Abstract

Diabetes mellitus is a degenerative disease that has an increasing prevalence worldwide and Indonesia. In Bali, the prevalence is 5.9% of the total population. This number will continue to increase as the change of lifestyle and eating pattern in the community. This study aims to prove the risk factors that related to the incident of type 2 diabetes mellitus in out clinic patient of Sanglah Hospital.This was an observational analytic study using unmatched case control study. The respondents in this study consisted of case and control with 1:2 ratio.  Case was type 2 DM  outpatients that matched diagnostic criteria or type 2 DM patients that had been treated and visited Sanglah hospital. Control was non diabetic outpatients that came to sanglah hospital. The number of respondents was  84 respondent  that were determined using consecutive sampling.The instruments   were questioners, medical records, and fasting blood sugar levels. Univariate, bivariate (chi-square) and multivariate (multiple logistic regression) analysis  were used in this study. The result of this study showed the OR value of activity levels   was   4.298 (95% CI = 1.646 – 11.226. The OR value of fat consumtion patterns was 7.171 (95% CI=1.986-11.226). The OR of karbohidrat consumtion was 0.224 (95% CI: 0.071-0.703).   The result also showed that fat consumtion pattern had a chance to cause diabetic by 6.7  compared to activity levconsumtion of carbohidrat to cure diabetes at Sanglah Hospital, Denpasar.The health institutions should improve promotive and preventive measures to diabetic and non diabetic patients.

Abstract

Background and purpose: The introduction of an online referral system aims to improve the quality and costs control of the National Health Insurance (NHI) or Jaminan Kesehatan Nasional. This study aims to determine the impact of the NHI online referral system implementation on access and quality of health services in Gianyar District. Methods: This is an explorative qualitative study conducted in Gianyar, Bali, Indonesia. Data was collected through semistructured in-depth interviews to a total of 26 informants recruited purposively. They were including policy makers in the health office and the NHI implementing organization (BPJS), heads of public health centres (puskesmas), private health facilities and referral healthcare facilities; and 13 NHI participants. Data collection was undertaken from May to August 2019. The interview results were thematically analysed based on the Health Care Reform Control Knobs Framework. Results: The study identifies several positive impacts of the online referral system, which include: the closer distance of patients accessing services, the easier it is to obtain information on services at referral health facilities, and the easier the hospital to promote its service products, the higher assurance of patients being accepted by the referral hospitals, patients with special conditions are better accommodated, the easier the administrative procedure, improved time, cost and working procedure efficiencies, increased punctuality of the services, and guaranteed data security that reduces the risk of lost or damaged referral documents. On the other hand, the online referral system has also resulted in negative impacts, some of them were system inconsistencies, information and communication technology (ICT) constraints, service discontinuity, service injustices and loss of patients’ loyalty. Conclusion: NHI online referral system generally have positive impacts on access and quality of health services, however, it is necessary to improve several aspects of the online referral system to better accommodate user needs and to optimize service.

Abstract

Background and purpose: Mother-to-child transmission of HIV has been one of the problems in HIV prevention. In Indonesia, private practice midwives (PPM) play an important role in maternal and child health services. The roles of PPM in HIV vertical transmission prevention through PMTCT have not yet clearly understood. This research aims to analyse the roles of PPM in Bali Province in the PMTCT program as well as the PPM’s characteristics, knowledge, and attitudes about the program. Methods: This study was a descriptive cross-sectional survey, conducted from March to May 2020. The study population was PPM in Bali Province. The samples obtained were 404 out of a total of 1,736 PPM (23% response rate). The variables studied included socio-demographic characteristics, services and resources characteristics, knowledge, attitudes, and the role of PPM in four PMTCT prongs. Data was collected via self-filled online questionnaires distributed through Indonesian Midwives Associations branches in every district/city in Bali Province. Data were analysed descriptively and differences in roles in PMTCT were analysed using the chi-square test. Results: Of the 404 PPM, 66.1% served childbirth (deliveries), 25.4% had served HIV-positive women, and 18.3% had served HIV-positive pregnant women. As many as 95.5% of PPM played a good role in prong 1, and 99.0% of PPM, who had served HIV positive patients, played a good role in prong 2, 3 and 4. Statistically significant determinants for good implementation of prong 1 of PMTCT by the PPM were indicated by ownership or access to PMTCT information media (p=0.026), experience of serving HIV positive women (p<0.00), and having served higher number of PMTCT patients (p=0.019). Conclusion: The PPM in Bali Province has played a good role inPMTCT, with a significant difference for good PMTCT implementation in prong 1 determined by the availability of information media, a history of serving HIV-positive women, and the number of PMTCT patients who have been served.

Abstract

Background and purpose: The Birth Insurance Program or called Jaminan Persalinan (Jampersal) aims to reduce maternal and newborn deaths. Bangli is one of the districts in Indonesia with low utilisation of Jampersal program. The study aims to explore factors associated with the low utilization and uptake of Jampersal Program in Bangli District. Methods: This was a qualitative study conducted between April- May 2019. The study involved 24 informants who were selected purposively. The informants were 18 pregnant women, two community leaders, three health workers and one Bangli Health Office’s staff. Data were collected through in-depth interviews which explore factors associated to utilisation of Jampersal using PRECEDE-PROCEED Framework. Data were analyzed by thematic analysis. Results: We found that informants who used Jampersal had sufficient knowledge about Jampersal, were exposed to the program informed by health workers and community leaders, and were supported by their families. Whereas, those who yet to utilize Jampersal had a poor understanding on Jampersal service mechanisms, lack of spousal support in preparing Jampersal requirements and private midwives had not signed the Jampersal MoU. Conclusion: Low utilisation of Jampersal Program in Bangli District partially influenced by low understanding of the program and in-optimal involvement of midwifery networks. The local health office should further expand cooperation with the private midwife network for optimum utilization of Jampersal, elucidation of the Jampersal technical guidelines to health workers, as well as technical support for participants in completing required documents.

Abstract

Background and purpose: Health financing aspects of the public health centres (PHC) may influence the achievement of PHC’s programs and its minimum service standards (SPM). The District Health Account (DHA) is a model used to describe and evaluate health financing at the PHC level. The purpose of this study was to analyze health financing mechanism from both the government and national health insurance in the Karangasem District using the DHA Model. Methods: This study was an evaluation research with a case study approach using the DHA Model. Evaluations were carried out at two PHC, Manggis II PHC and Abang I PHC, between March and April 2018. Data were collected from the Karangasem District Health Office and the PHCs. These include PHC realization data, report on program achievements and SPM. The research instrument used for the data collection and analysis was the DHA extraction form and pivot table which includes information on cost sources, financing managers, service providers, functions, programs, types of activities, budget lines, levels of activities and beneficiaries. Results: We found the source of budget in both PHC was dominated by the district budget (APBD) and the national budget (APBN). The institutions which manage the budget and provide service were predominantly the Health Office and PHC. The most frequent financing functions was for governance and administration, while the largest types of activities covered was the indirect activities. Budget spending for operational costs was higher than for maintenance and investment. Realization of the activity for community program efforts (UKM) and prevention program (UKP) was lower than for the capacity building program. Conclusion: The DHA model was able to identify patterns of health budget allocation by health agencies. The use of the DHA model to analyze health financing should be adopted routinely in order to provide evaluation reports for health agencies at the district level.

Abstract

Background and purpose: Case manager services in inpatientrooms have gained popularity in many hospitals in Indonesia,however there are challenges in its implementation. This study aims to explore ways to optimize the characteristics of the case manager, case management practice and outcome of the case manager in cases in inpatient wards based on the Sanglah Hospital standards. Methods: The study utilized a two-stage Delphi study design,started with a qualitative data collection and followed by aquantitative survey. Data collection was done in May-June 2020 based on the concept of case manager characteristics, practice and outcomes. The qualitative data collection through in-depth interviews involved 9 key informants. It aimed to explore case manager services based on qualifications, competencies, ethical standards, workload, hierarchy and guidelines, implementation of case management and indicators of effectiveness of case management at Sanglah General Hospital. The results of the indepth interviews with thematic analysis informed the Delphi survey instrument development. The Delphi survey aimed to generate agreement on the characteristic of the case manager, case management practice and outcome of the case manager among 21 expert panelists (case managers) in the inpatient wards of Sanglah Hospital. This Delphi survey was conducted in 3 rounds, the results were analyzed based on the median value and quartile deviation (QD). Results: The three rounds of the Delphi survey showed that there was an agreement regarding the qualifications of the case manager, competency development, mechanism for appointment and placement of case managers, case manager guidelines, organizational structure, job descriptions and authorities, information of the presence and role of case managers, screening indicators according to characteristics of inpatient cases, case management target measurement tools, case manager service evaluation indicators as well as the activity feedback loop. Conclusion: Optimization of the case manager service should focus on aspects of human resources, developing guidelines and work systems as well as evaluation indicators and feedback mechanism.

Abstract

Introduction: Female sex workers (FSWs) remain an important key population affected by HIV&AIDS in Indonesia. Indirect FSWs, in particular, are exposing to higher risk comparing to direct FSWs due to their characteristics and working conditions that could be barriers to safe sex or consistent condom use. This study aimed to identify association between sociodemographic characteristics, work venue, and knowledge on HIV&AIDS toward condom use among indirect FSWs. Methods: This was a cross-sectional study using secondary data from a survey on indirect FSWs conducted by Center for Public Health Innovation (CPHI), Udayana University, Indonesia during August to October 2017. About 171 indirect FSWs were eligible for this analysis. The dependent variable in this study was consistent condom use during the last month. Independent variables consisted of socio-demographic characteristics, work venue, and knowledge on HIV&AIDS. Data were analysed using univariate and multivariate analysis. Results: About half of indirect FSWs (50.17%) reported using condom consistently with their clients during the last month. Indirect FSWs were more likely to use condom consistently if they completed senior high school or higher (OR=3.49; 95%CI=1.42-8.57), but less likely to use condom if they were currently or ever married compared to single ones (OR=0.23; 95%CI=0.06-0.82). Those who worked in karaoke, cafe, and bar were less likely to use condom consistently compared to those in massage parlor, spa, and beauty salon (OR=0.09; 95%CI=0.03-0.26). Conclusions: Level of consistent condom use was low, indicating the need to intensify HIV prevention to indirect FSWs. Significant determinants of consistent condom use included marital status, educational level, and work venue. This study informed that providing HIV&AIDS information and improving negotiation skill that fit with characteristics of indirect FSWs and giving institutional support at workplace is crucial to ensure consistent condom use among indirect FSWs.

Pada Penelitian ini dilakukan mekanisme metode asesmen risiko pada situs web SMP Negeri 1 Blahbatuh. situs web tersebut merupakan situs web resmi dari sekolah untuk mengikuti kegiatan belajar mengajar yang dikelola oleh pihak sekolah. Abstract

Nutritional status, nutrient density, and dietary diversity of children under five during the COVID-19 pandemic in Bangli Regency, BaliBackground: Stunting in children caused the lack of nutritionally balanced food intake in the first two years of life. During the COVID-19 pandemic, the fulfillment of balanced nutrition for children must still be considered to maintain body resistance to avoid COVID-19 infection.Objective: This study was conducted to find out the nutritional status of children and also the knowledge and attitudes of mothers regarding the practice of balanced nutrition based on local food in the COVID-19 pandemic situation. Methods: Cross-sectional study with 202 children aged 23-59 months located in Susut District, Bangli Regency, Bali Province. Data collection includes anthropometric measurements and food recall. Characteristics, knowledge, and attitudes of mothers were conducted by interview using a questionnaire. Results: The nutritional status of subjects showed that the average weight for height z-score (WHZ) was 0.29 ± 1.37, height for age z-score (HAZ) was 0.56 ± 2.38, and weight for age z-score (WAZ) which is -1.18 ± 2.03. The average density of energy intake in the adequate (1.66±0.456), as well as protein density (4.12±0.884 g/100 calories), vitamin B12 (0.225±0.457 mg/100 kcal), and vitamin A (71.7 ±104.6 mg/100 kcal). The majority (61.9%) of subjects consumed less than five food groups in the last 24 hours. The average knowledge of respondents is 69.5±15.3, which is 35.1% in well nutrition knowledge, while most respondent’s attitude has in the good category. Conclusions: Overall children are in normal nutritional status, but it is necessary to attention to the density of vitamins and minerals which are still low, and the lack of food diversity. Suggestion to increase education on balanced nutrition practices based on local food to mothers and families so that they can help grow and also maintain children's health during the COVID-19 pandemic.

Abstract

Finally, with the growing and high number of internet and social media users, it could be concluded that Indonesia is a potential country for the development of internet-social media based health intervention.18 Currently, there are abundant programs and/or campaign using internet and social media based as platforms for health information and promotion, however most of them were developed without proper evaluation design to measure its effectiveness. Universities and research centers in the country could become a potential partner for any developers for developing programs and/or campaigns that utilize internet and social media in standardized and scientific ways.
