Abstract

The stability of healthcare around the world is seriously threatened by nurse turnover intention, which is a primary predictor of actual turnover. The necessity to comprehend contributing elements is highlighted by the 47.7% new nurse turnover rate in Korea, which costs over USD 33,733 per nurse. This comprehensive review synthesizes quantitative, qualitative, and mixed-methods evidence to map factors that influence nurse turnover intention across hospital settings. Using a PRISMA-ScR approach, a literature search for "factors of nurse turnover" in Scopus produced 2,274 articles; 13 of these met inclusion criteria and were subjected to thematic analysis. Four primary areas were identified: leadership style (transformational, toxic, bullying), psychosocial aspects (work-life balance, job satisfaction, distress, and social support), and organizational factors (work environment, mentorship). Work-life balance and job satisfaction reduce risk by 30–40%, while a supportive work environment and adequate resources reduce turnover by 10–20%. While toxic or bullying leadership significantly raises turnover intention (β=0.51), transformational leadership enhances retention by 0.082 per unit. There are significant regional differences; for instance, 81.4% of Kenyan nurses plan to quit because their pay is less than $500 USD, which increases the probability of turnover three to five times. The use of cross-sectional research (76.9%) and the lack of worldwide representation are examples of gaps in the literature. To increase retention, holistic approaches that incorporate transformational leadership training, mental health assistance, healthy workplaces, and equitable pay are crucial.

Abstract

Marketing strategy is important in marketing management. Hospitals need to develop effective strategies to increase outpatient service visits. This study aims to analyze the relationship between the Segmenting, Targeting, and Positioning (STP) strategy with outpatient revisits at Waled Hospital, Cirebon Regency. The design used was quantitative descriptive with a cross-sectional approach in 110 patients (Slovin, 10% error tolerance) who were selected by consecutive sampling. Primary data were collected using a Likert scale questionnaire that was tested for validity (p<0.05) and reliability (?>0.60). The analysis included univariate for perception and bivariate using Chi-Square test (?=0.05) and Spearman Rank test (?=0.05). The results of the respondent's characteristic test showed that it was not related to the outpatient revisit of Waled Hospital. The results showed positive perceptions of segmenting (easy access, services for all ages, easy flow) and positioning (quality is considered good, many polyclinics, affordable prices), while targeting was relatively weak (innovation and promotion are less attractive; routine treatment is low). Although the proportion of repeat visits was higher in the "good" category in each variable, the Spearman Rank test showed a significant relationship between segmenting (p=0.320), targeting (p=0.287), and positioning (p=0.491) and repeat visits. These findings indicate the need to focus attention on other components of healthcare and a follow-up evaluation of hospital marketing strategies to ensure alignment with patient needs and perceptions.

Abstract

Background Inpatient nurses play a key role in maintaining healthcare service quality. High job demands require effective leadership to sustain nurses’ work motivation and ensure excellent service delivery. Objective To examine the relationship between leadership styles, nurses’ work motivation, and excellent service performance in inpatient settings. Methods This study employed a scoping review based on PRISMA guidelines. Thirteen national and international articles published between 2021 - 2025 were analyzed using narrative synthesis. Results Transformational, democratic, participative, and supportive leadership styles were positively associated with nurses’ work motivation. Work motivation acted as a mediating factor strengthening the relationship between leadership and excellent service performance, reflected in increased empathy, proactive behavior, and adherence to service standards. Conclusion Effective leadership significantly enhances nurses’ work motivation and excellent service performance. Leadership approaches emphasizing support and participation are essential to improve nursing service quality. Keywords: leadership style, work motivation, nursing performance, excellent service

Abstract

Program Jaminan Kesehatan Nasional (JKN) melalui BPJS Kesehatan bertujuan meningkatkan akses masyarakat terhadap pelayanan kesehatan. Seiring meningkatnya jumlah peserta, kepuasan pasien menjadi indikator penting dalam menilai kualitas pelayanan kesehatan. Kepuasan pasien dipengaruhi oleh berbagai faktor yang berkaitan dengan pelayanan kesehatan, karakteristik pasien, dan sistem pelayanan BPJS. Penelitian ini bertujuan untuk mengidentifikasi faktor-faktor yang memengaruhi kepuasan pasien pengguna BPJS Kesehatan. Penelitian ini menggunakan desain scoping review dengan pendekatan Population, Concept, Context (PCC). Population adalah pasien pengguna BPJS Kesehatan, Concept adalah faktor-faktor yang memengaruhi kepuasan pasien (faktor pelayanan rumah sakit, faktor dari dalam diri pasien, dan faktor pelayanan BPJS), dan Context adalah pelayanan kesehatan di rumah sakit serta sistem pelayanan BPJS. Pencarian artikel dilakukan pada database PubMed, Scopus, Cochrane Library, dan Google Scholar menggunakan kata kunci terkait BPJS, patient satisfaction, dan health services. Proses seleksi artikel dilakukan menggunakan PRISMA-ScR. Hasil scoping review menunjukkan bahwa kepuasan pasien dipengaruhi oleh faktor pelayanan rumah sakit seperti kualitas pelayanan, sikap tenaga kesehatan, fasilitas, dan waktu tunggu. Faktor dari dalam diri pasien meliputi karakteristik sosiodemografi dan persepsi pasien, sedangkan faktor pelayanan BPJS berkaitan dengan sistem rujukan dan prosedur administrasi. Faktor pelayanan rumah sakit merupakan faktor yang paling dominan memengaruhi kepuasan pasien. Penelitian menyimpulkan bahwa kepuasan pasien pengguna BPJS Kesehatan dipengaruhi oleh faktor pelayanan rumah sakit, karakteristik pasien, dan sistem pelayanan BPJS, dengan faktor pelayanan rumah sakit sebagai faktor yang paling dominan.

Abstract

Kualitas pelayanan kesehatan berperan penting dalam menentukan kepuasan pasien dan keberlanjutan layanan rumah sakit. Evaluasi pelayanan rawat inap eksekutif diperlukan karena tingginya ekspektasi pasien serta tuntutan peningkatan mutu layanan. Penelitian ini bertujuan untuk menganalisis kualitas pelayanan rawat inap eksekutif di Rumah Sakit Budi Rahayu Pekalongan menggunakan metode SERVQUAL dan Importance Performance Analysis (IPA). Penelitian kuantitatif dengan desain cross sectional ini melibatkan 160 pasien sebagai responden melalui kuesioner pada dimensi kehandalan, daya tanggap, jaminan, empati dan bukti fisik. Hasil menunjukkan tingkat kepuasan pasien sebesar 98,84%, dengan tingkat kesesuaian tertinggi pada dimensi jaminan (100%), diikuti daya tanggap (99,41%), kehandalan (98,96%), empati (98,35%), dan bukti fisik (95,57%). Hasil IPA mengidentifikasi indikator prioritas perbaikan pada Kuadran I ada pada dimensi kecepatan administrasi, empati petugas, ketersediaan perawat standby 24 jam, dan fasilitas fisik di ruang rawat inap eksekutif. Upaya perbaikan perlu dilakukan secara sistematis melalui penguatan manajemen sumber daya manusia, optimalisasi proses administrasi berbasis digital, serta peningkatan komunikasi tenaga kesehatan berbasis patient-centered care. Selain itu, peningkatan kualitas dan keselamatan fasilitas melalui implementasi K3RS juga menjadi prioritas. Seluruh upaya tersebut perlu diintegrasikan dalam siklus peningkatan mutu berkelanjutan untuk meningkatkan kualitas pelayanan dan kepuasan pasien secara optimal.

Abstract

Latar Belakang: Klaim Pending BPJS Kesehatan merupakan salah satu permasalahan yang sering terjadi dalam proses pembiayaan pelayanan kesehatan di rumah sakit dan dapat berdampak pada aspek finansial serta mutu pelayanan. Tujuan: Penelitian ini bertujuan untuk mengidentifikasi dan menganalisis faktor-faktor penyebab pending claim BPJS Kesehatan pada pasien rawat inap di rumah sakit. Metode: Penelitian ini menggunakan desain scoping review dengan sumber data dari Google Scholar, PubMed, Scopus, dan ScienceDirect. Seleksi artikel dilakukan berdasarkan kriteria inklusi dan eksklusi menggunakan pendekatan PRISMA, dan diperoleh 7 artikel yang relevan untuk dianalisis. Hasil: Hasil sintesis menunjukkan bahwa faktor administratif dan koding merupakan penyebab dominan klaim pending, terutama terkait ketidaklengkapan dokumen klaim dan ketidaktepatan pengkodean diagnosis serta tindakan medis. Selain itu, faktor klinis, teknis (sistem informasi), dan sumber daya manusia juga berkontribusi terhadap terjadinya pending claim. Kesimpulan: Pending claim BPJS Kesehatan merupakan permasalahan multidimensional yang memerlukan pendekatan komprehensif melalui peningkatan kualitas dokumentasi medis, akurasi koding, optimalisasi sistem informasi, dan penguatan kompetensi SDM.

Abstract

Abstract Communication in healthcare services is an important indicator in determining the quality of hospital services, particularly in improving patient satisfaction. This study aims to analyze the role of communication, both patient-centered communication (PCC) and therapeutic communication, on patient satisfaction in hospitals through a scoping review. The scoping review was conducted based on the PRISMA-ScR guidelines using the PCC framework, which includes population (patients receiving hospital services), concept (healthcare communication such as PCC, therapeutic communication, and interpersonal communication), and context (hospital services). Article searches were conducted through PubMed, Scopus, Cochrane Library, and Google Scholar databases using the keywords (“patient-centered communication” OR “therapeutic communication”) AND (“patient satisfaction”) AND (“hospital”). A total of 18 articles met the inclusion criteria and reported that effective communication has a significant relationship with increased patient satisfaction across various hospital service units, including inpatient, outpatient, and emergency departments. The most influential aspects of communication include empathy, clarity of information, patient involvement, and the ability of healthcare providers to build interpersonal relationships and trust. In addition, other factors such as response time, service quality, and cleanliness also contribute to patient satisfaction, although their influence is not as strong as communication. These findings indicate that improving the communication skills of healthcare providers is an important strategy to enhance service quality and patient satisfaction in hospitals.
