Abstract

The assignment of diagnosis codes and procedures must be done correctly to ensure the quality of medical records is met. What is often forgotten when giving a diagnosis code is the secondary code or external cause. This research aims to determine the recording, implementation and level of accuracy of external cause codes. Descriptive research method with a cross sectional design. The research sample was 48 injury medical record files. The research results showed that external cause information was documented on triage forms, emergency department assessments, initial adult inpatient nursing assessments, integrated patient progress notes, and discharge summaries. External cause coding is carried out by a coder with a medical record background in accordance with the SOP for coding implementation using electronic ICD-10 contained in SIMRS. The level of accuracy of the external cause code up to the 5th character (Category A) is 0%, the 4th character (Category B) is 38%, the 3rd character (Category C) is 27%, and the 1st character (chapter) (Category D) of 35%. The reason for the inaccuracy is because SIMRS does not accommodate coding up to the fifth character and the SPO does not yet indicate the officer's activity steps to determine up to the 5th character sub-category. It can be concluded that the accuracy of external cause coding at RSU PKU Muhammadiyah Bantul falls into the Fair criteria with an accuracy range of 20-40%.

Abstract

The quality of diagnosis codes is an important part of the hospital management level. The impact of a lack of quality diagnosis codes is the potential to reduce hospital income. A clinical coding audit needs to be conducted to review and analyze discovered errors and attempt to trace their source. The purpose of this research is to audit the code. The research approach was carried out through a quantitative descriptive approach. The research population was inpatient medical records of BPJS Health patients. Sample calculations use Slovin, with a margin of error of 10%. In this study, the clinical coding audit involved 3 coders in each hospital, coding experts from senior practitioners from Type B Hospitals and experts from academics. The results of the clinical code audit showed that the timeliness aspect was 100%, accuracy was 92.5%, completeness was 91.0%, relevance, and legitimacy were 87.5% each, while the lowest was in the reliability aspect at 80.0%. The results of the legibility aspect are in line with the results of relevance. This shows that determining clinical codes for case mix purposes must be supported by complete documentation of the patient's medical records.

Abstract

The spread of infectious diseases is a global problem that requires effective approaches for prediction and management. In recent years, Big Data technology has become a major concern in the healthcare field due to its ability to quickly collect, store and analyze large and diverse volumes of data. This opens up new opportunities to improve prediction and management of the spread of infectious diseases. This research aims to investigate the role of Big Data technology in predicting and managing the spread of infectious diseases. We want to identify effective methods for using big data to predict disease spread patterns and manage responses to them. The research method used in this research is a qualitative method in the form of literature analysis about the use of Big Data technology in the health sector, case studies of the implementation of Big Data systems to predict the spread of disease. The research results show that Big Data technology can improve predictions of the spread of infectious diseases by integrating data from various sources, including clinical, geographic, demographic and social data. Integrated Big Data systems can provide a better understanding of the factors that influence the spread of disease and enable faster and more effective decision making in responding to outbreaks. The conclusion of this research is that it confirms that Big Data technology has great potential in improving the prediction and management of the spread of infectious diseases. By effectively leveraging big data, we can improve our understanding of the dynamics of disease spread and implement more timely and efficient intervention strategies. Therefore, further investment and development in Big Data technology in the health sector is essential to strengthen capacity to face global health challenges.

A good medical record must be qualified: accurate, complete, contain clear data, contain new information data that shows the patient's current status, informative, and accountable. Medical record management greatly affects the quality of health services. Storage room (filing) where medical record documents are stored, because medical records are confidential and have legal aspects, physical security is the responsibility of health facilities Alignment is a form of medical record management that functions as a sequencing to put medical records on storage shelves so that they can be found again when needed by patients, medical personnel or other needs. The alignment used is like an alphabetic system or based on numerical systems such as the straight numerical system (straight Numerical System), middle number system (Middle Digit Filing System) and final number system (Terminal Digit Filing System). Aisyiyah Siti Khotijah Salam primary clinic Magelang Regency is a primary clinic with health workers consisting of general practitioners, specialists, midwives, nurses and administrative personnel. For medical recording personnel themselves, it is not adequate, so there are problems related to medical record management. In this community service activity, training was carried out related to the alignment of medical record files, previously participants were given material related to the alignment of medical records. The result of this study is the improvement of the alignment system at Pratama Aisyiyah Siti Khotijah Salam Magelang Clinic.
