Abstract

Aims: Natural disasters can have an impact on the material (physical), psychological and social aspects. The traumatic experience is a psychological impact on victims that are often found. traumatic experience or traumatic stress if it lasts for 6 months will result in PTSD (Post Traumatic Stress Disorder). There are various interventions to treat PTSD, this intervention is known as trauma healing. Trauma healing can be one of the appropriate rehabilitation tools for disaster victims, especially to heal post-disaster trauma. The purpose of the study is to find out what trauma healing interventions are in handling PTSD in post-disaster victims. Method: The method used is a narrative review with PRISMA 2009 guidelines. The search is based on Garuda, EBSCO, Proquest, and PubMed. The studies reviewed were RCT research, quasi-experiment, and qualitative research in English and Indonesian with full text, as well as intervention studies within the last 7 years of publication from 2015  to 2021. Result: Trauma healing that can be given and proven effective to treat or reduce PTSD symptoms is CBT (Cognitive Behavioral Therapy), remembrance therapy, dhikr and play therapy. The results of the search used turned out to be for CBT therapy and play therapy were more effective in reducing PTSD in children, while dhikr was effective in adults, and remembrance therapy was very suitable for adults who experienced PTSD with other physical problems such as hypertension. Conclusions: Therapy as post-disaster trauma healing to overcome PTSD is divided into two, namely therapy for children and adults. Therapy for children is playing therapy and CBT, while for adults there is dhikr, and remembrance therapy is used for adults who experience physical disorders such as hypertension.

Abstract

AbstrakTingkat kepatuhan dan partisipasi pasien Penyakit Jantung Koroner (PJK) jantung pasca revaskularisasi dalam menjalani rehabilitasi jantung masih sangat rendah yang dipengaruhi oleh berbagai hambatan. Hal ini dapat menyebabkan terjadinya kekambuhan penyakit serta menurunkan kualitas hidup pasien. Penelitian ini bertujuan untuk mengetahui hambatandari pasien PJK dalam menjalani rehabilitasi jantung di Poliklinik Jantung RSUP Dr. Hasan Sadikin Bandung. Metoda penelitian menggunakan deskriptif kuantitatif dengan sampel sebanyak 42 responden yang menggunakan accidental sampling selama 1 bulan. Instrumen penelitian menggunakan kuisioner Cardiac Rehabilitation Barrier Scale dari Grace et.al tahun 2011 yang telah diterjemahkan. Analisa data menggunakan distribusi frekuensi dan rerata skor. Hasil penelitian diperoleh bahwa hambatan berdasarkan aspek logistik menjadi hambatan dengan rata-rata skor (mean) tertinggi yaitu 2,29. Selanjutnya adalah hambatan berdasarkan aspek waktu dengan rerata skor 2,24, kemudian berdasarkan aspek pelayanan kesehatan dengan rerata skor 2,19 dan yang paling rendah memperoleh rerata skor adalah berdasarkan status fungsional pasien yaitu 2,14. Jarak dan ketidaktahuan pasien mengenai rehabilitasi jantung menjadi dua hambatan yang memiliki rerata skor tertinggi yaitu 2,52 dan 2,38. Hambatan berdasarkan aspek logistik yaitu jarak menjadi hambatan yang paling banyak terjadi. Maka dari itu, peneliti menyarankan untuk dapat menerapkan rehabilitasi jantung dengan setting rumah yang disesuaikan dengan karakteristik pasien. Kata Kunci: Hambatan, PJK, Rehabilitasi Jantung AbstractBarrier Of Coronary Artery Disease (Cad)Patient Who Undergoing Cardiac Rehabilitation At Cardiac Clinicrsup Dr. Hasan Sadikin Bandungthe. The level of adherence and participation of patients coronary artery disease (CAD) post revascularization  who undergoing cardiac rehabilitation was still very low which is influenced by various barriers.  This may lead a recurrence of disease  and decrease of the quality of life . This research aims to know about barrier of CAD patient who undergoing cardiac rehabilitation in Cardiac Clinic RSUP Dr. Hasan Sadikin Bandung. This research used a descriptive quantitative method and involved 42 respondent were taken by accidental sampling technique which held in a month.  The research instrument used Cardiac Rehabilitation Barrier Scale from Grace et.al in 2011 that has been translated into Indonesian.  Analysis of  data used  distribution of frequencies and mean score. The results obtained that the barrier  based on logistic aspect with mean 2.29 become highest mean score . Then the barriers based on  aspect of time with mean value score 2.24, then based on the aspect of health care with mean score 2.19 and the lowest mean score is based on the patient's functional status 2.14. Distance and patient's ignorance about cardiac rehabilitation came in two barriersand the  highest mean value was 2.52 and 2.38. The barriers based on the logistic aspect was  distance, the most happened in cardiac rehabilitation. Researchers suggested that for apply the home based cardiac rehabilitation with adjusted of characteristic patient.  Keyword: Barriers, CAD, cardiac rehabilitation

Abstract

ARDS di Indonesia menjadi penyakit yang mengancam jiwa karena menyebabkan respon inflamasi akibat adanya kelainan baik langsung atau tidak  langsung pada paru, Prone position telah dilakukan selama bertahun-tahun untuk meningkatkan oksigenasi pada pasien terpasang ventilator dan support manajemen untuk pasien dengan ARDS. Metode yang digunakan dalam Systematic review dengan desain Penelitian yang menjadi kriteria dengan Randomize Control Trial, dengan tahun penerbitan artikel pada 2010-2020,  penelitian kuantitatif, berbahasa Inggris dan Indonesia. Dari 4 artikel peneliti menemukan adanya 2 kategori Prone position yang berhubungan dengan peningkatan oksigen yaitu, lama waktu penggunaan tehnik Prone position dan Prone position dengan kombinasi. Simpulan didapatkan ada pengaruh dari Prone position terhadap peningkatan oksigenasi, terlebih bila penggunaan Prone position jangka panjang akan menurunkan tingkat mortalitas pasien ARDS.

Abstract

ABSTRACTIntroduction: The rise in COVID-19 cases and fatalities indicates that nurses working in primary health care face certain challenges in administering treatment and preventative measures. Purpose: Identify the obstacles faced by the nurses of primary health care in their efforts to conduct prevention and treatment of the COVID-19 outbreak in Bandung, Indonesia. Methods: Correlational descriptive quantitative design with a survey approach was employed in this research. Total sampling was used to collect 120 nurses that made up the study's sample. Result: The findings showed that nurses experienced high obstacles 58 nurses (48.3%) in carrying out tracing tasks, 61 nurses (50.8%) in implementing monitoring tasks, 34 nurses (28.3%) in carrying out education and counseling tasks, and 60 nurses in the duty of referral selection.  Multivariate analysis indicated nurses' updating knowledge of monitoring task difficulties is the most crucial determinant with a p-value (p = 0.038). Conclusion: In carrying out nursing duties during the pandemic, updating knowledge is very useful for breaking the chain of transmission and handling pandemics in general. Nurses at PHC should improve their knowledge and handling skills related to the pandemic by attending training and seminars related to handling the COVID-19 outbreak. ABSTRAKPendahuluan: Kenaikan angka kasus dan kematian akibat COVID-19 pada tahun 2020-2021 menyebabkan permasalahan pada perawat dalam melaksanakan tugas, untuk menurunkan angka tersebut perawat harus bekerja maksimal, namun perawat sepertinya mengalami hambatan sehingga hambatan tersebut harus diidentifikasi. Tujuan: Mengidentifikasi hambatan yang dialami oleh perawat puskesmas dalam melakukan upaya pencegahan dan penanggulangan wabah COVID-19 di Kota Bandung, Indonesia. Metode: Deskriptif kuantitatif korelasional dengan pendekatan survei dengan sampel penelitian sebanyak 120 perawat diambil dengan teknik total sampling. Hasil: Didapatkan perawat yang mengalami hambatan tinggi adalah sebanyak 58 perawat (48,3%) dalam melaksanakan tugas tracing. 61 perawat (50,8%) dalam melaksanakan tugas monitoring. 34 perawat (28,3%) dalam melaksanakan tugas edukasi dan konseling. Dan 60 perawat (50%) dalam melaksanakan seleksi rujuk. Kemudian hasil penelitian menunjukkan bahwa variabel yang paling berpengaruh yaitu updating pengetahuan perawat terhadap hambatan tugas monitoring (p=0,038). Kesimpulan: Dalam melaksanakan tugas di masa pandemi ini, updating ilmu sangat bermanfaat untuk memutus rantai penularan dan penanganan pandemi secara umum. Hendaknya perawat puskesmas meningkatkan pengetahuan dan kemampuan penanganan terkait pandemi dengan mengikuti pelatihan maupun seminar terkait penanganan wabah COVID-19.

Abstract

The level of teachers' disaster preparedness in schools is one thing that influences the impact of disasters on children with disabilities, especially earthquakes, which can occur at any time, including during school hours. Therefore, this study aims to determine the description of earthquake disaster preparedness for special school teachers. This study used a quantitative descriptive method with a total sample of 37 teachers at Cileunyi State SLB, and the total sampling technique used was this study. Preparedness measurement for SLB teachers uses instruments modified according to research needs. Data analysis with frequency distribution and results were grouped into ready and unprepared categories. The results showed that 86.5% of teachers were categorized as 'ready'. The best preparedness parameter is disaster knowledge, while the lowest is disaster warning. From the study results, it can be concluded that most SLB teachers are categorized as ready to face earthquake disasters. However, it is expected that efforts to improve aspects of disaster warning can be carried out, such as providing disaster warning tools, especially for children with disabilities.

Abstract

IMPROVİNG THE PREPAREDNESS OF CİBİRU WETAN PKK CADRES FOR DİSASTER RESİLİENCE DURİNG THE COVİD-19 PANDEMİC. Cibiru is a high-risk area for natural and man-made disasters, so efforts must be made to reduce the risk of disaster impacts. One effort that can be made is to strengthen the Cibiru Wetan village's resilience by increasing preparedness to deal with emergency cases caused by disasters, such as cases of respiratory arrest, cardiac arrest, trauma, and respiratory emergencies caused by COVID-19. Community service is performed to improve the preparedness of Cibiru Wetan PKK cadres for disaster-resilient village resilience during the COVID-19 Pandemic. The method of implementing community service by simulating basic life support, trauma event handling, prevention and handling of COVID-19 emergencies, and the production of health drinks made from natural ingredients such as ginger, honey, lime, cardamom, and turmeric to boost immunity. As a result, the Cibiru Wetan PKK cadres were able to simulate basic life support, first aid in the event of trauma, and respiratory disorders caused by COVID-19, as well as process natural ingredients into healthy drinks to increase immunity, thereby increasing the Cibiru Wetan village's resilience.

Abstract

The COVID-19 pandemic has had an impact on the teaching and learning process, namely online learning methods which have made several changes, especially in subjects that require practical competence. According to the International Council of Nurses, nursing students must have appropriate knowledge of competence in dealing with disaster problems. This study aims to describe student perceptions of the competency achievements of the Disaster Nursing Course using online learning methods at the Faculty of Nursing, Universitas Padjadjaran. The research design uses quantitative descriptive with a cross sectional approach using the Nurse Perception of Disaster Core Competencies Scale instrument. The population in this study numbered 155 students from class 2018 using the purposive consecutive technique. The data were analyzed using univariate analysis which measured student perceptions with results supporting or not supporting. The results of the analysis showed that more than half of the students had unfavorable perceptions, namely 85 students (54.8%) and supportive perceptions, namely 70 students (45.2%). The results of the sub variable critical thinking (62.6%) did not support, assessment (53.5%) did not support, communication (56.1%) did not support, and technical skills (51.6%) did not support. Students' perceptions of competency achievement in disaster nursing courses using online learning methods are not supportive, both in the total score of perceptions about the achievements of all core competencies and in sub-variables. Suggestions for educational institutions to hold seminars/training, practicum, and simulations related to the competence of disaster nursing courses in online learning methods.

Abstract

Aims: Nursing Services strive to provide a source of support for the patient’s family by implementing the Family Centered Care model in critical care rooms, focusing on providing family-centered nursing care services has not been fully applied in hospitals optimally. Objective: To Analyze the factors related to the implementation of family center care in critical nursing rooms at the Karawang district hospital. Methods: The research is a quantitative research with a correlational analytic descriptive research design with a cross-sectional design and the sample used is total sampling. The independent variables include Knowledge, Attitudes Bahavior and Facilities, while the dependent variable is Implementation of Family Center Care. Data analysis used univariate median tests, standard deviation or frequency, while bivariate tests used chi square and multivariate tests used logistic regression tests. Results: The results showed that the average implementation of Family Center Care was 64.00%, with the lowest implementation being 23% and the highest being 65%. From the bivariate results it was found that half of the respondents had good knowledge (57.4%), and more than half of the respondents had a supportive attitude (57.4%), complete facilities (68.5%), and good behavior (53.7%). The results of the logistic regression analysis showed that the implementation of Family Center Care by nurses in the critical care room was influenced by the most dominant factor, namely attitude, with a p-value (0.000) less than (0.05). Conclusion: In the study is that the implementation of Family Center Care by nurses in the Critical Care Room at Karawang Hospital is influenced by the attitude of the nurse, where from the supportive attitude of the nurse it can encourage the behavior of nurses towards the implementation of Family Center Care in the Critical Care Room at Karawang Hospital. With the implementation of Family Center Care properly, it will improve the quality nursing services.

Abstract

Background: The process of grieving and depression can affect the treatment process and the condition of patients with coronary heart disease. The process experienced by the patient can be uncomplicated grief, complicated grief and depression. It is important to distinguish between uncomplicated grief, complicated grief and depression in response to normal and healthy loss and with response to the stresses of personal disorders that require more intensive intervention. The nurse is responsible for helping the patient go through the grieving process so as to prevent the adverse effects on the patient, so that the grieving and depression process are needed to determine the problems and to plan the nursing treatment.Objective: This study aimed to identify the process of grieving and depression in patients with coronary heart disease in the Intensive Room of Hospital Dr. Hasan Sadikin Bandung.Methods: This research method was quantitative descriptive. The study population of patients with coronary heart disease was an average of 37 people per month. Consecutive Sampling technique was carried out for one month with 34 respondents. Data collection technique used in this sresearch was the Grief / Depression Assessment Inventory questionnaire that had been tested for validity with the face validity and content validity. The collected data were analyzed by analytic descriptive method using frequency distribution.Result: The results showed that from 34 respondents, most of them (94.1%) had complicated grief while few of them (5.9%) had uncomplicated grief. The highest response to grieving and depression was in the domain of attitude, thought, feeling, and loss.Conclusion: Based on the results of the study, most patients with coronary heart experienced complicated grief. Therefore it was important for nurses to facilitate patients to be able to consult with treating doctors, psychologists, religious leaders or psychiatrists.Keywords: Coronary Heart Disease, Depression, Grieving

Abstract

The weaning of mechanical ventilation is a complex process and depends on many factors. The failure to wean mechanical ventilation may lead to prolonged duration of mechanical ventilation, which may increase the risk of ventilator associated pneumonia (VAP), morbidity, mortality, increased hospital costs and lower quality of hospital services. In the year 2016, in ICU RSUD Prof.Dr.Margono Soekarjo Purwokerto, in average there is a 20% failure of mechanical ventilation, if not followed up will adversely affect the patient. Factors investigated in this study included patient, nurses, collaboration and organization. The purpose of this study is to analyze the factors that affect implementation of the weaning of mechanical ventilation in ICU Room Prof.Dr.Margono Soekarjo General Hospital Purwokerto.The method used in this research was descriptive quantitative analytic design with cross sectional approach on 47 mechanical ventilation weaning activities. The sample selection was conducted by accidental sampling technique. Measurements for nurses were carried out using validated values including knowledge, experience and collaboration. While the observation sheet included the implementation of standard operating procedures for mechanical ventilation weaning, collaboration, patients APACHE II score and weaning results.The result of bivariate test showed that there were influence of patient (0.000), nurse (0,021), collaboration (0,024) and organization (0,011) to mechanical ventilation weaning activity. The result of logistic regression test showed that the patient was the most influential factor on mechanical ventilation weaning activity with 87% probability.Nurses should pay more attention to patients especially with high APACHE II scores (≥20) because of the risk of mechanical ventilation weaning failure. Assessment of weaning readiness must be carried out daily with appropriate assessment. While hospitals need to revise policies on mechanical ventilation weaning procedure and continue education and training programme related to patients criticality and collaboration.
