Abstract

Program Audit Maternal dan Perinatal (AMP) merupakan program yang dibentuk untuk menurunkan angka kematian ibu dan bayi dengan cara menggali faktor penyebab kematian yang dapat dicegah. Di Kabupaten Cianjur program ini belum dapat terlaksana dengan optimal, hal ini dapat terlihat dari angka kematian ibu dan bayi yang masih cukup tinggi dari tahun ke tahun. Tujuan penelitian ini untuk menganalisis pelaksanaan Program Audit Maternal dan Perinatal di Kabupaten Cianjur.Desain penelitian menggunakan rancangan kualitatif yang disajikan secara deskriptif eksploratif. Informan utama adalah 4 orang tim AMP dan Informan triangulasi 9 orang. Data dikumpulkan dengan teknik wawancara mendalam. Analisis data menggunakan analisis isi. Hasil penelitian menunjukkan bahwa implementasi program AMP belum optimal: tim AMP belum melibatkan lintas sektor dan sesi pembelajaran belum berkelanjutan, pemantauan dan evaluasi tidak menyeluruh dan tidak ada tindak lanjut. Sosialisasi belum sampai pada semua bidan, tidak ada pelatihan tim pengkaji. Dana terbatas untuk melakukan pertemuan pembahasan kasus, formulir kasus kematian diperbanyak oleh bidan. Dinas Kesehatan belum mempunyai Standar Operasional Prosedur (SOP) program AMP dan Surat Keputusan (SK) tim AMP belum ditandatangani Bapak Bupati. Disarankan untuk melakukan advokasi dengan bagian Satuan Kerja Perangkat Daerah (SKPD) untuk menindaklanjuti draft SK, melakukan evaluasi dan tindak lanjut secara berkala terhadap pelaksanaan sosialisasi AMP revisi oleh Puskesmas, membuat SOP program AMP, meningkatkan komitmen dan tanggungjawab pelaksana program dengan pemberian reward. Kata Kunci : Implementasi, Program Audit Maternal-Perinatal, komunikasi, sumber daya, disposisi, struktur birokrasi.

Abstract

Penelitian ini bertujuan untuk mendeskripsikan dan menganalisis manajemen perubahan dalam rangka kesiapan untuk proses transformasi dari puskesmas BLUD holding menuju BLUD mandiri, faktor yang mendorong dan menghambat, serta strateginya. Penelitian menggunakan pendekatan kualitatif. Informan adalah mantan Direktur BLUD Holding, Kepala Puskesmas. Kepala TU dan Bendahara dari Puskesmas Sokorejo dan Puskesmas Buaran, serta Dinas Kesehatan Kota Pekalongan. Data diperoleh menggunakan wawancara dan dokumentasi. Metode analisis data adalah analisis kualitatif. Hasil penelitian (1) Manajemen perubahan dalam rangka kesiapan untuk proses transformasi dari puskesmas BLUD Holding menuju BLUD Mandiri memiliki beberapa tahap, yaitu penetapan status UPT bagi 14 puskesmas, penetapan perwali, pencabutan status BLUD holding, dan penetapan puskesmas BLUD mandiri. Dari keempat tahap ini yang sudah dilakukan adalah penetapan status UPT dan pencabutan status BLUD holding, dan status BLUD Puskesmas; sementara perwali masih dalam proses. Kesiapan manajemen yang dilakukan mencakup SDM, anggaran, sarana dan prasarana, serta persyaratan BLUD. Dari kesiapan ini, yang sudah dilakukan adalah kesediaan SDM meskipun distribusi belum merata, anggaran yang bersumber dari luar masih terbatas, minimnya biaya pemeliharaan sarana prasarana, serta terpenuhi persyaratan administrasi. (2) Faktor yang mendorong dan menghambat manajemen perubahan dalam rangka kesiapan untuk proses transformasi dari puskesmas BLUD holding menuju BLUD mandiri meliputi regulasi, kelembagaan, SDM, anggaran, sarana prasarana, dan masyarakat. (3) Strategi manajemen perubahan dalam rangka kesiapan untuk proses transformasi dari puskesmas BLUD holding menuju BLUD Mandiri mencakup peningkatan koordinasi antara puskesmas dan dinas kesehatan, peningkatan kualitas tenaga kesehatan dan meratakan distribusi tenaga kesehatan, meningkatkan cakupan kepesertaan JKN, dan meningkatkan pemenuhan sarana dan prasarana (khususnya biaya pemeliharaan kesehatan).

Abstract

Terjadinya turnover berupa pengunduran diri beberapa dokter dan ketidaktaatan dokter terhadap aturan-aturan yang ditetapkan di rumah sakit merupakan indikasi adanya penurunan loyalitas dokter. Dalam penelitian terdahulu disebutkan bahwa beberapa faktor yang mempengaruhi loyalitas karyawan diantaranya adalah kompensasi, motivasi, gaya kepemimpinan dan kepuasan karyawan. Tujuan dari penelitian ini adalah untuk melakukan analisis seberapa besar pengaruh kompensasi, motivasi dan gaya kepemimpinan transformasional terhadap kepuasan dan loyalitas dokter di RSUD dr. H. Soewondo Kabupaten Kendal. Desain penelitian ini adalah cross sectional dengan pendekatan kuantitatif. Pengumpulan data dilakukan melalui   pengisian kuesioner oleh 52 dokter yang bekerja di RSUD dr. H Soewondo Kendal ditambah data komposit dari SIMRS. Data yang terkumpul diolah dan diuji dengan menggunakan software Smart PLS 3.0. Hasil penelitian menunjukkan bahwa kompensasi, motivasi dan gaya kepemimpinan transformasional berpengaruh signifikan terhadap kepuasan dokter. Motivasi juga berpengaruh signifikan terhadap loyalitas dokter. Kompensasi dan gaya kepemimpinan transformasional berpengaruh tidak signifikan terhadap loyalitas dokter. Kepuasan dokter kepuasan dokter tidak terbukti secara signifikan memediasi pengaruh kompensasi, motivasi dan gaya kepemimpinan transformasional terhadap loyalitas dokter. Kata Kunci     : Kompensasi, Motivasi,  Kepuasan, Loyalitas, Dokter.

Abstract

Penelitian ini bertujuan untuk menganalisis nilai implementasi manajemen risikopada Instalasi Bedah Sentral Rumah Sakit dr. Kariadi Semarang pada masapandemi COVID-19 ditinjau dari variabel-variabel yang ditemukan dalam studipendahuluan. Penelitian dilakukan di Rumah Sakit dr. Kariadi Semarang padaperiode Maret sampai dengan April 2021. Penelitian ini adalah penelitian kualitatif di mana data penelitian dianalisis secara deskriptif. Adapun teknik analisis datamenggunakan analisis narasi dan tringulasi data. Subjek penelitian yaitu 3 oranginforman yang ditentukan dengan teknik purposive sampling. Informan utama (key informan) dalam penelitian ini adalah Kepala Sub Bagian Komite Mutu. Sedangkan yang menjadi informan adalah Penanggung Jawab Program Manajemen Risiko dan perwakilan Instansi/Unit. hasil penelitian dan pembahasan berkaitan dengan implementasi manajemen risiko pada masa pandemic di IBS dapat diambil kesimpulan bahwasanya penilaain dari impelmentasi program manajemen resiko dinilai cukup optimal. Hal ini dikarenakan masih terdapat kendala dalam pelaksanaannya seperti ketidak pahaman pegawai berkaitan dengan tentang manajemen risiko dimasa pandemi. Disamping itu sejak munculnya COVID-19 belum ada evaluasi yang berfokus pada manajemen risiko di rumah sakit. Selain itu, pemangku kebijakan sudah memiliki pemahaman baik, namun masih terdapat kendala sehingga memerlukan beberapa strategi. Selain itu, kendala berkaitan dengan pemantauan atau peninajaun berkala berkaitan dengan manajemen risiko adalah sistem review risk register belum terintegrasi dengan sistem Rumah Sakit. Sehinga dalam pemantauan dan hasil evaluasi yang dilakukan sub komite membuat E-sistem tersediri atau manual dari google shiff. Sedangkan untuk dimensi impelmentasi manajemen risiko lainnya sudah dinilai optimal, dimana seluruh pemangku jajaran Rumah Sakit ikut berpartisipasi dalam peningkatan manajemen risiko

Abstract

 The utilization of reproductive health services for brides and grooms is still far from the national target (80%), that is 15.8%. This service is deemed lacking in meeting the needs of the bride and groom with various factors. This study aims to analyze the factors that influence the need for reproductive health services for the brides in Brebes District. This research method is quantitative observational research with cross-sectional approach. The population of this research is all brides who have registered in all Religious Affairs Office in Brebes District are 528 people. Primary data were collected through interviews using a structured questionnaire with 165 respondents selected through cluster sampling techniques. Bivariate data analysis was performed by Chi-Square test and multivariate data analysis was performed by logistic regression test. The results of this study indicate that there is a relationship between sex, knowledge, norms and culture of the community, and service technology (p-value <0.05) with the reproductive health service needs for brides and grooms. Meanwhile, the level of education, attitudes, and risk factors are not related to the reproductive health service needs for brides and grooms (p-value> 0.05). The strongest factor affecting the reproductive health service needs for brides and grooms is knowledge (OR = 2.736, 95% CI = 1,383-5,414). The recommendations of this study are service providers conducting surveys and needs for reproductive health services for brides and grooms continuously, branding by involving community leaders and health cadres, and the use of service technology that is practical, interesting and not boring, and can be accessed wherever and whenever they are.

Abstract

Pemerintah Provinsi Jawa Tengah telah mengeluarkan Instruksi Nomor 1 Tahun 2020 tentang Pemberdayaan Masyarakat Dalam Percepatan Penanganan COVID-19 Di Tingkat Rukun Warga (RW) Melalui Pembentukan Satuan Tugas (SATGAS) Jogo Tonggo. Desa Sendangasri merupakan salah satu desa di Kabupaten Rembang yang menerapkan Jogo Tonggo tepatnya di Dusun Pelem. Pada awal bulan Juli dilaporkan terdapat 1 kasus positif covid-19 di Dusun Pelem. Artikel ini merupakan hasil penelitian kualitatif dengan pendekatan deskriptif. Hasil penelitian menunjukkan bahwa pelaksanaan Jogo Tonggo hanya mencakup dua bidang, yaitu bidang kesehatan dan bidang sosial keamanan. Tidak optimalnya pelaksanaan jogo tonggo dipengaruhi oleh beberapa faktor yaitu kurangnya komunikasi antar birokrasi dalam mensosialisasikan jogo tonggo, satgas belum memahami pedoman pelaksanaan program sehingga tidak mengetahui tugas, fungsi dan kewenangannya. Kewenangan pelaksana yang terbatas akibat tidak adanya SK serta adanya ketergantungan yang tinggi terhadap pemerintah desa dalam perencanaan program dan pemenuhan sumber daya.Regional Government of Central Java Province issued Instruction Number 1 of 2020 concerning Community Empowerment in Accelerating Handling of COVID-19 at the Rukun Warga (RW) through the Establishment of a satgas Jogo Tonggo. Sendangasri is  one of the villages in Rembang Regency, which implements Jogo Tonggo, precisely in Pelem Hamlet. In July, Pelem Hamlet has reported a positive case of Covid-19. This article is based on qualitative research with a descriptive approach. The results of this study indicate that the implementation of jogo tonggo in Pelem Hamlet covers two sectors namely the health sector and the social security sector. The implementation of jogo tonggo has not been running optimally due to lack of communication between the bureaucracy in socializing jogo tonggo program, the implementers does not understand the guidelines so they do not know their duties, functions and responsibilities. Limited authority of implementers because there is no organization decree. Theres is high dependence on village governments in program planning and fulfillment of resources.

Abstract

The infant mortality rate is the important things to measure health status and achievement of Sustained Development Goals (SDGs). It can be used to audit the cause of perinatal death and used to planning program to prevent the death. In addition, data on perinatal deaths must be recorded in terms of population administration. The problem is underreported perinatal death so that data not valid. This is because many government stakeholder such as Health Office, Administrative Village, Family Empowerment and Welfare (PKK), and the Information and Communication Department has reported and recorded perinatal death themself but not integrated. This study aims to develop a concept of integrated perinatal death report and record. This research is qualitative research with following steps advocation, initial assessment, and development models through interview, observation, and Focus Group Discussion methods. The result of this study is existing condition and concept model that can integrate of  perinatal death record and reported. The concept model in Semarang City, Indonesia can be done in two ways. First, data comes from the Health Office (SIM-KIA). Second, data comes from Administrative Village and Family Empowerment and Welfare (PKK). All of the data forwarded to the Integrated Data Integration System (SIDADU) in Information and Communication Service in Semarang City. To support the validity of data, the Health Office should identify all public and private health facilities related to perinatal care. The local government should release Circular Letter concerning the notification of services regarding the recording and reporting of perinatal deaths in Administrative Village.

Abstract

TITLEAnalysis of factors influencing dental nurse performances in UKGS program among Puskesmas in public health department of Bandung DistrictABSTRACTIn Bandung District data showed that many students suffer from bleeding gums, dental caries in students tend to rise nearly 90% in elementary school students, the average incidence of dental caries occur in the age group 6-12 years with DMF-T = 2,7. Where as the target of Healthy Indonesia 2010 on caries incidence is not more than two. Not optimal oral health care in children aged 6-12 years (primary school age), showed that the performance of dental nurses in UKGS care services was not optimal.This study was an explanatory research with cross sectional approach. Data were collected by interview and observation. The number of samples is 50 dental nurses.The results showed that the performance of dental nurses in the implementation of the District Health Office UKGS in Bandung is only about 58.0% and that have a good performance is only 42.0%. Variables that are directly related to the performance of dental nurses in the implementation of UKGS are training, knowledge and motivation The variables most affect the dental nurse performance of the implementation UKGS was training (RP 20.931) means that dental nurse with a good level of 21 times likely to have a better performance compared to the dental nurse with less knowledge, and the most influential factor was training. Means that dental nurses who have high training will have 21 times better performance compared with dental nurses with low training in the implementation of UKGS. Key words : Performance, dental nurse at implementation, UKGS

Abstract

A clinical practice guideline is a procedure implemented by group of a profession referred to National Guideline of Medical Service made by organisation of profession and approved by Director of a hospital. In providing health services to patients, radiology service must refer to the clinical practice guideline that is available in its service. The aim of this study was to explain the process of draft arrangement of the clinical practice guideline of radiology service at Ajibarang Public Hospital in Banyumas District. This was qualitative research aimed at investigating the arrangement process of the clinical practice guideline of radiology service collected from various sources and informants at Ajibarang Public Hospital in Banyumas District. Data were collected using methods of indepth interview, observation, and literature review, analysed using content analysis, and presented descriptively combined with matrix of interview results. In addition, data presentation was supported by results of field observation and literature review. The results of this research showed that viewed from aspects of resources, there were any barriers as follows: radiologist did not involve in the arrangement process and there was lack of communication between radiographer and an arrangement team of the clinical practice guideline of hospital. On the other hand, there was no barrier in the aspect of tariff pattern. Tariff of INA- CBGs have been used in a program of National Health Insurance and implemented since 1 January 2014. In the aspect of evidence availability, arranged clinical practice guideline was based on literature and textbook. A format of arranged clinical practice guideline draft consisted of title of action procedure, definition, indication, contra-indication, preparation, action procedure, post action procedure, levels of evidence, levels of recommendation, reviewers, indicators of action procedure of radiographer, and references. In conclusion, the arrangement process of the clinical practice guideline had been well implemented. However, radiologist had not involved. Director’s decree about arrangement team of the clinical practice guideline at Ajibarang Public Hospital needs to be revised. Radiology service needs to accomplish and determine final draft of the clinical practice guideline.

Abstract

COVID-19 has become a global pandemic in 2020. As of May 25, 2020 there were 22,750 confirmed cases of COVID-19 in Indonesia. Prevention and control of COVID-19 needs to be carried out by the village government which is in direct contact with the community. Supported by the issuance of Permendes No. 6 of 2020 concerning the Implementation Regulations for the COVID-19 Response Village Program. Meteseh Village is one of the villages in Kendal Regency that has a risk of spreading COVID-19. This research is a qualitative descriptive study using in-depth interviews using purposive technique with informants selected based on criteria and literature study. The results show that the implementation of the COVID-19 Response Village Program in Meteseh Village has not been running optimally due to unclear implementation standards in related policies, limited human resource capacity in program planning and resource management, especially in the aspect of controlling COVID-19 and the community attitudes that do not apply health protocols influenced by social, economic, political conditions. Therefore, Meteseh Village is advised to increase the role of village midwives to provide education about the handling of COVID-19 to program implementers, increase the participation of health workers, religious leaders and community leaders in Meteseh Village to assist in handling COVID-19, continue to provide informations of COVID-19 and conduct evaluation activities of program implementation including make a complete written report about program implementation.
