Abstract

Increasing patient loyalty through patient-provider relationships becomes the key factor in medical providers’ successes. Perceived value is the long-term value that customers provide to hospitals. The creation of added value for customers, and increasing the level of satisfaction will make a major contribution to the possibility of customers staying, and customers are increasingly valuable to service providers. This study explored the mediated relationship between patients’ perceived value, trust, and patient loyalty. This study aims to examine the perceived value and trust in patient loyalty both in hospitals and medical centers. This literature review study was searched from 3 databases, namely PubMed, Scopus, and Google Scholar published between January 1, 2018-May 31, 2022. Inclusion criteria were research on medical care loyalty, experience in the patient's perceived value of hospital services, and patient’s trust in the hospital. A total of 11 from 86 articles found were selected for review. Based on 11 articles that have been reviewed discussing customer value and trust in patient loyalty. Hospital and Medical Center as health provider has a relationship between patients’ perceived value of medical services, commitment to the patient-provider relationship, and patient loyalty. Furthermore, when the patient demonstrated higher levels of trust in a healthcare provider, the relationship of perceived value, commitment, and patient loyalty was also enhanced.

Abstract

Background: Discharge summary format is the one of the enhancement applied to Rekam Medis Elektronik system at Dr Kariadi Hospital. Discharge Summary data is very important to support clinical diagnosis when the patient is about to apply for medical checkup or when he is going to be transferred/referred to another health facility. Discharge summary is also required in claim process by a health insurance verifier or a third party authorized to cover the cost of patient care during hospitalization. Electronic discharge summaries is reliable and it reduces time to read patient histories and search for relevant examination results, helping to find faults, mistakes, or error, missing information faster. There are no published research data on how the quality of the electronic discharge summary is measured. The human, organizational, and technology suitability (HOT-fit) model is utilized to evaluate the implementation of the RME and e-discharge summary as well as the perceived usefulness and acceptance of the developed system.Purpose: Knowing that correlation between human, organization and technology characteristics with perceived usefulness, perceived ease of use of RME and discharge summary and acceptance of the systems, using HOT Fits models in Kariadi General Hospital Semarang Central Java.Method: The research was conducted to knowledge variable by 100 copies questionnaire that sent out to physician who assess inpatient. Ninety nine completed copies returned, indicating a valid response rate of 99%. The questionnaire was filled out by the respondent immediately after verifying the return summary using RME. Data analyzed using Pearson Correlation test. Statistical test was considered significant if p < 0.05.Results: Upon e-Discharge Summary testing, self-efficacy of the responden at 69,7% in the implementation of e-discharge summary  Perceived ease of use rated at 78.8%  as easy to use; while 76.8% Perceive of usefulness rate was achieved as respondents found themselves easy to navigate and fulfill all the requirements when verifying the electronic discharge summary and RME.Conclusion: There is a significant correlation between Human Organization Technology characteristics with Net Benefit in HOT Fit models. These correlation effect discharge summary acceptance. Suggestions; Extensive and comprehensive evaluation on the development of electronic discharge summary and RME is required to meet the Non-physician care provider achievement.Keywords: Electronic Medical Records; Electronic Discharge Summary; HOT – Fit Models.Pendahuluan: Pengembangan rekam medis elektronik di RSUP dr Kariadi dengan salah satu format yang dielektronikkan adalah ringkasan pulang (discharge summary). Data yang ada di ringkasan pulang sangat penting untuk mendukung diagnosis klinis saat pasien akan melakukan kontrol ulang atau saat akan dirujuk ke fasilitas kesehatan lainnya. Manfaat lain dari ringkasan pulang yang terisi lengkap sebagai persyaratan yang mempengaruhi proses klaim oleh verifikator asuransi kesehatan atau pihak ketiga yang berwenang untuk menanggung biaya perawatan pasien selama rawat inap. Ringkasan pulang elektronik mengurangi waktu membaca riwayat pasien dan pencarian hasil pemeriksaan yang relevan, membantu menemukan kesalahan atau informasi yang hilang dengan lebih cepat dan lebih andal. Belum ada data penelitian yang terpublikasi tentang bagaimana kualitas ringkasan pulang elektronik diukur. Kerangka kerja manusia, organisasi, dan kesesuaian teknologi (HOT-fit) digunakan untuk mengevaluasi implementasi RME dan ringkasan pulang elektronik serta persepsi manfaat dan penerimaan terhadap sistem yang dikembangkan.Tujuan: Mengetahui Hubungan antara karakteristik Human Organization Technology dengan Net Benefit ringkasan pulang elektronik dan RMEMetode: Penelitian deskriptif korelasi dengan jumlah sampel 100 orang dokter penanggung jawab pasien (DPJP) dan Asisten DPJP yang menjalankan praktek di bangsal rawat inap Rumah Sakit Umum Pusat dr Kariadi Semarang. Kuesioner diisi oleh responden saat itu juga setelah melakukan verifikasi ringkasan pulang menggunakan RME. Uji korelasi menggunakan uji Pearson Correlation. Uji statistik dianggap bermakna jika p < 0,05.Hasil: Responden yang mengganggap dirinya mampu menggunakan ringkasan pulang elektronik (self efficacy) sebanyak 69,7% responden. Persepsi Kemudahan Penggunaan ringkasan pulang elektronik sebanyak 78,8% responden mudah memahami saat melakukan ujicoba ringkasan pulang elektronik. Sedangkan persepsi penerimaan Ringkasan pulang elektronik dan RME sebanyak 76,8% responden merasa terampil saat melakukan verifikasi ringkasan pulang elektronik. Penelitian ini Karakteristik Human, variabel kompatibilitas pengguna berhubungan secara bermakna dengan persepsi kemudahan penggunaan ringkasan pulang elektronik (p = 0,000). Karakteristik Organization, variabel Dukungan top manajemen berhubungan bermakna dengan persepsi manfaat dan persepsi kemudahan penggunaan ringkasan, variabel Kompetensi Tim Pengembang berhubungan bermakna dengan persepsi kemudahan penggunaan ringkasan  pulang elektronik (p < 0,05). Karakteristik Teknologi berhubungan bermakna dengan persepsi manfaat dan kemudahan penggunaan ringkasan pulang elektronik. Persepsi Net Benefit berhubungan bermakna dengan penerimaan ringkasan pulang elektronik oleh penggunanya (p < 0,05).Simpulan: Terdapat hubungan bermakna antara karakteristik human organization technology dengan net benefit serta penerimaan pengguna terhadap ringkasan pulang elektronik dan RME. Saran: Evaluasi pengembangan ringkasan pulang elektronik serta RME secara menyeluruh dan komprehensif untuk melihat penerimaan profesional pemberi asuhan selain dokter.

Abstract

Semarang merupakan kota dengan jumlah kasus COVID-19 tertinggi di Jawa Tengah. Guna mencegah penyebaran COVID-19, maka diberlakukan kebijakan Pembatasan Kegiatan Masyarakat (PKM). Namun, masih terdapat banyak masyarakat yang tidak patuh terhadap kebijakan tersebut sehingga jumlah kasus COVID-19 di Kota Semarang semakin meningkat. Penelitian ini dilakukan untuk mengetahui faktor-faktor yang memiliki hubungan dengan kepatuhan masyarakat terhadap kebijakan PKM di Kota Semarang. Jenis penelitian yang digunakan adalah kuantitatif dengan desain cross-sectional. Sebanyak 140 responden didapatkan melalui metode accidental sampling. Analisis data dilakukan secara univariat dan bivariat menggunakan uji statistik non-parametrik. Hasil penelitian menunjukkan bahwa sikap (p=0,039), sarana prasarana (p=0,002), pengawasan (p=0,002), dukungan tokoh masyarakat (p=0,000), dukungan kewenangan (p=0,018), kejelasan kebijakan (p=0,000), dan konsistensi kebijakan (p=0,040) merupakan faktor-faktor yang memiliki hubungan dengan kepatuhan masyarakat terhadap kebijakan PKM di Kota Semarang.

Abstract

Rumah Sakit Umum (RSU) Pembina Kesejahteraan Umat (PKU) Muhammadiyah Gubug adalah rumah sakit yang telah memperoleh akreditasi utama pada tahun 2019 dan menjadi rumah sakit yang merawat pasien Covid-19, meskipun telah terakreditasi ketersediaan linen di RSU PKU Muhammadiyah masih mengalami kekurangan serta keterlambatan dalam pendistribusian linen. Tujuan penelitian adalah untuk memperoleh informasi terkait faktor input, proses, dan output dalam pengelolaan linen di ruang isolasi Covid-19 di RSU PKU Muhammadiyah Gubug Kabupaten Grobogan. Penelitian ini merupakan penelitian kualitatif dengan desain studi deskriptif observatif. Pengumpulan data dilakukan dengan wawancara indepth interview dengan teknik purposive sampling. Subjek penelitian terdiri dari 5 informan utama dan 5 informan triangulasi. Aspek yang diteliti pada penelitian ini mulai dari input, proses, sampai pada ouput dalam pengelolaan linen. Hasil penelitian ini menunjukkan bahwa pengelolaan linen masih belum optimal pada semua aspek, pada input masih terdapat kekurangan petugas laundry, dana dan sarana prasarana serta SOP belum berjalan optimal. Pada aspek proses menunjukkan bahwa perencanaan, pengorganisasian, pelaksanaan, monitoring dan evaluasi belum berjalan optimal, sehingga output yang dihasilkan masih tidak sesuai dengan standar yaitu kekurangan stok linen bersih dan terjadi keterlambatan pendistribusian linen karena stok linen yang terbatas, sehingga perlu dilakukan perbaikan pada aspek input dan proses pengelolaan linen.

Abstract

The cost of COVID-19 services can be a burden in itself that can affect hospital budgets. Services for COVID-19 patients at RSUP dr. Kariadi has so far been running well and smoothly, but there is no detailed data and information regarding COVID-19 financing, the process of submitting a COVID-19 financing claim and its implications for finances and services as well as leadership policies at RSUP dr. Kariadi so that activities are needed to analyze the costs of COVID-19. The purpose of the study was to analyze the financing of COVID-19, the process of submitting a claim for COVID-19 and its implications for finance and services as well as leadership policies at RSUP dr. Kariadi. The research approach used in this research is descriptive. Collecting data through document observation and interviews. The sample was selected using simple random sampling and the sample size was using the Slovin formula. The results showed that there was a negative difference of Rp.-1,832,258.63 between the average real cost of COVID-19 services and those claimed for outpatients, and there was a positive difference of Rp.61,695,327.45 for inpatients. The difference is caused by the claim system using cost per day. The biggest real cost component is the cost of laboratory examinations. The process for submitting a COVID-19 claim is in accordance with applicable regulations and all submissions declared eligible and paid. From the results of the Chi-Square test, it can be seen that there is no relationship between age and real costs in inpatient care and vice versa in outpatient care, length of days of treatment affects real costs, there is a relationship between comorbidities and real costs in SL3 and SL2, and there is no the relationship between co-morbidities with real costs in SL1 and outpatient. COVID-19 has caused the number of hospital visits and revenues in general to decline. Hospital leadership implemented several policies in dealing with the implications of COVID-19, including shifting budget posts, controlling services, zoning services, limiting non-COVID-19 patient services, transferring facilities and infrastructure, and optimizing the claim process. The research results are expected to be used as input in policy making and budget planning.  Keywords: Financing Analysis, COVID-19

Abstract

Health services require quality service and good patient safety on an ongoing basis, and accreditation is considered a powerful method to improve the quality of health services. This study aims to describe and analyze the role of leadership and motivation in achieving accreditation at RS X Pekalongan. This research is a qualitative descriptive study. Information was obtained through in-depth interviews, observation, and document review. The sampling technique used purposive sampling, with the number of main informants as many as 19 respondents and triangulation informants as many as 4 respondents. Based on the research results, all parties in the hospital play a role in the accreditation process and all employees have their respective roles. The leadership of the director in accreditation is very good, as evidenced by the design of tasks that can be carried out properly, and in terms of increasing employee motivation, the director has provided rewards both material and non-material, as well as training according to standards. . In maintaining the results of its accreditation, the hospital instills a sense of responsibility for accreditation to all employees. Hospital owners provide moral support to hospitals by participating in a series of accreditation survey events, providing useful input during meetings and also approving funding proposals related to accreditation. Keywords: Accreditation, leadership, motivation, achievement of accreditation

Abstract

ABSTRACTThe form of excellent service quality is often discussed in previous studies, including through indicators; tangibles, responsiveness, assurance, empathy and reliability. In this study, the service quality variable used is different from previous studies, namely using the Kennedy & Young theory, which uses indicators; availability, responsiveness, convenience and on time. The purpose of the study was to analyze the effect of patient perceptions of service quality on outpatient satisfaction of the nuclear medicine unit at the radiology installation of RSUP Dr. Kariadi Semarang. This type of research is observational with a quantitative approach, namely research that intends to test hypotheses. The research analysis unit is an outpatient who comes to get services from the nuclear medicine unit at the radiology installation of RSUP Dr. Kariadi Semarang in February 2022, as many as 314 patients, a sample size of 86 respondents was taken through simple random sampling.The results show that: the service quality variable affects the overall satisfaction variable, with an R2 value of 0.389. The service quality variable has an effect on the expectation variable with an R2 value of 0.456. The service quality variable affects the experience variable with an R2 value of 0.454. The conclusion of this study is that the service quality variable has a positive effect on patient satisfaction in the Outpatient Nuclear Medicine Unit, Radiology Installation, Dr. Kariadi Semarang.

Abstract

Medical consumable supply management is done in a continuous cycle, or a logistics cycle, in accordance with the goals of logistics management. The planning, budgeting, purchasing, storing, distributing, withdrawing, destroying, and controlling of the requirements come first. Logistics management strongly emphasize inventory and material movement from the point of origin to consumption to satisfy the demands of consumers and users. In other words, the logistics system aims to provide the most excellent service while minimizing costs. Inventory is a vital component of every organization such as hospital. This study aims to understand the management procedure in the hospital installation's logistics unit. The study used a narrative review methodology. The publications compiled for this study were found using the keywords "Logistics Management," "Hospital Logistics Management," "Medical Consumables," and "Logistics Management" in Scopus, Science Direct, ProQuest, SpringerLink, Google Scholar, and Emerald Insight. Consumable healthcare supplies the amount of the materials used was 11 articles. The findings demonstrate the significance of each element to the operation of logistics management. The study's findings indicate that the logistics management process for managing medical consumables has an impact and can serve as a model for the distribution management system for managing medical supplies based on supply chain management evaluations.

Abstract

Adanya pandemi COVID-19 dapat menurunkan kinerja keuangan rumah sakit akibat perubahan tatanan layanan. RSUD Kelet sebagai rumah sakit BLUD Provinsi Jawa Tengah harus memiliki tingkat kemampuan keuangan BLUD yang baik sehingga mampu meningkatkan kualitas pelayanan. Penelitian ini dilakukan untuk mengetahui tingkat kemampuan keuangan BLUD RSUD Kelet Provinsi Jawa Tengah pada masa pandemi COVID-19 berdasarkan likuiditas, solvabilitas dan rentabilitas keuangan BLUD pada masa pandemi COVID-19. Jenis penelitian yang digunakan adalah kuantitatif. Data yang digunakan adalah data sekunder dari laporan pola pengelolaan keuangan badan layanan umum daerah (PPK BLUD) RSUD Kelet Provinsi Jawa Tengah tahun 2019 sampai dengan tahun 2021. Pengolahan data dengan menghitung nilai persentase Current Ratio, Total Assets to Debt Ratio, Return On Assets (ROA) dan Return On Equity (ROE). Metode analisa data dengan deskriptif. Hasil penelitian ini menunjukkan bahwa nilai current ratio pada tahun 2020 sebesar 485,37% dan tahun 2021 sebesar 598,27%. Nilai current ratio ini masih memenuhi standar BLUD. Nilai Total Asset to Debt tahun 2020 sebesar 33.572,60% dan tahun 2021 sebesar 10.657,99%. Nilai Total Asset to Debt ini masih memenuhi standar BLUD. Nilai Return On Assets (ROA) tahun 2020 sebesar 3,15% dan tahun 2021 sebesar 1,92%. Nilai Return On Assets (ROA) belum mencapai standar. Nilai Return On Equity (ROE) tahun 2020 sebesar 3,16% dan tahun 2021 sebesar 1,94%. Nilai Return On Equity (ROE) belum mencapai standar. Dapat disimpulkan bahwa likuiditas dan solvabilitas keuangan BLUD RSUD Kelet Provinsi Jawa Tengah selama pandemi COVID-19 sudah baik. Namun rentabilitas keuangan BLUD RSUD Kelet Provinsi Jawa Tengah belum baik.

Abstract

An increasing number of Indonesians are seeking medical treatment abroad, causing the government to lose potential revenue streams from the health services industry. People in border regions have a wonderful opportunity to use health treatment facilities in neighboring countries because of their close proximity to them. One such community is in the Entikong District, which is next to Sarawak, Malaysia. The purpose of this study is to examine the factors that influence Entikong District residents' use of Sarawak health services. This study is an observational quantitative study with a cross-sectional design. The research population consists of 9,337 residents of the Entikong District aged 20 to 50. A proportional random sampling method was used to pick 100 respondents for the research sample. This research was conducted from September to October 2022. The data were analyzed using the chi-square test and logistic regression. The findings showed that income (p=0.012), reference group (p=0.026), and perceptions of health services (p=0.009) were associated with the use of health services by the residents of Entikong District in Malaysia, however education (p=0.961) and occupation (p=0.589) were not. There is a relationship between income, reference groups and perceptions of health services with utilization of health services. There is no relationship between education and work with service utilization. People with high incomes have more potential to take advantage of Sarawak health services. It is hoped that the Entikong Community Health Center, the Sanggau District Health Office, and the Sanggau District Government will be able to provide quality primary healthcare facilities and referral healthcare facilities in border areas to increase competitiveness with Sarawak health service facilities.
