Abstract

In the fluid restriction program, patients with chronic renal failure will cause thirst. The patient controls this thirst by drinking. As a result, if it is not controlled, it can increase the accumulation of fluids which can cause edema and shortness of breath. Thirst needs to be controlled so that patients can adhere to fluid restriction programs and reduce the risk of complications. One of the ways used to overcome thirst is by gargling boiled water. This case study aims to obtain an overview and results of the application of gargling with boiled water to reduce thirst in CRF patients who are undergoing hemodialysis. The research method used a case study with a nursing care approach in the application of evidence-based nursing practice to one managed patient who was given an intervention by gargling boiled water for three meetings. The application of gargling with boiled water can reduce the level of thirst in chronic kidney failure patients undergoing hemodialysis from moderate thirst (4-6) to mild thirst (1-3), with an average length of time to hold thirst for 45 minutes.

Abstract

ABSTRACT Diabetes mellitus is a metabolic disease that occurs due to decreased tissue sensitivity to insulin or inadequate beta cells in producing insulin. High blood sugar levels can cause several complications that attack the body's organs, such as hypertension and acute kidney injury (AKI). Therefore, an intervention is needed to minimize the occurrence of complications, namely Diabetes Self Management Education (DSME) and acupressure massage. This study aims to determine the results of the Diabetes Self Management Education (DSME) nursing care intervention and acupressure massage therapy on blood glucose levels and self-management in patients with type 2 diabetes mellitus. This research uses descriptive research with a case study design using a nursing process approach. by conducting assessments, compiling nursing diagnoses, nursing interventions, implementation, and evaluation. This study shows that the nursing problems that arise in patients are knowledge deficits related to lack of exposure to information and blood glucose instability related to insulin resistance. The results of applying Diabetes Self Management Education (DSME) to clients show an increase in knowledge related to diabetes mellitus diet. Meanwhile, the results of applying acupressure massage showed a decrease in blood glucose levels in patients with type 2 diabetes mellitus. Abstract

Type 2 diabetes mellitus is classified as a chronic metabolic illness that has the potential to produce several complications, with peripheral neuropathy being among the most common. Peripheral neuropathy is characterized by sensory disturbances such as tingling, burning sensation, pain, and numbness in the lower extremities, which, if left untreated, may progress to diabetic foot ulcers and even amputation. One of the non-pharmacological interventions to prevent and reduce neuropathy is foot exercise, which improves blood circulation and enhances peripheral nerve function. To determine the effect of foot exercise on neuropathy levels among elderly patients with type 2 diabetes mellitus at Colomadu II Primary Health Cente: The research adopted a quasi-experimental method with a pretest and posttest control group design. A total of 68 respondents were recruited using purposive sampling, consisting of 34 respondents in the experimental group and 34 in the control group. The intervention was carried out for 3 weeks with a frequency of 3 times per week, lasting ±30 minutes per session. Neuropathy levels were measured using the Toronto Clinical Scoring System (TCSS). Data were analyzed utilizing paired sample t-test and independent sample t-test.The outcomes revealed a significant decrease in neuropathy levels in the experimental group, with the mean score decreasing from 10.56 before the intervention to 7.21 after the intervention (p = 0.001). In contrast, the control group showed no significant changes (p = 0.162). Furthermore, the independent sample t-test revealed a significant difference between the experimental and control groups after the intervention (p = 0.001 < 0.05). Foot exercise has a significant effect in reducing neuropathy levels among elderly patients with type 2 diabetes mellitus. Foot exercise can be recommended as a simple, low-cost, and practical non-pharmacological intervention that patients can perform independently to prevent diabetic neuropathy complications.

Abstract

Stroke often causes aphasia, a language disorder that hinders verbal and nonverbal communication and reduces the quality of life of patients. This study aims to evaluate the effectiveness of face expression exercises and oral motor exercises in improving face symmetry and communication skills in patients with aphasia stroke. A case study was conducted on two non-hemorrhagic stroke patients with aphasia at Dr. Soehadi Prijonegoro General Hospital in Sragen. The intervention was carried out for three consecutive days, twice a day, in the form of a combination of face expression and oral motor exercises using a mirror. The evaluation was conducted using the Sunnybrook Face Grading System and observation of oral motor function. The results showed an increase in the Sunnybrook score from 40 (grade IV, moderate to severe impairment) to 55 (grade III, mild to moderate) in the first subject, and from 47 (grade III, mild-moderate) to 65 (grade II, near normal) in the second subject. Both experienced an increase in face symmetry, tongue and lip muscle strength, and the ability to pronounce simple words. Face expression exercises and oral motor exercises proved to be effective as a simple, safe, and low-cost intervention to support communication recovery in stroke patients with aphasia.
