and TrkC (6KZC) as protein target, compared to their native ligand. Results: 7,8-DHF may increase intracellular endogenous antioxidants mainly GST and stimulate TrkC to activate further neuron survival signaling. 7,8 DHF has a much lower bond energy (-8.1 Kcal/mol) when it binds to GST compared to the native ligand (-5.9 Kcal/mol). Besides, binding affinity between 7,8-DHF-TrkC was -9 Kcal/mol, while native ligand-TrkC was -10.6 Kcal/mol. This study showed that there were the same amino acid residues between 7,8-DHF-GST and 7,8-DHF-TrkC, compared to their native ligand. Conclusion: As an adaptive response to hypoxia caused by ischemic stroke, these findings are likely to induce protective mechanism through indirectly TrkC activation which regulates neurogenesis and increasing intracellular endogenous antioxidants.

Abstract

Background: Stroke, particularly ischemic stroke, is one of the leading causes of death worldwide. Ischemic stroke causes a failure of oxidative phosphorylation and ATP synthesis, resulting in high levels of reactive oxygen species (ROS), neuroinflammatory responses, and apoptosis, all of which result in cell death. Neuroprotective agents are given to prevent the infarct area from expanding. Objective: This study aims to predict an in silico interaction by 7,8-dihydroxyflavone as neuprotective agent through TrkB signaling and inhibiting Glutaminase activity. Methods: In silico simulation with 7,8-dihydroxyflavone (DHF) as neuroprotective agent using PubChem, RCSB, Biovia Discovery Studio, PyRx, and PyMol software. This study analyzes the pharmacokinetics, pharmacodynamics, and protein-ligand interactions between 7,8-DHF as a ligand with TrkB (4AT5) and Glutaminase (5JYO) as protein target, compared to their native ligand. Results: 7,8 DHF binds to 4AT5 and 5JYO with lower bond energy (-9.4 Kcal/mol and -6.3 Kcal/mol, respectively) than the native ligand (-5 Kcal/mol and -5.9 Kcal/mol, respectively). It means that 7,8-DHF may increase protective mechanism. Conclusion: These findings tend to increase downstream signaling pathways, leading to increased TrkB expression, which induces protective mechanisms, and decreased glutamate expression, which reduces glutamate toxicity.

Abstract

Stroke is a serious health problem faced by almost all over the world. The occurrence of ischemic stroke could affect the gray matter volume (GMV) in the human brain. In Magnetic Resonance imaging of an ischemic stroke with a FLAIR sequence, a white lesion known as the white matter hyperintensity (WMH) is often found. We studied correlation between gray matter volume (GMV) and white matter hyperintensity (WMH) in ischemic stroke patients using fluid attenuated inversion recovery (FLAIR) sequences. The MRI data were taken randomly from ten ischemic stroke patients (mean age 46.9 years) and ten normal patients (mean age 36.8 years). FLAIR axial image parameters, i.e. TE (time echo), TR (time repetition), and TI (time inversion) were analyzed. The results reveal that the TE and TR values in FLAIR images of normal and ischemic stroke patients are classified as long TE (more than 70 ms), long TR  (more than 1500 ms) and long TI (more than 1700 ms). The mean GMV of ischemic stroke patients (1.371 cm3) is smaller than the GMV of normal patients (2.267 cm3). This is related to the presence of white matter hyperintensity (WMH) in ischemic stroke patients
