Abstract

Introduction: Asthma is a disease marked by bronchial hyperresponsivity. It is commonly seen in children and often affects their quality of life. Many factors affect asthma, including breastfeeding. Bioactive and immunological components of maternal milk have a protective effect on allergic asthma. However, some studies denied this, stating that it had no significant association to asthma. This study aimed to analyze the correlation between breastfeeding duration and the severity of allergic asthma described by the frequency of asthma attacks, sleep disorder, and activity limitation.Methods: This analytic observational study used a cross sectional approach. The subjects were pediatric outpatients aged 1 – 5 years old with allergic asthma at Pediatric Clinic of Dr. Soetomo General Hospital Surabaya and Private Pediatric Clinic from September 2019 to July 2020. Subjects were assessed based on the history of exclusive breastfeeding duration, frequency of asthma attacks, sleep disorder, and activity limitation through an interview using questionnaire.Results: Through the data obtained from 62 respondents, duration of exclusive breastfeeding showed no significant relation to the frequency of asthma attack (rs = -0.227, p = 0.076), sleep disorder (rs = -0.214, p = 0.095), and activity limitation (rs = -0.055, p = 0.672).Conclusion: There was no correlation between the duration of exclusive breastfeeding and the frequency of asthma attack, sleep disorder, and activity limitation.

Abstract

Introduction: The interaction between genetic and environmental factors plays an important role in shaping the early development of allergies, including exposure to cigarette smoke. This study aimed to determine the relationship between second-hand smoke exposure and early allergic manifestations in children. Methods: This was an observational analytic study with a cross-sectional design. Pediatric patients who suffered from allergies in the allergy outpatient clinic at the Department of Pediatrics, Dr. Soetomo General Hospital, Surabaya were included in this study. Data were obtained through medical records. Results: This study involved 190 patients and showed that 50% of the patients have families who smoke at home, the most common early manifestations were at 11-20 months old (32.10%), and the most common early manifestations were respiratory manifestations (98.42%). There was a significant relationship between the presence of a family member who smoked at home with the age of early manifestation, respiratory manifestation, and skin manifestation in allergic pediatric patients (all p < 0.05). There was no significant relationship between the presence of family members who smoked at home with gastrointestinal manifestations in allergic pediatric patients (p = 0.301). Conclusion: There was a significant relationship between the presence of family members who smoke at home with age, respiratory tract, and skin manifestations in allergic pediatric patients.

Abstract

Introduction: The mechanism of nerve damage in ENL patients is important to be revealed because the problem of nerve damage is not getting enough attention in handling ENL patients. Some pro-inflammatory mediators that have been studied are risk factors for nerve damage in ENL.Aim: This study aim in knowing the mechanism of nerve damage in the relation with its risk factors, which are CD64 neutrophil, IL-17, MMP-9, and iNOS with PGE-2 and IL-10 protective factor and IL-10 in ENL patients.Method: This is an analytic cross-sectional design study with tissue samples from 30 leprosy patients with ENL in Dr. Soetomo General Hospital Surabaya. Samples from biopsy examined using IHC method to evaluate S-100, IL-10, IL-17, PGE-2, iNOS, MMP-9, and CD64 neutrophil expressions. The result calculated within 20 view field with 1000 times magnification.Result: CD64 neutrophil (p=0.022, r=-0.417), IL-17 (p=0.001, r=-0.560), iNOS (p=0.005, r=-0.502) and MMP-9 (p=0.025, r=-0.409) has significant correlation with S-100. There is no significant correlation between IL-10 (p=0.061) and PGE-2 (p=0.407) with S-100.Conclusion: Expressions of IL-17, CD64 neutrophil, MMP-9, and iNOS in tissue correlate with nerve damage in ENL patients.

Abstract

Background Expression of breast milk is a strategy to ensures continuous breastfeeding during temporary separation of infant from the mother. Handling and storage of expressed breast milk (EBM) should be based on established recommendations because these practices can affect milk quality and safety. Objective To describe perceptions and practices of EBM handling and storage, compare with commonly used recommendations and associated factors influencing these practices among mothers in Jakarta, Indonesia, Methods Mothers who practiced expressed breastfeeding (EBF) to their 6-12 weeks-old infants completed questionnaires about their peceptions and practices on how they handle and storage EBM. The differences in perceptions and practices about expressed breastfeeding management were analyzed using the Chi-square or Fisher's exact test. Results Among 30 mothers, 14/30 gave EBF exclusively, 19/30 used breast pump, and 26/30 expressed ³6 times/day. Infants in this study received expressed milk 5-10 times per day. Maternal age and resource of information were factors significantly associated with mother’s perceptions, while maternal age, education, and parity were significantly associated with the practices of EBM handling and storage. Conclusion This study reveals inconsistent perceptions and practices on expressed milk handling and storage by the mothers. They have good knowledge about the theoretical aspects of expressed breastfeeding, but some other techniques are poor, which can be detrimental to the health of the infants. Further study is needed to find more efficient and optimal methods of expressed milk handling and storage, necessitating thoughtful consideration of better practices and recommendations.

Abstract

NPSLE diagnosis is still challenging because of many SLE-related and non-SLE-related processes that can be presented in patient. The report of NPSLE in Indonesia is still limited. This study aim to describe the clinical features, nutrition status, and laboratory characteristics of Neuropsychiatric Systemic Lupus Erythematosus (NPSLE) and compared to non NPSLE case in Indonesian children. The study is a retrospective cohort study. Data were collected from the complete medical record of Juvenile Systemic lupus Erythematosus (jSLE) patients 2016 - 2020 at the Allergy Immunology Outpatient clinic at Dr. Soetomo General Academic Hospital. We include all patients with ages ranging from age 0-18 years old with a diagnosis of Systemic lupus Erythematosus (SLE). The diagnosis fo SLE based on American College of Rheumatology (ACR) criteria 1997 and Neuropsychiatric (NP) manifestations were classified using the standardized nomenclature and case definitions for the 19 NP manifestations linked to SLE developed in 1999 by the ACR ad hoc Committee. Disease activity SLE was defined according to the American Mexican-Systemic Lupus Erythematosus Disease Activity Index (Mex-SLEDAI) criteria. Statistical analysis conducted in this study was descriptive analysis, paired T-test (NPSLE vs. non-NPSLE as the dependent variable), Fischer exact test, and Pearson Chi-square test using SPSS ver. 21. A total of 90 patients with juvenile SLE were enrolled, but only 71 patients were eligible as participants with complete medical records obtained. Mex-SLEDAI score was significantly higher on NPSLE compared to non-NPSLE (p=0.001).

Abstract

Introduction: The rising prevalence of food allergies in both developed and developing countries has emerged as a significant public health concern. Food allergies can result from various foods, with common triggers in children including cow's milk, eggs, soy, wheat, fish, seafood, peanuts, tree nuts, and other dietary staples. These allergies can lead to diverse clinical manifestations affecting multiple organs and systems, posing severe health challenges, particularly for children. Objective: This study aims to outline the characteristics of pediatric patients with food allergies treated at Dr. Soetomo Hospital, Surabaya. Methods: A descriptive observational approach using a cross-sectional method was employed, analyzing data from patient medical records. The sample consisted of total cases meeting inclusion criteria. Results and Discussion: Among the patients, 61.1% were female, with an average age of 6.07 ± 3.44 years. Common allergenic foods included bananas (70.4%), tilapia fish (68.5%), and mackarel tuna (59.3%). Notably, 57.4% of children exhibited sensitivity to two or more allergen groups. The predominant clinical symptoms were rhinitis (92.6%) and chronic recurrent cough (70.4%). Conclusion: Pediatric food allergy patients are predominantly female, around six years old on average, with bananas, tilapia, and mackarel tuna identified as frequent allergens. Respiratory symptoms, particularly rhinitis and chronic cough, were the most common clinical presentations

Abstract

Juvenile dermatomyositis (JDM) is a rare chronic autoimmune disease belonging to idiopathic inflammatory myopathies. Pathological skin lesions and proximal weakness primarily characterize this entity, but clinical symptoms can be heterogeneous. Children are more likely to have long-term complications such as lipodystrophy, calcinosis, and vasculopathy. Calcinosis is one of the characteristic sequelae of JDM, despite recent advances in the treatment of JDM, about one-third of patients still develop dystrophic calcinosis. In low-income countries, the availability of medicines is very limited. In our case, a 7-year-old female diagnosed with JDM presented with calcinosis. Aggressive and adequate treatment with steroids, methotrexate, hydroxychloroquine, and aluminium hydroxide can treat the complications. It is challenging to establish an early diagnosis, treatment, prevention of long-term complications, and improved prognosis of JDM, which then will improve the patient’s quality of life, especially in low income countries with limited drug availability.

Abstract

Highlights: The Bacillus Calmette-Guérin (BCG) vaccination has been widely implemented in Indonesia, but cases of spinal tuberculosis (TB) in children can still be found. There are significant and moderate relationships between BCG vaccination and the occurrence of spinal TB in children.   Abstract Introduction: Spinal tuberculosis (TB) is an extrapulmonary form of TB that affects the spine. The Bacillus Calmette-Guérin (BCG) vaccination program, which has been implemented to prevent TB, should have prevented this type of TB. However, cases of this disease in children can still be found. This study aimed to determine whether there was a relationship between BCG vaccination and the occurrence of spinal TB in children. Methods: This case-control analytic observational study was performed based on medical record data. The study subjects of each group, both case and control, were taken from pediatric TB patients at Dr. Soetomo General Academic Hospital, Surabaya, from 2017 to 2021. Patients with malnutrition and/or human immunodeficiency virus (HIV) infection were excluded. Statistical analyses were performed using Fisher’s exact test. Results: There were 9 patients in total within each group. Most were 12-17 years old, and the ratio of male by female was 1.25:1. There were 4 out of 9 (44%) spinal TB patients and 9 of 9 (100%) patients with no spinal TB who all received BCG vaccination. The statistical test indicated a significant (p=0.029) and moderate (C=0.527) relationship between BCG vaccination and the occurrence of spinal TB in children. Conclusion: The Bacillus Calmette-Guérin vaccination should be able to prevent the new occurrence of spinal TB in children.

Abstract

Atopic dermatitis (AD) is chronic inflammation of skin manifested as recurrent itchy redness patches. Avoiding precipitating factors is significant key of successful treatment, but not always possible. House dust mites (HDM) are the most common but not easily proven as aeroallergen. Skin prick test (SPT) is an effective diagnostic modality in recognizing type 1 hypersensitivity reaction. Positive result on SPT should be confirmed by clinical relevance to find the true causative factor. The aim of the study was to analyze the clinical relevance of HDM as AD causative factor in Surabaya, Indonesia. This is a descriptive observational study. Sixty samples were taken from SPT medical records in Allergy-Immunology Division outpatient care at Dr. Soetomo General Academic Hospital period of 2017 - 2019. All samples that qualify inclusion criteria were included. Data were compiled in data sheet, categorized as nominal, and presented as clinical relevance. This study reported that AD is more common in 21-30 years old (70%), Javanese (93.33%), female (783.3%). Most of them had history of itchy redness patches (83.33%), dry skin (55%), self and family history of atopic disease (93.33%; 53.33%). History of suspected HDM allergen was 50%, positive SPT results were 68,33%. Clinical relevance had shown that in 37 patients (61,67%), 17 out of 23 (73.92%) patients did not suspect HDM as allergen but has positive SPT results. It may conclude that HDM should be considered as hidden potent causative factor of AD in Surabaya, Indonesia.

Abstract

Background: Ramadan fasting is one religious challenge for children with type 1 diabetes mellitus (T1DM) which required specific management and strict monitoring. Fructosamine, which reflects glycemic control within 2-3 weeks, is an ideal parameter of glycemic control in Ramadan fasting. This study aimed to analyzed the effect of Ramadan fasting on fructosamine level of children with T1DM.Subjects and Method: This prospective cohort study included children aged under 18 years old diagnosed wit T1DM for >6 months at Dr. Soetomo Hospital, Surabaya, Indonesia in Ramadan 1438 H (May-June 2017). The independent variable from this study was Ramadhan fasting and the depen­dent variables were fructosamine and HbA1c. The sampling technique was total sampling. Forty-two children were included and devided into 27 children in the fasting and 15 children in the non-fasting group. Fructosamine and HbA1c levels were measured before, mid- and after Ramadan and the comparison between groups were analyzed using the independent T-test.Results: The mean age of all the subjects was 8.95 (SD=3.2) years old, 47.6% were males. The mean (SD) HbA1c level of all the subjects was 10.42 (1.16) %. There was a mild reduction of mean fructosamine level after Ramadan in the fasting group. There was no significat differece I fructsamie level before (Mean= 556.8; SD= 180.39; p= 0.620), mid (Mean= 568.07; SD= 175.37; p= 0.520), and after Ramadhan between groups (Mean= 539.41; SD= 173.04; p= 0.410) respectively. The average fructosamine levels after Ramadan was positively and strongly related to HbA1c (r=0.69; p<0.001). There was no case of severe hypoglycemia and diabetic ketoacidosis in both groups.Conclusion: There is a reduction of mean fructosamine level after Ramadan fasting in children with T1DM although it isn’t statistically significant.Keywords: type 1 diabetes, children, fasting, Ramadan, fructosamineCorrespondence: Nur Rochmah.
