Abstract

Indonesia is a country with high-risk polio transmission. Administering inactivated polio vaccination (IPV) after drops-polio or oral polio vaccination (OPV) was a strategy stopping polio transmission because its ability to ward off poliovirus which was very dangerous. The IPV coverage was very low and have never reached the target since it was launched in 2016. The study purpose was analyzing determinants factors of IPV adherence. A cross-sectional survey conducted in 31 districts in Central Java Province. Sample was 685 children aged 11-23 months and selected using purposive and accidental sampling method. Collecting data by interview using structured questionnaire and observation with maternal and child health (MCH) Handbook to identify immunization status. The collected data was analyzed univariate, bivariate, and multivariate. As many as 74.3% of children received IPV. Partially, variables of delivery history, immunization status, knowledge, attitudes, sources of information, and service satisfaction were related to IPV adherence. Simultaneously, immunization status, attitudes, and sources of information influenced IPV adherence with an overall effect 35.2%. Immunization status as the dominant influencing factor. Efforts were needed reducing resistance to IPV through psychological approach that was motivating and personal, also using of social media as a medium for positive information and communication, in addition to improve immunization service system.

Abstract

Background: Exclusive breastfeeding coverage in Bengkulu City has decreased over the last three years. Mothers' confidence in their abilities and good mental health become an essential part of supporting exclusive breastfeeding.Objective: This study aims to examine the differences between self-efficacy and mental health of mothers based on their breastfeeding status, located in Bengkulu City.Methods: The researchers employed a cross-sectional approach to analyze the study. The sample consisted of 72 mothers with infants aged 0-6 months. The researchers selected the samples using a purposive sampling technique. Data collection was carried out using a digital questionnaire (via Google Forms) through validity and reliability tests. Data were analyzed using an independent t-test. The mean self-efficacy score of partially breastfeeding mothers is lower than that of exclusively breastfeeding mothers. The mean mental health score of partially breastfeeding mothers is higher than that of exclusively breastfeeding mothers.Results: The results of the independent t-test found that the average self-efficacy score of exclusively breastfeeding mothers was 55.58, and the average mental health score of partially breastfeeding mothers was 10.32.Conclusion: There was a difference in the self-efficacy and mental health scores of partially and exclusively breastfeeding mothers. A high self-efficacy score indicates high maternal confidence. Meanwhile, high mental health scores indicate that mothers experience symptoms of psychological disorders. The achievement of successful exclusive breastfeeding is closely related to boosting self-assurance and preserving a mother's mental well-being.

Abstract

Introduction: The right marketing strategy is not only limited to promoting products, but also arranging segmentation, namely dividing product marketing based on target customers and target markets. Hospitals really need a marketing strategy to face all changes and challenges both internal and external to a company.Purpose: To analyze segmentation and determine the target market for the establishment of Bunda Mulya Hospital in Bogor RegencyMethods: Quantitative and qualitative research approaches (mixed methods). The quantitative approach used in this research is analytical observational with a cross sectional design. The qualitative approach in this research uses a grounded theory type of design. Market segmentation analysis was carried out to determine the appropriate target market for the establishment of Bunda Mulya Hospital through geographic, demographic, psychographic and behavioral approaches. The research subjects were the people of Bogor Regency, the Chair of PERSI Banten Province, Deputy Chair II of PERSI West Java Province, the Bogor Regency Health Service, and the Main Director of the Bunda Mulya Pratama Clinic.Results: The appropriate target market for establishing Bunda Mulya Hospital is the community around Bunda Mulya Hospital (travel time < 30 minutes) with an age range of teenagers - early adults (12-45 years), female, with a predominant high school/equivalent education level. by private workers, families of 2-4 people with total family income between 3-5 million. The public hopes to get fast treatment and complete specialist doctors. The community gets most of the information from neighbors/cadres/friends and visits 1-5x/year. The public provides suggestions regarding class I inpatient rates of <250,000, VIP class inpatient rates of <400,000, general practitioner consultation fees between 40,001-50,000, specialist doctor consultation fees between 100,001-125,000, normal delivery package costs between 1.5-2.5 million and The cost of an ultrasound examination is between 150,000-250,000. Most of the people around Bunda Mulya Hospital are willing to undergo examinations at Bunda Mulya HospitalConclusion: Plans for establishing a new hospital should pay attention to the needs of people of productive age (26-45 years), especially women. The public expects to receive fast service and complete specialist doctors. Therefore, this becomes input for Hospital Management to be able to meet community needs. Hospital financing should pay attention to the UMR limit for Bogor Regency where costs are adjusted for middle and lower middle class people

Abstract

Padangsari Community Health Center is one of the community health centers in Banyumanik sub-district with a low percentage of SICENTIK's participation. This research aimed to conduct implementation observation research at SDN Pedalangan 1 and SDN Pedalangan 2. This research was a qualitative descriptive research that uses purposive techniques in selecting informants. It was held on November 2023 - April 2024. Data collection using in-depth interviews and literature study. The results of this research show that there are several differences in implementation between SDN Pedalangan 1 and SDN Pedalangan 2 shown by the place of this program was implemented, principal's role, division of labour, form letter, knowledge and experience of implementer, shared commitments, implementer's preparations, and evaluations of the program.  The study's findings demonstrate that the lack of Standard Operating Procedures (SOP), success indicators, and health center controls is the primary cause of the disparities between the two schools. Thus, in addition to creating disparities between the two institutions, it also results in wasteful and poor program execution. Then, during the program's implementation, it was found that the community health center and SDN Pedalangan 2's supervisor had acted in an unprofessional manner, which had led to a lackluster implementation of SICENTIK in schools and community health centers.

Abstract

Program Audit Maternal dan Perinatal (AMP) merupakan program yang dibentuk untuk menurunkan angka kematian ibu dan bayi dengan cara menggali faktor penyebab kematian yang dapat dicegah. Di Kabupaten Cianjur program ini belum dapat terlaksana dengan optimal, hal ini dapat terlihat dari angka kematian ibu dan bayi yang masih cukup tinggi dari tahun ke tahun. Tujuan penelitian ini untuk menganalisis pelaksanaan Program Audit Maternal dan Perinatal di Kabupaten Cianjur.Desain penelitian menggunakan rancangan kualitatif yang disajikan secara deskriptif eksploratif. Informan utama adalah 4 orang tim AMP dan Informan triangulasi 9 orang. Data dikumpulkan dengan teknik wawancara mendalam. Analisis data menggunakan analisis isi. Hasil penelitian menunjukkan bahwa implementasi program AMP belum optimal: tim AMP belum melibatkan lintas sektor dan sesi pembelajaran belum berkelanjutan, pemantauan dan evaluasi tidak menyeluruh dan tidak ada tindak lanjut. Sosialisasi belum sampai pada semua bidan, tidak ada pelatihan tim pengkaji. Dana terbatas untuk melakukan pertemuan pembahasan kasus, formulir kasus kematian diperbanyak oleh bidan. Dinas Kesehatan belum mempunyai Standar Operasional Prosedur (SOP) program AMP dan Surat Keputusan (SK) tim AMP belum ditandatangani Bapak Bupati. Disarankan untuk melakukan advokasi dengan bagian Satuan Kerja Perangkat Daerah (SKPD) untuk menindaklanjuti draft SK, melakukan evaluasi dan tindak lanjut secara berkala terhadap pelaksanaan sosialisasi AMP revisi oleh Puskesmas, membuat SOP program AMP, meningkatkan komitmen dan tanggungjawab pelaksana program dengan pemberian reward. Abstract

Type C "X" Specialized Hospital in Semarang experienced in vitro fertilization (IVF) and embryology laboratory consumable medical supplies (CMS) stockouts in early 2023, delaying medical care. Minimum-maximum stock level (MMSL) is an alternative pharmaceutical inventory procurement method. Compared to the conventional method, this study examined how well the MMSL method controls IVF medications and embryology laboratory CMS at Hospital “X” Department of Pharmacy. This study is a quantitative, secondary data-based, quasi-experimental, non-equivalent study without a control group. The study sampled all IVF medications and embryology laboratory CMS data from October to December 2023 at Hospital “X” with consistent supply. MMSL was implemented prospectively in January – March 2024. Twelve (12) IVF medications and fifty-six (56) embryology laboratory CMS were studied. Efficiency analysis included inventory value, inventory turnover ratio (ITOR), and stockout value; effectiveness analysis included stockout incidences. Statistics were tested using Wilcoxon signed-rank and McNemar tests. MMSL implementation led to increased inventory value of IVF medications (from IDR21,514,537.00 to IDR49,635,116.00, p=0.040, p<0.05), decreased inventory value of embryology laboratory CMS (from IDR117,421,582.00 to IDR105,318,804.00, p=0.005, p<0.05), and increased ITOR of embryology laboratory CMS (from 0.82 to 0.84 times, p=0.040, p<0.05). MMSL significantly improved IVF medications and embryology laboratory CMS inventory management efficiency.

Abstract

Background: Labor pain feels tremendous, and only 2-4% of mothers low pain during labor. Heat therapy is a non-pharmacological method. The heat therapy is still conventional, and this study uses a relaxation belt with more stable, dry heat media, and comfortable.Objective: This study aims to develop and test a relaxation belt for efforts to reduce labor pain intensity and increase β-endorphin levels in the primigravida of the active labor phase.Method: This study Research and Development (R&D). It consisted of 5 stages; stage I (literature study), stage II (product development), stage III (expert validity and phase I trials), stage IV (product revision and final product), and stage V (phase II trials).Results: The relaxation belt has been created, it has been validity experts test and field trials. The result that the relaxation belt is more effective in reducing pain and increasing β-endorphin levels than warm water compress (mean different pain labor 2.40; p<0.01; effect size 1.72 and mean different increasing β-endorphin levels 53,34; p<0.01; effect size 2.42).Conclusion: The relaxation belt is effective in reducing labor pain intensity and increasing β-endorphin levels.

Abstract

Berdasarkan data Puskesmas Padangsari, diketahui bahwa cakupan pelayanan balita sudah mencapai 100% atau sebanyak 1.167 balita pada tahun 2023. Namun, kasus penyakit pada balita masih cukup tinggi. Kegiatan pengabdian ini bertujuan untuk upaya peningkatan pengetahuan dan kemampuan masyarakat, khususnya ibu balita untuk menerapkan MTBS berbasis masyarakat di lingkungan rumah untuk penurunan angka kesakitan pada bayi dan balita. Kegiatan pengabdian ini dilaksanakan pada tahun 2024 dengan peserta kegiatan pengabdian masyarakat ini adalah ibu bayi dan balita, kader FKK, dan kader kesehatan di wilayah kerja Puskesmas Padangsari. Kegiatan dilaksanakan di Posyandu di Kelurahan Jabungan. Pada kegiatan tersebut disampaikan mengenai tanda bahaya pada balita, upaya perawatan di rumah untuk kasus diare, demam, dan batuk, perlindungan dari kekerasan dan kekerasan seksual bagi balita, dan lain sebagainya. Ibu balita di wilayah Puskesmas Padangsari khususnya di Kelurahan Jabungan menunjukkan partisipasi yang baik. Para ibu balita menunjukkan komitmen yang baik untuk melakukan pemantauan kesehatan balita mereka.

Abstract

Tuberkulosis (TB) merupakan salah satu penyebab utama kematian akibat penyakit menular di dunia, dengan angka insidensi dan mortalitas yang masih tinggi, khususnya di negara berkembang seperti Indonesia. Salah satu strategi yang direkomendasikan Organisasi Kesehatan Dunia (WHO) untuk meningkatkan keberhasilan pengobatan TB adalah melalui program Pengawas Minum Obat (PMO). PMO bertujuan untuk memastikan kepatuhan pasien dalam menjalani terapi, mengurangi risiko resistansi obat, dan meningkatkan keberhasilan pengobatan. Penelitian ini bertujuan untuk mengevaluasi peran dan efektivitas PMO berbasis komunitas dalam pengobatan TB melalui metode scoping review. Ulasan ini dilakukan menggunakan metode Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). Sebanyak 356 artikel diidentifikasi melalui basis data seperti PubMed, Scopus, dan Google Scholar. Setelah proses seleksi, 10 studi yang melibatkan 6.912 pasien TB paru memenuhi kriteria inklusi. Data yang diambil meliputi desain studi, lokasi, intervensi PMO, durasi pengobatan, dan hasil utama. Hasil menunjukkan bahwa PMO berbasis komunitas efektif dalam meningkatkan keberhasilan pengobatan hingga 85%-90%, mengurangi tingkat penghentian pengobatan hingga 50%, dan meningkatkan kepatuhan pasien. Dukungan emosional dari keluarga dan relawan komunitas menjadi faktor penting dalam keberhasilan terapi. Desentralisasi layanan melalui PMO berbasis komunitas juga memperluas akses pengobatan, terutama di wilayah dengan keterbatasan fasilitas kesehatan. PMO berbasis komunitas berperan penting dalam mendukung keberhasilan pengobatan TB, terutama di negara dengan beban TB yang tinggi. Untuk meningkatkan efektivitas program, diperlukan pelatihan berkelanjutan bagi PMO, strategi yang sesuai dengan konteks lokal, serta kolaborasi sektor publik dan swasta. Penelitian ini memberikan wawasan strategis bagi pengembangan program pengendalian TB yang lebih baik.

Abstract

Central Java Province is one of the provinces that often experiences disasters. During the period 1 May 2023 – 31 May 2023, 72 incidents of strong winds, 8 incidents of land moving, 107 incidents of flooding, 94 incidents of landslides, 6 volcanic eruptions, and 2 incidents of fire were recorded. In disaster emergencies, the health needs of mothers and children are often overlooked. This study aims to analyze the flow of maternal and child health referrals in health crisis management in the focus location regencies in Central Java Province. This research is descriptive research with a qualitative approach. Data was collected through Focus Group Discussion (FGD) in March 2023. The research locations were Semarang Regency, Brebes Regency, Grobogan Regency, and Klaten Regency. The sample selection technique used was purposive sampling. The integrated referral system is not yet running optimally. The health referral flow carried out is not yet specialized in treating mothers and children. Pentahelix collaboration is carried out in managing health crises. The supporting factors in disaster management are the existence of policies, the availability of infrastructure for evacuating victims, as well as coordination from various sectors. Meanwhile, the inhibiting factors are the lack of trained human resources, limited logistics, and low public awareness regarding disasters. Thus, there is a need to develop a special referral pathway for maternal and child health involving multi-sectors, increase public awareness of disaster management, and strengthen Pentahelix collaboration in health crisis management.
