Abstract

Pasien hemodialisis harus menjalankan berbagai pengobatan untuk mengurangi gejala yang dirasakannya. Dalam menjalankan pengobatan tersebut, pasien hemodialisis perlu menjalankan self-management. Berbagai penelitian menunjukan bahwa banyak pasien hemodialisis yang tidak patuh dalam menjalankan self-management. Literasi kesehatan merupakan salah satu faktor penting yang menentukan kepatuhan pasien dalam pengobatan. Penelitian ini merupakan penelitian deskriptif untuk mengidentifikasi literasi kesehatan pasien hemodialisis. Tekhnik sampel yang digunakan adalah Concecutive sampling dengan jumlah 129 orang. Kriteria inklusi Pasien hemodialisis yang memiliki kesadaran penuh disertai tanda-tanda vital stabil. Kuesioner yang digunakan dalam penelitian ini dikembangkan dari HLS-EU-Q47 pada pasien dialisis yang dikembangkan oleh Martin et.al. (2012). Hasil penelitian ini menunjukan bahwa sebagian besar sebagian besar responden memiliki literasi kesehatan yang cukup yaitu 96 orang (74,4%) dan 33 orang memiliki literasi yang baik (25,6%).  Item yang dirasakan cukup mudah oleh responden adalah mencari informasi kesehatan di semua ruang lingkup, memahami informasi dalam pencegahan penyakit, menilai informasi kesehatan di semua lingkup, serta menggunakan informasi dalam promosi kesehatan. Kesimpulan hasil penelitian ini adalah sebagian besar pasien hemdoialisis memiliki literasi kesehatan yang cukup, tetapi masih ada beberapa item yang dirasakan masih sulit untuk dilakukan pasien. Perawat diharapkan dapat membantu untuk meningkatkan literasi pasien dengan memberikan edukasi, motivasi dan evaluasi secara menyeluruh dan berkesinambungan.

Abstract

Gagal ginjal terminal yang dialami pasien hemodialisis dapat menimbulkan berbagai perubahan dalam kehidupannya. Persepsi yang positif terhadap penyakit dapat membantu pasien hemodialisis dalam menerima keadaannya dan meningkatkan motivasi untuk menjalankan berbagai tindakan pengobatan. Apabila pasien hemodialisis memiliki persepsi yang negatif terhadap penyakit, maka akan cenderung mudah mengalami berbagai masalah psikologis. Penelitian ini dilakukan untuk mengetahui persepsi terhadap penyakit pada pasien hemodialisis di Bandung. Penelitian ini merupakan penelitian deskriptif kuantitatif yang dilakukan kepada pasien hemodialisis di salah satu Rumah Sakit di Bandung. Teknik sample yang digunakan adalah consecutive sampling sebanyak 126 orang. Kriteria inklusi dalam penelitian ini adalah pasien gagal ginjal yang menjalani hemodialisis dan memiliki tanda-tanda vital yang stabil. Instrumen yang digunakan untuk mengukur persepsi terhadap penyakit adalah kuesioner persepsi penyakit singkat (Brief-IPQ) yang dikembangkan oleh Broadbant, et.al. tahun 2005, dan sudah dilakukan back translate ke dalam bahasa Indonesia. Hasil penelitian menunjukkan bahwa sebagian besar pasien hemodialisis memiliki persepsi terhadap penyakit yang negatif (50,4%). Sebagian besar pasien merasakan berbagai dampak penyakit terhadap kehidupannya dan mengalami perubahan secara emosional semenjak mengalami gagal ginjal terminal. Persepsi terhadap penyakit yang negatif pada pasien hemodialisis dapat mempengaruhi kualitas hidup, angka kesakitan dan capaian pengobatan yang dijalaninya. Oleh karena itu, dibutuhkan dukungan keluarga dan sosial. Petugas kesehatan khususnya perawat diharapkan dapat memberikan edukasi dan konseling pada pasien hemodialisis untuk meningkatkan persepsi pasien terhadap penyakit.

Abstract

Gangrene is one of Diabetes Mellitus (DM) complication causing tissue death in wound and decreased peripheral sensation. Priority management of gangrene is through tissue reperfusion and wound care with modern dressing to enhance healing and increase periphery circulation. This research aimed to report wound healing progress and peripheral sensation gangrene in a patient who was given hydrogel and antimicrobial dressing. Research design used a descriptive case study approach. Sample involved one inpatient in West Java regional public hospital who was diagnosed with gangrene, had necrotic wounds, and poor peripheral perfusion, currently treated with modern dressing hydrogel and antimicrobial dressing. The instrument used is Bates Jensen Wound Assessment Tools (BJWAT), lowest score shows significant wound healing progress. Decreasing wound healing score using BJWAT questionnaire from 41 to 29, peripheral sensation (+), and monofilament score from 0/9 to 5/9 after the intervention. Modern dressing improved wound healing and peripheral sensation in gangrene pedis patient. Thus, it is expected to examine effectiveness of hydrogel and antimicrobial dressings on healing scores and peripheral sensation in gangrene pedis patients more extensively.

Abstract

ABSTRAKPemerintah Indonesia melalui Stranas Percepatan Pencegahan Anak Kerdil (stunting) 2018-2024 memprioritaskan pencegahan dan penurunan prevalensi stunting dengan menyasar berbagai penyebab langsung dan tidak langsung yang memerlukan kerjasama dan koordinasi lintas sektor di seluruh tingkatan pemerintah, swasta, dunia usaha dan masyarakat, terutama di Posyandu. EGrowth Monitoring System (EGMS) yang dirancang ini berfungsi untuk mengukur tumbuh kembang bayi dan balita, terutama mendeteksi terjadinya stunting dan gizi buruk sejak dini agar dapat dilakukan upaya perbaikan secara tepat. Implementasi produk inovasi ini menggunakan sensor ultrasonik dan load cell untuk mengukur tinggi badan dan berat badan bayi dan balita, di mana rentang error pembacaan sensor-sensor tersebut secara berturut-turut adalah 0,01 – 4,36% dan 0,00 – 1,43%.Kata kunci: stunting, e-growth monitoring system, berat badan, tinggi badan, posyandu ABSTRACTIndonesian government, through National Strategy for Preventing Child Stunting 2018-2024, prioritize both prevention and reduction of stunting prevalence by targeting the causes. It requires inter-sector coordinations between government, private companies, businesses, and citizens, especially in local clinics. E-Growth Monitoring System (EGMS) is proposed to measure the growth of infant and toddler, specifically to detect a possible stunting and bad nutrition as early as possible in order to devise an improvement effectively. The implementation of this innovation employs the use of both ultrasonic sensor and load cell to measure the body height and weight of infant and toddler. Its expected margin of error for the sensors are 0.01 – 4.36% and 0.00 – 1.43% for ultrasonic sensor and load cell, respectively.Keywords: stunting, e-growth monitoring system, weight, height, posyandu

Abstract

ABSTRAK Pasien Penyakit Ginjal Kronik harus melakukan self-management termasuk pada pengaturan nutrisi yang harus dikonsumsinya. Hal ini menyebabkan pasien PGK mengalami kebingungan dan ketakutan untuk mengkonsumsi makanan. Selain itu, Pasien PGK sering mengalami mual dan muntah akibat dari peningkatan kadar ureum dan kreatinin sehingga dapat mengurangi asupan nutrisi sesuai dengan kebutuhan. Kondisi tersebut dapat menyebabkan pasien mengalami malnutrisi yang dapat memperburuk kondisinya. Keluarga memiliki peran yang penting dalam memberikan dukungan kepada pasien PGK dalam menjalankan manajemen nutrisi. Program pengabdian kepada masyarakat ini dilakukan untuk memberdayakan keluarga dalam memberikan dukungan yang adekuat kepada pasien PGK untuk melakukan manajemen nutrisi. Program pengabdian ini dilakukan dengan memberikan edukasi kepada keluarga pasien PGK mengenai manajemen nutrisi dengan menggunakan metode ceramah dan tanya jawab serta menggunakan berbagai media edukasi. Program ini dievaluasi dengan menggunakan pre-post test. Terdapat peningkatan total skor pre-post test sebesar 29,8 % yang mengindikasikan adanya peningkatan pengetahuan keluarga mengenai manajemen nutrisi pasien PGK. Program pengabdian kepada masyarakat dengan menggunakan metode ceramah dan tanya jawab dapat meningkatkan pengetahuan keluarga dalam manajemen nutrisi pasien PGK. Tenaga kesehatan diharapkan dapat menlakukan edukasi kepada pasien dan keluarga secara berkesinambungan dan berkelanjutan terkait dengan manajemen nutrisi pada pasien PGK. Kata Kunci: Manajemen Nutrisi, Pemberdayaan Keluarga, Penyakit Ginjal Kronik  ABSTRACT Patients with chronic kidney disease (CKD) are required to perform self-management, which includes managing their nutritional intake. This causes CKD patients to experience confusion and fear of consuming food. In addition, CKD patients often experience nausea and vomiting due to increased levels of urea and creatinine, which can reduce nutritional intake according to their needs. These conditions can lead to malnutrition in patients, potentially worsening their condition. Families play a crucial role in assisting CKD patients with their nutritional management. The goal of this community service program was to enable families to effectively support CKD patients in their nutritional management. We conducted this community service program by educating families of CKD patients about nutritional management through lectures, question-and-answer sessions, and various educational media. We evaluated this program using a pre-post test. There was an increase in the total pre-post test score of 29.8%, indicating an increase in family knowledge regarding nutritional management for CKD patients. Conclusion: Community service programs using lecture and question-and-answer methods can improve family knowledge in nutritional management for CKD patients. We expect health workers to continuously and sustainably educate patients and families about nutritional management in CKD patients.  Keywords: Nutrition Management, Family Empowerment, Chronic Kidney Disease

Abstract

Introduction: Congestive Heart Failure (CHF) is a condition in which the heart is unable to pump blood effectively to meet the body’s needs, resulting in decreased cardiac output. This leads to fluid accumulation in the lungs due to increased pulmonary venous pressure, which subsequently triggers pulmonary edema. Pulmonary edema causes impaired oxygen diffusion in the alveoli, resulting in dyspnea as the primary complaint in CHF patients. The main nursing problem that arises is impaired gas exchange, indicated by an increased respiratory rate, decreased oxygen saturation, and the use of accessory respiratory muscles. To help address impaired gas exchange in CHF patients, non-pharmacological therapies such as the use of a hand-held fan and the implementation of an upright sitting position (orthopnea position) can be utilized. Purpose: To evaluate the effectiveness of the hand-held fan and orthopneic position interventions in CHF patients experiencing shortness of breath. Methods: This study employed a case study approach based on observation, interviews, and medical record reviews. The interventions hand-held fan and orthopneic position were administered over three days, with each session lasting 5 minutes. Results: Following the interventions, respiratory rate decreased from 30 breaths per minute to 25 breaths per minute, and oxygen saturation increased from 95% to 98%. The patient reported feeling more relieved, comfortable during breathing, and able to sleep more easily, although occasional shortness of breath persisted. Discussion: The interventions are physiologically beneficial: the orthopneic position helps reduce venous return to the heart, thereby lowering pulmonary pressure, while hand-held fan therapy alleviates the perception of breathlessness through sensory stimulation of the face, which activates the V2 branch of the trigeminal nerve. Conclusion: The combination of hand-held fan therapy and the orthopneic position is effective in relieving dyspnea, reducing respiratory rate, and improving oxygen saturation in patients with congestive heart failure

Abstract

Hemodialysis patients' self-management is critical for maintaining their quality of life and minimizing complications. Patients' health literacy and perception of the disease are essential for their acceptance of the condition and treatment. However, the relationship between these variables and self-management among hemodialysis patients remains unclear. This study aims to identify the relationship between health literacy, perception of disease, and self-management among hemodialysis patients. This correlational study was cross-sectional design, with 129 hemodialysis patients consecutively recruited from a hemodialysis unit of a tertiary hospital in West Java, Indonesia. Data were collected using the Brief-Illness Perception Questionnaire (Brief-IPQ), the European Health Literacy Survey Questionnaire (HLS-EU-Q47), and the End Stage Renal Disease Adherence Questionnaire (ESRD-AQ). The data were analyzed using Pearson correlation analysis. Most of the patients had sufficient or excellent health literacy (74.4%), negative illness perception (50.4%), and low self-management practices (71.3%). Pearson’s correlation analysis revealed a positive correlation was found between health literacy and self-management practices (r = .189; ρ=0.032). Additionally, there was a negative correlation between illness perception and health literacy (r = -.27; ρ= .002) as well as between illness perception and self-management practices (r = -.762; ρ= .000). Positive illness perception and better self-management practices correlate with a higher level of health literacy. These findings highlight the importance for healthcare staff to facilitate positive illness perceptions and self-management practices, and to consider these factors as vital aspects in developing self-management education programs for ESRD patients.

Abstract

Malaria is an infectious disease that caused by Plasmodium infection in red blood cells. The main clinical manifestations that appear in malaria are fever with shivering, shaking, and sweating and weakness. This condition can cause severe hyperthermia, which is at risk of causing brain damage, seizures, or delirium. This study aims to see the characteristics of fever and implementation in patients with malaria relapse. A 41-year-old man with a history of malaria four months ago. During the first malaria, the patient experienced fever to shivering and sweating as well as muscle pain and fatigue every morning. In malaria relapse, complaints of fever and chills every morning were exacerbated by complaints of dizziness as if being hit and weakness. Laboratory examination results found Plasmodium vivax gametocyte and schizont stages. The patient received paracetamol, cefixime, dexamethasone, and DHP Frimal therapy as well as Tepid Water Sponge therapy. After three days of treatment with a combination of pharmacological therapy and tepid water sponge, there was a decrease in temperature from 38.4°C with warm extremities, sweating, headache and shivering to 37.8°C on the second day and 36.7°C on the third day with warm extremities and no shivering, no headache and more fit. The administration of a combination of pharmacological and non-pharmacological tepid water sponge therapy in patients with malaria relapse was able to reduce hyperthermia. Further researchers are expected to be able to examine more widely the effectiveness of actions to reduce hyperthermia in adult patients with malaria.

Abstract

Pneumothorax is a condition in which air enters the pleural cavity of the lungs, potentially disrupting the respiratory process. A common clinical manifestation is severe shortness of breath during inspiration. Pursed-lip breathing is a non-pharmacological therapy that can relax the respiratory muscles and reduce dyspnea. This study aims to examine the application of pursed-lip breathing in a patient with pneumothorax and its effect on respiratory status. Mr. X, a 70-year-old fisherman, was admitted to the hospital with complaints of severe shortness of breath and difficulty breathing. The patient was medically diagnosed with right-sided pneumothorax due to ruptured bleb, suspected to be caused by chronic active pulmonary tuberculosis. A chest tube thoracostomy (CTT) procedure had been performed. The patient had a 55-year history of smoking 1.5-2 packs of cigarettes per day. This case report employed a nursing care approach. The intervention administered was pursed-lip breathing, conducted over three days with three repetitions per session. The evaluation focused on respiratory rate, oxygen saturation, and the level of dyspnea based on the Modified Borg Scale and the Modified Medical Research Council (mMRC) Dyspnea Scale. Evaluation results showed a decrease in respiratory rate from 25 to 21 breaths per minute and an increase in oxygen saturation from 94% to 97%. Additionally, dyspnea severity decreased from a Borg score of 3 to 2 and from mMRC grade 2 to grade 1 following the intervention. Pursed-lip breathing can improve respiratory status and reduce dyspnea, suggesting its potential as a non-pharmacological therapy for patients with pneumothorax.

Abstract

Hipertensi merupakan penyakit yang dapat menyebabkan kerusakan pada organ vital dan bahkan dapat mengancam jiwa. Hipertensi perlu dikontrol secara rutin untuk mencegah komplikasi yaitu dengan menerapkan self-care. Self-care profile terdiri dari tiga domain yaitu motivasi, self-efficacy, dan perilaku self-care. Tujuan penelitian ini yaitu mengetahui gambaran motivasi, self-efficacy, dan perilaku self-care pada pasien hipertensi. Jenis penelitian ini yaitu deskriptif kuantitatif. Sampel dalam penelitian ini sebanyak 219 responden yang diambil menggunakan teknik purposive sampling. Kriteria inklusi yaitu pasien hipertensi dengan kondisi umum baik, mampu berkomunikasi, membaca, dan menulis dengan baik. Instrumen yang digunakan yaitu High Blood Pressure Self-Care Profile (HBP-SCP). Hasil penelitian menunjukkan rata-rata usia responden yaitu 57,38 tahun, mayoritas berjenis kelamin perempuan (79%), dan pendidikan terakhir SD (40,2%). Domain motivasi, self-efficacy, dan perilaku self-care tergolong dalam kategori sedang. Skor rata-rata pada domain motivasi 55,35, kemudian diikuti skor domain self-efficacy 51,37 dan skor domain perilaku self-care 48,56. Hal ini menunjukkan bahwa skor domain motivasi paling tinggi dibandingkan dengan domain self-efficacy dan perilaku self care. Self-monitoring tekanan darah, aktivitas fisik, diet rendah lemak merupakan indikator dengan skor terendah, sehingga tenaga kesehatan diharapkan dapat memberikan promosi kesehatan untuk meningkatkan motivasi dan self-efficacy, terutama perilaku self-care terhadap self-monitoring tekanan darah, aktivitas fisik, dan diet rendah lemak.
