Abstract

Good medical record management will affect the quality of health services. A good medical record must be qualified: accurate, complete, contain clear data, contain new information data that shows the patient's current status, informative, and accountable. Medical record management greatly affects the quality of health services. Storage room (filing) where medical record documents are stored, because medical records are confidential and have legal aspects, physical security is the responsibility of health facilities Alignment is a form of medical record management that functions as a sequencing to put medical records on storage shelves so that they can be found again when needed by patients, medical personnel or other needs. The alignment used is like an alphabetic system or based on numerical systems such as the straight numerical system (straight Numerical System), middle number system (Middle Digit Filing System) and final number system (Terminal Digit Filing System). Aisyiyah Siti Khotijah Salam primary clinic Magelang Regency is a primary clinic with health workers consisting of general practitioners, specialists, midwives, nurses and administrative personnel. For medical recording personnel themselves, it is not adequate, so there are problems related to medical record management. In this community service activity, training was carried out related to the alignment of medical record files, previously participants were given material related to the alignment of medical records. The result of this study is the improvement of the alignment system at Pratama Aisyiyah Siti Khotijah Salam Magelang Clinic.

Abstract

The transformation towards Integrated Primary Healthcare (ILP) in Indonesia necessitates robust risk management to ensure service quality and patient safety. However, manual documentation and reporting at Community Health Centers (Puskesmas) hinder real-time risk response, creating a critical gap. This study aims to develop 'Kasir-Risk,' a digital prototype featuring an Early Warning System (EWS) dashboard, to mitigate risks and support accreditation standards. Employing a qualitative approach and prototyping method within the System Development Life Cycle (SDLC), this research involved 56 respondents from Puskesmas and the District Health Office of Bantul. Data was collected via Focus Group Discussions (FGD), observation, and documentation studies. The needs analysis revealed an urgent demand for an integrated tool for risk handling. The resulting Kasir-Risk application is a structured, secure, and data-driven system. Its design encompasses three main components: a functionality module for risk calculation and notifications, a user experience-based EWS dashboard, and a relational database. Built with modern technologies like Laravel, React.js, and MySQL, the prototype facilitates the entire risk management cycle—from identification and analysis to evaluation, mitigation, and reporting. This study concludes that Kasir-Risk serves as a viable digital solution for enhancing risk mitigation, real-time monitoring, and data-driven decision-making in primary healthcare transformation.

Abstract

Data on bed management in Indonesia is currently managed in an integrated manner by the Ministry of Health through the Integrated Referral System (SISRUTE) on the Satu Sehat website. Hospital bed management data can be presented in the Barber-Johnson Graph. The Barber Johnsons graph is a good tool. It makes it easier for managers to monitor the efficiency of bed use, because TOI (Turn Over Interval), LOS (Length Of Stay), BTO (Bed Turn Over), and BOR (Bed Occupation Rate) are presented in one graph. The problem currently faced by society is that the management of beds in healthcare facilities is not yet efficient and is still done manually. In some hospitals, they still describe the Barber-Johnson Chart in Microsoft Excel. Therefore, innovation is needed to develop the Barber-Johnson Graph Calculator application Johnson is based on an Early Warning System (EWS) called Klakson. The existence of efficient bed management will demonstrate quality service. This is certainly expected to impact improving public health in Indonesia. This research uses action research, with a qualitative data collection approach. Data analysis uses content analysis. The results of need identification show that hospitals, health centers, and clinics that provide inpatient care need a Klakson application based on the Early Warning System (EWS) in the form of notifications and sound to support early decisions. The results of the feasibility study show that users are accustomed to using computers, software, and hardware available to support the development of web-based information systems. Information system development using prototype methodology. The prototype test results using the Blackbock method show that the function/reaction is according to the actor, the system works well and can meet user needs. The development of the web-based Klakson application using prototype methodology stages has been tested in DIY clinics and shows that the system can meet user needs.

Abstract

Code precision has received significant attention due to the increased utilization of encoded procedural data. Coding errors have been documented in multiple research investigations. This study aims to assess the variables that affect coding quality. The prevalence of diabetes has increased substantially in the past two decades and is a significant cause of morbidity and mortality. Method: This study was conducted in 2 hospitals in East Java, Indonesia, that were selected through simple random sampling from a population of hospitals meeting the predefined inclusion criteria. The bed capacities of these hospitals are 211, with details of 62 and 149, respectively, for the specialised ones. The sample in this study was 60 medical record files taken randomly in 2022 in the case of diabetes mellitus. The result showed coding quality testing uses six elements: reliability, accuracy, relevancy, timeliness, completeness, and legibility. Data analysis was carried out analytically using the Fisher Exact test. The results of the study from 60 samples showed that four elements were significant out of a total of 6 aspects of coding quality elements. The four essential elements consisted of Accuracy (p=0.001), Reliability (p=0.001), Completeness (p=0.046), and Legibility (p=0.046). Reliability elements also impact coding accuracy or vice versa (p=0.001); Completeness also affects Legibility and vice versa (p=0.046). The odds ratio value of each component shows that Reliability and Accuracy are 8.782, which means that Reliability can increase Accuracy 8 times and vice versa. Meanwhile, completeness and legibility are at 3.818, which means completeness also increases legibility by three times and vice versa. The Hospitals should consider four significant coding quality elements, including completeness, accuracy, reliability, and legibility, for use in coding audits. Timeliness and Relevance were insignificant.

Abstract

Mental health stigma remains a major obstacle to accessing psychological services in urban communities. This study employs a Systematic Literature Review guided by PRISMA to examine public stigma, self stigma, structural barriers, and digital factors influencing help seeking behaviors. Out of 684 identified publications, 62 articles met the criteria and were thematically synthesized. The findings indicate that stigma is reinforced by cultural norms, social expectations, and gaps in mental health literacy. Economic barriers, limited workforce distribution, and fragmented service systems further restrict access. While digital mental health platforms offer new opportunities, digital inequalities and privacy concerns reduce adoption among vulnerable groups. The study highlights that reducing stigma and improving access require multidimensional approaches involving policy reform, community based interventions, mental health education, and safe, inclusive digital solutions. These findings provide an evidence based foundation for designing systemic interventions to strengthen urban mental health ecosystems in Indonesia

Abstract

Background: Clinical coding accuracy is a vital factor in the financial stability of hospitals under the BPJS Health casemix system. Errors in clinical coding frequently lead to pending claims, which hinder the hospital's cash flow. Objective: This study aims to analyze the underlying causes of casemix coding issues in pending claims for BPJS Health patients at Hospital X. Method: This research employed a descriptive analytical method with a qualitative approach. Data were collected through in-depth interviews with five informants, consisting of two clinical coders, one casemix coordinator, and two physicians, as well as document observations of medical records and claim regulations. Result: The findings indicated a pending claim rate of 10.2%, with 33.3% of these cases caused specifically by clinical coding errors. The primary causes were categorized into three factors: (1) Human Resources, involving low coder competency in ICD-10/9-CM guidelines and medical pathognomonics; (2) Documentation Process, including incomplete medical resumes and a lack of clinical evidence provided by physicians; and (3) System, characterized by the infrequent socialization of the latest claim regulations and technical agreements. Conclusion: Accuracy in casemix coding is highly dependent on the synergy between comprehensive clinical documentation and the coder's expertise. Hospitals are recommended to implement continuous professional training and routine medical audits to minimize financial risks.
