Abstract

Background: The clinical supervisor's roles influence the quality of clinical education as a teacher and their interaction with students. Underlined the importance of refreshment on clinical teaching skills for clinical supervisors, the 3rd Webinar series of IAMHPE has inspired the recommendations on essential elements in the clinical education of medical and health profession education.Gaps: Recent studies have highlighted the complexity of facilitating clinical education and the socio-cultural influence on clinical supervisor-student relations. As a gap, Indonesia has a hierarchical and collectivist culture and thus the potential to hinder improving students' learning development during clinical education.Recommendation: An understanding of the Gaps between literature and Indonesia's socio-cultural influence on clinical education, we develop several recommendations, including 1) Building Clinical Supervisor-Student Relations that support students' development; 2) Making role modeling explicit and integrating into teaching and learning;3) Facilitating the gradual development of students' clinical abilities, and 4) Re-conceptualization of feedback in clinical education as a “dialogue”.

Abstract

Menyusui memberi manfaat luar biasa bagi bayi karena ASI mengandung nutrisi yang penting dan terbaik untuk menunjang kesehatan, pertumbuhan, dan perkembangan bayi secara optimal .Meningkatnya ibu pekerja di Indonesia dapat mempengaruhi angka cakupan pemberian ASI ekslusif. Dukungan menyusui dapat meningkatkan konsentrasi ditempat bekerja karena berkurangnya kekhawatiran ibu terhadap nutrisi bayi sehingga produktivitas pekerja tinggi. Tujuan penelitian ini adalah untuk menggali secara mendalam dukungan yang diperoleh dalam memberikan  ASI pada ibu menyusui pekerja industri di pabrik tekstil dan garmen. Jenis penelitian iniadalah deskriptif kualitatif. Teknik pengambilan sampel menggunakan purposive sampling. Jumlah sampel pada penelitian ini adalah 26 informan. Penelitian dilakukan dengan wawancara. Hasil penelitian pada informan didapatkan bahwa dukungan yang diterima ibu adalah dukungan keluarga yang terdiri dari dukungan dengan memberikan bantuan fisik dan dukungan emosional. Ketika bekerja ibu menerima dukungan dari rekan kerja dan perusahaan.

Abstract

Abstract: Psychoeducation can reduce the risk of mental health problems in postpartum mothers. This systematic review aims to identify the effectiveness of providing psychoeducation in postpartum psychological disorders. Design using PRISMA checklist and CASP. Data were extracted from the English journal, RCT, which reports psychoeducation as exposure. Results, of the 13 studies that systematically reviewed mental health and psychoeducation issues, 11 passed the Critical Appraisal test. However, only 4 had a magnitude of effect indicating that psychoeducation was a protective factor against psychological disorders of postpartum mothers and 7 others showed a magnitude of chance (p <0.05).Abstrak: Psikoedukasi dapat menurunkan resiko terjadinya gangguan kesehatan mental pada ibu postpartum. Systematic review ini bertujuan untuk mengidentifikasi efektivitas pemberian psikoedukasi pada gangguan psikologis postpartum. Desain menggunakan PRISMA checklist dan CASP. Data diekstraksi dari jurnal yang bahasa Inggris, RCT, yang melaporkan psikoedukasi sebagai exposure. Hasil, dari 13 penelitian yang secara sistematis meninjau masalah kesehatan mental dan psikoedukasi, 11 lolos uji Critical Appraisal. Namun hanya 4 yang memiliki magnitude of effect yang menunjukan bahwa psikoedukasi merupakan faktor protektif terhadap gangguan psikologis ibu postpartum dan 7 lainnya menunjukan magnitude of chance (p < 0.05).

Abstract

Seksualitas merupakan sebuah domain penting dan kompleks dalam kualitas kehidupan. Aktivitas seksual melibatkan elemen fisik, psikologis, sosial, dan estetik. Hal ini sangat kompleks sehingga rentan mengalami masalah. Jika seksualitas perempuan terganggu, konsekuensi yang terjadi kemungkinan akan berpengaruh terhadap keharmonisan hubungan suami istri dan kelangsungan fungsi reproduksi seorang perempuan. Sejumlah penelitian menemukan efek samping yang ditimbulkan dari lamanya penggunaan suntikan DMPA. Pemakaian kontrasepsi suntik DMPA dalam jangka waktu yang lama dikaitkan dengan kemungkinan terjadinya disfungsi seksual. Mengetahui dan menganalisis risiko lama penggunaan kontrasepsi suntik DMPA dengan disfungsi seksual pada perempuan usia reproduktif di Kota Kendari. Penelitian observasional analitik menggunakan rancangan cohort retrospective. Subyek dalam penelitian ini adalah semua perempuan usia reproduktif yang menggunakan kontrasepsi DMPA di Kota Kendari Jumlah sampel dalam penelitian ini sebayak 196 perempuan usia subur dengan teknik pengambilan sampel secara accidental sampling. Variabel bebas yaitu lama penggunaan Depo Medroxy Progesterone Acetate dan variabel terikat yaitu disfungsi seksual perempuan. Subjek penelitian ini adalah perempuan usia subur 15-49 tahun pengguna kontrasepsi suntik Depo Medroxy Progesterone Acetate. Analisis data dilakukan dengan analisis deskriptif untuk mengetahui distribusi frekuensi dan proporsi, bivariabel dengan uji Chi-square dan multivariable menggunakan uji regresi logistik. Hasil penelitian ini ditemukan dari seluruh responden penelitian sebanyak 39 % perempuan mengalami difungsi seksual. Dari penelitian ini juga diketahui bahwa prevalensi klien DMPA ? 2 tahun mengalami disfungsi seksual sebesar 58 %. Risiko disfungsi seksual pada klien DMPA ? 2 tahun menggunakan kontrasepsi DMPA ? 2 tahun 2 kali lebih besar dibandingkan klien pengguna DMPA < 2 tahun (RR = 2,1 95 % CI 1,62-2,84). Usia ibu, usia suami dan usia pernikahan merupakan faktor risiko yang cukup penting dalam menyebabkan disfungsi seksual. Pada hasil multivariabel usia perempuan merupakan faktor yang berkontribusi paling besar memengaruhi kejadian disfungsi seksual, pada usia reproduksi yaitu pada kelompok usia 35-49 tahun berisiko tinggi (p=0,001 < 0,05) mengalami disfungsi seksual

Abstract

Abortus menjadi masalah yang penting dalam kesehatan masyarakat karena berpengaruh terhadap morbiditas dan mortalitas maternal. Di Indonesia, belum ada data yang komprehensif tentang kejadian abortus, berbagai data yang ada sebelumnya berdasarkan survei dengan cakupan yang relatif terbatas. Abortus yang tidak aman bertanggung jawab terhadap 11% kematian ibu di Indonesia. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan kejadian abortus di Indonesia. Penelitian ini dilaksanakan dengan metode observasional menggunakan disain studi cross sectional. Data penelitian diperoleh dari data Survei Demografi dan Kesehatan Indonesia 2002-2003. Analisis data dilakukan secara bertahap yaitu: analisis univariat, analisis bivariat dan analisis multivariat dengan regresi logistik. Hasil menunjukkan bahwa risiko abortus meningkat seiring dengan peningkatan usia ibu. Wanita dengan paritas 0-2 berisiko lebih tinggi untuk mengalami abortus dibandingkan wanita dengan paritas 3 atau lebih (OR=5,2, IK 95%=3,49-7,89). Wanita yang bekerja berisiko 2,7 kali lebih tinggi untuk mengalami aborsi daripada wanita yang tidak bekerja (OR= 2,7 , IK 95%= 2,10-3,58). Selain itu, risiko abortus meningkat pada wanita yang menikah pada usia 30 tahun atau lebih (OR=1,8, IK95%= 1,30-2,48). Risiko abortus tidak berhubungan bermakna dengan riwayat abortus sebelumnya, tingkat pendidikan dan tingkat sosial ekonomi. Abortion has become a main problem in public health because of its impact on maternal morbidity and mortality. There is no comprehensive data on abortion in Indonesia. So far, data were based on survey with limited coverage. Unsafe abortion is responsible to 11 percent of maternal mortality in Indonesia. This research objective is to determine factors related to abortion in Indonesia. The study was an observational method with cross sectional design. These data are gathered from Indonesian Demography and Health Survey 2002–2003. Data analysis used univariate, bivariate and multivariate analysis with logistic regression. The risk of an abortion increased in line with the increasing of maternal age. Women with 0-2 parity have higher risk than multiparity (OR=5.2, IK95%=3.49-7.89). Employed women have higher risk than unemployed (OR=2.7, IK95%=2.10-3.58). The risk of abortion increased among women married at 30 years old or over (OR=1.8, IK95%=1.30-2.48). There is no significant association between history of abortion, education, socioeconomic and risk of abortion.

Abstract

Objective: Knowing the perspective of the community watching reproductive health promotion activities through traditional art performances. Adolescent reproductive health promotion activities are carried out through the use of local wisdom in Bali, namely through the performance of traditional arts. The information conveyed through this promotional media contains informative messages about adolescent reproductive health so that it is no longer taboo to talk about and is easily accepted by the community. Methods: Qualitative research using an exploratory approach. The research location is in Tegallalang Village, Gianyar Regency, Bali. The informants were the people who watched the performance activities as many as 9 people. Retrieval of data through focus group discussions. Thematic data analysis. Result: The community's perspective on the performance and the message conveyed in the performance. The benefits of staging traditional arts according to the community watching are that socialization through art is easier to accept, traditional art performances can preserve culture and art, can be useful for society, namely reminding adolescents not to have premarital sexual behavior. The message received by the community is that there are limitations in dating, how to choose friends, carry out parental advice, anticipation of teenagers not having premarital sex and the impact of premarital sex. Acceptance of message content by the community, namely that the language is easy to understand, easily accepted by the community and becomes an example so that it is not imitated. Conclusion: Good interpretation of reproductive health promotion activities through the performance of traditional arts is related to the benefits and messages received by the community. The implication of the research results in the form of traditional art media as a health promotion media has the potential to educate the public in a positive way.

Abstract

Several public health center temporary closures due to antigen positive health workers in Daerah Istimewa Yogyakarta have been reported. Behind the scene, there was confusion among health workers when they were not sure about sources of transmission and concerns about wider transmission among health workers and their families. This paper reports the Yogyakarta City Health Office initiative to build health workers’ awareness and to protect health centers protected from transmitting all contagious conditions in the future, learning from the experience of the covid 19 pandemic. Kota Yogyakarta Health authority and Universitas Gadjah Mada held two focus group discussions to document (1) the experience of responding to cases of reactive antigens in health workers who need to receive future follow-up from the experiences of 18 puskesmas heads and elements of the health office, and (2) the puskesmas Covid 19 screening and testing capacity for the community and health personnel use. This report raises the importance of safe workplace operational rules that prevent disease transmission and urgent actions to enforce these rules in puskesmas and health offices.

Abstract

Background: Three delays models in referring the mother to health facility are the most important determinants in maternal mortality. Based on the culture in West Sumba, most of the people still follow patrilinear patrimony. This culture limiting women to make decisions such as the decision to determine where to get health services, plan the number of children and bird spacing. The first model should be immediately prevented from causing the next delay which is late in making a family decision and too late to recognize danger sign in pregnancy that affects maternal mortality. Purpose: The purpose of this study was to explore the role of decision-makers in delayed maternal referrals. Methods: Qualitative research using the phenomenological design on 5 mothers who experienced a delay in referral at Padediwatu Primary Health Care. Data collected with in-depth interviews. Result: Decision maker in maternal referring was dominated by the husband. Obstacles in the referral process such as not having a vehicle, long distances and difficult road access, low economic status, and the culture that depend on others so maternal health status with complications is not a priority for the husband. Husband's work as a farmer and sailor that keeps the husband away from home or near to his wife for long periods was affected by the decision-making process of maternal referral. Conclusion: Decision-making in maternal referrals still emphasizes patrilinierculture, husbands have an important role in making maternal referral decision.

Abstract

Background : The Robson 10-group classification system categorizes pregnant women into ten distinct groups based on specific obstetric characteristics. As caesarean section rates rise globally, this trend has become an important public health concern. Between 2012 and 2015, RSUD Panembahan Senopati Bantul, a regional government hospital in Indonesia, reported caesarean section rates exceeding 30%. The aim of this study is to identify which Robson groups contributed most to the significant increase in cesarean deliveries.Objective : This study aims to determine the rate of change in the trend of caesarean section rates that occurred at Bantul Regional General Hospital during the 2021–2022 period using the Robson Classification.Methods : This study used a descriptive analytic design. The study population included all pregnant women who gave birth at RSUD Panembahan Senopati in Bantul from 2021 to 2022. Data were obtained from secondary sources using Case Report Form (CRF) instruments and processed using Robson classification. Additional analyses were conducted to determine the cesarean section rate within each group and identify reasons for cesarean delivery.Results  : Of the 2,295 study participants, 906 (39.8%, 95% CI 38.27–41.33) were pregnant women who delivered by C-section during 2021–2022. The C-section rate declined by 1.53% overall over the two-year period (95% CI 1.08–2.12). However, several Robson groups showed rising C-section trends. According to the Robson classification, group 5 represented the largest proportion of the population (25.1%, 95% confidence interval [CI] 23.09–27.12) and accounted for the highest proportion of cesarean deliveries (9.9%), followed by group 4 (6.2%, 95% CI 3.22–8.27). Groups 2, 4, and 7 experienced rising C-section trends from 2021 to 2022, with Group 4 showing the greatest increase (8.5%), followed by Group 2 (4.4%, 95% CI 1.2–6.75). Further evaluation identified failed labor induction as the leading cause of the increased C-section rate (14.6%, 95% CI 12.02–17.9), while fetal compromise/distress also played a significant role (10.6%, 95% CI 8.79–12.5).Conclusion : From 2021 to 2022, the caesarean section rate at RSUD Panembahan Senopati Bantul decreased by 1.53% (95% CI 1.08–2.12), though certain groups still had elevated rates. Groups 2, 4, and 5 accounted for the majority of cesarean deliveries during this period. Targeted interventions, including routine evaluations and improved management, are needed to reduce the overall C-section rate further.
