Abstract

Introduction: Perineal wounds are a very common complication of vaginal birth. Perineal trauma associated with childbirth is defined as damage to the skin, perineal muscles, as well as the sphincter and anal epithelium complex. This study aims to determine the effect of PRP use on perineal wounds.  Method: A single-center, controlled, non-randomized, non-blinded clinical trial was designed in patients with perineal wounds after vaginal delivery. PRP was taken from each patient's serum (autologous). Perineal wound healing after intervention was assessed using the Redness, Oedema, Ecchymosis, Discharge, Approximation (REEDA) scale. Perineal wound pain was assessed using the Visual Analog Scale (VAS). Result: The current study involved 56 samples, 28 people per group. The average age of the patients was 28.38 ± 6.280 years. The highest education was high school, 42.9%, while the lowest was elementary school (21.4%). The highest VAS scale was 3 (42.9%) and the lowest was 1 (5.4%). The prevalence of the highest degree of perineal wounds in general was degree 2 (98.2%). The highest score was in the moderately healed group on PRP administration (92.9%). In contrast to the control group, the mildly healed category was 21.4% and not healed 14.4%.  Conclusion: There is a significant relationship between the PRP to healing perineal wounds in vaginal delivery patients at the Seberang Padang Health

Abstract

Introduction: Polycystic ovary syndrome involves the endocrine system, metabolic system, as well as the reproductive system contributing to anovulatory infertility. Polycystic ovary syndrome affects 5-20% of reproductive age and 6-12% of adolescents. Oxidative stress is known to play an important role in the pathogenesis of PCOS resulting in excessive formation of ROS (malondialdehyde). ROS can be controlled by the antioxidant inositol and has minimal side effects.  To determine the effectivity of inositol antioxidant on malondialdehyde serum levels in female students Medical faculty, Andalas University with clinical PCOS. Methods: This study is a clinical trial research with a pure experimental study design (true experimental design). There were 23 samples for the experimental group and 23 samples for the control group selected by probability sampling. Normality test using Shapiro-Wilk test. Data analysis using Independent T test and Mann Whitney Test. Results: There was a significant decrease in malondialdehyde serum levels in PCOS students who consumed inositol therapy for 3 months. Conclusion: There is an effect of inositol antioxidant consumption on malondialdehyde serum levels in female students Medical faculty, Andalas University with clinical polycystic ovary syndrome.

Abstract

Introduction: Ovarian cancer is the primary cause of cancer-related mortality in women, with epithelial ovarian cancer (EOC) ranking as the second greatest cause of death among gynaecological malignancies.  The rising use of plastics has resulted in increased microplastic exposure among the population.  This exposure aligns with recognised risk factors for ovarian cancer, including genetic, hormonal, and lifestyle elements. This study aimed to investigate the relationship between microplastics in blood samples and the stage of epithelial ovarian cancer at Dr. M. Djamil General Hospital, Padang. Methods: Between August 2024 and January 2025, a cross-sectional analytical correlational investigation was carried out.  Forty individuals with histopathologically verified epithelial ovarian carcinoma were consecutively recruited.  Blood samples were examined for di(2-ethylhexyl) phthalate (DEHP) microplastics utilizing Liquid Chromatography–Mass Spectrometry (LCMS).  Data were evaluated utilizing descriptive statistics and the Chi-square test, with p < 0.05 being significant. Results: The average age of patients was 49.1±10.59 years, with the majority being multiparous, married, and of normal weight.  The histological subtypes comprised mucinous (37.5%), serous (30%), endometrioid (20%), and clear cell (12.5%).  Microplastics were identified in 30% of blood samples, with the greatest prevalence observed in mucinous (40%) and endometrioid (37.5%) subtypes. Statistical study indicated no significant correlation between microplastic occurrence and the stage of epithelial ovarian cancer (p = 0.869). Conclusion: Microplastics were identified in the blood of around one-third of patients with epithelial ovarian cancer, present across all histological subtypes. However, no substantial link was identified between the presence of microplastics and the cancer stage.

Abstract

Latar belakang: Rinosinusitis kronis (RSK) merupakan inflamasi pada mukosa hidung dan sinus paranasal. Rinosinusitis kronis berdasarkan perbedaan ekspresi sitokin dan pola remodeling inflamasi dibedakan menjadi dua fenotipe, RSK dengan polip dan RSK tanpa polip. Transforming growth factor–?1 (TGF–?1) merupakan salah satu sitokin yang berperan pada remodeling jaringan.       Tujuan: Mengetahui ekspresi gen TGF–?1 pada RSK dengan polip dan RSK tanpa polip. Metode: Penelitian analitik komparatif menggunakan desain potong lintang (cross sectional comparative study) pada 12 responden RSK dengan polip dan 12 responden RSK tanpa polip. Sampel diambil saat operasi Bedah Sinus Endoskopi Fungsional (BSEF) pada jaringan polip dan mukosa sinus etmoid atau maksila. Semua sampel dilakukan pemeriksaan ekspresi gen dengan metode Real Time Polymerase Chain Reaction (RT-PCR). Data dianalisis dengan SPSS, dikatakan bermakna jika p<0,05. Hasil: Ekspresi TGF-?1 pada RSK tanpa polip lebih tinggi (18,63±24,58) dibandingkan RSK dengan polip (2,82±4,02). Secara statistik perbedaan antara RSK polip dan RSK tanpa polip ini bermakna (p<0,05). Kesimpulan: Terdapat peningkatan ekspresi TGF-?1 pada RSK tanpa polip yang bermakna secara statistik dibandingkan dengan RSK dengan polip.

Abstract

Latar Belakang: Pendahuluan: Karsinoma nasofaring memiliki rekurensi atau metastasis jauh, yang menandakan suatu adanya aktifitas sel yaitu proliferasi sel. Penilaian adanya suatu proliferasi sel memerlukan biomarker, salah satunya Ki67 yang sampai saat ini sudah dikenal baik sebagai penanda proliferasi sel. Tujuan: Mengetahui peran Ki67 pada karsinoma nasofaring. Tinjauan Pustaka: Karsinoma nasofaring adalah tumor ganas sel skuamosa yang terjadi di daerah perbatasan epitel nasofaring dimana epitel kubus beralih menjadi epitel skuamosa. Pada stadium dini tidak mudah dideteksi karena secara anatomis terletak jauh di dalam kepala. Ki67 berhubungan erat dengan mitosis dan aktivitas proliferasi tumor ganas serta kinerja pengobatan kanker nasofaring. Kesimpulan: Ekspresi Ki67 yang tinggi merupakan penanda untuk prognosis buruk pada karsinoma nasofaring. ABSTRACT Background: Introduction: Nasopharyngeal carcinoma has recurrence or distant metastasis, which indicates a cell activity, namely cell proliferation. Assessment of the presence of a cell proliferation requires biomarkers, one of which is Ki67 which has been established as a marker of cell proliferation. Objective: To determine the role of Ki67 in nasopharyngeal carcinoma. Literature Review: Nasopharyngeal carcinoma is a malignant squamous cell tumor that occurs in the border area of the nasopharyngeal epithelium where the cuboidal epithelium turns into the squamous epithelium. At an early stage, it is difficult to detect because it is anatomically located deep in the head. Ki67 is closely related to the mitotic and proliferative activity of malignant tumors and the treatment performance of nasopharyngeal carcinoma. Conclusion: High Ki67 expression is a marker for poor prognosis in nasopharyngeal carcinoma.

Abstract

Latar Belakang: Karsinoma nasofaring (KNF) adalah keganasan yang berasal dari sel epitel nasofaring. Kelangsungan hidup dan prognosis pasien KNF sangat tergantung pada stadium tumor. Pemahaman yang baik tentang faktor-faktor yang mempengaruhi prognosis dan perkembangan penyakit sangat penting dalam pengelolaan KNF. Salah satu faktor yang berpotensi memainkan peran kunci dalam hal ini adalah enzim cyclooxygenase-2 (COX-2). Peningkatan aktivitas COX-2 dapat memfasilitasi sel-sel tumor untuk memperoleh berbagai kemampuan biologis yang berkontribusi pada pertumbuhan dan perkembangan tumor. Tujuan dari penelitian ini adalah untuk mengetahui perbedaan ekspresi enzim COX-2 berdasarkan stadium karsinoma nasofaring. Metode: Penelitian ini merupakan penelitian cross-sectional analitik komparatif numerik tidak berpasangan dua kelompok satu kali pengukuran pada jaringan tumor dalam bentuk blok parafin dari hasil biopsi pasien KNF dan dilakukan pemeriksaan imunohistokimia untuk menilai ekspresi COX-2 pada jaringan tumor KNF yang dibagi menjadi kelompok stadium dini dan stadium lanjut. Data dianalisis secara statistik dengan program komputer dan dinyatakan bermakna jika p<0,05. Hasil: Pada penelitian ini didapatkan rerata ekspresi COX-2 pada KNF stadium lanjut (69,59±11,75) lebih tinggi dibandingkan dengan KNF stadium dini (47,29±31,04), namun hasil uji statistik menunjukkan bahwa perbedaan tersebut tidak signifikan secara statistik (p=0,28). Kesimpulan: Terdapat kecenderungan peningkatan ekspresi COX-2 pada stadium lanjut dibandingkan dengan stadium dini, namun perbedaan ini tidak signifikan secara statistik.

Abstract

Background: Congenital Heart Disease with Pulmonary Hypertension (CHD-PH) increases Right Ventricular (RV) afterload, which may lead to subclinical myocardial impairment before overt systolic dysfunction becomes apparent. Early detection is crucial to prevent progression to right heart failure. Global Longitudinal Strain (GLS) by echocardiography is sensitive for identifying subclinical RV dysfunction despite preserved conventional systolic parameters, but the role of molecular biomarkers remains unclear. Soluble Suppression of Tumorigenicity 2 (sST2) has been proposed as a potential biomarker for subclinical RV dysfunction. This study aims to compare sST2 levels in CHD-PH patients with and without subclinical RV dysfunction as assessed by RV-Global Longitudinal Strain (RV-GLS). Methods: This cross-sectional study included adult CHD-PH patients (≥18 years) at the Integrated Heart Center, RSUP M. Djamil Padang, from January to June 2025. All participants underwent right heart catheterization and had preserved RV systolic function, as assessed by conventional echocardiography (Three-Dimensional Right Ventricular Ejection Fraction [3D RVEF] ≥45%). RV-GLS was assessed using Speckle-Tracking Echocardiography (STE), and serum sST2 levels were measured. Patients were categorized into subclinical RV dysfunction (GLS > –20%) and without subclinical RV dysfunction (GLS ≤ –20%) groups. Statistical analysis was performed to compare sST2 levels between groups. Results: Thirty-four patients were included (82% female, mean age 37.8 ± 15.6 years), with secundum Atrial Septal Defect (ASD) as the most common etiology (71%). Median sST2 levels in patients with CHD-PH without subclinical RV dysfunction were 15.65 (6.05–40.90) ng/mL, with a GLS of –20.50 (–27.20 to –20.00)%. In patients with CHD-PH with subclinical RV dysfunction, median sST2 was 15.15 (5.25–47.60) ng/mL, with a GLS of –11.80 (–19.90 to –6.20)%. No statistically significant difference in sST2 levels was observed between groups (p = 0.89). In contrast, patients with CHD-PH with subclinical RV dysfunction exhibited significantly higher indexed Pulmonary Arterial Resistance (PAR) and pulmonary-to-Systemic Vascular Resistance ratio (PAR/SVR ratio), indicating increased pulmonary vascular load despite preserved conventional RV systolic function. Conclusion: sST2 levels did not differ significantly between CHD-PH patients with subclinical RV dysfunction and those without. Subclinical RV dysfunction was associated with higher pulmonary vascular load, as reflected by increased indexed PAR and PAR/SVR ratio, despite preserved conventional RV systolic function. From this may conclude that subclinical RV myocardial impairment in CHD-PH is more closely related to hemodynamic afterload than to molecular stress biomarkers alone. These findings suggest that sST2 alone may have limited utility as a biomarker for detecting subclinical RV myocardial impairment in this population.

Abstract

Background: Gingivitis is an inflammatory condition of the gingiva caused by dental plaque accumulation. Scaling root-planing (SRP) is the gold standard therapy for gingivitis; however, its long-term effectiveness may be limited due to bacterial recolonization and plaque re-accumulation after treatment. Red Palm Oil (RPO) (Elaeis guineensis) contains bioactive compounds with potential as an adjunctive therapy following SRP. Objective: This study aimed to analyze the effect of topical RPO application as an adjunctive therapy after SRP on changes in clinical parameters and salivary Matrix Metalloproteinase-8 (MMP-8) levels in gingivitis patients. Methods: This study utilized a randomized controlled trial design. A total of 40 gingivitis patients were randomly assigned into two groups: the control group (n=20), receiving SRP with placebo application, and the RPO group (n=20), receiving SRP followed by topical RPO application. The evaluated parameters included Gingival Index (GI), Oral Hygiene Index-Simplified (OHI-S), and salivary MMP-8 levels. Statistical analyses were performed using parametric and nonparametric tests according to data distribution. Results: Significant differences in GI and OHI-S changes were observed between the control and RPO groups (p<0.05), with a greater reduction in clinical parameter scores in the RPO group. Analysis of salivary MMP-8 levels showed a decrease after intervention in both groups; however, the difference between groups was not statistically significant (p>0.05). Conclusion: Topical application of Red Palm Oil as an adjunctive therapy after SRP has the potential to improve clinical periodontal parameters in gingivitis patients. However, the additional effect of RPO on changes in salivary MMP-8 levels has not demonstrated a statistically significant difference compared with SRP alone.

Abstract

Background: Optimal growth and development in toddlers require appropriate food for growing toddlers. Stunting indicates inadequate nutritional intake in children. Purpose: To determine the risk factors and determinants associated with stunted infant development growth. Method: A descriptive quantitative with analytical survey specially focusing on cross-sectional method. The study took place from September 2024 to July 2025 in Dadok Tunggul Hitam Community Health Center, Padang. The independent variables studied were toddler age, gender, birth weight history, exclusive breastfeeding history, number of siblings, mother's age, mother's education, mother's occupation, family income, and gadget use, while the dependent variable was toddler growth. Using random sampling and the Slovin formula, 86 respondents meeting the inclusion and exclusion criteria were selected and categorized into two groups: suspect and normal. Data analysis began with univariate tests to describe the distribution of respondent characteristics, followed by bivariate analysis using the chi-square test and multivariate analysis with logistic regression. Results: The logistic regression results show that the most influential factor on the occurrence of suspicious developmental events is gadget use, especially in the high use category, with an increased risk of up to 18 times (Exp(B) = 18.125; p = 0.000). Toddler age also plays a significant role, with an approximately 5-fold increase in risk (Exp(B) = 5.406; p = 0.008). The number of siblings and family income also contribute each increasing the likelihood of developmental disorders by approximately 3–4 times (p < 0.05). Conclusion: Gadget use is the factor most strongly linked to growth delays in toddlers.
