Abstract

Introduction: Recurrent diarrhea occurs repeatedly within a short period, between one and three months or more than once per year. Diarrhea is a leading cause of toddler deaths globally, including in Indonesia with 100,000 deaths annually. Identifying factors predicting recurrent diarrhea significantly impacts prevention efforts. Objectives: This research aims to create a prediction model for the incidence of recurrent diarrhea in toddlers at the Tanah Kali Kedinding Community Health Center, Surabaya. Methods: This research method was a retrospective observational analytical study with a case-control design, involving 80 toddlers (40 case samples and 40 control samples) selected through purposive sampling. The independent variable was recurrent diarrhea in toddlers. The dependent variables were birth weight, nutritional status, measles immunization status, vitamin A suplementation, breastfeeding patterns, and mother's hand washing habits. The instrument used was a questionnaire. Data analysis used simple and multiple logistic regression. Results: The research showed that the incidence of recurrent diarrhea was related to malnutrition status (p = 0.008, OR = 20.184), measles immunization status (p = 0.029, OR = 9.599), breastfeeding pattern (p = 0.002, OR = 15.270), and maternal hand washing habits (p = 0.001, OR = 15.917). Meanwhile, factors that were not significantly related included birth weight (p = 0.397) and vitamin A suplementation (p = 0.456) Conclusions: Factors that influenced the formation of a prediction model for the incidence of recurrent diarrhea in toddlers were malnutrition status, measles immunization status, breastfeeding patterns, and mother's hand washing habits.

Abstract

Background: CLABSI accounts for 70% of all hospitalized bloodstream infections, especially in premature infants. CLABSI in neonates can compromise the immune system and future quality of life. Purpose: This study aims to identify determinant factors associated with CLABSI in neonates.  Methods: This research was a scoping review study of three international databases namely Science Direct, PubMed, and ProQuest from January 2021-April 2024. Keyword selection refers to PCC (Populations, Concepts, Categories), with the keywords used "factor" AND "associated" AND "CLABSI" AND ("neonate" OR "newborn") AND "hospital." The selection process is depicted with PRISMA-ScR. Results: A total of 14 articles met the criteria and objectives of the study. The articles obtained        represented   all continents, which were conducted in the Americas (29%), Europe (29%), Asia (29%), and Africa (13%). Risk factors for CLABSI in neonates are grouped into two, namely intrinsic and extrinsic factors. Prevention of CLABSI in neonates can be done by modifying extrinsic factors, namely with proper aseptic technique, supervision, and management strategies. Conclusion: The determinant factors of CLABSI in neonates include intrinsic factors (gestational age of <27 weeks or preterm, comorbidities, being born by cesarean section, low birth weight, being previously treated with antibiotics, and having underlying surgical conditions) and extrinsic factors (the type of catheter, the amount of catheter lumen, long duration of insertion, insertion location at the femoral and internal-jugular access site, parenteral nutrition, and large microbial colonization at the insertion site).

Abstract

Satuan Tugas Imunisasi Ikatan Dokter Anak Indonesia secara periodik melakukan kaji ulang jadwal imunisasi dengan menyelaraskan dengan buku pedoman imunisasi di Indonesia edisi ke 7 tahun 2024, dan sumber-sumber lainnya. Jadwal imunisasi selalu dievaluasi ulang karena beredarnya vaksin baru yang telah disetujui oleh BPOM dan vaksin yang tidak tersedia lagi di Indonesia. Terdapat beberapa perubahan dalam jadwal imunisasi rekomendasi IDAI tahun 2024 yaitu ketersediaan jenis vaksin, seperti pada pneumococcal conjugate vaccine (PCV), vaksin rotavirus, vaksin varisela, vaksin dengue, dan vaksin human papillomavirus (HPV). Agar dalam pelaksanaan lebih mudah, disertakan juga tabel jadwal imunisasi tahun 2024 di akhir.

Abstract

Pandemi Covid-19 berdampak pada berbagai aspek, terutama pelayanan imunisasi. Pelayanan imunisasi terganggu karena banyak orang tua yang khawatir membawa anaknya untuk imunisasi ke puskesmas. Saat ini, di akhir pandemi Covid-19, aktivitas dan layanan kesehatan seharusnya mulai beroperasi kembali secara normal dan kecemasan ibu akan mulai berkurang karena kasus Covid-19 menurun dan protokol kesehatan mulai longgar. Penelitian ini bertujuan untuk menganalisis korelasi antara tingkat kecemasan ibu dan kunjungan imunisasi anak pada akhir pandemi Covid-19. Metode yang digunakan adalah observational analytics dengan pendekatan cross sectional. Total sampel adalah 100 ibu di Wonokusumo Surabaya yang memiliki anak usia 0-2 tahun dengan teknik sampling menggunakan purposive sampling. Analisis data dilakukan dengan menggunakan uji korelasi Rank Spearman. Hasil penelitian menunjukkan bahwa mayoritas ibu tidak memiliki kecemasan, yaitu 86 (86%) ibu. Sementara itu, mayoritas anak rutin berkunjung untuk imunisasi, yaitu 52 (52%) anak. Hasil analisis uji Korelasi Rank Spearman menunjukkan p = 0,903. Dapat disimpulkan bahwa tidak ada hubungan antara tingkat kecemasan ibu dan kunjungan imunisasi anak pada akhir pandemi Covid-19.

Abstract

The COVID-19 pandemic is yet to be over. Although the number of pediatric patients is relatively small, their clinical manifestations differ from the adult group. Multisystem Inflammatory Syndrome in Children (MIS-C) is one of the most recently discussed clinical manifestations. The clinical features of this case resemble those of Kawasaki disease or toxic shock syndrome with limited diagnostic methods. Patients experience gastrointestinal, mucocutaneous, cardiovascular, hematological, and neurological symptoms. There was also an increase in inflammatory parameters and cardiac disorders, which will also be confirmed by electrophysiological and radiological examinations. Management consists of the administration of immunoglobulins, steroids, and some anti-cytokines. When the patient goes into shock, fluid resuscitation is the mainstay of therapy. Until now, the number of MIS-C patients is relatively small compared to the number of COVID-19 cases, and the number of deaths is very minimal as well.

Abstract

Culex spp. is a vector for Japanese Encephalitis (JE) and filariasis in Indonesia. JE is caused by Japanese Encephalitis Virus (JEV). JEV attacks the central nervous system and causes inflammation of the brain, lifelong disability, and even death. Filariasis is a chronic infectious disease that can cause permanent disability. Transmission of JE and filariasis can increase due to the density of Culex spp mosquitoes. The purpose of this study was to identify the distribution of mutation variations in the Voltage Gated Sodium Channel (VGSC) and acetylcholinesterase-1 (ace-1) genes in Culex spp. mosquitoes in Surabaya. Mutation variations of the VGSC Culex spp. gene show that there are three genotypes, namely wildtype (LL), heterozygous (LS, FS), and homozygous (FF, SS). Heterozygous (FS) and homozygous (FF, FS) genotypes caused by point mutations in codon 1014 of the VGSC gene are found in the Culex spp. mosquito population in Surabaya. Heterozygous (LF) genotype was not detected in the Culex spp. mosquito population in Surabaya. Another study in Nigeria showed that homozygous/FF genotypes (45.1%) were more prevalent than heterozygotes/LF (36.3%) and wildtypes/LL (18.6%).

Abstract

Activated monocytes/macrophages and T lymphocytes that generate a cytokine storm are thought to play a critical role in the development of dengue. Cytokine storms, characterized by the excessive production of proinflammatory cytokines, can result in cellular dysfunction and organ failure, often contributing to the severity of dengue in affected patients. Interleukin-18 (IL-18), like IL-1beta, is a proinflammatory cytokine released during inflammation triggered by inflammasome activation. Increased IL-1beta and IL-18 during dengue virus infection (DVI) are known to worsen the host's vascular permeability, increasing hemostasis disorders and potentially, all of which are important elements in the pathophysiology of dengue fever. This study investigates the association between IL-1beta levels, IL-18 levels, age, dengue virus serotype, and the severity of dengue virus infection, aiming to understand how these factors interact and influence disease outcomes. This study is an observational cross-sectional design from 59 DVI patients in Dr. Soetomo General Hospital, Surabaya with positive dengue PCR results. Both IL-1beta and IL-18 levels reveal no significant relationship with the patient's age, infection status, and dengue virus serotype. A weak negative significant relationship between IL-1beta levels and the severity of DVI, indicating an inverse relationship between IL-1beta levels and the severity. Further studies are required to investigate the function of these cytokines in severe dengue.

Abstract

Maternal compliance as determining factor in increasing the coverage of double immunization injections for children <2-years-old at the Medokan Ayu Health Center in Surabaya. Based on local monitoring data, the average immunized child is still below the target of 90% which is certainly at high risk of exposure to hepatitis-B, tuberculosis, whooping cough, and diphtheria in children. This study aimed to analyze the factors that affect maternal compliance in administering double immunization injections to children <2-years-old at the Medokan Ayu Surabaya Health Center. This was observational research with a cross-sectional study design. Data was collected using questionnaires distributed directly to respondents. The population in this study was mothers who have children <2-years-old at the Medokan Ayu Health Center in Surabaya. Sample selected using the Multistage Random Sampling technique. There was no influence between demographic factors on maternal compliance in administering double immunization injections to children <2-years-old at the Medokan Ayu Health Center in Surabaya (p=0.855). There were effects of perceived vulnerability, perceived severity, perceived benefits, perceived obstacles, belief in success (self-efficacy), and signals to action on maternal compliance in administering double immunization injections to children <2-years-old at the Medokan Ayu Surabaya Health Center (p< 0.05). Mothers who have perceived obstacles, as well as disbelief in the success of double immunization injections tend to disobey in providing immunization injections to their children. The latest program from the Health Center which is home immunization visits is a to increase the coverage of double immunization injections for children <2-years-old.

Abstract

Diphtheria, caused by Corynebacterium diphtheriae, is a significant public health threat, particularly in areas with low immunization coverage. Despite national immunization programs, sporadic outbreaks continue to occur, especially in East Java, which reported the highest number of cases in 2021. The disease is transmitted through respiratory droplets and can lead to severe complications if not diagnosed and treated promptly. Diphtheria mortality can be reduced with appropriate treatment, along with good immunization status. Diagnosis can be made both clinically and through laboratory tests, including culturing the diphtheria bacteria from swabs of affected tissues. This community service program aimed to enhance the capacity of healthcare workers in East Java, specifically in Sampang Regency, to manage diphtheria through training that included both theoretical and practical components. The training methods used included pre- and post-tests to assess knowledge, mini lectures on epidemiology, clinical symptoms, diphtheria vaccination, and management, along with case simulations to improve participants' practical skills. The program was attended by 42 participants from various healthcare professions, including doctors, nurses, health analysts, and surveillance officers The evaluation demonstrated a significant improvement in participants' knowledge after the training. This program contributed meaningfully to enhancing preparedness among local healthcare providers and is expected to support more robust early detection and response systems for diphtheria outbreaks in the future.
