Abstract

Background: Leprosy is one of the neglected tropical disease caused by Mycobacterium leprae. Endemicareas are still found due to incapability of MDT regimen alone to eradicate leprosy. The good understandingof high-risk population characteristic to plan elimination strategy of leprosy is needed, especially invulnerable populations such as mothers and children population. This study aims to find association betweensocioeconomic factors and the incidence of leprosy in maternal and children leprosy in endemic areas.Methods: This is a case-control study in endemic areas in Tuban Regency, East Java Province, Indonesia.The obtained data was done using structured questionnaire and direct measurement. Chi-square, t-test/Mann-Whitney test was used to assess the association between socioeconomic factors and the incidence ofleprosy in maternal and children leprosy in endemic areas.Results: 22 pairs of cases and 57 pairs of controls were analyzed. This study found that education isassociated with the incidence of leprosy. Significant results were observed in father’s education (p value=0.023) with the incidence of maternal leprosy and mother’s education (p value= 0.003) education) andfather’s education (p value= 0.013) with the incidence of children leprosy. Family income also associatedwith the incidence of maternal (p value= 0.040) and children leprosy (p value= 0.036). Occupation is notassociated with the incidence of leprosy.Conclusion : The result of this study shows that education and family income are related with the incidenceof leprosy in both, mothers and children population.

Abstract

Background: Leprosy is chronic infectious disease caused by Mycobacterium leprae. Endemic areas stillexisted with stable number of new cases including new cases of pediatric leprosy and female leprosy cases.Close contact and women’s role in household increase the risk of transmission of leprosy to their children.Population lived in endemic areas are prone to contract leprosy due to dysregulation of immune system. Thisstudy aims to analyze correlation of hematological parameters and immunity ration in maternal and pediatricleprosy in endemic areas.Methods: This is a cross-sectional study in endemic areas in Tuban Regency, East Java Province,Indonesia. The blood sample was taken from the subjects and underwent complete hematological parametertest (hemoglobin, red blood cells, white blood cells, platelets, hematocrit) and measurement of T cellsactivityratio(Th1/Th2 ratio and Th17/Treg ratio) by ELISA. Correlation test was done between blood testresultsand the immunity ratio of maternal and pediatric leprosy in endemic areas.Results: 33 pairs of maternal and pediatric leprosy cases were analyzed. This study found correlationbetween RBC (p value= 0.029) and HCT (p value= 0.038) with Th17/Treg ratio in children population. Nosignificant results observed in mothers population.Conclusion: The result of this study shows that hematological paramaters (HB and HCT) related to theTh17/Treg ratio and could play roles in the incidence of dysregulation of immune system in children leprosy.

Abstract

Background: Skin Prick Test (SPT) is good diagnostic tool of allergic disease such as Atopic Dermatitis (AD). Imported allergen extracts are expensive and not always available in Indonesia. New local SPT allergen extracts are potential as substitute. Objective: To compare SPT results of new local and imported allergen extracts (house dust mite, chicken, egg white, cow’s milk, and shrimp), and specific IgE in AD patients. Methods: This was clinical trial of 45 non lesional AD patients. The SPT of imported allergen extracts was performed in the first week, followed by the new local and specific IgE examination in the next one week. Result: Sensitivity (Sn) of new local compared with imported allergen extracts in this study was between 15.38%-84.61%, the highest point was house dust mite extract. Specificity (Sp) was ranged from 81.48%-93.75%, the highest rank was shrimp extract. Compared with IgE results, the Sn of new local and imported house dust mite extracts was good (85.7%; 92.9%). The Sp was good for egg white and cow’s milk new local extracts (86.4%; 84.4%). Conclusion: New local allergen extracts should be considered as a substitute of the imported diagnostic tool.

Abstract

In the National Standard for Accreditation of Hospitals (SNARS) 2018, patient safety is one of the standards of assessment. The purpose of this study was to assess the fair and open culture in the patient safety culture survey, with the aim of assessing the culture of reporting and learning in Dr. Soetomo Hospita, Surabaya. This research was an observational study with a total sample of 406 employees spread across 34 installations and 11 fields / sections, with the same proportion. Data was collected using Hospital Survey on Patient Safety Cuture issued by AHRQ (Agency for Healthcare Research and Quality), which consisted of 12 dimensions. The results showed that the majority of employees had assessed the 12 AHRQ dimensions well, but there were several dimensions that still received poor ratings such as openness of communication, bad = 25.4%, frequency of incident reporting, rarely = 27.8% and staffing, less 39.6%; it is necessary to develop a culture of not blaming if someone reports an incident, and changes in staffing to suit the workload. Tujuan penelitian ini adalah untuk menilai budaya adil dan terbuka dalam survei budaya keselamatan pasien, dengan tujuan menilai budaya pelaporan dan pembelajaran di RSUD Dr. Soetomo Surabaya. Penelitian ini merupakan studi observasional dengan total sampel 406 karyawan yang tersebar di 34 instalasi dan 11 bidang/bagian, dengan proporsi yang sama. Data dikumpulkan menggunakan Hospital Survey on Patient Safety Cuture yang dikeluarkan oleh AHRQ (Agency for Healthcare Research and Quality), yang terdiri atas 12 dimensi. Hasil penelitian menunjukkan bahwa sebagian besar karyawan telah menilai baik pada 12 dimensi AHRQ, namun terdapat beberapa dimensi yang masih mendapat penilaian buruk seperti keterbukaan komunikasi, buruk=25,4%, frekuensi pelaporan insiden, jarang=27,8% dan staffing, kurang=39,6%; maka perlu dikembangkan budaya tidak menyalahkan jika ada yang melaporkan insiden, dan perubahan susunan kepegawaian agar sesuai dengan beban kerja. Abstract

Probiotic’s protection in ileal mucosa of mice after lipopolysaccaride Escherichia coliBackground: Gastrointestinal infection is one of the major causes of morbidity in children. Diarrhea is the common manifestation. Diarrheal prevention needs a balance mucosal immune system. Probiotic used in prevention of gastrointestinal infection needs to be considered. The aim of the study is to prove probiotic protection in ileal mucosa after lipopolysaccaride (LPS) Escherichia coli O55:B5 inoculation anatomically and immunologically.Method: Experimental study with male Balb/c, age 10-12 weeks, body weight 30-40g and randomized into treatment group and placebo group. Treatment group received mix probiotic for 14 days and on day 15 were inoculated by LPS Escherichia coli O55:B5, and day 16-21 were given mix probiotic again. Placebo group received LPS Escherichia coli O55:B5 on day 15. Mice necropsy were conducted at day 22. Immunohistochemistry examination used to look for amount expression cell for IL-2, IFN-γ, TNF-α, IL-4, IL-5,IL-6, IL-10 dan TGF-β in ileal mucosa. Scanning electron microscope was used to see ileal mucosal structure. Statistic analysis used in this study was multivariate analysis.Results: Sixteen Balb/c mices were randomized into 2 groups, each group consist 8 mices. There was significant difference on amount of cells expression IL-5 (p=0.022), IL-6 (p=0.05), and also on amount of cells expression TGF-β (p=0.036). On probiotic- LPS group, there was no Th1 domination but on the other hand, Treg became dominant. Th1 and Th2 response were still balance. Structural damages occurred in LPS group and did not occur in probiotic-LPS group.Conclusion: Probiotic protection in ileal Balb/c mice mucosa after LPS Escherichia coli O55:B5 inoculation is through anatomy and immunology changes.Keywords: Probiotic, LPS, Escherichia coli, protection, immunologyABSTRAKLatar belakang: Infeksi gastrointestinal dengan manifestasi tersering diare merupakan salah satu infeksi yang sering terjadi pada anak. Pencegahan diare memerlukan keseimbangan sistem imun mukosa yang baik. Penggunaan probiotik untuk pencegahan terhadap infeksi gastrointestinal perlu dipikirkan. Penelitian ini bertujuan untuk membuktikan proteksi probiotik pada ileum mencit yang terpajan lipopolisakarida (LPS) Escherichia coli O55:B5 secara anatomis dan imunologis.Metode: Uji eksperimental dengan menggunakan mencit Balb/c jantan berusia 10-12 minggu, berat badan 30-40g dan dirandomisasi menjadi kelompok perlakuan dan kelompok kontrol. Kelompok perlakuan mendapatkan mix probiotik selama 14 hari, hari ke-15 mendapatkan LPS Escherichia coli O55:B5 dan hari ke-16-21 kembali mendapatkan mix probiotik. Kelompok kontrol mendapatkan LPS Escherichia coli O55:B5 saja pada hari ke-15. Seluruh mencit dinekropsi pada hari ke-22. Pemeriksaan imunohistokimia untuk melihat ekspresi sel penghasil sitokin IL-2, IFN-, TNF-α, IL-4, IL-5, IL-6, IL-10 dan TGF-ß di jaringan ileum. Scanning microscope electron (SEM) untuk melihat struktur mukosa ileum. Analisis statistik yang digunakan adalah analisis multivariat.Hasil: Enam belas ekor mencit Balb/c terbagi menjadi dua kelompok, masing-masing 8 mencit. Pada kelompok probiotik-LPS didapatkan perbedaan bermakna dibandingkan dengan kelompok LPS pada jumlah penghasil sitokin IL-5 (p=0,022), IL-6 (p=0,050) dan jumlah sel penghasil sitokin TGF-β (p=0,036). Pada kelompok probiotik-LPS, dominasi respon Th1 tidak terlihat lagi dan menimbulkan dominasi Treg pada kelompok ini. Posisi Th1 dan Th2 masih dapat dipertahankan secara seimbang. Kerusakan struktur yang terjadi pada kelompok LPS tidak didapatkan pada kelompok Probiotik-LPS.Simpulan: Proteksi probiotik pada mukosa ileum yang terpajan LPS Escherichia coli terjadi secara anatomi dan imunologi.

Abstract

BACKGROUND: Risk of acute respiratory infections in children less than 5 years of age is up to 95%. Zinc deficiency is one of the main risk factors. This study aimed to explore the effect of zinc on the bronchial mucosae inflammatory status expressed by nuclear factor (NF)-κB p105/p50, NF-κB p65, interleukin (IL)-8, and IL-1β.METHODS: Twenty-four Wistar rats were divided into 4 groups: normal zinc diet group without zinc supplementation (Z1), normal zinc diet group with zinc supplementation (Z2), zinc deficient diet group without zinc supplementation (Z3), and zinc deficient diet group with zinc supplementation (Z4). NF-κB p105/p50, p-NF-κB p65, IL-8, and IL-1β were measured by immunohistochemical staining.RESULTS: The inflammatory status of bronchial mucosae between Z1 and Z2 groups showed no difference (p=0.055). However, the inflammatory status of bronchial mucosae between Z3 and Z4 groups showed significant difference (p<0.01). Multivariate factorial design showed that zinc supplementation was beneficial when given to zinc deficient diet group with regard to decrease p-NF-κB p65, IL-8 and IL-1β levels (p<0.001) and increase dendritic cell (p=0.022).CONCLUSION: Zinc administration under conditions of zinc deficiency affects the inflammatory status, as shown by the decrease of p-NF-κB p65, IL-8 and IL-1β and the increase of NF-κB p105/p50.KEYWORDS: zinc, NF-κB, p105/p50, p65, IL-8, IL-1β, rat

Abstract

BACKGROUND: Airway epithelium is the first line of defense against a variety of exposures. Inflammatory processes, hyperresponsiveness and zinc deficiency cause epithelial damage. Zinc is involved in apoptosis and microtubule formation. However, its role in the integrity of bronchial mucosa and cilia is unclear.METHODS: To assess the effect of zinc on the integrity of the bronchial epithelium, 24 male Rattus norvegicus strain Wistar rats were randomized into four experimental groups: normal zinc diet group without zinc supplementation, normal zinc diet group with 60 ppm zinc supplementation, zinc deficient diet group without zinc supplementation, and zinc deficient diet group with 120 ppm zinc supplementation. Bronchial mucosal integrity was measured with the number of epithelial cells, and the number and length of cilia.RESULTS: Number of cell in normal zinc diet group was 8.8±1.82, while it was only 8.1±1.08 in zinc deficient diet group (p<0.001). Number of cilia per cell was 4.6±1.08 in normal zinc diet group, compared to 4.0±0.79 in zinc deficient diet group (p<0.001). Ciliary length also differ by 7.68±0.66 μm in normal zinc diet group and only 5.16±0.91 μm in zinc deficient diet group (p<0.001).CONCLUSION: Zinc supplementation of the normal zinc diet group affected the length of bronchial cilia. Zinc supplementation of the zinc deficient diet group affected the integrity of the bronchial epithelium, which was shown by the number and length of cilia, and the number of epithelial cells.KEYWORDS: zinc, bronchial epithelial integrity, cilia length, number of cilia, epithelial cell

Abstract

Latar belakang. Fomula berbasis isolat protein kedelai banyak digunakan untuk anak-anak dengan alergi susu sapi di Indonesia. Namun, diperlukan penelitian untuk mendapatkan gambaran penerimaan orangtua dan toleransi saluran cerna pada penggunaan formula isolat protein kedelai.Tujuan. Pertama, menentukan penerimaan orangtua terhadap pemberian suatu isolat protein kedelai pada bayi yang diduga mengalami alergi terhadap protein susu sapi. Kedua, mengetahui toleransi saluran cerna pada pemberian susu formula tersebut.Metode. Suatu studi pasca-pemasaran, prospektif, multisenter yang dilakukan di Jakarta, Bandung, Surabaya, Yogyakarta, Solo, dan Denpasar sejak September 2011 sampai April 2012. Subyek berusia antara 6 bulan hingga 1 tahun dengan gejala dugaan alergi terhadap protein susu sapi yang diberikan formula isolat protein kedelai dan diamati selama 4 minggu. Luaran yang diharapkan adalah penerimaan orangtua terhadap pemberian formula isolat protein kedelai dan toleransi saluran cerna terhadap pemberian isolat protein kedelai.Hasil. Diteliti 534 subyek yang dapat dianalisis selama periode penelitian. Mayoritas orangtua (84%) merasa puas dengan formula isolat protein kedelai, 83% orangtua berencana untuk melanjutkan pemberian susu formula karena berkurang (31,5%) dan hilangnya gejala yang diduga akibat alergi susu sapi (32,4%). Gejala klinis yang diduga akibat alergi terhadap protein susu sapi menurun pada setiap kunjungan berikutnya. Tidak ada efek samping serius yang dilaporkan selama periode penelitian.Kesimpulan. Penelitian ini menemukan tingkat penerimaan orangtua dan toleransi saluran cerna yang baik terhadap pemberian formula isolat protein kedelai kepada bayi dengan gejala sugestif alergi terhadap protein susu sapi. Formula isolat protein kedelai cukup aman dijadikan sebagai formula alternatif pengganti pada anak dengan alergi susu sapi.

Abstract

Perkembangan kognitif anak merupakan cerminan dari kompleksitas kerja sirkuit otak yang terbentuk sejak usia dini dan harus melewati periode kriitis pada tahun-tahun pertama awal kehidupan anak. Semakin kompleks sirkuit otak yang terbentuk, maka semakin besar kapasitas anak untuk belajar mengembangkan kemampuan kognitif. Saat ini berbagai studi mengkaitkan antara perkembangan otak dengan perkembangan sistem imun anak pada usia dini. Sistem imun selama ini lebih dikenal peranannya dalam sistem pertahanan tubuh untuk melawan organisme patogen, ternyata terbukti juga mempunyai peran khusus dalam proses perkembangan otak, baik dalam kondisi yang normal atau sehat, maupun pada kondisi patologis. Proses perkembangan otak dan sistem imun melibatkan berbagai sel dan molekul-molekul mediator yang sama, seperti protein dan sitokin. Keterkaitan perkembangan otak dan sistem imun terjadi secara dua arah dengan konsep mekanisme yang terprogram sejak usia dini, dimana aktivasi di usia dini akan berdampak jangka panjang terhadap kedua sistem tersebut. Berbagai studi saat ini juga membuktikan bahwa mikrobiota saluran cerna juga berperan khusus dalam sistem imun dan perkembangan otak. Memahami aktivasi sistem imun selama periode kritis perkembangan otak anak merupakan langkah yang strategis untuk dapat mengungkap patogenesis berbagai gangguan perkembangan, perilaku, dan kognitif pada anak dan juga terhadap berbagai cara dan metode terapi di masa depan.

Abstract

Background: Subjective diagnostic test of food allergy is hugely biased, resulting in irrational diet avoidance. Additional objective tools by skin prick test following food provocation test resulting more accurate cause and prevalence on population. Purpose: To evaluate the compatibility of clinical symptoms with skin prick test and provocation test for imunoglobulin E (IgE) mediated food allergy in Dr. Soetomo Hospital Surabaya. Methods: Cross sectional observational analytic study. Patient with allergy symptoms, diagnosed with food skin prick test followed by food provocation test. McNemar test and Kappa were used to analyze compatibility of skin prick test with food provocation test.  Sensitivity and spesificity test of skin prick test toward food provocation test were done. Results: McNemar test between skin prick test  and food provocation test are valued p<0.05 in all food allergen, except fruit p=0.607, but all Kappa test are low p<0.6. Fruit and chocolate showed low sensitivity  (0.307;0.409) but with high spesificity (0.823;0.8333); egg and shrimp showed high sensitivity (0.777;1.000) but low spesificity (0.548;0.352); cow’s milk, seafish, and chicken meat showed sensitivity (0.636;1.000;0.785) and high specifity (0.607;0.625;0.631); fresh water fish showed low sensitivity and spesificity (0.500;0.583). IgE mediated symptoms profile is dominated by cough except for shrimp, and non-IgE mediated also is dominated by cough  except seafish and shrimp. Conclusion: There is incompatibilty between result of skin prick test and food provocation test in IgE mediated food allergy. IgE mediated and non-mediated food allergy symptoms are dominated by airway, gastrointestinal tract, and skin symptoms. Food allergy symptoms are more related to IgE mediated than non-IgE mediated.
