Abstract

Background: Diabetes mellitus is a major risk factor for impaired wound healing. Conventional treatments are often expensive and carry a risk of side effects, necessitating the development of safe and effective topical therapy alternatives from natural ingredients. Arumanis mango leaves (Mangifera indica L.) are reported to contain bioactive compounds such as mangiferin and flavonoids, which have potential in the tissue regeneration process. Objective: To determine the effectiveness of an arumanis mango leaf extract ointment on the healing of incision wounds in male mice with alloxan-induced diabetes. Methods: This laboratory experimental study used 30 male BALB/c mice divided into 6 groups: normal, diabetic without treatment, diabetic with base ointment, and three diabetic groups treated with mango leaf extract ointment (concentrations of 5%, 10%, 15%). Diabetes was induced via intraperitoneal alloxan injection. Aseptic incision wounds were created on the mice's backs and topically treated with the ointment once daily for 14 days. Wound diameter and blood glucose levels were measured periodically. Data were analyzed statistically. Results: Groups treated with mango leaf extract ointment (10% and 15% concentrations) showed faster and complete wound healing (100%) by day 14, which was not significantly different from the normal group and was markedly better than the untreated diabetic group (39%). However, no significant reduction in blood glucose levels was observed in any diabetic group throughout the study. Conclusion: The arumanis mango leaf extract ointment is effective topically in accelerating incision wound healing in diabetic mice, without demonstrating systemic antihyperglycemic effects. This effect is presumed to be mediated by bioactive compounds acting locally at the wound site.

Abstract

Introduction: Diabetic ulcers are serious complications of diabetes mellitus that are difficult to heal due to hyperglycemia, oxidative stress, and infection. Beetroot (Beta vulgaris L.) contains betalains, flavonoids, and polyphenols which possess antioxidant and anti-inflammatory activities to accelerate tissue regeneration. Objective: This study aims to formulate beetroot ethanol extract into a gel preparation and test its effectiveness on wound healing in alloxan-induced diabetic rat models. Methods: This experimental study included beetroot extraction using 96% ethanol maceration, phytochemical screening, and gel formulation with varying extract concentrations (5%, 10%, and 15%). Thirty male rats were induced with alloxan (150 mg/kgBW) to reach a diabetic state, then given a 2 cm incisional wound. Rats were divided into 6 groups: Normal, Negative Control (gel base), Positive Control (Chloramphenicol), and extract gel groups of 5% (F1), 10% (F2), and 15% (F3). Wound length was observed for 14 days. Results: Phytochemical screening showed the extract was positive for alkaloids, flavonoids, saponins, steroids, and tannins. Physical evaluation of the preparation (pH, homogeneity, viscosity, and spreadability) met SNI standards. The effectiveness test results showed significant differences ($p < 0.05$) between treatment groups. The F3 group (15%) showed the best healing rate, comparable to the positive control and normal groups, with an average wound length of 0 mm by day 14. Conclusion: Beetroot ethanol extract gel has good physical stability, and the 15% concentration is the most effective in accelerating wound healing in diabetic rat models.

Abstract

Bervariasinya penggunaan obat akan mengakibatkan adanya perbedaan dalam biaya dan luaran terapinya sehingga diperlukan analisis efektivitas biaya penggunaan terapi metformin dan obat bahan alam untuk mengetahui penggunaan penanganan dalam mengatasi diabetes mellitus yang paling cost- effective. Penelitian ini bertujuan untuk mengetahui pilihan manakah yang lebih cost-effective antara metformin dan obat bahan alam pada pasien diabetes melitus. Penelitian ini merupakan penelitian eksperimental dengan rancangan deskriptif, dimana data yang diambil merupakan data dengan menggunakan uji coba tikus putih. Sampel pada penelitian ini sebanyak 6 kelompok dengan menggunakan uji coba tikus putih dengan masing masing kelompok sebanyak 18 ekor yang terdiri dari 3 ekor tikus putih yang menggunakan metformin dan 3 ekor yang menggunakan obat bahan alam. Berdasarkan hasil perhitungan nilai ICER pada penggunaan obat bahan alam dengan hasil dibagi menjadi tiga yaitu, OHT dengan nilai -210,45 Rp%, Jamu dengan nilai -70,72 Rp/%, dan Diabetadex - 1.489,16 Rp/%. Pada data ACER didapatkan nilai pada OHT dengan nilai sebesar 84,32 dengan efektifitas 47,43% dan biaya sebesar Rp. 4.000, Jamu memiliki nilai ACER sebesar 243,04 dengan efektifitas 16,45% dan biaya Rp.4.000, dan Diabetadex menunjukan nilai ACER tertinggi sebesar 518,81, dengan efektifitas 47,15% dan biaya Rp.24.467. Metformin memiliki nilai ACER paling rendah yaitu 11,09 dengan efektifitas 63,11% dan biaya Rp. 700,- penggunaan obat metformin. Berdasarkan hasil yang diperoleh dapat disimpulkan bahwa pengobatan yang cost-effective adalah Metformin.

Abstract

Irreversible skin aging begins in early adulthood due to intrinsic factors such as genetics and cellular metabolism and extrinsic factors such as ultraviolet exposure, smoking, pollution, and lifestyle, which trigger reactive oxygen species (ROS) and collagen damage, leading to wrinkles and reduced skin elasticity. This study aimed to formulate an optimal anti-aging cream from ethanol extract of porang tubers (Amorphophallus muelleri Blume) using variations in the HLB value of Span 60-Tween 80 emulsifiers. This quantitative experimental study used three cream formulas (F1: HLB 4.95; F2: HLB 5.7; F3: HLB 6.8) prepared from 3 kg of porang tubers obtained from Sirapit, Langkat. Evaluations included organoleptic properties, homogeneity, pH, viscosity, spreadability, adhesiveness, cycling-test stability, irritation, preference, and antioxidant activity using the DPPH method. The results showed that F2 was the optimal formula, with medium viscosity, spreadability of 6.2 cm², pH 5.8, stability for six cycles, and an IC₅₀ value of approximately 114 μg/mL, indicating moderate antioxidant activity. Formula F2 therefore has potential as a topical antioxidant preparation for preventing premature skin aging based on locally sourced tropical material.

Abstract

This research relies on the analysis of fat content from the food replacement product (x) and lotus root (nelumbo nucifera) fermentation by utilizing the Soxhlet method. The experiment was conducted by extraction using n-hexane solvent for six hours. The fat content was determined based on the difference in weight after extraction. The findings showed that the fat content in the food replacement product (x) reached 63.60%, while in the lotus root fermentation it was 89.19%. These high numbers are influenced by the composition of the ingredients used, the effectiveness of nonpolar solvents, as well as changes in the matrix structure due to fermentation which increases lipid accessibility. The fermentation process, which lasted for 22 days, enhances enzymatic effectiveness that damages cell walls, thereby facilitating the extraction of lipophilic compounds. The results of this study indicate that fermentation can increase the measurement of fat in plant-based food materials, but the figures obtained reflect the fat that was successfully extracted, not pure fat. According to regulations in Indonesia, there is not yet a maximum limit for fat content in foods for special nutritional needs.

Abstract

Management of Type 2 Diabetes Mellitus (T2DM) as a chronic pathology highly depends on the synergy between pharmacological interventions and lifestyle modifications. Patient health literacy plays a crucial role in determining the effectiveness of glycemic control and the prevention of long-term complications. This research aims to map the patient knowledge profile while identifying the determinant factors influencing it at Royal Prima Hospital, Medan. Utilizing a descriptive quantitative design with a cross-sectional approach, this study involved 100 respondents recruited through an accidental sampling technique. The research instrument employed an adapted version of the DKQ-24 questionnaire. Univariate analysis results confirmed that the majority of respondents (79%) possessed adequate understanding regarding their treatment regimen. Based on the Chi-Square test, a significant correlation was found in the gender variable ($p = 0.044$), while other demographic and clinical variables showed no meaningful association. Furthermore, binary logistic regression testing demonstrated a lack of simultaneous influence from all independent variables on the level of patient knowledge.

Abstract

Skin infections caused by this bacterium remain common, and antibiotic resistance continues to increase, creating a need for natural antibacterial sources. Andaliman leaves are one plant that can be used as a natural antibacterial active ingredient due to their content of flavonoids, tannins, saponins, terpenoids, and essential oils. This study aimed to determine whether n-hexane, ethyl acetate, and ethanol extracts of andaliman leaves exhibit antibacterial activity against Staphylococcus aureus. The study was conducted experimentally, beginning with successive extraction of andaliman leaves using n-hexane, ethyl acetate, and ethanol, followed by antibacterial activity testing using the disc diffusion method at concentrations of 20%, 30%, and 40%. The results showed that the n-hexane extract produced inhibition zones of 8.3±0.12 mm at 20%, 8.8±0.16 mm at 30%, and 9.3±0.04 mm at 40%. The ethyl acetate extract showed increased inhibitory activity with inhibition zones of 9.4 ± 0.28 mm at 20%, 12.7 ± 1.52 mm at 30%, and 16.0 ± 1.31 mm at 40%. The ethanol extract exhibited the highest antibacterial activity, with inhibition zones of 13.0±1.66 mm at 20%, 15.0±1.81 mm at 30%, and 19.4±1.12mmat40%.

Abstract

Skin aging is a natural process accelerated by free radical exposure, UV radiation, and environmental pollution, necessitating effective and safe anti-aging agents. This study aimed to evaluate the potential of red shoot leaves ethanol extract (Syzygium myrtifolium) as anti-aging through antioxidant activity and tirosinase enzyme inhibition assays in vitro. Extraction was performed by maceration using 70% ethanol (1:10) for 12 hours, centrifuged, sonicated, and concentrated using a rotary evaporator. Phytochemical skrining identified flavonoids, alkaloids, saponins, tannins, and triterpenoids. DPPH antioxidant assay showed inhibition percentages of 96.17–99.49% at concentrations of 200-800 ppm. Tirosinase inhibition test resulted in 82.54% inhibition. These results demonstrate that red shoot leaves extract has dual anti-aging potential through antioxidant protection and melanogenesis inhibition mechanisms. This study provides scientific basis for developing anti-aging cream formulations based on safe and sustainable local natural ingredients

Abstract

Functional foods provide additional health benefits beyond their nutritional value. One approach in the development of functional foods is through fermentation involving lactic acid bacteria (LAB). This study aims to isolate and characterize lactic acid bacteria produced from the fermentation of lotus root and food replacement X as alternative food ingredients. The study was conducted experimentally through a five-day fermentation process at 37°C. The results show that LAB was successfully isolated from the fermentation of lotus root with a colony count of 1.88 × 10⁵ CFU/mL at a 10⁻² dilution, while no LAB growth was found in food replacement X. Four pure isolates were obtained with colony characteristics of round shape, milky white color, non-motile, and catalase-negative. Two isolates were Gram-positive, while the other two were Gram-negative. All isolates showed the ability to survive in acidic pH conditions (pH 3), with isolate EDP 1 having the highest resistance. The results indicate that lotus root fermentation has the potential to be a source of LAB for functional food applications.

Abstract

Antibiotic resistance in represents a serious global health challenge, driving the search for alternative antibacterial agents from natural sources. The stem bark of the sikkam tree (Bischofia javanica Blume) has bioactive potential, yet systematic evaluation of its antibacterial activity based on solvent polarity remains limited. This study aimed to investigate the antibacterial potential of graded extracts of Sikkam stem bark against Staphylococcus aureus. Extraction was performed by graded maceration and antibacterial activity was assessed using the disc diffusion method, with amoxicillin and dimethyl sulfoxide serving as positive and negative controls, respectively. Phytochemical screening revealed the presence of phenolics, flavonoids, alkaloids, tannins, saponins, steroids/terpenoids, and quinones. Among the tested extracts, the ethyl acetate extract showed the highest yield (44.65%) and exhibited the strongest antibacterial activity, producing an inhibition zone of 13.75 ± 0.09 mm at concentration 500 mg/mL The ethanol extract also demonstrated strong and consistent inhibition zone of 12.90 ± 0.13 mm, while the n-hexane extract showed the lowest activity, with an inhibition zone of 10.86 ± 0.20 mm. This study indicates that ethyl acetate and ethanol extracts of sikkam stem bark have potential as natural antibacterial agents against Staphylococcus aureus.
